Epidemiology of necrotizing enterocolitis: a case control study.

All neonates with necrotizing enterocoltis cared for at Grady Memorial Hospital from July, 1977, through February, 1979, were compared with controls matched for birth weight and time of admission to the nursery, to examine risk factors which have been implicated in the etiology of the disease. Data on maternal history, birth history, and hospital course were uniformly collected and contrasted for 35 cases and 98 controls. Low birth weight was associated with an increased incidence of NEC and an increased case fatality rate. All babies 36 weeks or more at birth were diagnosed by seven days. Mor

Necrotizing enterocolitis following operation in the neonatal period.

Necrotizing enterocolitis (NEC) usually occurs in low birth weight infants who have had perinatal stress, and the mortality remains significant. There are a few reports of NEC in the postoperative period, especially in young infants. Nine neonates developed NEC following operations and form the basis of this report. The interval between operation and the diagnosis of NEC varied from 3 days to 4 mo. The surgical lesions included one case each of esophageal atresia, tetralogy of Fallot, supralevator rectal atresia with rectourethral fistula, and multiple intestinal atresias. Three babies had gas

CCL3 as a novel biomarker in the diagnosis of necrotizing enterocolitis.

OBJECTIVES: Neonatal necrotizing enterocolitis (NEC) is a common intestinal disease that threatens the lives of newborns and is characterized by ischemic necrosis of the small intestine and colon. As early diagnosis of NEC improves prognosis, the identification of new or complementary biomarkers is of great importance. In this study, we evaluate the diagnostic value of CCL3 in NEC and compare its effectiveness with other commonly used biomarkers, such as procalcitonin (PCT) and C-reactive protein (CRP). PARTICIPANTS AND DESIGN: Serum samples were collected from 64 patients with NEC and 38 jau

Necrotizing Enterocolitis in a Term Newborn: A Case Report.

This case report describes a 5-day-old term infant with necrotizing enterocolitis (NEC). The clinical presentation, diagnostic reasoning, current literature, hospital course, and follow-up are discussed. Necrotizing enterocolitis is a gastrointestinal emergency characterized by severe inflammation and ischemic necrosis of the intestinal mucosa. Usually a condition of prematurity, NEC primarily occurs in very low birth weight premature infants. Necrotizing enterocolitis can be managed medically or surgically, depending on the severity. Although the etiology of NEC is unknown, the clinical prese

Is mode of delivery associated with the risk of necrotizing enterocolitis?

BACKGROUND: The pathogenesis of necrotizing enterocolitis remains poorly understood. Different newborn bacterial colonization due to cesarean delivery as opposed to vaginal delivery has been implicated as one potential contributing factor. OBJECTIVE: We sought to determine whether mode of delivery is associated with the risk of necrotizing enterocolitis in neonates of women who were at imminent risk of delivery <32 weeks' gestational age. STUDY DESIGN: This is a secondary analysis of data from a randomized controlled trial of magnesium sulfate for the prevention of cerebral palsy. The parent

Timing of ostomy reversal in neonates with necrotizing enterocolitis.

INTRODUCTION: Most neonates with necrotizing enterocolitis (NEC) requiring laparotomy have bowel resection and intestinal diversion. At present, there is no consensus regarding the best time for enterostomy reversal. Our aim is to determine if there is any difference in outcomes of infants whose enterostomy was reversed early versus late. MATERIALS AND METHODS: We retrospectively reviewed medical records of patients with NEC admitted to the neonatal intensive care unit at a large urban children's hospital from 2002 to 2010. The patients underwent operative intervention with creation and subse

Innate immune signaling in the pathogenesis of necrotizing enterocolitis.

Necrotizing enterocolitis (NEC) is a challenging disease to treat, and caring for patients afflicted by it remains both frustrating and difficult. While NEC may develop quickly and without warning, it may also develop slowly, insidiously, and appear to take the caregiver by surprise. In seeking to understand the molecular and cellular processes that lead to NEC development, we have identified a critical role for the receptor for bacterial lipopolysaccharide (LPS) toll like receptor 4 (TLR4) in the pathogenesis of NEC, as its activation within the intestinal epithelium of the premature infant l

An update on necrotizing enterocolitis: pathogenesis and preventive strategies.

Necrotizing enterocolitis (NEC) is one of the most critical morbidities in preterm infants. The incidence of NEC is 7% in very-low-birth-weight infants, and its mortality is 15 to 30%. Infants who survive NEC have various complications, such as nosocomial infection, malnutrition, growth failure, bronchopulmonary dysplasia, retinopathy of prematurity, and neurodevelopmental delays. The most important etiology in the pathogenesis of NEC is structural and immunological intestinal immaturity. In preterm infants with immature gastrointestinal tracts, development of NEC may be associated with a vari

Preformed silastic silos in the management of necrotizing enterocolitis.

