Early Post-EVAR Rupture in a Patient with Early Type II Endoleak: A Borderline Treatment.

PURPOSE: The detection of type II endoleak one month after endovascular aneurysm repair (EVAR) is usually neglected or considered benign. Aim of this report is to present a case of post-EVAR rupture, due to type II endoleak, and discuss on pathophysiology and differential diagnosis. CASE REPORT: We present a case of a 67-year-old male who was treated with EVAR for an asymptomatic abdominal aortic aneurysm and four months later, he presented with a contained rupture due to a type II endoleak. The patient was treated conservatively without any intervention with satisfying result. CONCLUSIONS:

Laparoscopic Transarterial Embolization of Type II Endoleak after Branched/Fenestrated Thoracoabdominal Aortic Aneurysm Endovascular Repair.

Type II endoleak is the most frequent procedure-related complication during endovascular aneurysm exclusion. Actually, there is little controversy in the management of type I and III endoleak, while type II endoleak still generates conflicting reports about their timing and type of treatment. Currently, the intervention is needed only in case of sac enlargement but not in case of persistent endoleak alone. We report the case of a 77-year-old man treated with a custom-made branched/fenestrated endograft for a type III thoracoabdominal aortic aneurysm. A low-flow type II endoleak was detected at

The Roadside Technique for Type II Endoleak Embolization 4 Years after Endovascular Aortic Aneurysm Repair.

Endoleaks are the most common cause of reintervention after endovascular aortic aneurysm repair (EVAR). Type II endoleaks have been implicated as a risk factor for expansion and rupture. Several techniques have been described to manage type II endoleaks, being transarterial catheterization the most commonly used. In some cases this technique can be difficult or impossible to achieve. We report the use of a technique that offers a direct access to the aneurysm sac and the possibility of catheterization of the involved vessels or the embolization of the communication between them, even 4 years a

Ischemic Colitis Following Transarterial Embolization for Type 2 Endoleak of EVAR: Report of a Case.

A 71 year old man was diagnosed to have enlargement of abdominal aortic aneurysm due to type 2 endoleak two years after endovascular aneurysm repair (EVAR). 3D-CT demonstrated a type 2 endoleak that originated from the superior mesenteric artery that fed the inferior mesenteric artery and the right iliolumbar artery that flowed into the 4th lumbar artery. Transarterial embolization was performed by means of N-butyl-2-cyanoacrylate (NBCA). After the treatment, he suffered ischemic colitis that extended from the sigmoid colon to the descending colon. Conservative treatment was mainly performed,

Carbon Dioxide Angiography in Endovascular Aneurysm Repair May Overestimate the Presence of Type II Endoleak.

BACKGROUND: To evaluate the utility of carbon dioxide (CO2) angiography in the detection of type II endoleak (T2EL) during endovascular treatment of infrarenal abdominal aortic aneurysms (AAAs). METHODS: A retrospective analysis was conducted on a prospectively enrolled 20 consecutive patients with infrarenal AAAs treated with endovascular aneurysm repair (EVAR) and intraoperative completion imaging using both contrast media and CO2. The primary end points were the early intraoperative detection of T2EL and 30-day reintervention and mortality. The secondary end points were the confirmatory di

Endovascular Repair of Type IA Endoleak With Zone 0 TBE and Carotid-Carotid-Subclavian Bypass.

BACKGROUND: Type IA endoleaks are a serious complication of thoracic endovascular aortic repair. Consensus is limited on a superior management strategy. CASE SUMMARY: A 76-year-old man with thoracic endovascular aortic repair presented with dyspnea. Imaging revealed a thoracic aortic aneurysm with type IA endoleak compressing adjacent structures. He declined open arch repair and underwent right carotid-left carotid-left subclavian bypass and aortic arch zone 0 thoracic branch endoprosthesis (TBE) placement. The endoleak was eliminated, symptoms resolved, and imaging confirmed graft patency.

Influence of Hemostatic Disorder on Type II Endoleak Development After Endovascular Abdominal Aortic Aneurysm Repair.

Endovascular aneurysm repair (EVAR) is a widely used minimally invasive treatment for abdominal aortic aneurysms. However, postoperative type II endoleak (T2EL) remains a relevant complication associated with a risk of aneurysm rupture and the need for repeated imaging follow-up, resulting in exposure to ionizing radiation. Identification of biological factors predisposing to T2EL may improve risk stratification. This pilot study aimed to investigate whether disturbances in hemostasis are associated with early T2EL development after EVAR. A total of 103 patients treated with EVAR for symptomat

Endovascular Rescue of Type III Endoleak From an Abandoned Fenestration After Fenestrated Endovascular Aneurysm Repair.

BACKGROUND: Type III endoleak after fenestrated endovascular aneurysm repair of an abdominal aortic aneurysm is uncommon and difficult to treat when direct sac catheterization is not feasible. CASE SUMMARY: A 78-year-old man presented 1 year after fenestrated endovascular aneurysm repair with 4 fenestrations. During the initial procedure, a fenestration for a right accessory renal artery could not be bridged and was occluded with an embolization device, leaving an abandoned fenestration. Follow-up computed tomography angiography showed sac enlargement, perigraft contrast, and perianeurysmal h

Endovascular Salvage of Late Type IA Endoleak After Ascending Aortic TEVAR for Ruptured Penetrating Ulcer.

BACKGROUND: Ascending aortic penetrating atherosclerotic ulcers (PAUs) are rare acute aortic syndromes, where guidelines recommend surgical repair. In surgically ineligible patients, endovascular repair has been described; however, data on management of postoperative complications are limited. CASE SUMMARY: A frail 75-year-old woman presented with dyspnea, 1 month after emergency endovascular repair of a ruptured ascending aortic PAU. Computed tomography aortography (CTA) demonstrated a late type IA endoleak with associated pseudoaneurysm. Given her prohibitive surgical risk, a staged approac

Early and Mid-Term Outcomes of Isolated Type 2 Endoleak Refractory to an Embolization Procedure.

