Therapeutic Plasma Exchange as a Rescue Therapy in Sickle Cell Disease-Associated Fat Embolism Syndrome: Case Series and Literature Review.

Fat embolism syndrome (FES) in sickle cell disease (SCD) remains a significant diagnostic and therapeutic challenge, often associated with high morbidity and mortality. While red blood cell exchange (RCE) is the standard first-line therapy, some cases continue to deteriorate despite aggressive management. Emerging evidence suggests that therapeutic plasma exchange (TPE) may serve as valuable salvage therapy, potentially improving survival in refractory cases. We present two cases of FES in SCD, both marked by an attack of profound hypotension and multiorgan dysfunction, yet with distinct traje

Impact of time in therapeutic range (TTR) within the first 72 h on prognosis in patients with pulmonary embolism treated with unfractionated heparin.

Pulmonary embolism (PE) is a life-threatening condition often treated with unfractionated heparin (UFH) in intermediate high-risk patients. Activated Partial Thromboplastin Time (aPTT) is used to monitor UFH efficacy. We sought to evaluate the correlation between time in therapeutic range (TTR) and prognosis in patients with pulmonary embolism treated with unfractionated heparin. A prospective cohort study included 203 patients admitted to a tertiary care center between July 2019 and August 2024 with a confirmed diagnosis of intermediate risk PE treated with UFH. TTR was calculated based on aP

A Rare Presentation of HLH as acute encephalopathy, fever and Diabetic Ketoacidosis (DKA) followed by massive pulmonary embolism: Diagnostic and Therapeutic Challenges.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life‑threatening hyperinflammatory syndrome that often presents with nonspecific features, delaying diagnosis and treatment. Although fever, cytopenias, and organomegaly are classic, unusual presentations can obscure recognition. We report an exceptional case of secondary HLH manifesting as severe diabetic ketoacidosis (DKA) with cerebral edema in a young man with type 1 diabetes, later complicated by a massive saddle pulmonary embolism. CASE PRESENTATION: A 25‑year‑old man with a history of type 1 diabetes mellitus (HbA₁c 10.4%)

Successful nonsurgical therapeutic management in a case of early mechanical aortic prosthetic thrombosis and coronary embolism after a modified Bentall procedure.

Prosthetic valve thrombosis, although rare, is a life-threatening complication of valve replacement surgery. The authors present an atypical case of a modified Bentall procedure with the CarboSeal Valsalva™ conduit complicated by an early mechanical prosthetic aortic valve thrombosis and coronary embolism. The patient was successfully treated with an emergency percutaneous coronary angioplasty and intracoronary thrombus aspiration of the left anterior descending artery, followed by a systemic 10 mg bolus of tissue plasminogen activator followed by ultraslow (25 h) infusion of low-dose (25 mg),

Percutaneous Transhepatic Variceal Embolization as a Therapeutic Bridge for Variceal Bleeding: An Initial Alternative in the Absence of Endoscopy or Transjugular Intrahepatic Portosystemic Shunt (TIPS).

Variceal gastrointestinal bleeding in patients with liver cirrhosis is a potentially fatal complication that requires prompt hemostatic control. Standard management includes vasoactive therapy, antibiotic prophylaxis, and urgent endoscopic intervention, with transjugular intrahepatic portosystemic shunt reserved for selected high-risk or refractory cases. However, in resource-limited settings, timely access to endoscopy or definitive portal decompression may be delayed or unavailable, increasing the risk of hemodynamic deterioration and early mortality. We report the case of a 64-year-old woma

Enterococcus faecalis Aortic Valve Endocarditis Masquerading As Urinary Tract Infection: Diagnostic Delay, Septic Embolic Strokes, and a Fatal Therapeutic Impasse.

We present a case involving a 67-year-old man with multiple comorbidities who initially experienced two weeks of gastrointestinal symptoms that were managed as acute gastroenteritis. He later returned to the emergency department with altered mental status following a fall. Initial evaluation showed leukocytosis, findings consistent with a urinary tract infection, and elevated cardiac biomarkers. Head computed tomography revealed a right frontal lobe hypodensity, while imaging of the chest, abdomen, and pelvis showed diffuse lymphadenopathy. Blood cultures identified vancomycin-susceptible Ente

Pulmonary endarterectomy and lung resection achieving therapeutic success in rarely encountered large cavitary infarctions following pulmonary embolism: a case report.

BACKGROUND: Cavitary infarction following pulmonary embolism is a rare condition associated with high mortality. Surgical intervention is mainly indicated in cases where medical therapy has failed. Although previous reports have shown promising results of surgical management, we have found only 15 cases reported in the literature where surgery was undertaken. To the best of our knowledge, we report the first case in which pulmonary endarterectomy combined with lung resection were successfully performed to manage this rare high-risk condition. CASE PRESENTATION: 44-year-old man presented with

Rheolytic thrombectomy in patient with acute pulmonary embolism, heparin-induced thrombocytopenia and recent stoke. When percutaneous treatment is the only therapeutic alternative.

