Novel Subxiphoid Cut Down Approach to Facilitate Epicardial Catheter Ablation in an Obese Patient.

INTRODUCTION: Dry epicardial access to facilitate epicardium mapping and ablation is a well described technique with increasing use in modern electrophysiology. However, traditional percutaneous access can be challenging is obese patients or those with anatomical variations. METHODS AND RESULTS: We report on a case where a subxiphoid cut down was utilized for epicardial access in a patient with morbid obesity. This approach allows us to reach the epicardial space when traditional needles do not suffice and enables a more controlled access, potentially minimizing the risk of damage to nearby s

Epicardial Ablation of Supraventricular Tachycardias.

Supraventricular arrhythmias are the most common cardiac arrhythmias encountered; however, it is uncommon that supraventricular tachycardias require percutaneous epicardial access for successful mapping and ablation. There are particular scenarios where epicardial access and ablation should be considered. Certain accessory pathways particularly in the posteroseptal region may require epicardial access for successful ablation. These pathways may also be approached from within the coronary sinus system. In addition, tachycardias near the phrenic nerve in the right atrium or left atrium may requi

Electromechanical wave imaging vs electrocardiographic imaging: a direct comparison of non-invasive ventricular activation mapping modalities.

BACKGROUND: Precise non-invasive identification of the site of origin (SoO) of ventricular arrhythmias (VA) could inform ablation strategies. OBJECTIVE: To compare spatial accuracy of ultrasound-based electromechanical wave imaging (EWI) and ECG imaging (ECGI) to estimate the anatomical and axial (endo- vs epicardial) SoO of focal VA or pace maps employing contact mapping as gold standard. METHODS: Patients awaiting a catheter ablation procedure underwent preprocedural EWI and ECGI to non-invasively map the SoO of VE/VT or RV and LV pacing sites. A commercial CT-ECGI system was used to recon

Epicardial Ablation of Ventricular Tachycardia: a Review.

For over 3 decades, it has been known that reentry circuits for ventricular tachycardia (VT) are not limited to the subendocardial myocardium. Rather, intramural or subepicardial substrates may also give rise to VT, particularly in those with non-ischemic cardiomyopathy. Percutaneous epicardial mapping and ablation has been successfully introduced for the treatment of such subepicardial VT. Herein, we review the indications for epicardial ablation and the identification of epicardial VT by electrocardiographic and imaging modalities. We also discuss the optimal technique for epicardial access

Catheter ablation of ventricular tachycardia in patients with prior cardiac surgery: An analysis from the International VT Ablation Center Collaborative Group.

INTRODUCTION: Patients with prior cardiac surgery may represent a subgroup of patients with ventricular tachycardia (VT) that may be more difficult to control with catheter ablation. METHODS: We evaluated 1901 patients with ischemic and nonischemic cardiomyopathy who underwent VT ablation at 12 centers. Clinical characteristics and VT radiofrequency ablation procedural outcomes were assessed and compared between those with and without prior cardiac surgery. Kaplan-Meier analysis was used to estimate freedom from recurrent VT and survival. RESULTS: There were 578 subjects (30.4%) with prior c

A variant of pulmonary vein-gap reentrant atrial tachycardia with multiple epicardial conduction gaps.

A 66-year-old male had an atrial tachycardia (AT) during a first extensive pulmonary vein (PV) isolation (PVI) of persistent atrial fibrillation. Activation mapping during the AT using Rhythmia (Boston Scientific, Marlborough, MA, USA) exhibited a centrifugal pattern with the earliest activation at the left-sided carina, and conduction towards the inferior left atrium (LA) over the left PVI line. The post-pacing interval was similar to the tachycardia cycle length (TCL) upon entrainment from the LA roof, left-sided carina, and anterior, inferior, and septal LA, but was longer than the TCL upon

Efficacy and safety of combined endocardial/epicardial catheter ablation for ventricular tachycardia in Chagas disease: A randomized controlled study.

BACKGROUND: Epicardial mapping and ablation are frequently necessary to eliminate ventricular tachycardia (VT) in patients with Chagas disease. Nonetheless, there are no randomized controlled trials demonstrating the role of this strategy. OBJECTIVE: We conducted this randomized controlled trial to evaluate the efficacy and safety of combined epicardial ablation in patients with Chagas disease. METHODS: We randomized patients with Chagas disease and VT in a 1:1 fashion to either the endocardial (endo) mapping and ablation group or the combined endocardial/epicardial (endo/epi) mapping and ab

Entrance-Site Ablation in Atrial Tachycardia Involving Epicardial Conduction: A Case Series.

