OBJECTIVE: Common femoral endarterectomy (CFE) comprises the current standard-of-care for symptomatic common femoral artery occlusive disease. Although it provides effective inflow revascularization via a single incision, it remains an invasive procedure in an often-frail patient population. The purpose of this retrospective clinical study was to assess the morbidity and mortality of CFE in a contemporary cohort.
METHODS: Consecutive CFEs performed at a large, urban hospital were reviewed. Six-month mortality, local complications (hematoma, lymphatic leak, pseudoaneurysm, wound infection, and

Protamine sulfate is commonly used to reverse the anticoagulant effects of unfractionated heparin (UFH) during surgical procedures, but its administration can sometimes trigger severe adverse reactions, including life-threatening anaphylaxis. We present the case of a 77-year-old male undergoing carotid endarterectomy who developed profound hypotension and tachycardia following protamine infusion. Anaphylaxis was confirmed by elevated tryptase levels. This case emphasizes the importance of vigilant monitoring during protamine administration, particularly in high-risk patients, and highlights th

PURPOSE: The impact of carotid endarterectomy (CEA) on choriocapillaris (CC) perfusion was investigated using swept-source optical coherence tomography angiography (SS-OCTA) imaging before and after surgery in patients with clinically significant carotid artery stenosis (CAS).
METHODS: In this prospective observational study, patients with clinically significant CAS undergoing unilateral CEA had SS-OCTA imaging performed in both eyes before and within 1 week after surgery. The percent CC flow deficits (CC FD%) and CC thickness were assessed using previously validated algorithms. Multivariable

BACKGROUND: Carotid endarterectomy (CEA) reduces the risk of stroke in patients with internal carotid stenosis of 50-99 per cent. This study assessed national surgical practice through audit of CEA procedures and outcomes.
METHODS: This was a prospective cohort study of UK surgeons performing CEA, using clinical audit data collected continuously and reported in two rounds, covering operations from December 2005 to December 2007, and January 2008 to September 2009.
RESULTS: Some 352 (92·6 per cent) of 380 eligible surgeons contributed data. Of 19,935 CEAs recorded by Hospital Episode Statisti

INTRODUCTION: Forty-five per cent of stoke patients have a surgically accessible stenosis. The objective of our study is to describe the response to carotid endarterectomy (EA) in patients of advanced age compared to younger ones.
MATERIAL AND METHOD: Retrospective evaluation of the clinical history of all patients who underwent an endarterectomy in a tertiary hospital between January 1995 and December 2006. The patients were grouped into those 75 years or older and those less than this age. The incidence of peri-operative complications in the first month after surgery, and the long-term mort

Despite the earlier accepted notion that CEA should be delayed 4 to 6 weeks after a stroke, current evidence suggests that CEA may be performed safely earlier than this in most patients who have mild to moderate deficits once symptoms stabilize. The gray areas, however, remain gray, as outlined. Crescendo TIAs are urgent cases in the authors' practice; others advocate a more moderate delayed approach in such cases. Almost everyone agrees that propagating intraluminal thrombus is treated best with a moderate delayed approach that allows the thrombus to resolve first with anticoagulants. Acute c

Ocular symptoms can be the first sign of carotid artery stenosis. In our carotid endarterectomy patients group were ocular symptoms present in 15%, as the only sign (without accompanying neurological signs) in 10.5%. Amaurosis fugax was a dominant sign in 56 cases, blindness in 5, quadrantanopia in 3; trochlear and oculomotor nerve paresis in one case. Angiography finding of more than 60% ICA stenosis according NASCET criteria was an indication for surgery. The operative technique per se was a microsurgical endarterectomy with selective peroperative shunt application. The 30-d morbidity was 3%

The prevention of ischemic stroke by surgical means goes back half a century. Interventional treatment of significant carotid artery occlusive disease should be performed by the method that provides the least periprocedural risk and best durability while providing long-term freedom from stroke. The aim of this study was to analyzed the group of 108 patients following 130 carotid endarterectomies from the point of view of risk and clinical importance of restenosis and occlusion, and probability of recurrent ischemic stroke. Carotid endarterectomy represents an efficacious stroke-preventing stra

The outcomes of carotid endarterectomy (CEA) including long-term results in 121 patients (126 procedures) were retrospectively analyzed to identify the causes of operative morbidity. The angiographic internal carotid artery (ICA) stenosis was severe (> 70%) in 62 patients and moderate (50-70%) with ulceration in 64. The arterial wall was sutured primarily in 91 patients and with patch graft in 35. The outcomes 3 months after operation were good recovery in 86 patients, moderately disabled in 20, severely disabled in 11, and death in four. Three patients suffered operative morbidity (2.5%). Dur

