We report a case of an extragonadal germ cell tumor from the retrovesical region associated with Klinefelter's syndrome. The patient presented with symptoms of macroscopic hematuria and micturition pain. An x-ray and digital examination were suspicious for prostatic carcinoma. However, needle biopsy of the tumor revealed embryonal cell carcinoma. Serum alpha-fetoprotein was 10,700 ng. per ml. Preoperatively, combined cytotoxic chemotherapy was administered and the antineoplastic effect was excellent. Fine needle aspiration cytology was performed on six patients with malignant mediastinal germ cell tumor: 1 pure seminoma, 1 pure embryonal carcinoma, 1 pure yolk sac tumor and 3 mixed germ cell tumors containing teratoma. From 1957 to 1988 eleven cases of primary mediastinal germ cell tumor were referred to the Peter MacCallum Cancer Institute (PMCI). Four were seminomas, three were mixed germ cell tumors, two were embryonal carcinomas and two were teratocarcinomas. Two of the eleven patients were female. For seminoma, surgical debulking and post-operative irradiation produced the best results. Mediastinal doses ranged from 30 to 37.5 Gy. Serum lactate dehydrogenase and lactate dehydrogenase isoenzymes were determined as a screen for testicular germ cell neoplasia in 130 men with maldescended testes. A testicular tumor was found on clinical examination in 1 patient, which was revealed to be embryonal carcinoma, teratoma, yolk sac tumor and carcinoma in situ on orchiectomy. Testicular cancer is broadly divided into seminoma and non-seminoma types for treatment planning because seminomas are more sensitive to radiation therapy. Non-seminomatous germ-cell testis tumors represent a majority of all testicular neoplasms and include yolk sac tumor, embryonal carcinoma, choriocarcinoma, teratoma and undifferentiated tumors. Malignant neoplasms account for approximately 25% of neoplasms of the paratesticular tissues and most of them are sarcomas. BACKGROUND: The ovarian reserve is the quantity of non-growing primordial follicles (NGF) in paired ovaries. A low or high ovarian reserve at birth respectively may foreshadow early or late age at menopause. Somatic-type malignancies (STMs) arising in germ cell tumors are relatively rare, affecting 3% to 8% of patients. These neoplasms show aggressive clinical behavior and are frequently resistant to systemic therapy. In the current review, we provide a comprehensive clinicopathologic overview of these tumors, emphasizing new molecular findings that have improved our understanding of their pathogenesis. In all areas, we highlight special considerations related to the different histologic subtypes. We conclude with a brief mention of areas of uncertainty and areas that require further study.
