Electromyography in musculoskeletal pain: A reappraisal and practical considerations.

BACKGROUND: Patients with musculoskeletal pain (MSP) and local tenderness in the back and extremities are frequently referred to electromyography (EMG) laboratory to assess the integrity of the spinal nerve roots, peripheral nerves, and skeletal muscles. When focal muscle weakness and anatomical sensory deficits are clinically evident, this procedure is almost always abnormal. In some situations, when the presenting symptoms consist of local pain and tenderness without neuromuscular deficits, its diagnostic utility becomes questionable as illustrated in the present study. METHODS: EMG finding

Prospective evaluation of the clinical utility of laryngeal electromyography.

OBJECTIVES/HYPOTHESIS: To prospectively evaluate the clinical utility of laryngeal electromyography (LEMG) STUDY DESIGN: Prospective observational study. METHODS: The study involved 50 consecutive patients referred for LEMG. Laryngologists initially indicated diagnoses and treatment plans under the assumption of no access to LEMG. Patients then underwent LEMG by blinded examiners. LEMG results were reviewed by each patient's laryngologist. Diagnoses and treatment plans were either maintained or altered based on the LEMG results. RESULTS: The diagnosis changed 10% (5/50) of the time and treat

[The role of electromyography in cases of vocal cord paralysis].

BACKGROUND: Several pathologies can involve muscles that control vocal folds. The abnormality can affect peripheral nerves or central nervous system centers. Clinically, muscle function can be assessed by observing the movement of structures themselves or by recording electrical activity of these muscles using (electromyography-EMG). Since EMG is an invasive technique, its use is not very widespread in the diagnosis and management of voice disorders, Laryngeal EMG can be helpful in those patients with voice problems of suspected neurological or neuromuscular etiology. OBJECTIVE: Assess the ro

Cricopharyngeal electromyography: patterns of injury based on etiology.

OBJECTIVE: Although cricopharyngeal electromyography (CP-EMG) is recognized as a diagnostic tool for dysphagia assessment, few reports in the literature characterize CP-EMG abnormalities in relation to clinical presentation. The aim of this study was to review a large series of CP-EMG studies, and compare the CP-EMG results with the patients' diagnoses. METHODS: A retrospective review of all CP-EMG performed at our institution over a 10-year period was executed. CP-EMG findings were then compared with the patients' clinical history, focusing on potential etiologies of neurogenic injury. RESU

Comparison of quantitative techniques in anal sphincter electromyography.

Data comparing results and utility of different quantitative electromyographic (EMG) techniques are limited. In the present study, we analyzed the EMG signal from the external anal sphincter (EAS) muscle using three techniques of motor unit potential (MUP) analysis, and a technique of interference pattern (IP) analysis. We examined 56 patients with damage to the cauda equina or conus medullaris, and 64 control subjects. Using manual-MUP and multi-MUP analysis about 20 MUPs, using a single-MUP technique about 10 MUPs, and using turn/amplitude (T/A) analysis about 20 IP samples were obtained. Th

[Electromyography monitoring in the treatment of torticollis with botulinum toxin].

UNLABELLED: The new method of treatment chronic torticollis is injection into affected muscles botulinum's toxin. Some physicians in diagnostic procedure, treatment and monitoring use needle electromyography (nEMG). The aim of our study was the estimation of the value of nEMG in chronic torticollis treatment. We examined 34 patients with chronic torticollis 4 weeks after botulinum's toxin injection into affected muscles. We have been looking for denervations signs in affected muscles. We found denervations signs in 26 patients. In both muscles sterno-cleido-mastoideus and trapezius we found de

The role of quantitative electromyography in inclusion body myositis.

OBJECTIVE AND METHODS: Inclusion body myositis is said to have both myopathic and neurogenic features on electrophysiological tests. Twenty one studies from 20 patients with biopsy defined inclusion body myosis, 13 of whom had quantitative electromyography (qEMG), were reviewed to determine if this technique added diagnostic specificity (one patient had both needle EMG and a later study with qEMG before muscle biopsy). RESULTS: Excessive numbers of polyphasic motor unit potentials (MUPs) (> 12% per muscle) were seen in 11 of the 13 patients. In 10 of 13 patients, mean MUP duration was abnorma

Gait electromyography in children with myelomeningocele at the sacral level.