Introduction: A type 2 endoleak (EL2) remains the most prevalent complication of endovascular aortic repair (EVAR) for an abdominal aortic aneurysm (AAA). Methods: We conducted a retrospective, single-center analysis, including patients who underwent embolization for an isolated EL2 after EVAR. The study population was stratified into two groups: Group A, consisting of patients whose EL2 resolved after the first embolization procedure, and Group B, consisting of those with refractory EL2 (rEL2). The indication for EL2 treatment was aneurysmal sac growth amounting to >10 mm from the index EVAR.

Double thoracic branch endoprosthesis to repair type IA endoleak after zone 2 thoracic branch endoprosthesis.

Thoracic endovascular aortic repair has become the first-line treatment modality for descending thoracic aortic diseases. Coverage of the aortic arch branches is often required to extend the proximal seal zones during thoracic endovascular aortic repair. The thoracic branch endoprosthesis (TBE) is the first US Food and Drug Administration-approved branched device for thoracic endovascular repair allowing for incorporation of the left subclavian artery. Type IA endoleak after zone 2 TBE as well as its rescue options have not been well-described. In this report, we describe two cases of failed z

Endoleak in covered CP stent causes procedural failure during transcatheter closure of sinus venosus defects.

Transcatheter sinus venosus defect closure uses a long covered stent of appropriate length and diameter across the cavoatrial junction after balloon interrogation. The fabric in the covered stent creates a roof for the right upper pulmonary vein that closes the interatrial communication and redirects the vein into the left atrium behind the stent. A fabric tear in the covered stent may cause endoleak that will result in residual flows across the struts of the covered stent, causing procedural failure. This report highlights the identification of fabric leak by angiography and transesophageal e

Aortic Endovascular Stent IIIb Endoleak Combined With Sealed Aneurysm Rupture Mimicking a Paravertebral Retroperitoneal Tumor.

The aim of this paper is to present the unique case of a sealed ruptured abdominal aortic aneurysm (AAA) with simultaneous type IIIb endoleak of an endovascular stent, mimicking a paravertebral retroperitoneal tumor. A 75-year-old male was referred to the tumor service of our tertiary orthopaedic department suffering from intractable low back pain with an onset of six months. He had undergone repair of an infrarenal AAA with an endovascular stent 5 years ago. Imaging studies depicted a large retroperitoneal mass adjacent to L2 and L3 vertebrae. The stent's metal cage had no signs of wear or mi

Risk Factors for Early and Late Type Ib Endoleak Following Endovascular Abdominal Aortic Aneurysm Repair.

OBJECTIVES: A type Ib endoleak (T1bEL) is a postoperative complication that usually requires additional interventions following endovascular aortic aneurysm repair. Previous studies have focused on iliac artery tortuosity or common iliac artery (CIA) diameter. However, we investigated the various risk factors for early and late T1bELs more comprehensively. METHODS: This retrospective case-control study of a prospectively maintained database compared anatomical, demographic and technical factors between patients with early or late T1bELs and a control group. Early T1bEL was defined as a T1bEL

Surgery for retrograde dissection and endoleak repair following endovascular repair of type B aortic dissection.

This video tutorial presents a case of repair of a delayed-onset retrograde type A dissection after TEVAR with a left carotid-subclavian bypass for acute type B dissection. One of our key challenges was problematic brain and spinal cord protection in a frail patient with distal dissection, and dissection involving an ectasic brachiocephalic trunk extending into the right carotid artery with a significantly increased risk of stroke and paraplegia.  We demonstrate the repair of the dissection using the frozen elephant trunk technique and we also present sequential brain and visceral perfusion di

An Uncommon Case of Type III Endoleak Treated with a Custom-made Thoracic Stent Graft.

Endovascular aortic repair (EVAR) has been shown to be a valid and minimally invasive alternative to open abdominal aortic aneurysm repair. A major shortcoming for EVAR is the need to submit patients to regular follow-up to detect potential complications such as endoleak, limb occlusion, aneurysm expansion, aneurysm rupture, infection, structural failure, and migration. In this case report, we describe an uncommon case of late type III endoleak due to complete detachment of the stent-graft main body segment from its suprarenal uncovered fixation stent. It was treated with a custom-made Relay(®

Bridging stent repair of type III endoleak causing aortocaval fistula after branched aortic endovascular repair.

A 62-year-old man presented to our department with abdominal pain and diarrhea for 3 weeks on a background of previous branched endovascular repair for a thoracoabdominal aneurysm. A triple-phase computed tomography scan of his abdomen and pelvis showed a large aortocaval fistula caused by a type III endoleak from a dislodged superior mesenteric artery stent. He was successfully treated with a BeGraft (Bentley Innomed, Hechingen, Germany) by using an endovascular technique.

Type II endoleak prevention with coil embolization during endovascular aneurysm repair in high-risk patients.

OBJECTIVE: This study evaluated endoleak level and size decrease of infrarenal abdominal aortic aneurysm (AAA) after endovascular aneurysm repair (EVAR) with coil embolization in patients at high risk for type II endoleak. METHODS: Between 2009 and 2013, 83 of 187 patients (44.3%) who underwent EVAR for AAA also underwent coil embolization of the aneurysm sac immediately after complete stent graft release because of risk factors for type II endoleak, including absence of a circumferential thrombus, two or more pairs of patent lumbar arteries, or a patent inferior mesenteric artery.
