Deformability of cord blood vs. newborns' red blood cells: implication for blood transfusion.

AIM: About 50% of premature neonates (PN) are treated with transfusion of packed red blood cells (PRBC) collected from adult donors, which has been suggested to potentially provoke PN pathologies, characterized as blood circulation disorders. RBC have properties that are key determinants of blood circulation, primarily the cell deformability. In previous studies we have shown that transfusion of RBC with reduced deformability impaired the transfusion outcome. Although RBC of PN (PN-RBC) are larger, and their microvessels are narrower than those of adults, their blood circulation is very effici

Variable degrees of suppression of hemoglobin S synthesis in subjects with hemoglobin SS disease on a long-term transfusion regimen.

The objectives of this study were to quantify the amount of blood required to suppress synthesis of hemoglobin S (HbS) in patients with hemoglobin SS on a long-term transfusion regimen and to evaluate factors that might contribute to variations in transfusion-induced patterns of responsiveness. Eleven patients with hemoglobin SS (age range, 2 years 4 months to 19 years 9 months) who had had a cerebrovascular accident were monitored during a period of 1 1/2 to 4 years for HbS percentages, reticulocyte percentages, the amount of erythrocytes infused, and weight. From these data the amount of blo

[Transfusion management and immuno-hematological follow-up after allogeneic hematopoietic cell transplantation: Guidelines from the Francophone Society of Bone Marrow Transplantation and Cellular Therapy (SFGM-TC)].

The French High Authority of Health (HAS) and National Drug Safety (ANSM) agencies recommendations issued in 2014, the French General Direction of Health (DGS) instruction published in November 2021, the French National Blood Bank (EFS) guidelines and the data available in the literature globally define "good transfusion practices" but provide little information about the immuno-hematological and transfusion management of patients who have received an allogeneic hematopoietic stem transplantation (allo-HCT). The aim of this workshop was to harmonize these practices in situations for which ther

[Autotransfusion of cord blood erythrocytes in newborns with malformations requiring early surgical intervention].

PURPOSE: To evaluate the clinical efficacy of cord blood erythrocytes autotransfiusion for the correction of anemia in the intra-and postoperative periods in infants requiring early surgical correction of congenital malformations. METHODS: Washed autoemythrocytes from placental umbilical blood were transfused for correction of intra and post-operative anemias. Umbilical blood assembly was carried out after extraction of the child and navel intersection by the occluded mean by a vein puncture distal (placental) end of a navel by the drainage needle which is a part of special transfiusion syste

Maternal blood donation for intrauterine transfusion.

Pregnancy affected by erythrocyte alloimmunization often requires intrauterine transfusion for fetal survival. Because blood donated by a random donor has been associated with an increased risk of disease transmission, maternal blood donation has been advocated as an alternative blood source for intrauterine transfusion. Collaboration between medical, nursing, laboratory, and nutrition personnel optimizes the safety and success of the procedure. This article describes the collaborative care of the woman participating in maternal blood donation for intrauterine transfusion.

Exome-Based Trio Analysis for Diagnosis of the Cause of Congenital Severe Hemolytic Anemia in a Child.

Inborn hemolytic anemia requiring frequent blood transfusions can be a life-threatening disease. Treatment, besides blood transfusion, includes iron chelation for prevention of iron accumulation due to frequent blood transfusions. We present the results of a clinical investigation where the proband was diagnosed with severe hemolytic anemia of unknown origin soon after birth. Transfusion was required every 4-6 weeks. After whole exome sequencing of the proband and his parents as well as a healthy sibling, we established that the proband had a compound heterozygous state carrying two rare varia

Multiple-ancestry genome-wide association study identifies 27 loci associated with measures of hemolysis following blood storage.

BackgroundThe evolutionary pressure of endemic malaria and other erythrocytic pathogens has shaped variation in genes encoding erythrocyte structural and functional proteins, influencing responses to hemolytic stress during transfusion and disease.MethodsWe sought to identify such genetic variants in blood donors by conducting a genome-wide association study (GWAS) of 12,353 volunteer donors, including 1,406 African Americans, 1,306 Asians, and 945 Hispanics, whose stored erythrocytes were characterized by quantitative assays of in vitro osmotic, oxidative, and cold-storage hemolysis.ResultsGW

Bacterial shock due to transfusion with Yersinia enterocolitica infected blood.

A fatal case (a 55-yr-old man) of bacterial shock and sepsis following a transfusion with erythrocytes infected with Yersinia enterocolitica serotype 03, is reported. The blood donor had slight diarrhea 6 days before the blood donation. A serum sample from the donor showed high titre of both IgG and IgM antibodies against Y. enterocolitica 03, indicating a recent infection. Y. enterocolitica 03 was isolated from blood cultures from the patient. The remaining portion of the transfused erythrocyte concentrate also yielded abundant growth of the same organism on direct plating of the material on

On the trail of blood doping-microRNA fingerprints to monitor autologous blood transfusions in vivo.