OBJECTIVE: Patients with sacral level myelomeningocele can be expected to maintain a high level of ambulatory status long into adulthood. Gait deterioration and knee pain reported in this population may be attributed to compensatory movements and increased recruitment of less affected muscle groups to achieve this desired level of ambulation. The objective of this study was to analyze the effect of the solid ankle-foot-orthoses (AFOs) on the muscular activity of selected muscles during walking. DESIGN: Cohort/outcome. SETTING: Laboratory. PATIENTS: Twenty four patients with sacral level mye

Intraoperative identification of laryngeal nerves with laryngeal electromyography.

The laryngeal nerves are at risk during thyroid surgery, and several techniques have been described for their intraoperative identification to minimize potential damage. Nerve protection is based on the electromyographic recording from the muscles innervated by the laryngeal nerves, and that electrical activity is picked up by various techniques. We evaluated an electrode attachment to the endotracheal tube that provides a stable method for continuous recording of the laryngeal electromyogram. In addition, we tested various modalities of electrical stimulation in the region where the nerves ar

Correlation between stroboscopy and electromyography in laryngeal paralysis.

Twenty patients with vocal fold motion impairment were reviewed to correlate the findings of electromyography (EMG) and stroboscopy. The causes of motion impairment were idiopathic, previous surgery with recurrent laryngeal nerve injury, neck and skull base trauma, and neoplasm. The EMG studies were analyzed to assess the status of innervation of the immobile vocal fold. The presence or absence of the mucosal wave prior to therapeutic intervention was determined with stroboscopic examination. Eight of 10 patients with EMG evidence of reinnervation or partial denervation were found to have muco

Cinedefecography and electromyography in the diagnosis of nonrelaxing puborectalis syndrome.

A prospective study was undertaken to assess the correlation between electromyography (EMG) and cinedefecography (CD) for the diagnosis of nonrelaxing puborectalis syndrome (NRPR). Clinical criteria for NRPR included straining, incomplete evacuation, tenesmus, and the need for enemas, suppositories, or digitation. EMG criteria included failure to achieve a significant decrease in electrical activity of the puborectalis (PR) during attempted evacuation. CD criteria included either paradoxical contraction or failure of relaxation of the PR along with incomplete evacuation. In addition, other eti

Role of electromyography in the diagnosis of motor neuron disorders.

Programming of electromyographic examination in motor neuron diseases is discussed taking into account application of appropriate techniques. The difficulties of correct interpretation of results are stressed. The stages of disintegration and reintegration of affected motor units are described as well as compensatory changes of surviving motor units. A detailed description of EMG dynamics of amyotrophic lateral sclerosis, late post-polio syndrome and of childhood spinal muscular atrophy is given.

[Electromyography of external laryngeal muscles in diagnosis of dysphonia].

Bioelectrical parameters of two groups of external laryngeal muscles, viz. superior muscles pulling up and inferior muscles pulling down the thyroid cartilage, were measured. Hypokinetic dysphonia was indicated by 1.2-1.6-fold increase of the pulse amplitude of superior muscles and hyperkinetic dysphonia was evidenced by 1.4-2.0-fold increase of that of inferior muscles. When the amplitude was identical on differed by less than 1.2 times, mixed dysphonia was suspected. In healthy people, the pulse amplitude of inferior muscles exceeded that of superior muscles by 1.2-1.4 times. This approach h

Laryngeal electromyography in the diagnosis of laryngeal nerve injuries.

A practical technique of electromyographic (EMG) sampling of the cricothyroid and thyroarytenoid (vocalis) muscles is described. Quantitative motor unit action potential (MUAP) analysis on a group of seven healthy subjects revealed a mean amplitude of 426 microV for the vocalis and 500 microV for the cricothyroid muscle. Mean MUAP duration for the vocalis muscle was 3.5 msec and 4.4 msec for the cricothyroid. Normal activation patterns are reviewed for each muscle. The results of EMG studies in 18 consecutive patients referred to the EMG laboratory with chronic hoarseness was reviewed. There w

Evoked serial electromyography in the evaluation of the paralyzed face.

Serial evoked electromyography (EEMG) is a reliable, objective, repeatable test of facial nerve function. It is very important in the initial patient evaluation in determining percent degeneration of the facial nerve. A response of 0-20% will usually result in incomplete return of facial function while responses of 60% or better will usually result in normal function. With viral facial paralysis (Bell's palsy, herpes zoster oticus), serial EEMG after several weeks has little value in predicting the final percent recovery of facial function. If there is no EEMG response, the diagnosis of viral
