Interventions to Improve Outcomes for High-Need, High-Cost Patients: A Systematic Review and Meta-Analysis.

BACKGROUND: Chronic disease patients who are the greatest users of healthcare services are often referred to as high-need, high-cost (HNHC). Payers, providers, and policymakers in the United States are interested in identifying interventions that can modify or reduce preventable healthcare use among these patients, without adversely impacting their quality of care and health. We systematically reviewed the evidence on the effectiveness of complex interventions designed to change the healthcare of HNHC patients, modifying cost and utilization, as well as clinical/functional, and social risk fac

Emergency medical services for children: a general pediatrician's perspective.

The EMSC movement is still in its infancy, and there is much that remains to be done. The primary care pediatrician plays a major role in the EMSC system and should continue to advocate for efficient, high-quality pediatric emergency care. In summary, there are several ways that the office-based pediatrician can and should become involved with EMSC: 1. Pediatricians should emphasize safe and injury prevention at each health maintenance visit throughout a child's life. 2. Pediatricians should encourage all parents to become certified in BLS/CPR. Ideally, training in CPR should be provided durin

[The hospital emergency plan].

STRUCTURE: The hospital emergency plan consists of a basic plan and an appendix. The basic plan deals with the general aspects of emergency operation and the special aspects of external and internal emergencies. The appendix contains special instructions, e.g., emergency action orders, staff alert lists, material lists, and situation plans. CONTENT AND IMPLEMENTATION: External emergencies (e.g., mass casualty incidents) and internal emergencies (e.g., fire and other environmental threats) should be regarded. Once a defined experienced physician decides to activate the emergency plan, the hosp

A pictorial review of non-traumatic adrenergic crisis.

Non-traumatic adrenal crisis is a rare but critical diagnosis to make in emergency settings due to grave consequences. Various pathologies can present as acute crisis, such as spectrum of endocrine imbalance, ranging from catecholamine excess in pheochromocytomas to acute adrenal insufficiency related to glandular dysfunction. Critical manifestations may be due to structural causes related to adrenal hemorrhage, especially when they are bilateral. Oncological complications such as vascular invasion, tumoral bleed, rupture, and hormonal dysfunction can occur. Due to non-specific clinical presen

Down the Rabbit Hole: Emergency Department Medical Clearance of Patients with Psychiatric or Behavioral Emergencies.

Patients presenting with behavior or psychiatric complaints may have an underlying medical disorder causing or worsening their symptoms. Misdiagnosing a medical illness as psychiatric can lead to increased morbidity and mortality. A thorough history and physical examination, including mental status, are important to identify these causes and guide further testing. Laboratory and ancillary testing should be guided by what is indicated based on clinical assessment. Certain patient populations and signs and symptoms have a higher association with organic causes of behavioral complaints. Many medi

Approach to metabolic alkalosis.

Metabolic alkalosis is a common disorder, accounting for half of all acid-base disturbances in hospitalized patients. It is the result of an increase in bicarbonate production, a decrease in bicarbonate excretion, or a loss of hydrogen ions. Most causes of metabolic alkalosis can be divided into 4 categories: chloride depletion alkalosis, mineralocorticoid excess syndromes, apparent mineralocorticoid excess syndromes, and excess alkali administration. Treatment is usually supportive and based on cause of the alkalosis.

Visit-level acuity and resource-based relative value unit utilization in a pediatric emergency department.

OBJECTIVE: There is currently limited data on the distribution of evaluation and management (E&M) codes and resource utilization in pediatric emergency departments. We sought to ascertain the following: (1) the distribution of visit-level acuity among patients who sought care in our pediatric emergency department (PED); (2) mean relative value units per physician hour (RVUs/h) as a measure of health care provider productivity; (3) the extent of correlation between the mean number of patients seen per hour and RVUs/h; and (4) the difference in RVU's generated using observation codes versus a hi

Airport emergency management: The art of strict rules and flexible minds.

For businesses large and small, the world over, maintaining operational continuity is an economic imperative. This paper suggests that airports provide a good case study from which to learn, due to their emergency-oriented nature; their nonstop tango with disruptions of many origins; their experience in moving from crisis to normal operations as quickly as possible; and because of their multifaceted functions and stakeholders. With this in mind, the article illustrates how the various challenges faced by airport operators align with those faced by businesses more generally. The paper argues th

How do emergency care providers and law enforcement officers navigate the competing imperatives of saving lives and serving justice? A qualitative case study in a Ghanaian secondary hospital.

OBJECTIVE: Emergency care departments often navigate the dual imperatives of providing timely medical care and cooperating with police as law enforcement officers (LEOs) involved in injury-related cases. Using Strauss' negotiated order theory, we examined the engagements between clinicians and the police at the Upper West Regional Hospital in Ghana, analysing the structural and negotiation contexts shaping their interactions. DESIGN: We used a qualitative case study design. SETTING: The study was conducted at the Upper West Regional Hospital in Ghana. PARTICIPANTS: Participants included cli

Evaluation of a Statewide Policy to Improve Post-Overdose Care in Emergency Departments and Subsequent Treatment Engagement.

OBJECTIVE: To evaluate the impact of a statewide treatment standards policy for post-overdose emergency department (ED) care on services provided and subsequent treatment engagement. METHODS: This pre-/post-study used electronic health record data and surveillance data from Rhode Island. Outcomes were compared for patients attending EDs for opioid overdose before (03/1/2015-02/28/2017) and after (04/01/2017-03/31/2021) policy release. RESULTS: Overall, 2,134 patients attended 2,891 ED visits for opioid overdose. Compared to pre-policy, visits post-policy more often included initiation of bup

A program for volunteers accompanying older patients with cognitive dysfunction to improve the quality of emergency department care: A pilot study.

Admission to an emergency department (ED) is challenging for older patients with cognitive dysfunction (PWCD). Targeted patient-oriented approaches to improve the care for PWCD are needed. The aim of this pilot study was to design and evaluate a program for volunteers to support PWCD in the ED. Volunteers (N = 9) first received a training and during the following six months (N = 90 shifts), they accompanied PWCD (N = 112) during their stay. Results showed that the training increased volunteers' knowledge and expertise, but not shift-related self-efficacy. The most frequent strategies applied w

Subsequent Consent and Blameworthiness.

Informed consent is normally understood as something that a patient gives prior to a medical intervention that can render it morally permissible. Whether or not it must be given prior to the intervention is debated. Some have argued that subsequent consent-that is, consent given after a medical intervention-can also render an otherwise impermissible act permissible. If so, then a patient may give her consent to an intervention that has already been performed and thereby justify a physician's (paternalistic) act retroactively. The purpose of this paper is to argue that even if subsequent consen

Optimizing Emergency Department Care for People With Bleeding Disorders: A Scoping Review of Barriers and Interventions for Improvement.

BACKGROUND: Emergency department (ED) care is critical for managing acute bleeding events in people with bleeding disorders. Despite international guidelines recommending haemostatic treatment within 30-60 min, delays and deviations from best practices are common and associated with poorer outcomes. OBJECTIVE: Identify barriers to guideline-concordant ED management of bleeding disorders, evaluate interventions, assess clinical impact, and highlight knowledge gaps to inform future research. METHODS: Five databases (MEDLINE, Embase, Scopus, Web of Science, Cochrane Library) were searched from
