[Changes in vestibular function and furosemide test after experimental endolymphatic hydrops in guinea pigs].

The present study reports a successful anial model of endolymphatic hydrops (EH) in guinea pigs (n = 48) through a surgical obliteration of the endolymphatic sac and duct. Electronystagmography was calibrated by optokinetic nystagmus. During the formation of EH the vestibular functions (n = 16) were dynamically observed by ratation test. After eight weeks (n = 25) the furosemide tests with different doses were carried out by sinusoidal angular acceleration test. The results showed that the excitability of vestibular function in the operated ears was enhanced for the first week after operation,

Peripheral labyrinthine causes of dizziness.

A patient's complaint of dizziness must be defined specifically. Many dizzy patients do not have labyrinthine or balance system disease. The patient with dysequilibrium, on the other hand, often has a balance system disease but not necessarily a labyrinthine disorder. The patient with vertigo most likely has labyrinthine disease. Vertigo is accompanied by nystagmus, which can be identified only when the examiner specifically looks for it. Although vertigo is the classic symptom of labyrinthine disease, not all labyrinthine diseases have associated vertigo. Careful history taking, physical exam

Recovery nystagmus in Ménière's disease.

Spontaneous nystagmus occurs during a Meniere's attack although the literature indicates that the direction can be variable. Previous observations made during the acute and recovery stages of a Meniere's attack suggested that the direction of the spontaneous nystagmus was consistent with the primary-secondary sequence of nystagmus that occurs with relatively prolonged stimulation of the normal vestibulo-oculomotor system. To evaluate this nystagmus pattern further, spontaneous nystagmus was monitored in eight patients using DC electronystagmography during an acute Meniere's attack. All showed

[Quantitative analysis of horizontal and vertical optokinetic nystagmus and optokinetic after-nystagmus in the cat].

This study reported on the horizontal optokinetic nystagmus (OKN) and vertical OKN of cats under the same conditions with quantitative parameters. Using the search coil method, the horizontal and vertical OKN was investigated in 5 alert cats in an upright position. As the optokinetic stimulus, a stepped random dot pattern was used. We recorded the quantitative parameters in both the horizontal and vertical OKN (for the direct pathway parameters, initial fast rise and fast fall; for the indirect pathway parameters, steady state slow phase velocity [SPV] and the optokinetic after-nystagmus [OKAN

[Functional study in early strabismus: translucent screens and electro-nystagmography].

The symptoms of infantile strabismus are triggered by fixation, chiefly monocular fixation. On the excluded eye, "occlusion deviations" (elevation, esodeviation, extorsion) are well observed through a "translucent" unilateral cover. These deviations are super-imposed upon a position of rest without fixation which may be an orthoposition, well observed through bilateral translucent occluders. They are caused by a rupture of binocular retinal stimuli which does not mean rupture of fusion (ex: the darkening Bielschowsky test). Electro-oculographic studies confirm the prevalence of adduction durin

Vertical saccades and the "rider artifact".

Quantitative studies of vertical saccades are sparse, due to a lack of reliable recording techniques. In this study, electro-oculographic (EOG) and photo-electronystagmographic (PENG) techniques showed that up-directed saccades were faster than down-directed ones. In the EOG recording, however, the velocity of the up-directed saccade was correlated to the size of an artifact caused by movement of the upper eye lid. Further investigations are needed to analyse the characteristics of vertical eye movements.

Evaluation of vertiginous children.

Our objective was to evaluate, using a structured approach method, the history and findings in vertiginous children as compared with a group of healthy children. We evaluated 24 vertiginous children (15 girls, 9 boys) with episodes of true vertigo of unknown etiology and 12 healthy age- and gender-matched controls. A detailed medical history was obtained using a structured approach technique. Vertiginous and healthy children underwent general and otoneurologic examinations, including audiogram, electronystagmography, and tympanometry, at the Helsinki University Hospital ENT clinic. Dose-related effects of alcohol on dynamic posturography and oculomotor measures.

Four healthy male volunteers < 30 years of age participated in a blinded study of placebo versus low (0.45 g/kg lean body weight = LBW), medium (0.80 g/kg LBW) and high (1.05 g/kg LBW) dose ethanol ingestion to investigate its effect upon gaze and posture control. Serial electronystagmography and computerized platform posturography were performed at different points along each subject's blood alcohol concentration (BAC) curve as measured by breath analysis. Smooth pursuit and positional testing revealed subtle abnormalities at sub-intoxicating BACs. In addition, instability on posturography wa

An effective maneuver of positional test by turning of the head and body together.

OBJECTIVE: To find the effective maneuver of a positional test. METHODS: We performed three different maneuvers of a positional test in 86 vertiginous patients in supine-lying position in the following order: (1) "body-only maneuver": a patient turned only his body to one side while keeping his head still; (2) "head-only maneuver": a patient turned only his head to one side while keeping the body still; (3) "head-and-body maneuver": a patient turned his head and body together to one side. Eye movement during the whole procedure was recorded by two-dimensional electronystagmography. RESULTS:

Urea test and vestibular dysfunction in suspected Ménière's disease.