BACKGROUND: Treatment of atrial tachycardia (AT) involving epicardial conduction remains challenging. While endocardial exit sites can serve as targets for radiofrequency (RF) ablation, this approach is sometimes ineffective and may result in treatment failure. METHODS: This retrospective study systematically investigated entrance-site ablation for 10 ATs involving epicardial conduction in nine patients who were unresponsive to initial exit-site ablation. The entrance site-identified using high-resolution endocardial mapping and entrainment as the most downstream site where the postpacing int

Epicardial Ablation of Ventricular Tachycardia.

Epicardial mapping and ablation via a percutaneous subxiphoid technique has been instrumental in improving the working understanding of complex myocardial scars in various arrhythmogenic substrates. Endocardial ablation alone may not be sufficient in patients with ischemic cardiomyopathy, dilated cardiomyopathy, arrhythmogenic right ventricular cardiomyopathy, hypertrophic cardiomyopathy, and Chagas disease to prevent recurrent ventricular tachycardia. Multiple observational studies have demonstrated greater freedom from recurrence with adjunctive epicardial ablation compared with endocardial

High-resolution/Density Mapping in Patients with Atrial and Ventricular Arrhythmias.

High-definition/ultra-high-definition mapping, owing to an impressive increase of the point density of electroanatomic maps, provides improved substrate characterization, better understanding of the arrhythmia mechanism, and a better selection of the ablation target in patients with atrial and ventricular arrhythmias. Despite the scarce comparative data on ablation results versus standard mapping, ultra-high-definition mapping is increasingly used by the electrophysiology community.

Epicardial Catheter Ablation of Ventricular Tachycardia.

Over the last two decades, epicardial catheter ablation has evolved into a practical approach for treatment of ventricular tachycardia (VT). There are certain considerations when performing this procedure. First, presence of epicardial fat can diminish peak-to-peak electrogram amplitude and also impede radiofrequency energy delivery. Hence, epicardial VT ablation should be performed with cooled-tip radiofrequency using reduced irrigation flow within a relatively 'dry' pericardial milieu. Furthermore, catheter orientation is key when performing epicardial ablation. Lastly, hemo-pericardium rema

Advances in Hybrid Management Strategies for Arrhythmogenic Cardiomyopathy Progression in an Amateur Athlete.

BACKGROUND: Arrhythmogenic cardiomyopathy (ACM) is a rare inherited disorder leading to ventricular arrhythmias and sudden cardiac death. CASE SUMMARY: A middle-aged male amateur cyclist with ventricular arrhythmias complicated by myocardial infarction with non-obstructive coronary arteries during fast ventricular tachycardias recurrence underwent a successful endocardial ablation, resulting in a prolonged arrhythmia-free period. ACM was suspected, although its diagnostic criteria were not met at that time. In 2024, ventricular tachycardia recurred, prompting multidisciplinary re-evaluation.

Right ventricular outflow tract endocardial unipolar substrate mapping: implications in risk stratification of Brugada syndrome.

Brugada syndrome (BrS) is a complex arrhythmogenic disease displaying electrical and micro-structural abnormalities mainly located at the epicardium of the right ventricular outflow tract (RVOT). It is well-known that fibrosis, fatty infiltration, inflammation and reduced gap junction expression have been demonstrated at the epicardial anterior aspect of the RVOT providing the arrhythmogenic substrate for ventricular arrhythmic events in BrS. A number of models have been proposed for the risk stratification of patients with BrS. Endocardial unipolar electroanatomical mapping is an emerging too

[Estimation of location and size of myocardial infarction from body surface potentials using the ECG inverse solution method].

This paper describes a non-invasive mathematical method for estimating the locations and sizes of myocardial infarction using body surface electrocardiographic mappings. The inverse calculation is the theoretical basis of our method of estimation. First, the boundary integral equations were used to relate body surface and epicardial potential distributions. Next, a spherical harmonic expansion was used to solve the equations in order to obtain the epicardial potentials from the body surface potentials. The validity of the method was assessed by animal experiments and the clinical application.
