A clinical study of viper bite poisoning.

Thirty-one consecutive cases of snake bite were studied in the Benue Valley, Nigeria where carpet viper (Echis carinatus) bite constitutes a serious health problem. E. carinatus was responsible for 26 cases, the night adder (Causus maculatus) for three and the puff adder (Bitis arietans) for two. There were two fatalities, both late admissions following E. carinatus poisoning. One patient died after a subarachnoid haemorrhage and the other after tissue necrosis and the complications of a tightly applied tourniquet. Another fatality after E. carinatus bite was suspected in a patient who dischar

A best-worst scaling statistical design of experiment study to model healthcare workers' prioritization of snake antivenoms available in sub-Saharan Africa for treating snakebite envenoming: Perspectives from Ghana.

INTRODUCTION: In low- and middle-income countries (LMICs) with tropical or subtropical climates and a high poverty rate, the risk of snakebite envenoming (SBE) is high. Healthcare workers, together with patients, are the end-users of antivenom. This study aimed to assess healthcare workers' antivenom prioritization by providing quantitative evidence to guide policymakers to make better decisions to improve the procurement and supply of antivenoms, enhance the effectiveness of snakebite treatment, and improve patient care quality in Ghana. METHOD: We gathered data through an interview-based su

First draft of the genomic organization of a PIII-SVMP gene.

The evolutionary pathway of highly toxic proteins expressed in snake venom glands from proteins without toxic function and expressed in non-parotid tissues remains poorly understood. Here we examine gene structure of a representative of a venom protein with an ADAMs metalloproteinase evolutionary origin. The structure of the 15,652 bp Echis ocellatus pre-pro EOC00089-like PIII-SVMP gene was assembled from PCR-amplified sequences of overlapping genomic fragments. The gene comprises 12 exons interrupted by 11 introns. In a homology model of the EOC00089-like protein, the insertion of introns int

Effectiveness of the electronic Community Health Information System on maternal continuum of care completion in rural East Shewa Zone, Eastern Ethiopia: A prospective controlled quasi-experimental study.

Maternal mortality in Ethiopia remains high, and strengthening the continuum of maternal care is essential for reducing preventable deaths. The electronic Community Health Information System was introduced to improve service delivery through decision support, client tracking, and electronic referrals, but evidence of its effect in routine settings is limited. This study assessed the effect of the eCHIS's Reproductive, Maternal, Newborn, Child Health and Nutrition module intervention on completion of the maternal continuum of care. A prospective controlled pre-post quasi-experimental study desi

Identification and phylogeny of Arabian snakes: Comparison of venom chromatographic profiles versus 16S rRNA gene sequences.

Identification of snake species is important for various reasons including the emergency treatment of snake bite victims. We present a simple method for identification of six snake species using the gel filtration chromatographic profiles of their venoms. The venoms of Echis coloratus, Echis pyramidum, Cerastes gasperettii, Bitis arietans, Naja arabica, and Walterinnesia aegyptia were milked, lyophilized, diluted and centrifuged to separate the mucus from the venom. The clear supernatants were filtered and chromatographed on fast protein liquid chromatography (FPLC). We obtained the 16S rRNA g

Snake Venom Protease Detection and Inhibition in Serum.

Snake bites remain major threats with only limited diagnostic and therapeutic options. Current antibody-based antivenoms show only limited effectiveness against local tissue damage and require complex manufacturing and cold chain logistics. To overcome these limitations, a fluorescence-based assay was established to enable the sensitive detection of snake venom metalloprotease (SVMP) and serine protease (SVSP) activities in crude viper venoms from Crotalus atrox, Bothrops jararaca, and Echis carinatus. The assays showed species-specific activity profiles with detection limits in the sub-microg

Varespladib Inhibits the Phospholipase A2 and Coagulopathic Activities of Venom Components from Hemotoxic Snakes.

Phospholipase A2 (PLA2) enzymes are important toxins found in many snake venoms, and they can exhibit a variety of toxic activities including causing hemolysis and/or anticoagulation. In this study, the inhibiting effects of the small molecule PLA2 inhibitor varespladib on snake venom PLA2s was investigated by nanofractionation analytics, which combined chromatography, mass spectrometry (MS), and bioassays. The venoms of the medically important snake species Bothrops asper, Calloselasma rhodostoma, Deinagkistrodon acutus, Daboia russelii, Echis carinatus, Echis ocellatus, and Oxyuranus scutell

Protective effects of a natural herbal compound quercetin against snake venom-induced hepatic and renal toxicities in rats.

Echis pyramidum is a highly poisonous viper snake. Previous studies have shown acute phase hepatic and renal toxicities of Echis pyramidum venom (EPV) in rats. This study reports the protective effects of a natural herbal compound quercetin (QRC) on EPV-induced hepatic and renal toxicities in rats. A singly injection of EPV (4.76 mg/kg) caused significant increase in serum biomarkers of liver and kidney function. Pre-treatment of QRC (10 mg/kg) significantly reduced the toxic effects of EPV on functional impairment in liver and kidneys of rats. Administration of QRC also reversed EPV-induced i

Extreme Procoagulant Potency in Human Plasma of Venoms from the African Viperid Genera Atheris, Cerastes, and Proatheris and the Relative Efficacy of Antivenoms and Synthetic Enzyme-Inhibitors.