Twenty-four consecutive patients underwent urea testing as part of a diagnostic work-up for vertigo and/or fluctuating hearing loss. Latent and positional nystagmus were evaluated by electronystagmography before and after urea ingestion. Of 10 patients with no nystagmus pre urea, only one exhibited slight latent nystagmus post urea. Of 14 patients with latent or positional nystagmus prior to the urea test, two lost their nystagmus, four showed marked decrease in slow component velocity, and eight were unchanged. No correlation could be made with cochlear response. Direction changing positional

A Rare Case of Hearing Impairment due to Cerebello-Pontine Angle Lesion: Trigeminal Schwannoma.

Schwannoma of the trigeminal nerve is a rare condition. Even rarer is hearing loss occurring as a result of this lesion. The aim of this study is to highlight this rare cause of hearing impairment. Here we report the clinical features and findings of the imaging and audiological investigations of a case of trigeminal schwannoma diagnosed at our institution. Our patient presented with headache, giddiness, tinnitus, left-sided facial weakness, left-sided hearing loss, right-sided hemiplegia, and unintelligible speech. Radiological studies revealed a large well-defined mass lesion in the left cer

Saccadic and smooth-pursuit eye movement in neurootological diagnostic procedures.

The origin of the generation of saccadic and smooth-pursuit eye movements has not been proved. The goal of the study reported here was to search for the connection of visuooculomotor reactions--saccades and eye-tracking test--with cerebellar damage. Using electronystagmography, we tested 11 patients treated with aminoglycosides because of pneumonia. After a mean of 21 days of pharmacological therapy, a neurological examination revealed cerebellar disability in all cases. On the basis of our previous studies, we suspected cortical Purkinje cell damage. In all tested patients, saccadic movements

[Bechterew phenomenon in the human].

BACKGROUND: Vestibuloocular and vestibulospinal reactions following the bilateral loss of vestibular function are designated as Bechterew's phenomenon. In vestibular physiology, under experimental conditions, this phenomenon has played an important role for understanding the mechanisms underlying vestibular compensation. However, clinical reports about this phenomenon in humans are extremely rare. PATIENTS AND METHODS: Bechterew's phenomen was observed throughout every stage of its development in a patient with consecutive bilateral loss of vestibular function. The disorder was documented by

[The phenomenon of the fixation suppression of caloric nystagmus in the diagnosis of vestibular disorders].

Fixation nystagmus suppression in caloric tests was used in examination of 56 patients suffering from chronic remittent labyrinthopathy, acute labyrinthopathy, vestibular neuronitis, otosclerosis, adhesive otitis media, vertebrobasilar insufficiency, motor disease, chronic remittent vestibulopathy and neurinoma of the VIII nerve. The bithermal test (BT) with electronystagmography was made to calculate the fixation suppression index (FSI). Complete suppression of the caloric nystagmus in at least one BT test appeared to be the most frequent variant of the response (24 cases). This was not depen

Vertigo and the anterior inferior cerebellar artery syndrome.

We present two patients with clinical features of infarction in the distribution of the anterior inferior cerebellar artery (AICA) who had vertigo as an isolated symptom for several months prior to infarction. Both had risk factors for cerebrovascular disease and other episodes of transient neurologic symptoms not associated with vertigo. At the time of infarction they developed vertigo, unilateral hearing loss, tinnitus, facial numbness, and hemiataxia. MRI identified hyperintense lesions in the lateral pons and middle cerebellar peduncle on T2-weighted images. Audiometry and electronystagmog

The aetiology of dizziness and how to examine a dizzy patient.

Dizziness embraces various sensations of spatial disorientation. A common symptom, it has been experienced by at least one third of the population by the age of 65. Peripheral causes include disorders of the labyrinth and vestibular nerve such as Ménière's disease, benign paroxysmal positional vertigo, acute vestibulopathy (vestibular neuronitis) and acoustic neuroma. The most common lesions of the central nervous system that cause dizziness are infratentorial ischaemia, mechanical trauma, multiple sclerosis and cerebellar atrophy. Other aetiological factors include intoxications, psychogenic

Persisting nystagmus following vestibular nerve section for Menière's disease.

Unilateral vestibular nerve section (VNS) creates a state of acute dysequilibrium which resolves by a process of central compensation. This disturbance resolves quickly and central compensation is complete generally within a month with resolution of symptoms and signs. The course of central compensation following VNS will be similar to that seen after labyrinthectomy because the detachment of hemi-labyrinthine input that is achieved by both will be identical. Six patients are presented who have undergone VNS at least 2.7 years ago (Average 3.5 years); all of them have persisting spontaneous pe
