Managing failed vital pulp therapies in mature permanent teeth in a retrospective cohort study, with success and survival rates of managing protocols.

Despite advancements in vital pulp therapy (VPT), a subset of cases fails to achieve desired outcomes. This study based on a previous large-scale cohort study involving 1257 VPT-treated teeth, aiming to describe the demographic data and clinical characteristics of all failed cases and their management protocols. Clinical records/images of 105 failed cases treated by a single endodontist (2011-2022) were examined, including 10 extracted teeth. Asymptomatic cases with PDL widening received no intervention, while others underwent management protocols, including (selective) RCT and (tampon) re-VPT

Radix molaris is a hidden truth of mandibular first permanent molars: A descriptive- analytic study using cone beam computed tomography.

BACKGROUND: Cone-beam computed tomography (CBCT) could be more beneficial in clinical situations that involve the determination of root canal morphology. AIM: The aim of the study was to ascertain the prevalence of radix molaris (paramolaris-RP and entomolaris-RE) in a subpopulation of Saudi Arabia using CBCT. METHOD: A total of 700 CBCT scans of mandibular permanent first molars were included in this study. All CBCT scans were interpreted by two trained dentists and an endodontist. Mandibular permanent first molars with fully developed roots and closed apices were only included. Computed To

Successful regenerative endodontic treatment in a tooth with incomplete root apex and posttreatment apical periodontitis: A case report.

This case report describes the procedure and outcome of regenerative endodontic treatment (RET) in a tooth with incomplete root apex and posttreatment apical periodontitis. A 44-year-old patient was referred to the endodontist because of a periapical lesion on tooth #21 and a recent episode of acute periapical abscess. On clinical and radiographic examination, this tooth presented with tenderness to percussion and palpation, periapical radiolucent lesion, external apical resorption, and incomplete apex formation. After coronal access, the filling material was removed, and the canal was gently

Antibacterial and antifungal activity of endodontic intracanal medications.

BACKGROUND AND AIMS: The sterilization of the entire root canal system represents the main goal of every endodontist, given the fact that the control of the microbial flora is the key point of every root canal treatment. The diversity of microorganisms found inside the root canal and also the resistance of some bacterial species to intracanal medications led to a continuous development of new endodontic products. The present study focuses on the comparison of the antibacterial and antifungal properties of different endodontic products, two commercially available, one experimental plant based e

Efficacy of conventional and digital radiographic imaging methods for diagnosis of simulated external root resorption.

This in vitro study evaluated and compared the efficacy of conventional (Kodak F-speed (Insight), Kodak) and a digital (DRS Gnatus System, Gnatus) radiographic imaging for diagnosis of simulated external root resorption cavities. Human mandibles containing teeth were covered with bovine muscle slices in order to simulate the soft tissues. Nine teeth out of each group of teeth were investigated. Initially, three periapical radiographs of each tooth were taken using a tube shift technique with mesial and distal angulations in both methods. All teeth were subsequently extracted and had 0.7 and 1.

Avulsions and intrusions: the controversial displacement injuries.

Avulsions and intrusions are the most complicated and controversial displacement injuries of permanent teeth. Clinical guidelines published by authorities such as the American Association of Endodontists, the Royal College of Surgeons of England and the International Association of Dental Traumatology are inconsistent. While a certain amount of inconsistency might be expected, some of these guidelines recommend treatments that are experimental or have not incorporated research information from the past 5 years, and in one case the guidelines incorrectly describe the nature of Hank's balanced s

Status of viral hepatitis in the world community: its incidence among dentists and other dental personnel.

Viral hepatitis is a major public health problem, occurring endemically in all areas of the world. The prevalence of the disease is influenced by numerous factors which may be able to modulate its onset. Study of the epidemiology of viral hepatitis in different geographical, ethnic, social and genetic groups as well as immunological and individual factors has contributed much to our understanding of the disease. Hepatitis viruses are classified into A (infectious hepatitis), B (serum hepatitis) and non-A, non-B. The transmission of hepatitis B virus (HBV) is a potential hazard in dental practi

Clinical aspects of pulp stones: A case report series.

Pulp stones (PSs) are calcified masses that can be found in the pulp cavity of any deciduous or permanent tooth. They can be observed in healthy, diseased, and even unerupted or impacted teeth. Calcifications within the pulp may lead to a poorer outcome of a root canal procedure, as they can block access to the root canals, and hinder their subsequent cleaning and shaping. The paper describes 4 clinical cases of denticles located in the coronal and radicular pulp. Therapeutic methods of PS removal with different techniques and instruments are discussed. Specialist root canal treatment (RCT) wa

Regenerative Endodontics for Adult Patients.

The goal of endodontics is to save teeth. Since inception, endodontic treatments are performed to obturate disinfected root canals with inert materials such as gutta-percha. Although teeth can be saved after successful endodontic treatments, they are devitalized and therefore susceptible to reinfections and fractures. The American Association of Endodontists (AAE) has made a tremendous effort to revitalize disinfected immature permanent teeth in children and adolescents with diagnoses including pulp necrosis or apical periodontitis. The American Dental Association (ADA) in 2011 issued several

Herpes Zoster Involving the Second Division of the Trigeminal Nerve: Case Report and Literature Review.

Herpes zoster along the maxillary division of the trigeminal nerve is a rare condition that is caused by reactivation of the varicella zoster virus that resides within the trigeminal ganglion after the primary infection of chickenpox. The disease may be manifested as a toothache during its prodromal stage. The active stage of the disease is characterized by the appearance of a vesicular rash. Postherpetic neuralgia is a common complication of herpes zoster after resolution of the facial and intraoral symptoms. There is increasing evidence for herpes zoster patients to develop stroke later in l

Cone-beam Computed Tomography: Anatomic Analysis of Maxillary Posterior Teeth-Impact on Endodontic Microsurgery.

INTRODUCTION: It is imperative that the endodontic surgeon be knowledgeable of the anatomic dimensions of the surgical site. If cone-beam computed tomography is not available because of location/cost, it is prudent for the clinician to consult a knowledge base for the surgical site. An understanding of the root thickness of maxillary premolars and molars at the preferred level for root resection (3.0-3.6 mm), bone thickness over these roots, and the proximity of each root apex to the maxillary sinus will help the surgeon before and during the surgical procedure. METHODS: Cone-beam computed to

Comparative evaluation of clinical and radiographic outcomes of three different scaffolds in regenerative endodontic treatment - A randomized clinical trial.

BACKGROUND: Regenerative endodontic procedures (REPs) have emerged as a biologically based treatment modality for immature nonvital permanent teeth, promoting continued root maturation and periapical healing. Platelet-rich fibrin (PRF), concentrated growth factor (CGF), and collagen hydroxyapatite-based scaffolds such as SynOss Putty have been used as scaffolds during regenerative therapy; however, comparative evidence regarding their effectiveness remains limited. AIM: To evaluate and compare the clinical and radiographic outcomes of PRF, CGF, and SynOss Putty as scaffolds in regenerative en
