Development of cochlear spiral ligament fibrocytes of the common marmoset, a nonhuman model animal.

Spiral ligament fibrocytes generate potassium gradients, which hair cells require to convert mechanical sound waves into electrical palsy. Together with the stria vascularis, they regulate endolymph electrolyte homeostasis. Developing spiral ligament fibrocytes and generating endocochlear potential with an appropriate endolymph ion composition are essential for hearing. Understanding spiral ligament fibrocyte development is useful for studying age-related and genetic hearing loss, as well as for regenerative therapy and cochlear immunology. Despite interspecies differences, most studies of coc

Rupture of Reissner's membrane during acute endolymphatic hydrops in the guinea pig: a model for Ménière's disease?

CONCLUSION: The changes in cochlear function during a destructive acute endolymphatic hydrops were relatively small. This might be consistent with the hypothesis that an endolymphatic hydrops is a marker of disordered inner ear homeostasis rather than the cause of the clinical symptoms of Ménière's disease. OBJECTIVE: Assessment of cochlear function during induction of a destructive acute endolymphatic hydrops. MATERIALS AND METHODS: During repetitive microinjections of 0.5 microl of artificial endolymph at a rate of 50 nl/s the 2f1-f2 and f2-f1 cochlear microphonics distortion products (CMD

Etiology, pathophysiology of symptoms, and pathogenesis of Meniere's disease.

Endolymphatic hydrops is the pathologic feature associated with Meniere's disease. The development of endolymphatic hydrops appears to arise from multifactorial inheritance with alteration of endolymphatic homeostasis. Various factors associated with the phenomenon of hydrops include functional or anatomic obstruction of endolymphatic flow, malabsorption of endolymph, genetic anomalies, vasodilation, allergy, viral infection, and autoimmunity.

Meniere's Disease Patients with Distinct Sac Pathoanatomic Findings React Differently to Endolymphatic Duct Blockage.

OBJECTIVES: To explore the possible difference in response to endolymphatic duct blockage (EDB) treatment in patients with Meniere's disease (MD) with distinct pathoanatomic characteristics of the sac. METHODS: In a total of 24 patients with MD receiving EDB treatment, the dynamics of the vertigo attack, hearing, vestibular function, and endolymph hydrops (EH) before surgery and 40 months following surgery in patients with normoplastic extraosseous portion of endolymphatic sac (eES) were compared with that in patients with atrophic eES. RESULTS: A higher prevalence of complete vertigo contro

Clinical implications of experiments on alteration of the labyrinthine fluid pressures.

Hearing is affected by changes in hydrostatic pressure of the perilymph, but very high pressures are required to produce a significant loss. Among the observed findings in cats are a rapidly reversible decrease in the sensitivity of the cochlear microphonic that is greater for low frequencies, harmonic distortion and reduction of dynamic range as measured by the input-output function for the cochlear microphonic, and a change in the summating potential from negative to positive. These changes are attributed to a temporary reduction in the size of the cochlear duct due to a shift of water molec

Apical lesions of the cochlea in idiopathic endolymphatic hydrops and other disorders: pathophysiological implications.

Apical lesions of the cochlea may be caused by direct trauma, impairment of blood supply, contamination of perilymphatic fluid and endolymphatic hydrops. It is proposed that endolymphatic hydrops causes apical lesions when endolymph or its components leak into the perilymphatic fluid and pass through the helicotrema to degrade the tissue fluid environment of the sensory and neural structures in this area.

[Otosclerosis involving the capsule and inner ear. Morphological findings (author's transl)].

The problem of otosclerotic inner ear damage is considered on the basis of morphological findings. It is pointed out that the disease of the otic capsule certainly also affects the underlying sense organ. This arises through disturbance of the perilymphatic electrolytes, venous stasis and thus changes in the circulation, disturbed regulation of endolymph and, more rarely, the direct effect of the focus on the auditory nerve or calcification of the mobile membranes.

"Light cupula" involving all three semicircular canals: A frequently misdiagnosed disorder.

Though benign paroxysmal positional vertigo (BPPV) is the most common vestibular disorder causing positional vertigo, patients with typical positional vertigo in which the findings of positional nystagmus do not meet the diagnostic criteria for BPPV are often encountered in the clinic. Recently a concept of the light cupula was introduced, which accounts for some of positional vertigo. Under a normal condition in which the specific gravity of the cupula is same as that of the surrounding endolymph, semicircular canals (SCCs) are not influenced by the gravity. The light cupula, which indicates

Perfusion of the inner ear with streptomycin.

Any medical or surgical treatment of Meniere's disease is always suspect because the natural history is so variable and the pathologic physiology is not well-known. Even the occurrence rate in one and both ears is not known. It is certainly true that endolymphatic hydrops, caused by the failure of absorption of endolymph in the endolymphatic sac, is the principal pathology in the classical, fully developed Meniere's disease. While the endolymphatic shunt operation is correct in theory, in most ears, it does not work because the surgeon does not always locate the lumen of the sac and when locat

Competitive antagonism of fluorescent gentamicin uptake in the cochlea.