The African viperid snake genera Atheris, Cerastes, and Proatheris are closely related, similar in size, but occupy extremely divergent ecological niches (arboreal in tropical rainforests, fossorial in deserts, and swamp-dwelling, respectively). Their venoms have not previously been subjected to comparative analyses for their action upon the coagulation of blood, most notably with significant data deficiencies from Atheris and Proatheris. In contrast, the closely related genus Echis is well-documented as capable of producing potent procoagulant effects. In light of this, we set out to compare

A polygeneric immunogen composed of 22 venoms from sub-Saharan African snakes to expand the neutralization scope of the EchiTAb-plus-ICP antivenom.

Recent research suggests that a polygeneric immunogen made from the venoms of the most medically important viperid and elapid snakes in sub-Saharan Africa could elicit a broader antibody response in horses compared to the current EchiTAb-plus-ICP antivenom, especially against neurotoxic elapid venoms. To test this, 25 horses that have been regularly immunized to produce this antivenom were reimmunized with an immunogen containing 22 venoms from various snake species from the genera Bitis, Echis, Dendroaspis, and both spitting and non-spitting Naja. The plasma collected from these horses was pr

Interaction of echicetin with a high affinity thrombin binding site on platelet glycoprotein GPIb.

Echicetin, a protein isolated from Echis carinatus snake venom, inhibited platelet aggregation and secretion induced by low concentrations of thrombin ( < 0.2 U/ml), by binding to platelet glycoprotein Ib (GPIb). The inhibition was not observed when the platelets were stimulated with higher concentrations of thrombin ( > 0.2 U/ml). Echicetin competed with thrombin for binding to the high affinity site on GPIb. Thrombin also inhibited 50% of the binding of 125I-echicetin to the platelets.

In vitro snake venom detoxifying action of the leaf extract of Guiera senegalensis.

The extract of the leaves of Guiera senegalensis was found to detoxify (in vitro) venom from two common northern Nigerian snake species, Echis carinatus and Naja nigricollis, in separate experiments. There was a remarkable reduction in the mortality of albino mice after intra-peritoneal (i.p.) administration of reconstituted venom incubated with the extract, when compared to those challenged with the venom only. The survival of the animals exposed to the venom incubated with the different concentrations of the extract was used as the in vitro detoxification parameter.

Micro-ELISA for detecting and assaying snake venom and venom-antibody.

Enzyme-linked immunosorbent assay (ELISA) can detect venom levels down to 1-5 ng/ml. The assay is specific; no cross-reaction of clinical importance was found in a range of 14 different types of venom. Both venom and venom-antibody levels were monitored in mice. Specific antibody was detected in human serum more than 2 years after an accidental bite by Echis carinatus. ELISA should clarify many basic problems of envenoming in man.

[Defibrination syndrome after snake bites].

A defibrination syndrome developed in two men (aged 51, and 29 years old) within two hours of having been bitten by vipers Echis carinatus and Agkistrodon halys, respectively. In both the syndrome was characterized by afibrinogenemia with prolongation of the thrombin time, presence of fibrin monomers and split products of fibrinogen. Haemorrhagic signs included oral mucosa bleeding and macrohematuria. Specific serum was administered 9 and 16 hours, respectively, after the bite and achieved normalization of all clotting values within 48 hours.

Gene tree parsimony of multilocus snake venom protein families reveals species tree conflict as a result of multiple parallel gene loss.

The proliferation of gene data from multiple loci of large multigene families has been greatly facilitated by considerable recent advances in sequence generation. The evolution of such gene families, which often undergo complex histories and different rates of change, combined with increases in sequence data, pose complex problems for traditional phylogenetic analyses, and in particular, those that aim to successfully recover species relationships from gene trees. Here, we implement gene tree parsimony analyses on multicopy gene family data sets of snake venom proteins for two separate groups

Differential evolution and neofunctionalization of snake venom metalloprotease domains.

Snake venom metalloproteases (SVMP) are composed of five domains: signal peptide, propeptide, metalloprotease, disintegrin, and cysteine-rich. Secreted toxins are typically combinatorial variations of the latter three domains. The SVMP-encoding genes of Psammophis mossambicus venom are unique in containing only the signal and propeptide domains. We show that the Psammophis SVMP propeptide evolves rapidly and is subject to a high degree of positive selection. Unlike Psammophis, some species of Echis express both the typical multidomain and the unusual monodomain (propeptide only) SVMP, with the

Quantifying venom in African snakes: Insights into protein content, yield and body size associations.

Snake venoms are complex mixtures primarily composed of toxic proteins used during prey capture and defence. There is limited knowledge concerning the protein concentration of snake venom and the biases of different protein determination methods. Here, we assess the ability of the Qubit protein assay, bicinchoninic acid (BCA) assay, Bradford assay and NanoDrop spectrometry (A280 with a mass extinction coefficient of one) to accurately quantify protein concentrations of toxins isolated from venom, including three-finger toxins and phospholipase A2. The Bradford assays severely underestimated th

Investigating Snake-Venom-Induced Dermonecrosis and Inflammation Using an Ex Vivo Human Skin Model.

Snakebite envenoming is a neglected tropical disease that causes >100,000 deaths and >400,000 cases of morbidity annually. Despite the use of mouse models, severe local envenoming, defined by morbidity-causing local tissue necrosis, remains poorly understood, and human-tissue responses are ill-defined. Here, for the first time, an ex vivo, non-perfused human skin model was used to investigate temporal histopathological and immunological changes following subcutaneous injections of venoms from medically important African vipers (Echis ocellatus and Bitis arietans) and cobras (Naja nigricollis a
