Background/Objectives: Sudden cardiac arrest (SCA) is a severe medical condition involving the cessation of the heart's mechanical activity. Following the chain of survival, which includes early recognition and calling for help, early initiation of cardiopulmonary resuscitation (CPR), early defibrillation, and post-resuscitation care, offers the greatest chances of saving a person who has experienced SCA. The aim of this study was to analyze cases of out-of-hospital cardiac arrest (OHCA) and assess the actions taken by bystanders. Methods: The input for analysis consisted of 49,649 dispatch re

BACKGROUND: Patient at Risk (PAR) is an online database using an emergency information form (EIF) that parents of children with special health care needs (CSHCN) complete to store their child's medical information. Emergency medical service (EMS) providers can then access PAR during medical emergencies or for pre-planning. PAR was active for over 10 years but use by patients and EMS was limited.
OBJECTIVE: To determine EMS perspectives on why PAR was not used to its full potential and how to improve PAR for EMS.
METHODS: Seven questions regarding PAR were developed and presented in focus gro

BACKGROUND AND OBJECTIVES: Prehospital Advanced Life Support (ALS) is important to improve patient outcomes in children with seizures, yet data is limited regarding national prehospital variation in ALS response for these children. We aimed to determine the variation in ALS response and prehospital administration of antiepileptic medication for children with seizures across the United States.
METHODS: We analyzed children <19 years with 9-1-1 dispatch codes for seizure in the 2019 National Emergency Medical Services Information System dataset. We defined ALS response as ALS-paramedic, ALS-Adv

BACKGROUND: We aimed to describe and analyze the pre-hospital emergency medical service (EMS) in Beijing and provide information for the government and medical institutions to optimize EMS.
METHODS: We collected all pre-hospital emergency data in Beijing from 2008 to 2017. The chief complaint in each case was classified according to the Medical Priority Dispatch System (MPDS). The sites' administrative districts were determined through geo-encoding of addresses and then classified into four functional regions. We analyzed the demand for EMS, emergency response times (ERT), and disease spectru

OBJECTIVE: Early bystander cardiopulmonary resuscitation (CPR) and rapid automated external defibrillator (AED) use are key determinants of survival following out-of-hospital cardiac arrest (OHCA). Smartphone applications may enhance dispatcher-assisted CPR (DA-CPR) and facilitate early AED access; however, evidence regarding their effectiveness in standardized simulation settings remains limited. This study evaluated the effectiveness of a smartphone application for DA-CPR and AED response during simulated OHCA.
METHODS: This randomized simulation-based study was conducted using standardized

INTRODUCTION: Emergency telephone triage must rapidly recognise cardiac arrest, severe respiratory distress, chest pain and stroke, yet current systems struggle to balance under- and over-triage. trIAje will evaluate present performance and develop an artificial intelligence (AI) model to improve triage in an emergency medical system (EMS).
METHODS AND ANALYSIS: A retrospective cohort will include all 2020-2024 calls initially coded as unconsciousness, cardiac arrest, respiratory difficulty, non-traumatic chest pain or suspected stroke. Dispatch records, mobile electronic health records and t

This report describes a case of hypokalemia-induced hemiplegia and altered mental status masquerading as an acute large vessel occlusion (LVO) stroke, and provides a brief discussion of stroke mimics. An air ambulance was dispatched to rendezvous with a ground emergency medical services (EMS) crew to manage a 75-year-old female patient with altered mental status and left-sided weakness. The patient was transported to a comprehensive stroke center (CSC), where neuroimaging failed to demonstrate evidence for acute stroke, although the patient was found to have severe hypokalemia. The patient's s

OBJECTIVES: Emergency medical services (EMS) commonly use standardized call interrogation systems to determine response strategies. The findings of caller interrogation are typically combined with local clinical and operational guidelines to determine EMS response plans. These plans dictate assigned resource type and response priority, often with associated response time targets. It is unclear whether EMS response plans have been prospectively aligned with patient outcomes and evaluated in terms of diagnostic accuracy or predictive performance measures. This review examines evidence on the pro

