Posterior fossa meningiomas intimately involved with the endolymphatic sac.

Approximately 10% of all intracranial meningiomas occur in the cerebellopontine angle (CPA). Although their clinical presentation depends on their site of origin, most are first seen with nonspecific otologic symptoms like those of acoustic neuroma. We present two cases of CPA meningioma that were intimately involved with the endolymphatic sac and had symptoms and signs suggestive of endolymphatic hydrops.

Endolymphatic sac-mastoid shunt surgery. A nonspecific treatment modality?

The reason for the effectiveness of endolymphatic sac-mastoid shunt surgery in the treatment of patients with Meniere's disease is still open for debate. In a double-blind study, published in 1981, we could not demonstrate any difference between the effect of a simple mastoidectomy and a regular endolymphatic sac-mastoid Silastic sheet shunt. However, a significant reduction in symptoms could be demonstrated in both groups, and 70% of patients in both groups could be classified as successes. The patients were reexamined 3 years after surgery, and it was still not possible to demonstrate any di

Cochleostomy or endolymphatic sac surgery for advanced Meniere's disease.

The operation of cochleostomy is described. The surgical technique is vital for the true assessment of results. If the cochlear duct is punctured, the risk to hearing is unacceptably high. If only the osseous spiral lamina is punctured, the results are acceptable in the short term, though the rationale cannot be explained. In the management of difficult patients with more advanced Meniere's disease, some variant of cochleostomy should probably be included in the therapeutic options. The results of endolymphatic sac surgery are significantly better in a matched group of patients.

Results of endolymphatic sac surgery in advanced Meniere's disease.

A rational surgical approach to Meniere's disease eludes us because of the uncertainty of the pathophysiology and the variability in the natural history of the disease. The results of different surgical techniques are difficult to compare because of differences in patient selection, methods of analysis, and length of follow-up reported. This study reviews long-term results in twenty-four ELS procedures in which the test group and parameters of evaluation are carefully described. The overall rate of vertigo control is 75 percent and incidence of significant hearing improvement is 25 percent. Th

Endolymphatic sac surgery for hearing conservation in Meniere disease.

Thirty-five patients with Meniere disease underwent endolymphatic sac decompression and drainage with a Silastic sac-mastoid shunt. On the basis of patients' responses to a questionnaire and on analysis of preoperative and postoperative audiometric studies, we found the method to be an effective means for conservation or improvement of hearing and for relief of vertigo in patients with positive glycerol tests. Of the 35 patients, eight had negative glycerol tests and did not benefit from surgery (P less than .001). Of the 27 patients with positive glycerol tests, 70.4% (19) had no further vert

[A review of endolymphatic sac-related surgeries and their possible mechanisms].

Endolymphatic sac-related surgeries are a series of effective procedures to treat patients with Ménière's disease. However, there is controversy since they were proposed in 1927. Although most studies have shown the effectiveness of endolymphatic decompression and endolymphatic shunting, the mechanisms of them are still unclear. Some studies suggested that these procedures work because the endolymphatic hydrops relieves after surgery. However, the new one proposed recently, endolymphatic duct blockage, seems to contradict to the relief theory. Therefore, this article reviewed the mechanisms of

Current Treatment of Endolymphatic Sac Tumor of the Temporal Bone.

An endolymphatic sac tumor (ELST) is a rare, indolent but locally aggressive tumor arising in the posterior petrous ridge. Patients present with sensorineural hearing loss and tinnitus. As the tumor progresses, patients may experience vertigo, ataxia, facial nerve paresis, pain and otorrhea. Most patients present in their 4th or 5th decade with a wide age range. Patients with von Hippel-Lindau disease have an increased likelihood of developing ELST. Histologically, ELST is a low-grade adenocarcinoma. As it progresses, it destroys bone and extends into adjacent tissues. The likelihood of region

Coexpression of erythropoietin and its receptor in endolymphatic sac tumors.

OBJECT: Von Hippel-Lindau (VHL) disease is characterized by multiple tumors in specific organs. The cell of origin and the reason for the particular organ distribution of the tumors remains unknown. Endolymphatic sac tumor (ELST) is one of the lesions associated with VHL disease. Data from previous studies of VHL disease-associated hemangioblastomas (HBs) and renal cell carcinomas (RCCs) have indicated that VHL gene deficiency causes coexpression of erythropoietin (Epo) and its receptor (Epo-R), which facilitates tumor growth. METHODS: The authors studied ELSTs from five patients with VHL ger

Effect of acetazolamide on cation concentration in the endolymph of the endolymphatic sac.