Thirteen patients with internal carotid occlusion underwent 14 primary external carotid revascularization procedures over a 31 month period. Ten patients had obliteration of their internal carotid stump combined with patch angioplasty of the external carotid artery, and 3 had vein bypasses from the common carotid artery to the external carotid artery. Eleven patients were symptomatic with either amaurosis fugax or hemispheric transient ischemic attacks. Two patients were asymptomatic. All patients had serial carotid noninvasive tests (B-mode ultrasonography, spectral analysis, and oculoplethys

We studied the neurologic complications after carotid endarterectomy to develop recommendations for prevention. From 1973 through June 1981, 195 carotid endarterectomies were performed on 184 patients. Carotid endarterectomy was performed using general anesthesia with routine use of a Javid shunt. There were no postoperative deaths. Three major, disabling strokes (1.5%) and four (2%) minor, permanent neurologic deficits occurred. The neurologic deficits were analyzed according to cause: (1) inadequate cranial inflow in four patients, (2) fluctuation in blood pressure requiring treatment in one

Staging of bilateral carotid endarterectomies 1 to 6 weeks apart has been recommended because of presumed excessive morbidity chiefly related to respiratory problems, hypertension, and neurological deficits. Since data regarding the timing of the second procedure are lacking, an analysis of 79 consecutive patients undergoing bilateral endarterectomies staged from 6 days to 34 months apart (median interval, 52 days) was performed. In addition to postoperative neurological deficits, however, transient perioperative mean systolic and diastolic blood pressures (SBP and DBP) were compared after eac

We report on three patients in whom ipsilateral blindness developed following carotid endarterectomy. Fundoscopic examination of the retina documented ischemia and the visual field defects were permanent. In one patient, the internal carotid artery was patent and thought to be the conduit for embolization to the eye. Two patients had chronic occlusion of the internal carotid artery, and blindness resulted from atheromatous emboli to the eye through the external carotid artery in one and from postoperative thrombosis of the external cartoid artery in the other. Our experience calls attention to

Five patients studied by the authors and 28 mentioned in the literature indicate that recurrent stenosis occurs in no less than 0.6% of patients after carotid endarterectomy. The pathology of the recurrent stenosis was stated in only 10 cases indicating atherosclerotic disease in various stages of development in 7 and a fibrous intimal hyperplasia in 3. Correlation between risk factors for the development of atherosclerosis and the pathology of the recurrent disease was poor. Six patients developed recurrent disease despite postoperative prophylactic oral anticoagulation. Surgical technique ap

BACKGROUND: Chronic thromboembolic pulmonary hypertension (CTEPH) is rare in children, and published experience with pulmonary endarterectomy (PEA) is limited.
METHODS: We retrospectively reviewed six patients (<18 years) who underwent seven PEA procedures at our center between December 2011 and September 2025. Clinical characteristics, perioperative findings, hemodynamic outcomes, and follow-up data were analyzed.
RESULTS: Median age was 13 years (range, 2-17), and five patients were female. Five had a history of pulmonary embolism, while one presented with a hydatid cyst. Risk factors for

RATIONALE: Extracranial carotid artery stenosis is a major cause of stroke. Carotid endarterectomy with patch angioplasty is a widely used surgical technique to reduce the risk of stroke in people with carotid stenosis. Patch materials include biological materials, like autologous vein or bovine pericardium, and synthetic materials, such as polytetrafluoroethylene (PTFE), Dacron, polyurethane, and polyester. This review is an update of one first published 30 years ago and last updated in 2021, and it has been conducted in line with current Cochrane guidance. In this update, we incorporated an

We review the technique for carotid endarterectomy (CEA) and direct carotid access for distal thrombectomy after attempted proximal thrombectomy in the setting of tandem occlusions. A patient in their 70s presented with right facial droop and drooling and was found to have critical left carotid stenosis with filling defect in the cavernous segment of the left internal carotid artery consistent with vessel occlusion, Thrombolysis in Cerebral Infarction (TICI) 0, and left M2 middle cerebral artery (MCA) occlusion. After multiple attempts with different wire shapes guided by microcatheter injecti

Spontaneous retroperitoneal hematoma (SRH) is a rare complication of anticoagulation therapy. Presentation may vary from limb paresis to hypovolemic shock due to blood loss. The optimal treatment is controversial. It can be managed conservatively or surgically. We report a case of a 73-year-old man presenting with progressively worsening abdominal pain and severe pain radiating to his left lower limb twenty-five days after his pulmonary endarterectomy (PEA) surgery. He was on anticoagulation per our institutional protocol for PEA patients. Investigations revealed a large, spontaneously occurri

BACKGROUND: Carotid endarterectomy may significantly reduce the risk of stroke in people with recently symptomatic, severe carotid artery stenosis. However, there are significant perioperative risks that may be minimised by performing the operation under local rather than general anaesthetics. This is an update of a Cochrane Review first published in 1996, and previously updated in 2004, 2008, and 2013.
OBJECTIVES: To determine whether carotid endarterectomy under local anaesthetic: 1) reduces the risk of perioperative stroke and death compared with general anaesthetic; 2) reduces the complic
