A 14-year-old girl with cerebral palsy (spastic diplegia) underwent examination due to a chief complaint of right foot pain, and was diagnosed with a stress fracture of the central one third of the navicular bone. The fracture was considered to have developed due to repeated loading on the navicular bone as a result of an equinus gait. Therefore, she underwent osteosynthesis and Achilles tendon lengthening to correct the equinus deformity. Following our review of the current literature, we did not identify any reports of stress fracture of the navicular bone in cerebral palsy. We believe that i

BACKGROUND: Despite the large number of studies on the recurrence after surgery for equinus foot deformity in cerebral palsy (CP) patients, only a few investigations have reported long-term recurrence rates. Furthermore, little is known on the interval between the recurrent surgeries and the factors that lead to early recurrence. This study aimed to assess the overall recurrence after surgery for equinus foot deformity in patients with CP and to assess the factors associated with recurrence. We also aimed to determine the predisposing factors for early recurrence.
METHODS: The medical records

Clubfoot is a common and complex, multi-segmental, 3-dimensional, congenital foot deformity. The segmental deformities have been recognized for millennia. They are cavus of the forefoot/midfoot, adductus of the midfoot, varus of the hindfoot, and equinus of the ankle, often referred to by the acronym CAVE. However, description of the deformities and their correction have varied over time with changes in consensus terminology for foot deformities and the understanding of foot biomechanics. In this narrative review, the most current terminology for foot deformities and understanding of foot biom

Although poliomyelitis is not encountered in the Western countries, it is still endemic in the underdeveloped world. Because the foot commonly is involved, an understanding of the causative factors and the available options of treatment is essential before surgical intervention. It is important to note that inadequate or improper surgical procedures potentially can lead to more disability; a well-planned approach to the foot problem, after considering the patient as a whole and understanding the principles involved, is the best option.

For equinus deformity in spastic hemiplegia, correction by the White slide technique has been studied in a group of 35 patients followed up for 14 to 20 years. It is a simple, effective method of lengthening the calcaneal tendon, and is free from significant complications with an acceptable rate of recurrent deformity. The majority of patients achieved a heel-toe gait.

Achilles tendon pathologic conditions are implicated in contributing to the development of many diabetic foot complications including diabetic foot ulceration and Charcot neuroarthropathy. Surgical correction of the diabetic equinus deformity has been studied as an isolated or adjunctive treatment when dealing with difficult-to-close diabetic foot ulcerations or when surgically addressing the diabetic Charcot neuroarthropathy foot or ankle. This article reviews the most common indications, complications, and surgical procedures for equinus correction by either a tendo-Achilles lengthening or g

We compared the effects of different methods of surgical correction of equinus gait in children with spastic cerebral palsy (CP) using pre- and postoperative measurements of gait, electromyography (EMG), range of motion (ROM), and dynamic ankle motion. Operative results significantly improved all variables that produced more normal ankle motion in subjects, although no differences were observed between surgical methods. We conclude that preoperative gait analysis can assist in surgical planning regardless of surgical method for Achilles lengthening and can provide an objective measure of resul

It is essential to distinguish congenital vertical talus from calcaneovalgus deformity in infants as the proper treatment must begin at an early age. The key clinical finding in the differential diagnosis of flexible and rigid flatfoot conditions is the range of motion at the subtalar joint. In rigid flatfoot conditions, subtalar joint motion is impossible because of the rigidity of the deformity. Also, in the rigid type of deformity, the calcaneus will be in an equinus position.

MATERIAL AND METHODS: 38 feet in 36 patients who underwent a progressive foot correction without osteotomies, with an Ilizarov frame have been reviewed. There were 23 equinovarus, 2 equinovalgus, 13 equinus deformities. In 20 patients, the deformity had relapsed after one to four previous operations. The mean follow-up was 22 (12-35) months. The mean age was 17 years (6-68). There were 15 females and 21 males.
RESULTS: All but 4 patients were satisfied with the result, although 15 still had pain during daily activities. The shape of the feet was distinctly improved in all patients, although r

Surgery for severe equine cavo-varus is complex and must be individualized. The interindividual phenotypic variability demands a personalized planning of each foot to be operated. The study's primary goal was to evaluate the function and satisfaction of a series of patients with severe equinus cavo-varus deformity who underwent a triplanar tarsectomy and transposition of the posterior tibial tendon in a single stage surgery after a patient specific 3D biomodel planning. A series of 12 feet (5 patients bilaterally) operated with this technique was analyzed. The cohort comprised 2 females (28.5