To report the combined use of rheolytic thrombectomy (RT) and inferior vena cava (IVC) filter placement in the setting of acute pulmonary embolism (PE), heparin-induced thrombocytopenia (HIT) type II (HIT-II) and recent ischemic stroke. A 66-year-old man with an HIT-II and recent ischemic stroke was referred to our institution from a secondary regional center for acute PE and left deep vein thrombosis (DVT), confirmed at chest computed tomography (CT) and lower limb ultrasound, respectively. RT was attempted because intravenous heparin anticoagulation was contraindicated by the patient's medic

Successful therapeutic plasma exchange for a patient with sickle cell disease and fat embolism syndrome after a failure of a response to red cell exchange transfusion.

BACKGROUND: Fat embolism syndrome (FES) is a rare complication, which was reported mostly with milder forms of heterozygous sickle cell disease (SCD). It may present in a catastrophic way with multi-organ failure, particularly involving the pulmonary and neurological systems. Diagnosis is often missed or delayed; and the standard recommended treatment is red cell exchange (RCE) transfusion, which has sub-optimal results, such as debilitating long-term neurological complications. Recently, few reports suggested that the addition of Therapeutic Plasma Exchange (TPE) might further improve the out

Concomitance of Pericardial Tamponade and Pulmonary Embolism in an Invasive Mucinous Lung Adenocarcinoma with Atypical Presentation: Diagnostic and Therapeutic Pitfalls-Case Report and Literature Review.

The invasive mucinous adenocarcinoma of the lungs (LIMA) is an uncommon histological subtype of the mucinous adenocarcinoma. In this article, we present the case of a patient with a very high cardiovascular risk profile, diagnosed with LIMA, pericardial tamponade due to secondary dissemination, and pulmonary embolism, whose management rouses many challenges. Despite receiving the correct anticoagulant and antiaggregant therapy, our patient developed repeated acute major cardiovascular events leading to a fatal outcome. To gather additional information on LIMA and the above cluster of pathologi

High dose of trimethoprim-sulfamethoxazole and daptomycin as a therapeutic option for MRSA endocarditis with large vegetation complicated by embolic stroke: a case report and literature review.

Large cardiac vegetation carries a poor prognosis and high mortality risk, especially if associated with methicillin-resistant Staphylococcus aureus (MRSA) infection. We share our experience of a rare and complicated large cardiac vegetation which had a favourable outcome with combination antibiotic treatment alone. A 35-year-old HIV-negative, HCV-positive male patient with a previous history of methicillin-susceptible S. aureus endocarditis showed MRSA mitral valve endocarditis with large vegetation, complicated by embolic stroke. The strain was soon identified by PCR but only after culture d

Opening the window: Ischemic postconditioning reduces the hyperemic response of delayed tissue plasminogen activator and extends its therapeutic time window in an embolic stroke model.

It has been reported that ischemic postconditioning (PC) changes the reperfusion pattern in permanent or transient models of stroke and confers neuroprotection. However, the effects of PC and subsequent use of tissue plasminogen activator (tPA) for the treatment of embolic stroke have not yet been investigated. Rats were subjected to stroke by injection of a preformed clot into the middle cerebral artery and randomly assigned to vehicle (saline 0.1 ml/100 g), tPA (3 mg/kg), PC only or PC+tPA (3 mg/kg). tPA was injected at 6 h after embolic stroke and PC was conducted at 6.5 h after ischemia by

Non-Traumatic Fat Embolism Syndrome in Sickle Cell Disease Complicated by Bone Marrow Necrosis Successfully Treated with Therapeutic Plasma Exchange: A Case Report.

BACKGROUND: Non-traumatic fat embolism syndrome (NT-FES) is a rare but frequently fatal complication of sickle cell disease (SCD) that is often associated with bone marrow necrosis. A delayed diagnosis or lack of standard criteria leads to a higher mortality rate. Firstline therapy in NT-FES is red cell exchange transfusion (RCE); however, therapeutic plasma exchange (TPE) is considered to be an adjunct or potential alternative in recent times. CASE PRESENTATION: We report a case of a middle-aged male with HbSS sickle cell disease and α-thalassemia trait who presented with a vaso-occlusive cr

Tranexamic acid vs. embolization of the meningeal artery as an adjunctive therapeutic regime to reduce the recurrence rate after surgical relief of chronic subdural hematomas (TABASCO)-a randomized controlled trial.

BACKGROUND: Chronic subdural hematoma is a persistent, bloody to serous fluid retention in the subdural space between the dura mater and arachnoid mater, usually caused by an initial trauma, and is one of the most common traumatic intracranial hemorrhages in western industrialized nations. In the event of a compressive effect on the brain, the hematoma is usually relieved by means of a burr hole trephination. In view of the high postoperative recurrence rate, conservative treatment methods have been investigated both as a competitor to surgery alone in cases where the indication for surgery is

Investigation of the different parameters contributing to bubble sticking inside physiological bifurcations.

Gas embolotherapy (GE) is a developing medical method which can be utilized either as an autonomous therapeutic method to treat vascularized solid tumors, or it can be combined with other medical procedures-such as high-intensity focused ultrasound-to improve their efficiency. This paper is dedicated to investigating the different parameters which influence bubble lodging inside human vasculature via 2D-modeling of bubble dynamics in arteries' and arterioles' bifurcations which are potential sticking positions. Values used in the simulations are in accordance with the non-dimensional physiolog