Autologous blood doping refers to the illegal re-transfusion of any quantities of blood or blood components with blood donor and recipient being the same person. The re-transfusion of stored erythrocyte concentrates is particularly attractive to high-performance athletes as this practice improves their oxygen capacity excessively. However, there is still no reliable detection method available. Analyzing circulating microRNA profiles of human subjects that underwent monitored autologous blood transfusions seems to be a highly promising approach to develop novel biomarkers for autologous blood d

Processing of stored packed red blood cells using autotransfusion devices decreases potassium and microaggregates: a prospective, randomized, single-blinded in vitro study.

The aim of the study was to compare the potential of autotransfusion devices to reduce non-infectious complications related to transfusion of long-stored packed red blood cells (PRBC; n= 57), such as changes in electrolytes, blood cells and the load of free microaggregates. Following a baseline measurement, a blood pool of three PRBC was divided into three equal volumes and washed with either the Haemonetics Cell Saver (HCS) or the continuous autotransfusion system (C.A.T.S), using the quality (CATS(quality)) and emergency (CATS(emergency)) mode. After the washing procedure, measurements for e

Low-cost molecular methodology for blood group antigens identification and genotyping contribution to transfusion efficacy in multiple transfused patients.

BACKGROUND: Blood transfusions usually result in the production of alloantibodies, complicating subsequent transfusions. Many blood group systems, in addition to ABO and Rh, can lead to the production of irregular antibodies in multiple transfused patients. OBJECTIVES: The aim of this work was to standardize a molecular biology methodology for identified some alleles of KEL, FY, JK and DI blood group system; the transfusion efficacy of chronically transfused patients with phenotype-matched blood was also evaluated. METHODS: A PCR-SSP was developed and validated using Sanger sequencing. The g

Practice Guidelines for blood component therapy: A report by the American Society of Anesthesiologists Task Force on Blood Component Therapy.

In 1994, the American Society of Anesthesiologists established the Task Force on Blood Component Therapy to develop evidence-based indications for transfusing red blood cells, platelets, fresh-frozen plasma, and cryoprecipitate in perioperative and peripartum settings. The guidelines were developed according to an explicit methodology. The principal conclusions of the task force are that red blood cell transfusions should not be dictated by a single hemoglobin "trigger" but instead should be based on the patient's risks of developing complications of inadequate oxygenation. Red blood cell tran

Red blood cells with reduced deformability are selectively cleared from circulation in a mouse model.

Donated red blood cells (RBCs) collected for blood transfusions progressively lose their deformability due to natural aging and cold storage in blood bags. This loss accelerates circulatory clearance via mechanical sensing by the spleen, leading to RBC retention and entrapment. Although reduced deformability is known to shorten RBC circulation time, the extent to which splenic clearance distinguishes and removes RBCs with altered deformability is poorly understood. Here, we show that subpopulations of donor RBCs with a deformability distribution distinct from endogenous recipient's RBCs are se

Successful treatment by exchange transfusion of a young infant with sodium nitroprusside poisoning.

Although sodium nitroprusside (SNP) is often used in pediatric intensive care units, cyanide toxicity can occur after SNP treatment. To treat SNP-induced cyanide poisoning, antidotes such as amyl nitrite, sodium nitrite, sodium thiosulfate, and hydroxycobalamin should be administered immediately after diagnosis. Here, we report the first case of a very young infant whose SNP-induced cyanide poisoning was successfully treated by exchange transfusion. The success of this alternative method may be related to the fact that exchange transfusion not only removes the cyanide from the blood but also a

Transfusion of type-A and type-B blood to cats.

Although nearly all domestic shorthair and longhair cats have type-A blood (greater than 99%), the frequency of blood type B in various feline breeds ranges from 0 to 59%. All blood-type-B cats have strong natural alloantibodies, predominantly of the IgM class, whereas blood-type-A cats have low alloantibody titers of the IgG and IgM classes. We therefore studied the efficacy and safety of transfusing 20 ml of matched and mismatched 14C-potassium cyanate-labeled blood to cats. In autologous and allogeneic matched transfusions of blood-type-A and type-B cats, the half-life of labeled erythrocyt

Investigation of risk factors associated with erythrocyte engraftment after ABO-incompatible hematopoietic stem cell transplantation.

ABO-incompatible hematopoietic stem cell transplantations (HSCTs) are widely practiced; however, the delay in erythrocyte engraftment can be problematic. While erythrocyte engraftment is usually indicated by an increase in reticulocyte levels without the need for erythrocyte transfusions, the disappearance of recipient-derived anti-A/B isoagglutinin and detection of donor-derived A/B antigens can also be used as other parameters. We conducted a retrospective analysis of 68 ABO-incompatible HSCTs, focusing on major and bidirectional mismatch. We analyzed known clinical risk factors associated w

Successful implementation of a patient blood management programme in a lower middle-income state.

BACKGROUND AND OBJECTIVES: Transfusions are common, but their use is decreasing in some countries as a result of increased risk awareness and the implementation of patient blood management (PBM), an evidence-based approach to optimize patient outcomes. This study aimed to detail the implementation of PBM in a Brazilian state and its impact on transfusion rates and associated costs. MATERIALS AND METHODS: The PBM implementation involved several strategies: medical education, haematology consultation services, provision of intravenous iron and other medications, establishment of PBM and periope
