Early Functional and Radiological Outcomes of Radial Head Replacement in the Management of Radial Head Fractures: A Prospective Study.

Background Radial head fractures, particularly Mason type III and IV, often require radial head replacement (RHR) to restore elbow stability and function when fractures are unreconstructable. This study evaluates the short-term functional and radiological outcomes of RHR in managing complex radial head fractures Methods A prospective study was conducted from January 2023 to January 2025 at Government Medical College, Amritsar, India, involving 25 patients with Mason type III (n=21, 84%) and type IV (n=4, 16%) radial head fractures treated with monopolar cemented RHR. Functional outcomes were a

Long-term outcomes of a cemented bipolar radial head prosthesis: a large retrospective study.

BACKGROUND: In acute radial head fractures (RHFs), a radial head prosthesis can be considered if open reduction and internal fixation are not technically feasible. This study provides the largest long-term clinical and radiologic follow-up study of cemented bipolar Judet prostheses implanted to manage acute RHFs, either isolated and associated with other elbow fractures. We compare their functional and radiographic outcomes to test the hypothesis that the bipolar design can achieve similar results in both groups. METHODS: We reviewed data of 63 consecutive patients implanted with a bipolar pr

Radial head resection versus prosthetic arthroplasty in terrible triad injury: a retrospective comparative cohort study: a retrospective comparative cohort study.

AIMS: The crucial role of the radial head in the stability of the elbow in terrible triad injury is acknowledged. This retrospective study aims to compare the results of resection of a severely comminuted radial head with or without prosthetic arthroplasty as part of the reconstruction for this injury. PATIENTS AND METHODS: The outcome of radial head resection was compared with prosthetic arthroplasty in 29 and 15 patients with terrible triad injuries, respectively. There were ten female patients (34.5%) in the resection group and six female patients (40%) in the prosthesis group. The mean ag

3D printing-assisted fabrication of a patient-specific antibacterial radial head prosthesis with high periprosthetic bone preservation.

We present a novel fabrication and surgical approach for anatomical reconstruction of a fractured radial head using a patient-specific radial head prosthesis (RHP) made of polymethylmethacrylate (PMMA) bone cement. To this end, the use of PMMA bone cement for prosthesis fabrication was initially investigated using computational modeling and experimental methods. The RHP was fabricated through casting of PMMA bone cement in a silicone mold in the operating room before implantation. To enhance the precision of bony preparation for replacement of the radial head, a patient-specific surgical guide

Linked semiconstrained and unlinked total elbow replacement in juvenile idiopathic arthritis: a case comparison series with mean 11.7-year follow-up.

BACKGROUND: Few series report the results of total elbow replacement (TER) in patients with juvenile idiopathic arthritis (JIA). Most report the use of a linked implant. There are theoretical benefits to using an unlinked prosthesis, and thus we report our experience of the clinical benefit and survivorship of both this implant and a linked semiconstrained prosthesis. METHODS: There were 21 elbows replaced in 14 JIA patients (12 women and 2 men; 14 unlinked, 7 linked). Mean age at surgery was 39.5 years (range, 26-52 years). Mean clinical follow-up was 11.7 years (range, 5.4-17.6 years). RES

[Secondary reconstruction of stumps above the elbow. Five cases].

The authors report a serie of five patients with five stumps above the elbow who needed a secondary reconstruction to allow or facilitate a prosthesis. They used in the first case an expansion of the latissimus dorsi flap and of the axillary and prepectoral region in order to free the stump of the humerus sutured on the thorax. In the second case, a free parascapular flap covered an unstable scar of the clavicula after a scapulothoracic amputation. In the third case, the transfer of a free flap of fibula associated with a pedicled latissimus dorsi flap had allowed the elongation of the stump o

A modified posterior approach to the elbow for total elbow replacement.

Fifty-nine consecutive primary total elbow replacements were performed with the modified posterior approach. The approach differs from other described approaches. The fascia and periosteum over the subcutaneous border of the ulna are preserved, and dissection is carried out on either side of the ulna. This enables a more secure repair of the posteromedial and posterolateral muscle compartments. The ulnar nerve is mobilized to prevent any injury. The distal humerus and proximal ulna can be fully exposed by this approach, giving wide access so necessary for accurate positioning of the prosthesis

Radial head replacement or repair for the terrible triad of the elbow: which procedure is better?

BACKGROUND: The terrible triad of the elbow comprises an ulnar coronoid process fracture, a radial head (RH) fracture and posterior dislocation of the elbow. It is considered severe by many clinicians because it is difficult to repair and has a poor prognosis; therefore, it deserves more attention. METHODS: Thirty-nine patients with terrible triad of the elbow characterized by a Mason type-III RH fracture were randomly divided into either an RH repair group (n = 19) or an RH replacement group (n = 20). The complication rates, radiological outcomes, Mayo Elbow Performance Score and range of mo

Elbow arthritis.