Acetazolamide (ACTZ), a carbonic anhydrase inhibitor, has been reported to decrease the endolymphatic sac (ES) DC potential (ESP) in the guinea pig. To assess the involvement of cation transport in the ESP change by ACTZ we examined the effect of ACTZ upon the K+ and Na+ activities of the ES endolymph in the guinea pig using ion-sensitive microelectrode. ACTZ (10 mg/kg), a dose that produces the ESP maximum reduction, produced a significant increase in Na+ activity of the ES endolymph with no change in K+ activity. The results suggest that Na+ transport may be directly or indirectly involved i

Effect of locally applied drugs on the endolymphatic sac potential.

In Ménière's disease, an inner ear disorder related to an endolymphatic hydrops, an alteration of the functioning of the endolymphatic sac has been proposed. The endolymphatic sac is assumed to be involved in the secretion/resorption of endolymph. The epithelial transport systems have been indirectly studied by the recording of the endolymphatic sac transepithelial potential (ESP) in control conditions and after the local injection of drugs such as diuretics that have been proposed in the treatment of Ménière's disease. The ESP was recorded, in vivo, in guinea pigs up to 150 minutes after the

Endolymphatic sac tumor: a case report and review of the literature.

BACKGROUND: Papillary tumors of the temporal bone are very rare but aggressive neoplasms. In the past, a middle-ear origin was presumed. Only recently convincing evidence exists that these tumors in fact arise from the endolymphatic sac. METHODS: We present a case of an endolymphatic sac tumor (ELST) with detailed clinical, imaging, operative, and pathologic data. The literature on this rare tumor type is reviewed. RESULTS: This 63-year-old woman had a progressive mass lesion in the temporal bone for a period of more than 35 years, resulting in unilateral fifth to eleventh cranial nerve pals

Electron microscopic studies of the endolymphatic sac in Ménière's disease.

Five specimens of the endolymphatic sac taken from patients with Ménière's disease during epidural drainage surgery were examined under the electron microscope. The clinical history and histopathological findings are presented. The most common and prominent finding was perisaccular fibrosis of the endolymphatic sac which appears to disturb the free absorption of endolymph in the endolymph in the endolymphatic sac resulting in endolymphatic hydrops. Well-preserved epithelial cells of the endolymphatic sac could only be found in one of the specimens.

[Pathology of the endolymphatic duct and sac in inner-ear hydrops].

The morphological substrate of Menière's disease is the endolymphatic hydrops. That can be idiopathic or secondary as a sequence of a labyrinthine disease of another cause. It is possible to coordinate function loss of the sensory end-organs and pathohistological findings, but not all failures are light-microscopically explainable. The reason for the hydrops is certainly a disturbance in the endolymph physiology. For the pathogenesis are important, besides the biochemistry of the inner ear-fluids, problems of the membranes and pathological changes on ductus and saccus endolymphaticus and perim

"Endolymphatic sac tumour": A case report with review of literature.

Endolymphatic sac tumour (ELST) is a non-metastasizing low grade adenocarcinoma of endolymphatic sac origin. It is also known as Heffner tumour, low grade adenocarcinoma of endolymphatic sac origin and aggressive papillary middle ear tumour. These tumours are closely associated with Von Hippel Lindau (VHL) disease. Here we report a case of Endolymphatic sac tumour in a 63 yr old lady who presented with left sided facial palsy. Since the tumour was highly vascular and required preoperative embolization, initial clinicoradiological diagnosis was Jugulotymphanic paraganglioma. Histopathology show

Tricellulin Expression and its Deletion Effects in the Endolymphatic Sac.

OBJECTIVES: Tricellulin is a tight junction (TJ)-forming protein that participates in the sealing function of tricellular TJs. Tricellulin-knockout (Tric-/-) mice show progressive hearing loss with degeneration of hair cells in the cochlea without physiological or physical disorders. In the present study, we investigated the tricellulin expression and its deletion effects in the endolymphatic sac (ES) using Tric-/- mice. MATERIALS AND METHODS: The ES epithelia from wild-type (WT) mice were laser-microdissected, and RT-PCR was performed. The ES sections from Tric-/- and WT mice were immunostai

Endolymphatic sac tumor: case report and review of the literature.

UNLABELLED: Endolymphatic sac tumor (ELST) is a rare neoplasm which can be encountered sporadically or in Von Hippel-Lindau (VHL) disease. Here we report a sporadic case of ELST in 31-year-old man. Neither the symptoms nor a family history of VHL disease were found in the patient. CT imaging demonstrated an expansile lytic lesion of the mastoid process of the left petrous bone. MR scanning revealed a 5.2 cm × 4.7 cm × 4.2 cm mass which showed hyperintensity on T1- and T2-weighted images. Histologic sections showed a papillary, cystic or glandular architecture.
