Mechanisms of Abrupt Loss of Virus Control in a Cohort of Previous HIV Controllers.

Elite and viremic HIV controllers are able to control their HIV infection and maintain undetectable or low-level viremia in the absence of antiretroviral treatment. Despite extensive studies, the immune factors responsible for such exclusive control remain poorly defined. We identified a cohort of 14 HIV controllers that suffered an abrupt loss of HIV control (LoC) to investigate possible mechanisms and virological and immunological events related to the sudden loss of control. The in-depth analysis of these subjects involved the study of cell tropism of circulating virus, evidence for HIV sup

Effectiveness of Mindfulness-acceptance-commitment based approach for Rumination, Cognitive Flexibility and Sports Performance of Elite Players of Beach Soccer: A Randomized Controlled Trial with 2-months Follow-up.

BACKGROUND/OBJECTIVE: There is little research on the effectiveness of new approaches to psychology, including mindfulness-acceptance-commitment, especially in team disciplines. Therefore, this study compared mindfulness-acceptance and commitment-based approaches to rumination, cognitive flexibility, and sports performance of elite beach soccer players during a two-month follow-up. METHODS: The research design consisted of a randomized controlled trial (RCT), with follow-up. Thus, 34 players of the premier league of beach soccer were randomly divided into intervention and control groups based

Expansion of stemlike HIV-specific CD8+ T cells and limited viral epitope diversity characterize durable posttreatment control of HIV.

Individuals who durably control HIV after cessation of antiretroviral therapy (ART) are of great interest for HIV cure efforts, but the correlates of posttreatment control remain incompletely understood. Here, we show across a diverse cohort of posttreatment controllers (PTCs), posttreatment noncontrollers (NCs), and spontaneous elite controllers (ECs) that robust expansion of HIV-specific CD8+ T cells and reduced viral reservoir diversity were strongly associated with durable (>1 year) viral suppression after ART cessation. In addition, HIV-specific CD8+ T cells in PTCs exhibited a stemlike m

Canonical Wnts Mediate CD8+ T Cell Noncytolytic Anti-HIV-1 Activity and Correlate with HIV-1 Clinical Status.

CD8+ T cells do not rely solely on cytotoxic functions for significant HIV control. Moreover, the noncytotoxic CD8+ T cell antiviral response is a primary mediator of natural HIV control such as that seen in HIV elite controllers and long-term nonprogressors that does not require combined antiretroviral therapy. In this study, we investigated the biological factors contributing to the noncytotoxic control of HIV replication mediated by primary human CD8+ T cells. We report that canonical Wnt signaling inhibits HIV transcription in an MHC-independent, noncytotoxic manner and that mediators of t

Reevaluation of possible outcomes of infections with human immunodeficiency virus.

Several lines of evidence indicate that HIV infection can result in several possible incomes, including a very small proportion of individuals whose HIV replication is controlled after treatment interruption (known as HIV posttreatment controllers) or spontaneously without any treatment (known as HIV elite controllers). Both types of individuals are HIV RNA negative but HIV DNA positive, with living virus which can be stimulated ex vivo. A review was conducted to assess the literature on yet rarer cases with detectable integrated HIV DNA without HIV infectious virus in HIV-seropositive or -neg

The Influence of Menstrual Cycle Phase and Urinary Incontinence on Potential ACL Injury Risk Factors with a Focus on Hip Strength and Postural Control in Elite Female Team Sport Athletes: A Pilot Study.

To improve understanding of anterior cruciate ligament (ACL) injuries, this study investigated the effect of menstrual cycle (MC) phase on ACL injury risk factors in elite female team sport athletes with and without urinary incontinence (UI). Additionally, associations between endogenous sex hormones, MC-related symptoms, and these risk factors were investigated. Ten elite female athletes (24.2 ± 3.6 years, BMI 23.2 ± 1.3 kg/m2, 10.9 ± 1.8 training hours/week) completed three testing sessions across three MC phases, determined using the three-step method. Assessments included static and dynami

Recapitulating the qualities of HIV-specific CD8 + T cells from spontaneous controllers.

