In throwing activities, the elbow is sometimes stressed to its biomechanical limits. In this article, forces, torques, angular velocities, and muscle activity about the elbow are reviewed for the baseball pitch, the football pass, the javelin throw, the windmill softball pitch, the tennis serve, and the golf swing. The elbow goes through rapid extension in baseball pitching (about 2400 degrees/s) and rapid flexion in the javelin throw (about 1900 degrees/s). During baseball pitching, the elbow joint is subject to a valgus torque reaching 64 Nm, and requires proximal forces as high as 1000 N to

Osteochondritis dissecans of the elbow is a localized condition of the articular surface that is commonly seen in the young athlete. This disorder refers primarily to lesions of the capitellum and can be difficult to treat. Although trauma and ischemia play significant roles, the exact etiology remains unknown. The natural history is poorly understood and long-term sequelae include degenerative arthritis. The integrity of the articular surface and the stability of the lesion can be carefully evaluated with MRI and arthroscopy. Management is based mainly upon these two factors, yet no good univ

Lateral epicondylitis is a diagnostic term that describes a pattern of pain and localized tenderness at the lateral epicondyle of the distal humerus. The disorder was originally termed tennis elbow in 1883. This term remains in use despite the fact that most affected people are not tennis players. The incidence of lateral epicondylitis is equal among men and women. The average peak age distribution is 42 years (range, 30-50 years). Acute onset of symptoms is much more common in young athletes, and the chronic, recalcitrant pattern most often occurs in older people. This paper discusses the path

The authors describe a rare case of recurrent posterior post-traumatic subdislocation of the elbow in a male aged 40 years. From a clinical point of view, it is a highly disabling condition, due to lesion of the ulnar portion of the radial collateral ligament. It is produced at minimum degrees of flexion movement in the elbow and depending on the entity of capsuloligamentous injury it may occur with episodes of instability, recurrent subdislocation or recurrent dislocation. Treatment is surgery and capsuloligamentous reconstruction according to the Osborne-Cotteril method, which has allowed fo

Among the entrapment neuropathies, ulnar neuropathy at the elbow is second only to carpal tunnel syndrome in frequency; however, diagnosis and management are considerably more difficult in ulnar lesions than in carpal tunnel syndrome. Electrodiagnosis is the most important means of identifying and localizing ulnar neuropathies at the elbow, but even sophisticated techniques may sometimes fail to confirm diagnosis and localization preoperatively. Mild lesions are best managed conservatively. More severe lesions require surgical intervention. Simple decompression is now preferred over transposit

Osteochondritis dissecans of the elbow remains one of the leading causes of permanent elbow disability in adolescents and young adults engaged in throwing sports or gymnastics. The insidious onset of lateral elbow pain and restriction of full extension should alert the physician and prompt further investigation. Early recognition and appropriate treatment may allow for the prevention of long-term sequelae. Conservative care following early detection provides the best opportunity for a complete recovery. Surgical management at this point consists primarily of excision or removal of the osteocho

Craniosynostosis may occur in conjunction with limb and visceral anomalies in more than 100 syndromes and may include anomalies of the elbow. Apert's, Pfeiffer's, Crouzon's, and Saethre-Chotzen syndrome have been linked with anomalies of the elbow, but the incidence and severity of such anomalies is unknown. A prospective radiographic study was undertaken to establish the types, incidence, and severity of elbow anomalies in patients with either Apert's, Pfeiffer's, Crouzon's, or Saethre-Chotzen syndrome attending the Craniofacial Centre at Great Ormond Street Hospital during a 12-month period.

Proximal radioulnar translocation is an extremely rare injury. Only seven cases have been reported previously. The authors report the case of a 6-year-old girl whose translocation was undiagnosed for more than 2 weeks. Definitive treatment required open reduction and use of a radiocapitellar pin to stabilize the proximal radius. At the 2-year followup, the patient has no symptoms and has resumed full activity with nearly full range of motion. To ensure early diagnosis of rare types of elbow dislocations, it is important to recognize the lack of supination and disruption of the radiocapitellar

A series of 31 patients who had elbow arthroscopy has been reviewed. Good results have been achieved after removal of loose bodies or post-traumatic hematoma. A danger of injury to periarticular structures due to the use of arthroscopic instruments, especially with pathologically decreased joint volume has been pointed out.

Athletes expose their elbows to significantly high forces, in addition to multiple repetitive motions. Athletic injuries about the elbow include ligamentous, tendon, articular, and nerve injuries. The proper diagnosis requires a thorough knowledge of the anatomy, pathomechanics, and physical findings associated with these injuries. Treatment entails a methodical progression from nonoperative to operative intervention.

We investigated the characteristics of hemophilic arthropathy of the elbow joint in 32 patients 5-12 years of age who had been followed-up from infancy. Restriction of forearm rotation and elbow extension was the most common finding in the joints with recurrent hemarthrosis. Limitation of rotation resulted mainly from hypertrophy of the radial head. Advancement of bone age was also commonly observed in the joints with hemarthrosis. Mild to moderate hemophilic arthropathy was associated with greater advancement of bone age than was severe arthropathy. There was a discrepancy between the growth

Ulnar nerve entrapment is the second most common compressive neuropathy in the upper extremity because of its anatomy and superficial location. Major aetiological factors in the development of ulnar neuropathy of the elbow are compression, inherent anatomical structures, or lesions within the cubital tunnel. Extrinsic nerve compression may be elicited by acute or recurrent trauma. Nerve mobility may be impeded by congenital deformities. Ulnar nerve dysfunction has been associated with metabolic conditions, certain occupations and athletes involved in repetitive overhead activities. Ulnar nerve

A case of a recurrent dislocation of the elbow joint in a 13-year-old female is described. The patient sustained a medial epicondylar fracture of the humerus and consulted us four months later. There was no deformity but marked lateral instability of the elbow was recognized. It could be fully flexed and hyperextended ca 20 degrees. The elbow joint could be easily dislocated by pronation of the hyperextended elbow. Roentgenogram demonstrated an apparent non-union of the medial epicondylar fracture. The fragment was repositioned so that the medial collateral ligament became taut. There was no r

Of 27 patients who underwent magnetic resonance (MR) imaging of the elbow, 11 underwent elbow arthroscopy and/or an open surgical procedure. Surgical findings were compared with those from MR imaging. Five healthy volunteers also underwent MR imaging to demonstrate anatomic relationships. Transchondral fracture (osteochondritis dissecans) was identified in three of the 11 patients and was proved at surgery. Loose bodies were suspected at MR imaging in the three patients but were found in only two. One complete avulsion of the ulnar collateral ligament (UCL) and four cases of intact, thickened

Twelve cases of recurrent dislocation of the elbow have been studied with special reference to the cause of instability, the operative findings and the results of treatment. We consider the major cause to be insufficiency of the posterolateral capsule and ligaments, and therefore the best treatment is a posterolateral capsulorraphy.