Patients with elbow arthritis typically present with complaints of pain and stiffness. Rheumatoid arthritis is the most common cause of elbow arthritis, followed by posttraumatic arthritis and primary osteoarthritis. Nonoperative management consisting of oral analgesics, intra-articular steroid injections, physical therapy, and splinting may provide symptomatic relief in the majority of patients. If these modalities fail, operative treatment is guided by the severity of disease as well as several patient-related factors such as age, activity level, and expectations. Total elbow arthroplasty ca

Single-stage revision of peri-prosthetic infection following total elbow replacement.

This study reviews the predisposing features, the clinical, and laboratory findings at the time of diagnosis and the results of single-stage revision of prosthetic replacement of the elbow for infection. Deep infection occurred in six of 305 (1.9%) primary total elbow replacements. The mean follow-up after revision was 6.8 years (6 months to 16 years) and the mean age at the time of revision was 62.7 years (56 to 74). All six cases with infection had rheumatoid arthritis and had received steroid therapy. The infective organism was Staphylococcus aureus. Four of the six elbows had a developed r

Acute Joint Blockage due to Abrasion-Related Dislocation of a Silastic Radial Head Prosthesis: A Histological Examination after 14 Years of Durability.

The implantation of a radial head prosthesis can take place as a therapeutic option after radial head fracture. There are various implants for this purpose. Many studies and case reports about silastic radial head prosthesis implantation describe foreign body reactions with accompanying synovitis and poor functional results. A few studies have investigated the reason for the material failure and the accompanying synovitis. The case report presented shows an unusually long durability of an in situ 14-year silastic radial head prosthesis. 14 years after implantation, a previously full-time worki

Design of intramedullary prostheses to prevent bone loss: predictions based on damage-stimulated remodelling.

The adaptation of bone around intramedullary fixated prostheses, such as the femoral component of the hip joint or the radial component of the elbow joint, is well documented in follow-up studies. Bone adaptation takes the form of proximal bone atrophy accompanied, in some cases, by distal bone hypertrophy. A mechanistic model has been formulated to predict bone adaptation based on the concept that the continuous processes of damage and repair regulate bone adaptation. We apply the model to investigate the significance of two features of intramedullary prosthesis design on bone adaptation: pro

[Fresh fractures of the radial head: results with the Judet prosthesis].

PURPOSE OF THE STUDY: Communitive fractures of the radial head are a therapeutic challenge when fixation is not possible. Secondary sequelae including ulnar valgus, ascension of the radius, osteoporosis of the humeral condyle and biomechanical impairment of the elbow cannot be avoided with resection or the Swanson prosthesis. The Judet prosthesis, with its floating cup, is a technically attractive solution, but the question is whether it can avoid the secondary effects observed with resection or the Swanson prosthesis. MATERIAL AND METHODS: We have used the Judet prosthesis since August 1995

[Endoprosthetics of the radial column after trauma].

Non-displaced fractures of the radius head are in most cases treated conservatively. Open reduction and anatomical internal fixation of displaced radius head fractures is the method of choice. In comminuted fractures of the radius head (Mason type III and type IV) replacement with a radius head prosthesis achieves joint stability and prevents secondary complications, such as valgus elbow deformity and proximal radial migration. Modern anatomically formed prostheses show promising results in the medium-term view. Typical complications after radius head replacement are limited range of motion in

Massive Upper Extremity Hemihyperplasia Secondary to Neurofibromatosis.

CASE: A 30-year-old woman with type 1 neurofibromatosis presented with enlargement of the right upper extremity. The condition began in childhood and progressed until the limb reached a nonfunctional and burdensome size from elbow to fingertips. Radiographs showed elongated, thinned bones, and CT demonstrated diffuse soft tissue infiltration. Above-elbow amputation was performed. Pathology revealed diffuse neurofibromatosis of skin, subcutaneous tissues, and muscle without malignant change. Postoperatively, she was fitted with an aesthetic prosthesis and reported marked physical and emotional

Short- to medium-term outcomes of radial head replacement arthroplasty in posttraumatic unstable elbows: 20 to 70 months follow-up.

BACKGROUND: The radial head is considered the main stabilizer of the elbow when the medial collateral ligament and lateral ulnar collateral ligament have been compromised. Radial head replacement (RHR) is indicated for patients with irreparable or non-united radial head fractures associated with elbow stiffness or instability. The present study aimed to analyze the clinical results after treatment with titanium radial head prostheses, repair of torn soft tissue constraints, and early mobilization of the elbow. METHODS: From 2002 to 2008, 13 patients with radial head fractures were included. R