PURPOSE: Studies in spontaneous controllers of HIV and, more recently, post-treatment controllers have shown that effective HIV-specific CD8 + T cell responses may mediate control of the virus in the absence of antiretroviral therapy. The purpose of this review is to first discuss the unique features of HIV-specific CD8 + T cells in spontaneous controllers. We will then explore how qualities of these cells might be harnessed using T cell engineering strategies. SUMMARY OF RECENT FINDINGS: Several recent studies have deepened our understanding of HIV-specific CD8 + T cell responses in spontane

Chinese HIV Controllers Exhibit Different Immunophenotypic Profiles and Viral Reservoir Dynamics Compared to Normal Progressors: A Real-World Cohort Study.

HIV controllers (HICs) maintain low-level viremia and normal CD4+ T-cell counts without antiviral therapy (ART). This study aimed to characterized immunophenotypic and reservoir dynamics in Chinese HICs, revealing key distinctions from normal progressors (NPs). This prospective single-center study enrolled HICs (ART-free HIV RNA < 2000 copies/mL ≥ 12 months) and age/sex-matched NPs served as controls at PUMCH (2000-2024). Longitudinal immunophenotyping of PBMCs was performed and used PCR to detected total HIV-1 DNA. Among 1792 PLWH, 46 (2.5%) HICs (median age 31 years, 69% male) were enrolled

Circulating immune landscape and immune signatures in spontaneous HIV controllers.

A subset of people with HIV, termed HIV controllers (HIC), maintain low viral loads without antiretroviral therapy. To identify the immune cell architecture of HIV control, we profiled peripheral blood from 54 HIC (including 21 elite controllers, EC) and 1,044 non-controllers (non-HIC) in the 2000HIV study (NCT03994835) using high-dimensional cytometry and confounder-adjusted regression analysis. Both HIC and EC exhibited distinct innate immune profiles compared to non-HIC, marked by reduced frequencies of CCR5+ NKT and TCRγδ1+ cells. EC further showed increased neutrophils and TCRγδ2+ cells,

Massive Release of CD9+ Microvesicles in Human Immunodeficiency Virus Infection, Regardless of Virologic Control.

BACKGROUND: The role of extracellular vesicles (EVs) in human immunodeficiency virus (HIV) pathogenesis is unknown. We examine the cellular origin of plasma microvesicles (MVs), a type of ectocytosis-derived EV, the presence of mitochondria in MVs, and their relationship to circulating cell-free mitochondrial deoxyribonucleic acid (ccf-mtDNA) in HIV-infected patients and controls. METHODS: Five participant groups were defined: 30 antiretroviral therapy (ART)-naive; 30 ART-treated with nondetectable viremia; 30 elite controllers; 30 viremic controllers; and 30 HIV-uninfected controls. Safety and efficacy of drug-eluting stents versus heparin-bonded stents in treatment of femoropopliteal peripheral artery disease: study protocol for a multicentre, prospective randomised controlled trial in China (ELITE trial).

INTRODUCTION: Endovascular therapy has emerged as a prominent strategy for managing femoropopliteal peripheral artery disease, offering acceptable safety and efficacy compared with open surgical bypass. Both paclitaxel-eluting stents and heparin-bonded covered stents have exhibited enhanced clinical outcomes compared with bare metal stents. However, there is currently a lack of level I evidence comparing the safety and efficacy of paclitaxel-eluting stents and heparin-bonded covered stents. Therefore, the primary objective of this study is to systematically evaluate the efficacy and safety out

HIV Proviral Burden, Genetic Diversity, and Dynamics in Viremic Controllers Who Subsequently Initiated Suppressive Antiretroviral Therapy.

Curing HIV will require eliminating the reservoir of integrated, replication-competent proviruses that persist despite antiretroviral therapy (ART). Understanding the burden, genetic diversity, and longevity of persisting proviruses in diverse individuals with HIV is critical to this goal, but these characteristics remain understudied in some groups. Among them are viremic controllers-individuals who naturally suppress HIV to low levels but for whom therapy is nevertheless recommended. We reconstructed within-host HIV evolutionary histories from longitudinal single-genome amplified viral seque

Immune checkpoint landscape in CD4⁺ T cells stratifies HIV-infected individuals by clinical progression.

Immune checkpoint (IC) pathways play a central role in modulating HIV-specific T cell responses and may influence the degree of viral control. Here, we performed a comparative transcriptomic analysis of CD4⁺ T cells from HIV-infected individuals classified as elite controllers (EC), viremic controllers (VC), and chronic progressors (CP), aiming to identify IC signatures associated with viral control. ECs exhibited distinct expression profiles, characterized by significant upregulation of CEACAM1 and CD274 (PD-L1), and downregulation of CD200, TIGIT, CTLA4, BTLA, and ADGRG1 relative to CPs. VC

HLA-B*57 elite suppressor and chronic progressor HIV-1 isolates replicate vigorously and cause CD4+ T cell depletion in humanized BLT mice.

