Removal of embedded or malpositioned inferior vena cava (IVC) filters can require advanced techniques. Historically, operators have feared IVC filter device fracture but have lacked reliable methods to manage embolization of liberated fragments. We report the case of a 70-year-old male who presented with abdominal pain and bilateral lower extremity edema and was found to have iliocaval thrombosis secondary to a malpositioned and embedded IVC filter. A novel embolic protection strategy using the ProTrieve Sheath (Stryker Peripheral Vascular) was instrumental in capturing a liberated fragment re

BACKGROUND: Transcatheter aortic valve implantation (TAVI) is associated with a high incidence of cerebrovascular injury. As these injuries are thought to be primarily embolic, neuroprotection strategies have focused on embolic protection devices. However, the topographical distribution of cerebral emboli and how this impacts on the effectiveness of these devices have not been thoroughly assessed. Here, we evaluated the anatomical characteristics of magnetic resonance imaging (MRI)-defined cerebral ischemic lesions occurring secondary to TAVI to enhance our understanding of the distribution of

We present a case of symptomatic carotid artery thrombosis treated with catheter intervention under proximal occlusion and flow reversal embolic protection. Although catheter intervention is contraindicated in carotid artery thrombosis due to the risk of distal embolization, the introduction of proximal occlusion embolic protection devices allow interventionalists to use catheter intervention where it was previously deemed too high a risk. A Gore Flow Reversal device was used in a 57-year-old male with obesity, uncontrolled type I diabetes mellitus, hypertension, hyperlipidemia, and prior stro

Percutaneous mechanical circulatory support (pMCS) devices are commonly being utilized for hemodynamic support in patients undergoing high-risk percutaneous coronary interventions or stabilization for those in cardiogenic shock. Left ventricular (LV) to ascending aorta (Ao) rotodynamic pumps such as the Impella devices allow for rapid hemodynamic stabilization or support in such instances. The use of such devices is contraindicated in patients with known LV thrombus. However, it remains unclear on how to manage patients who develop an LV thrombus while on prolonged Impella support. While there

BACKGROUND: Distal net-type embolic protection filters (EPDn) are widely used during endovascular interventions to reduce embolic complications. However, the impact of mesh design on filtration performance and flow dynamics remains poorly defined.
METHODS: A controlled in vitro experimental model simulating arterial flow under physiological pulsatile conditions was developed. Four EPDn were evaluated. A standardized bolus of 140 μm microspheres was injected. Filtration performance was assessed using capture efficacy (CE), overall efficacy (OE), and particle leakage fraction (PLF), while hemod

BACKGROUND: Posterior circulation strokes comprise approximately 20-25% of all strokes of ischemic origin. Strokes affecting this area carry a significantly higher risk for subsequent stroke or death as compared to anterior circulation strokes. Embolic protection device (EPD) use for carotid artery stenosis has translated into percutaneous interventions of proximal vertebral artery (VA) stenosis. However, the use of EPDs when treating intracranial lesions has yet to be studied and may not be feasible as the vessel caliber is frequently smaller than in existing devices.
OBJECTIVE: The aim of t

Stroke is the third most common cause of death in the United States. Approximately 30% of all ischemic strokes are secondary to carotid artery disease. Carotid endarterectomy (CEA) is considered the gold standard for the treatment of carotid artery stenosis. Endovascular techniques such as carotid angioplasty and stenting (CAS) have been proposed as possible alternatives for high-risk patients, but not until the advent of embolic protection devices (EPD) were they considered an acceptable alternative to surgery. There are currently two types of cerebral protection strategies employed: distal p

The field of interventional cardiology has grown dramatically over the past 30 years, including recent advances in the area of peripheral vascular intervention. The phenomenon of no-reflow, characterized by a reduction in coronary blood flow during vascular intervention, has been associated with adverse outcomes in both saphenous vein graft and carotid artery interventions. Distal embolic protection devices, such as the FilterWire EZ System, have been shown to reduce the incidence of no-reflow and adverse cardiac events in patients undergoing saphenous vein graft interventions. Recently approv

BACKGROUND AND PURPOSE: To evaluate the periprocedural outcome after carotid artery stenting with embolic brain protection (EBP+) versus without embolic brain protection (EBP-).
METHODS: We retrospectively reviewed data from a prospective nonrandomized database of 357 patients who underwent carotid artery stenting in the neuroradiology division of our institution from 1999 to 2009. One hundred five patients underwent angioplasty and stenting without distal protection, whereas 252 were treated with distal protection. Patients were analyzed according to their EBP status (+ or -) for the primary

BACKGROUND: Carotid angioplasty and stenting (CAS) is an alternative to carotid endarterectomy. CAS outcomes and risk factors affecting postoperative complications in women are not well defined. We sought to determine the effect of sex on particle size captured by embolic protection devices, comorbidities influencing embolization, and results after CAS.
METHODS: Embolic debris from 188 consecutively collected carotid embolic protection devices were analyzed using photomicroscopy and imaging software. Patient comorbidities, preoperative cerebrovascular symptoms, and perioperative outcomes (cer

