Lactate-albumin ratio improves combined predictive value of qSOFA and MEWS for 30-day mortality in ICU patients with sepsis: A retrospective cohort study.

Evaluating disease severity and predicting adverse outcomes using various risk prediction tools in early disease stages is essential to reduce sepsis-related mortality. Unfortunately, there is still no clear consensus on the best score. The present study aimed to develop and validate a multivariable risk prediction model for 30-days mortality by combining the lactate-to-albumin (L/A) ratio, Modified Early Warning Score (MEWS), and quick Sequential Organ Failure Assessment (qSOFA) in sepsis patients admitted to the intensive care unit (ICU). This retrospective study included ICU patients with s

Dynamic rough set learning for reliable early warning in industrial time-series systems.

Industrial early-warning systems require models that can detect transitional risk states before failure while also explaining uncertainty in the resulting decisions. Existing machine-learning and deep-learning approaches can achieve strong predictive performance, but they often provide limited information about whether a time-series window is certainly normal, certainly critical or only ambiguously classified. This paper addresses this problem by proposing a dynamic rough set learning framework for uncertainty-aware early warning in industrial time-series systems. The proposed method converts

Clinical frailty scale at ICU discharge predicts ICU readmission and post-ICU mortality: A retrospective single-center study.

Despite successful discharge from the intensive care unit (ICU), a substantial number of patients remain at risk of ICU readmission or death. Identifying high-risk individuals at the time of ICU discharge is essential for planning post-ICU care. This study aimed to assess the Clinical Frailty Scale (CFS) at ICU discharge as a screening tool for predicting ICU readmission and post-ICU mortality, and to compare its predictive performance with other commonly used scoring systems. We conducted a retrospective single-center study including adult patients (≥20 years) discharged from all ICUs to gene

Protein interaction mapping in C. elegans using proteins involved in vulval development.

Early recognition and timely escalation of care are critical for improving sepsis outcomes. This post-hoc analysis of a multi-site clinical trial examined whether the timing of ICU transfer following CONCERN Early Warning System (EWS) score escalation was associated with in-hospital mortality among patients later diagnosed with sepsis. Among 54 patients with CONCERN score changes prior to unanticipated ICU transfer, shorter score-change-to-ICU-transfer time intervals were significantly associated with lower odds of in-hospital death. A 36-hour threshold emerged as a potential inflection point;

Feasibility and usability of a combat medical early warning system for simulated mass casualty triage during a naval exercise - A pilot study.

INTRODUCTION: Mass casualty incidents (MASCAL) in modern naval large-scale combat operations (LSCO) present unique triage challenges due to the confined environments, operational constraints, and saturation of medical resources. Combat Medical Early Warning Systems (CMEWS)-integrating wearable biosensors with automated early warning scores (EWS)-may support dynamic triage, but their use in a naval military setting has not been previously studied. MATERIALS AND METHODS: This prospective observational pilot study was conducted aboard a French Navy amphibious helicopter carrier during the POLARI

Associations Between Frailty, Illness Severity, and Long-Term Mortality Among Older Adults Admitted to Municipal Acute Care.

OBJECTIVES: Important risk factors for mortality include frailty and illness severity. The National Early Warning Score 2 (NEWS2) is widely used to assess acute illness severity and mortality risk, but age and frailty's impact on the association between NEWS2 and mortality is less understood, particularly in nonhospital settings. This study aimed to examine the associations between frailty, NEWS2, and 1-year postdischarge mortality in older adults admitted to the Oslo Municipal In-Patient Acute Care (MipAC) unit. DESIGN: Prospective observational study. SETTING AND PARTICIPANTS: Patients adm

qREDS score compared with the REDS and NEWS2 scores to risk-stratify emergency department suspected sepsis patients for in-hospital mortality and 168-hour survival probability: a retrospective cohort study.

BACKGROUND: The Risk-stratification of Emergency Department suspected Sepsis (REDS) score requires external validation. The six dichotomous variables and lactate can be accurately extracted electronically unlike refractory hypotension (RH), where the recording of the fluid-bolus and blood pressure measurement may be inaccurate. The impact of excluding RH is not known. OBJECTIVES: To develop the quick-REDS (qREDS) score, by removing RH from the REDS score and to validate its prognostic performance in comparison to the original score and the National Early Warning Score (NEWS)2 score, for all-c

Development and validation of a dynamic early warning system with time-varying machine learning models for predicting hemodynamic instability in critical care: a multicohort study.

BACKGROUND: Hemodynamic instability, a life-threatening condition marked by circulatory failure, presents a significant challenge in intensive care unit (ICU) settings, often leading to poor patient outcomes. Traditional monitoring methods that rely on single parameters may delay diagnosis. Machine learning models offer a solution by integrating multiple clinical parameters to more dynamically and accurately predict instability. METHODS: We developed the Time-varying Hemodynamic Early Warning Score (TvHEWS), an AI-assisted model used to predict hemodynamic instability in intensive care unit (I

Deep learning early warning system for embryo culture conditions and embryologist performance in the ART laboratory.

Staff competency is a crucial component of the in vitro fertilization (IVF) laboratory quality management system because it impacts clinical outcomes and informs the key performance indicators (KPIs) used to continuously monitor and assess culture conditions. Contemporary quality control and assurance in the IVF lab can be automated (collect, store, retrieve, and analyze), to elevate quality control and assurance beyond the cursory monthly review. Here we demonstrate that statistical KPI monitoring systems for individual embryologist performance and culture conditions can be detected by artifi

Neonatal Early Warning Tools for recognising and responding to clinical deterioration in neonates cared for in the maternity setting: A retrospective case-control study.

BACKGROUND: All newborns are at risk of deterioration as a result of failing to make the transition to extra uterine life. Signs of deterioration can be subtle and easily missed. It has been postulated that the use of an Early Warning Tool may assist clinicians in recognising and responding to signs of deterioration earlier in neonates, thereby preventing a serious adverse event. OBJECTIVE: To examine whether observations from a Standard Observation Tool, applied to three neonatal Early Warning Tools, would hypothetically trigger an escalation of care more frequently than actual escalation of