UNLABELLED: Elite controllers or suppressors (ES) are HIV-1-infected patients who maintain undetectable viral loads without antiretroviral therapy. The mechanism of control remains unclear, but the HLA-B*57 allele is overrepresented in cohorts of these patients. However, many HLA-B*57 patients develop progressive disease, and some studies have suggested that infection with defective viruses may be the cause of the lack of high levels of virus replication and disease progression in ES. We therefore performed a comprehensive comparative in vivo and in vitro characterization of viruses isolated f

Multiple-cohort genetic association study reveals CXCR6 as a new chemokine receptor involved in long-term nonprogression to AIDS.

BACKGROUND: The compilation of previous genomewide association studies of AIDS shows a major polymorphism in the HCP5 gene associated with both control of the viral load and long-term nonprogression (LTNP) to AIDS. METHODS: To look for genetic variants that affect LTNP without necessary control of the viral load, we reanalyzed the genomewide data of the unique LTNP Genomics of Resistance to Immunodeficiency Virus (GRIV) cohort by excluding "elite controller" patients, who were controlling the viral load at very low levels (<100 copies/mL). RESULTS: The rs2234358 polymorphism in the CXCR6 gen

Unsupervised proteomic stratification of people living with HIV reveals inflammatory biotypes beyond conventional clinical classifications.

BACKGROUND: Conventional HIV-1 management relies on clinical phenotypes, assuming a uniform inflammatory status and overlooking biological heterogeneity. We aimed to employ a high-throughput proteomic approach to stratify people living with HIV (PLWH) based on plasma inflammatory profiles and evaluate their association with comorbidity burden. METHODS: This cross-sectional study included 30 PLWH (10 treatment-naïve, 10 ART-suppressed, and 10 elite controllers (EC)) and 10 seronegative controls. Unsupervised hierarchical clustering was applied to 87 inflammatory proteins (analyzed by proximity

MiRNA-103 downmodulates CCR5 expression reducing human immunodeficiency virus type-1 entry and impacting latency establishment in CD4+ T cells.

Activated-to-memory transitioning CD4+ T cells display elevated expression of the HIV-1 co-receptor CCR5 and are more prone to HIV-1 latent infection. Here, we show that p53-regulated miRNA-103 downmodulates CCR5 levels in CD4+ T lymphocytes. We reveal that miRNA-103 mimics, as well as Nutlin-3, an inhibitor of Mdm2-mediated p53 degradation, decrease CCR5-dependent HIV-1 infection. Using a dual-reporter virus, we subsequently validate that in transitioning CD4+ T cells, Nutlin-3 treatment decreases the frequency of both productively and latently infected cells via upregulation of miRNA-103. Current view on HIV-2 - epidemiology, diagnosis and treatment.

Human immunodeficiency virus 2 (HIV-2) is a retrovirus related to HIV-1. It is believed to have evolved from the simian immunodeficiency virus (SIV). The disease caused by HIV-2 is manifested by a slower progression compared to HIV-1. Data from West Africa show that 37% to 50% of HIV-2 infected people have undetected or very low viremia and if left untreated, have a much more gradual decline in CD4 T lymphocytes, thus longer survival. However, treatment should not be postponed even in this case, because the course of immune reconstitution inflammatory syndrome (IRIS) in people with HIV-2 is mo

Deep functional immunophenotyping predicts risk of cytomegalovirus reactivation after hematopoietic cell transplantation.

Cytomegalovirus (CMV) is the most common viral infection in hematopoietic cell transplantation (HCT) recipients. We performed deep phenotyping of CMV-specific T cells to predict CMV outcomes following allogeneic HCT. By using 13-color flow cytometry, we studied ex vivo CD8+ T-cell cytokine production in response to CMV-pp65 peptides in 3 clinically distinct subgroups of CMV-seropositive HCT patients: (1) Elite Controllers (n = 19): did not have evidence of CMV DNAemia on surveillance testing; (2) Spontaneous Controllers (n = 16): spontaneously resolved low-grade CMV DNAemia without antiviral t