Necrotizing enterocolitis (NEC) is sometimes complicated by abdominal compartment syndrome, a clinical syndrome characterized by multiple organ dysfunction that arises as a consequence of increased intra-abdominal pressure. The evolving clinical picture of NEC sometimes requires "second-look" operations done after initial abdominal exploration to more accurately gauge the optimal extent of surgery. Placing intestines in a preformed, spring-loaded, transparent Silastic silo, traditionally used in the staged treatment of gastroschisis, addresses both situations: decompression of the abdomen and

Probiotics for prevention of necrotizing enterocolitis in preterm infants.

BACKGROUND: Necrotizing enterocolitis (NEC) and nosocomial sepsis are associated with increased morbidity and mortality in preterm infants. Through prevention of bacterial migration across the mucosa, competitive exclusion of pathogenic bacteria, and enhancing the immune responses of the host, prophylactic enteral probiotics (live microbial supplements) may play a role in reducing NEC and associated morbidity. OBJECTIVES: To compare the efficacy and safety of prophylactic enteral probiotics administration versus placebo or no treatment in the prevention of severe NEC and/or sepsis in preterm

Total glutathione is not decreased in infants with necrotizing enterocolitis.

BACKGROUND: Glutathione (GSH) is the major intracellular antioxidant protecting against free radical-mediated damage. Oxidative stress is implicated in the pathogenesis of necrotizing enterocolitis (NEC), and damage could be enhanced by a relative deficiency of GSH. We hypothesized that infants with NEC would have lower levels of erythrocyte GSH when compared with controls. METHODS: Total erythrocyte GSH concentration (per gram of hemoglobin [g Hb]) was determined in blood samples from infants with NEC (n = 16) referred for surgical intervention. Nonseptic infants referred for other condition

Neonatal necrotizing enterocolitis in the absence of pneumatosis intestinalis.

Necrotizing enterocolitis (NEC) may be diagnosed radiologically when pneumatosis intestinalis or hepatic-portal gas are present on abdominal roentgenograms. Of 136 patients with strictly documented NEC, 19 (14%) never demonstrated specific roentgenographic evidence of NEC. The diagnosis in these 19 patients was, however, confirmed by a histopathologic examination of tissue obtained at the time of laparatomy or autopsy. In 13 of these infants gastrointestinal (GI) perforation developed 12 hours to eight days following the onset of symptoms, while in five patients ascites developed as the only r

[Blood in the feces as an aid to the diagnosis of necrotizing enterocolitis].

The presence of macro or microscopic blood in the stools of 300 premature babies was studied prospectively in relation with the future risk of necrotizing enterocolitis (NEC). The microscopic blood was investigated using the bililabstix reactive stripe. Four group were made up: group A babies, with no blood in their stools. Group B babies with microscopic blood in feces. Group C babies with macroscopic or gross blood and group D babies with initially microscopic and later gross blood in their feces. The frequency of NEC for these groups were as follows: group A 0.0%, group B 0.47%, group C 63.

Operative techniques for the treatment of neonatal necrotizing enterocolitis.

Operative management of infants with necrotizing enterocolitis and its complications requires a repertoire of surgical techniques, many of which are unique to pediatric surgery. Survival rates for infants with this once deadly illness are improving. Choice of the correct operative procedure and precise technical performance of the technique are mandatory for survival of the infant.

The emerging role of the gut virome in necrotizing enterocolitis.

Necrotizing enterocolitis (NEC) is the most common gastrointestinal emergency in neonates, particularly preterm infants. Many factors can lead to NEC, but microbial dysbiosis is one of the most important risk factors that can induce this disease. Given the major role of the gut virome in shaping bacterial homeostasis, virome research is a fledgling but rapidly evolving area in the field of microbiome that is increasingly connected to human diseases, including NEC. This review provides an overview of the development of the gut virome in newborns, discusses its emerging role in NEC, and explores

A Novel Role for Necroptosis in the Pathogenesis of Necrotizing Enterocolitis.

BACKGROUND & AIMS: Necrotizing enterocolitis (NEC) is a devastating disease of premature infants characterized by Toll-like receptor 4 (TLR4)-dependent intestinal inflammation and enterocyte death. Given that necroptosis is a proinflammatory cell death process that is linked to bacterial signaling, we investigated its potential role in NEC, and the mechanisms involved. METHODS: Human and mouse NEC intestine were analyzed for necroptosis gene expression (ie, RIPK1, RIPK3, and MLKL), and protein activation (phosphorylated RIPK3). To evaluate a potential role for necroptosis in NEC, the effects

Pentoxifylline for treatment of sepsis and necrotizing enterocolitis in neonates.

BACKGROUND: Mortality and morbidity due to neonatal sepsis and necrotizing enterocolitis (NEC) remain high despite the use of potent antimicrobial agents. Agents that modulate inflammation may improve outcomes. Pentoxifylline, a phosphodiesterase inhibitor, is one such agent. OBJECTIVES: Our primary objectives were :1.To assess the effect of intravenous pentoxifylline as an adjunct to antibiotic therapy on mortality and morbidity in neonates with suspected or confirmed sepsis.2.To assess the effect of intravenous pentoxifylline as an adjunct to antibiotic therapy on mortality and morbidity in