The purpose of this study was to evaluate the results of primary radical soft-tissue release of arthrogrypotic clubfeet in children less than 1 year of age. We performed a retrospective review of six patients (12 feet) who underwent radical release of clubfoot deformity. Primary surgery was performed at an average of 7.4 months and the average follow-up was 4.3 years. We graded our results using a modified functional clubfoot rating system. We had two excellent, four good, three fair and three poor results. Revision surgery was performed on one foot for residual equinus. All patients ambulated

A prospective study was carried out on 12 patients with postpoliomyelitis clawing of the hallux treated by a modified Jones procedure using Kirschner wire for the interphalangeal joint fusion. The purpose of this study was to evaluate the outcome of a modified Jones procedure in the treatment of postpolio claw hallux deformity. All the patients had symptoms related to the claw hallux deformity; foot biomechanics and gait were affected. Patients were assessed both pre- and postoperatively using Axer's criteria. Mean follow-up was 32 months. Ten patients had very good results and two patients ha

Equinus is the commonest deformity in cerebral palsy (CP). Many different surgical procedures have been described for the treatment of spastic equinus. In long standing equinus deformities the tibialis anterior muscle becomes elongated which is one reason for muscle weakness. Surgical tendon shortening of the tibialis anterior tendon was therefore introduced to rebalance muscle strength. All patients with CP who had a tibialis anterior tendon shortening (TATS) in combination with a tendo Achilles lengthening (TAL) were included in this study. A total of 29 patients had 30 surgical intervention

The Ilizarov fixator was used to correct chronic burn deformities of the foot and ankle. These contractures can be classified as "simple" or "complex" based on associated deformities and musculoskeletal function. The simple, undirectional deformities of equinus, cavus, rockerbottom, and toe dislocations are fairly easy to correct and maintain. Correction and maintenance of complex deformities with varus or valgus angulation, bone abnormality, or muscle loss are more difficult.

Equinovarus hindfoot deformity is one of the most common deformities in children with spastic paralysis ; it is usually secondary to cerebral palsy. Split tibialis posterior tendon transfer is performed to balance the flexible spastic varus foot and is preferable to tibialis posterior lengthening, as the muscle does not loose its power and therefore the possibility of a valgus or calcaneovalgus deformity is diminished. We retrospectively evaluated 33 consecutive ambulant patients (38 feet) with flexible spastic varus hindfoot deformity. Twenty-eight presented unilateral and five bilateral invo

In this study a generalised dynamic neural network (GDNN) was designed to process gait analysis parameters to evaluate equinus deformity in ambulatory children with cerebral palsy. The aim was to differentiate dynamic calf muscle tightness from fixed muscle contracture. Patients underwent clinical examination and had instrumented gait analysis before evaluating their equinus under anaesthesia and muscle relaxation at the time of surgery to improve gait. The performance of the clinical examination, the subjective interpretation of gait analysis results, and the application of the neural network

BACKGROUND: We present a patient with a subtotal traumatic supramalleolar amputation of the leg, which was initially treated by a vascular reconstruction with deliberate bone and soft-tissue shortening.
METHODS: To correct the ensuing complex deformity, which consisted of a varus hindfoot, leg length discrepancy and equinus, a staged reconstruction was planned. Initially, the hindfoot varus, in presence of a stiff ankle, was corrected by a supramalleolar osteotomy, followed by a Wagner distraction and finally a correction of the equinus.
RESULTS: After a relatively long period of normal func

The current understanding is that gastrocnemius equinus is caused by a pathologic tightening of the muscle leading to decreased ankle joint dorsiflexion. However, an alternative hypothesis is that it is a normal limitation of available muscle length. Passive muscular insufficiency of the gastrocnemius muscle acts on a pathologic foot, which is poorly prepared to accept the forces from the gastrocnemius-soleus complex. In this manner, a normal gastrocnemius muscle exerts abnormal forces across the foot. This alternative hypothesis leads to a different interpretation of the current research lite

The foot deformities in CMT follow certain general patterns; however, like the underlying motor failures that cause them, the deformities present in each patient are unique, and care must be individualized. There is no simple algorithm that can be applied to all patients. The hindfoot, forefoot, and toe deformities in CMT all ultimately are interconnected. As a general rule, it makes sense to address the plantar fascia, then proceed from the hindfoot to the forefoot in analyzing the deformities and in surgery. Releasing the plantar fascia may alter the amount of bony correction required in any

In the child between 4 and 12 years of age presenting with cavus deformity associated with spastic cerebral palsy, symptomatic feet may be improved by soft tissue release of plantar structures followed by appropriate tendon lengthenings or transfers and good cast correction. It is more common for the deformity to present in teenage patients, however, with a more rigid foot usually complicated by some degree of varus and equinus. In these situations, triple arthrodesis is often necessary and is a predictable procedure for correction and stabilization of the deformity.