Aminoglycosides enter inner ear hair cells via apical endocytosis, or mechanoelectrical transduction channels, implying that, in vivo, aminoglycosides enter hair cells from endolymph prior to exerting their cytotoxic effect. If so, circulating aminoglycosides likely cross the strial blood-labyrinth barrier and enter marginal cells prior to clearance into endolymph. We characterized the competitive antagonism of unconjugated aminoglycosides on the uptake of fluorescent gentamicin (GTTR) in the stria vascularis and kidney cells at an early time point. In mice, uptake of GTTR by kidney proximal t

Mice lacking the basolateral Na-K-2Cl cotransporter have impaired epithelial chloride secretion and are profoundly deaf.

In chloride-secretory epithelia, the basolateral Na-K-2Cl cotransporter (NKCC1) is thought to play a major role in transepithelial Cl(-) and fluid transport. Similarly, in marginal cells of the inner ear, NKCC1 has been proposed as a component of the entry pathway for K(+) that is secreted into the endolymph, thus playing a critical role in hearing. To test these hypotheses, we generated and analyzed an NKCC1-deficient mouse. Homozygous mutant (Nkcc1(-/-)) mice exhibited growth retardation, a 28% incidence of death around the time of weaning, and mild difficulties in maintaining their balance.

Complete omission of exon 21 from Slc12a2 transcripts in mice results in hearing loss.

Hereditary hearing loss is highly heterogeneous. SLC12A2 is linked to autosomal dominant nonsyndromic hearing loss, DFNA78, with all the pathogenic variants affecting the exon 21. The gene encodes a cotransporter NKCC1 crucial for regulating intracellular osmotic pressure and producing endolymph in the cochlea. We generated two mouse strains with heterologous Slc12a2 variants in the splice site of the exon 21 (Em1: NM_009194.3:c.2912-2 A > G and Em2: c.2912-4_2913del). Slc12a2Em2/Em2 mice with complete skip of the exon 21 showed reduced endolymph on postnatal day 1 (P1), reduced stria vascular

Constituents of the endolymphatic tubules as demonstrated by three-dimensional morphometry.

The various constituents of the tubules of the human adult intraosseous endolymphatic sac were stained with lectins and Millon reaction to reveal some of the carbohydrate and protein-containing substances in the endolymph. The amount of each substance was determined by digitizing sequential temporal bone sections. Differing amounts of the various identified substances were found in sacs from patients with Meniere's disease, patients with labyrinthine fibrosis, and controls.

Imaging of Temporal Bone.

Multidetector computed tomography has been the benchmark for visualizing bony changes of the ear, but has recently been challenged by cone-beam computed tomography. In both methods, all inner ear bony structures can be visualized satisfactorily with 2D or 3D imaging. Both methods produce ionizing radiation and induce adverse health effects, especially among children. In 3T magnetic resonance imaging, the soft tissue can be imaged accurately. Use of gadolinium chelate (GdC) as a contrast agent allows the partition of fluid spaces to be visualized, such as the bulging of basilar and Reissner's m

Estimation of Volume of Stria Vascularis and the Length of Its Capillaries in the Human Cochlea.

BACKGROUND: The stria vascularis (SV) is a vascularized epithelium that secretes endolymph and is located on the lateral wall of the membranous cochlea. The capillaries of SV directly influence the composition of the endolymph and hence the generation of impulses by the hair-cells that are auditory receptors and thus affect hearing. Therefore, the real morphology of the SV would be very important for understanding the hearing system. There are few reliable reports of the morphology of the human SV. AIMS AND OBJECTIVES: In this research, we have estimated the volume of the SV and total length

Scanning electron microscopic observation of dark cells after streptomycin perfusion of the vestibule in guinea pigs.

Hearing has been stabilized in the majority of patients studied in the treatment of Meniére's disease with streptomycin. This observation suggests that effects of streptomycin may ameliorate endolymphatic hydrops, possibly by attenuating the activity of secretory tissue. The purpose of this study is to observe the dark cells of the utricle in guinea pigs after streptomycin perfusion of the vestibule. Twelve pigmented guinea pigs weighing 250-350 grams were used in this study. The vestibules in five guinea pigs were perfused monolaterally with 150 micrograms of streptomycin in artificial perily

Bilateral Endolymphatic Hydrops in a Patient With Migraine Variant Without Vertigo: A Case Report.

OBJECTIVE: To use modern high-resolution inner ear imaging modalities to evaluate for endolymphatic hydrops (EH) in a patient with migraine-associated fluctuating hearing loss without vertigo spells or dizziness. BACKGROUND: EH has been well described in patients with Meniere's disease on both human temporal bone studies and modern high-resolution imaging; however, there is no study to date, to our knowledge, that examines the presence of EH in a patient with migraine and bilateral hearing loss. We present the MRI findings using a sequence for detecting EH in a unique case of a patient experi

In vitro growth of human endolymphatic sac cells: a transmission electron microscopic and immunohistochemical study in patients with vestibular schwannoma and Ménière's disease.

HYPOTHESIS: Human endolymphatic sac cells have been notoriously difficult to maintain in culture. It was hypothesized that an in vitro environment intended for growth of keratinocytes would also be suitable for human endolymph sac cells. BACKGROUND: Studies on cell physiology of human endolymphatic sac cells have been hampered by difficulties in maintaining them in culture. METHODS: Human endolymphatic sac cells were taken from 10 patients during translabyrinthine skull base surgery for vestibular schwannoma, one of whom also had Ménière's disease.