Stroke is one of the most significant causes of death and morbidity worldwide. Atherosclerotic stenosis of the extracranial internal carotid artery has been invoked as a common causative factor for stroke in over 20% of patients. Although medical therapy can help to prevent the onset and progression of carotid arteriosclerosis, open surgical carotid endarterectomy has been the gold standard for treating severe blockage. Carotid artery stenting has recently demonstrated efficacy in stroke prevention from atherosclerosis, and in high-risk surgical subgroups, has been shown to be associated with

OBJECTIVE: Persistent primitive hypoglossal artery (PPHA) is a rare vascular anomaly that can complicate carotid artery stenting (CAS) due to its role in posterior circulation perfusion. Embolic protection is required for both the anterior and posterior circulations, which increases procedural complexity. We present a novel approach utilizing double distal filter protection (DDFP) with a single guiding catheter.
CASE PRESENTATION: A 69-year-old man with symptomatic internal carotid artery stenosis (ICS) and coexisting PPHA underwent successful CAS using a DDFP strategy with a single guiding c

BACKGROUND: Our study aims to evaluate the technical feasibility and safety of retrograde popliteal artery access (RPA) combined with rotational atherectomy and drug-coated balloon (DCB) angioplasty for complex femoropopliteal lesions, specifically without the use of distal embolic protection devices (EPD).
METHODS: This single-center, single-operator, retrospective study enrolled 22 consecutive patients with TASC B and C femoropopliteal lesions treated between January 2021 and June 2025. All had severe claudication (Rutherford class 3) and calcified lesions. All underwent ipsilateral retrogr

Subclavian artery (SCA) stenting is an established treatment for symptomatic stenosis; however, the optimal strategy for embolic protection of the vertebral artery (VA) remains controversial, particularly in cases with preserved antegrade VA flow. We report a case of symptomatic left SCA stenosis treated using proximal VA protection with a sheathless balloon-guiding catheter (BGC) via a transradial approach. A 77-year-old man with a history of cerebral infarction and coronary artery disease presented with dizziness. Preoperative imaging demonstrated severe stenosis of the left SCA adjacent to

Carotid artery stenting has rapidly grown as an alternative to carotid endarterectomy for stroke prevention among selected patients with extracranial carotid artery stenosis. Development of mechanical embolic protection devices (EPDs) has been associated with improved clinical outcomes and is now a strongly advocated adjunct to the procedure. Characteristically, EPDs have been broadly defined into 3 primary categories, of which the distal filter elements have largely been the most developed and used. Improvements among the class of proximal balloon occlusion devices with flow reversal have a n

OBJECTIVE: To investigate ultrasonographic character of carotid plaques, and incidences of brain embolism in carotid angioplasty and stenting (CAS) and carotid endarterectomy (CEA).
MATERIALS AND METHODS: CEA (22/25 symptomatic lesions) and CAS (17/20 symptomatic lesions) between 2007 and 2010. Embolic protection devices (15 occlusion and 5 filtering devices) were used during CAS. Carotid plaques were classified into three categories (I: calcificated, II: intermediately echogenic, III: echolucent). Magnetic resonance imaging (MRI) was used to investigate brain emboli.
RESULTS: Ultrasonograph

Transcatheter aortic valve replacement (TAVR) is now the dominant form of aortic valve replacement in the United States. Continued innovation has allowed the technique to be safe and democratized. New advances will increase the number of patients eligible to receive this therapy while increasing safety and efficiency. Herein, the authors review new TAVR technologies, approaches to valve deployment, and dedicated devices for cerebral embolic protection and vascular closure.

Percutaneous coronary intervention of degenerated saphenous vein grafts remains relatively high risk when compared to native vessel interventions, despite advances in pharmacotherapy and embolic protection. This article discusses the phenomenon of distal embolization that seems to plague saphenous vein graft interventions, reviews device-based strategies for embolic protection, and offers a perspective on the utility of percutaneous saphenous vein graft intervention in both elective and acute settings.

INTRODUCTION: Carotid artery stenting (CAS) is typically performed using embolic protection devices (EPDs) as a means to reduce the risk of procedure-related stroke. In this study, we compared procedural morbidity and mortality associated with distal (D-EPD) vs. proximal (P-EPD) protection.
METHODS: MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials were queried from January 1998 through May 2015. Only studies comparing (D-EPD) and (P-EPD) were included. Two independent reviewers selected and appraised studies and extracted data in duplicate. Random-effects meta-analysis

OBJECTIVES: The sentinel cerebral embolic protection device has been used as prophylaxis of stroke and transient ischemic attack during transcatheter aortic valve replacement. However, there are no absolute criteria guiding cerebral embolic protection device placement. This study aimed to evaluate the impact of cerebral embolic protection on postoperative cerebrovascular events in patients at high risk for stroke.
METHODS: Between April 2021 and August 2023, 695 patients underwent transcatheter aortic valve replacement through a transfemoral approach. The cerebral embolic protection device wa