BACKGROUND: Dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) is a key strategy to reduce no-flow time in out-of-hospital cardiac arrest (OHCA), yet its impact on long-term survival remains various across emergency medical systems.
PURPOSE: We aimed to evaluate the association between DA-CPR and resuscitation outcomes in a nationwide cohort of OHCA patients presenting with an initial shockable rhythm.
METHODS: We conducted a retrospective observational study including all adult OHCA cases with ventricular fibrillation or pulseless ventricular tachycardia (VF/VT) in which resuscitati

BACKGROUND: Early identification and correct triage of stroke in the prehospital setting are essential for timely treatment and improved outcomes. In Region Zealand, Denmark, ambulance personnel are instructed to consult a neurologist when stroke is suspected to facilitate direct admission to a specialised stroke unit. However, little is known about what happens when emergency medical service (EMS) dispatchers correctly suspect stroke, but the patient is not admitted directly to a stroke unit. This includes whether a consultation with a neurologist occurs and on what basis patients are rejecte

Background/Case Presentation: An 18-year-old female college student shared an electronic message with her friends in which she discussed plans to harm herself through the ingestion of a highly toxic substance. Concerned friends activated emergency healthcare resources, who were dispatched to the individual's residence. Based on the known toxicity of the substance, locating the individual expeditiously to ensure her safety and well-being remained paramount. Upon arrival of collegiate-based health services, emergency medical services, and law enforcement, however, the individual was unable to be

AIM: Emergency medical services target to reduce time to cardiopulmonary resuscitation and defibrillation by alerting additional individuals to out-of-hospital cardiac arrest (OHCA). Multiple terms are used to describe these individuals, potentially causing confusion and hindering comparisons. This international consensus study aimed to establish standardised terminology.
METHODS: Forty-six interdisciplinary researchers from four continents participated in a symposium on "Community First Responders" with the objective of standardising relevant terminology. Initially, terms were proposed anony

A review of all railroad-related deaths and significant injuries that occurred in a medium-sized metropolitan area from January 1, 1979, to June 30, 1986, was conducted. Autopsy reports were obtained for each fatality, and pre-hospital data were retrieved for all railroad-related injuries resulting in emergency medical services dispatch. There were ten fatalities (24%) and 31 survivors. The average age was 31.2 years (range, 1 to 67). Thirty-seven (90%) were men. Eleven persons (27%) were intoxicated (average blood alcohol of 279 mg/dL; range, 140 to 460). Of the 30 survivors transported, hosp

INTRODUCTION: ST-segment-elevation acute myocardial infarction (STEMI) is a therapeutic emergency. Early reperfusion is the key to successful reperfusion. Guidelines recommend organizing regional networks. In France, this starts with a call to a medical dispatch center, the SAMU-centre 15. The aim of this study was to evaluate regional STEMI management using data collected from 2002 to 2010.
METHODS: Observational, prospective, multicenter survey. STEMI patient with chest pain lasting for less than 24hours managed by 40 mobile emergency and resuscitation service (SMUR) and 8 emergency medical

AIMS: How the prehospital chest pain triage contributes to inequities in cardiovascular outcomes remains poorly understood in patients with severe mental illness (SMI). We aimed to examine the association between SMI, emergency response, and cardiovascular health disparities in chest pain-related calls.
METHODS: This registry-based cohort study included first-time chest pain-related calls to the Copenhagen Emergency Medical Services (emergency number and out-of-hours service) during the study period 2014-2018. We compared patients with SMI to patients without SMI (controls). We examined emerg

CASE PRESENTATION: A 40-year-old woman was injured in a motor vehicle accident. Physician-staffed helicopter emergency medical service (HEMS) was dispatched, and after the HEMS physician performed thoracostomy and tracheal intubation to relieve the tension pneumothorax and hemorrhagic shock, her carotid artery became unpalpable. The physician then decided to perform prehospital resuscitative thoracotomy. Immediately after arriving at the hospital, an emergency laparotomy was performed. Intraoperative findings showed that a huge uterine fibroid had been avulsed from the uterine wall, and we per
