[Reflux esophagitis. Diagnosis and treatment].

Refluxoesophagitis was the topic of a meeting of gastroenterologists in Bergen this year for the purpose of working out consensus guidelines on the diagnosis and treatment of this common disease. To day upper gastrointestinal endoscopy is the most important diagnostic procedure. There was a general agreement on using Berstad's staging scheme, which comprises only three stages of oesophagitis. Twenty-four hour pH-monitoring may provide useful information on the reflux pattern, but the practical consequences of the examination are still uncertain. H2-receptor antagonists are indicated in mild ca

[Pseudocoronary syndrome in reflux esophagitis].

The authors have shown that among 173 patients with the insufficient closing function of the cardia there were 18 patients with pseudo-coronary syndrome related with reflux-esophagitis. Operations were performed on 14 of them with good effects. The surgical correction of insufficiency of cardia functions in most cases gives elimination of cardio-vascular disorders resulting from reflux-esophagitis.

[Reflux esophagitis: clinico-endoscopic aspects].

Though long well known, reflux oesophagitis is as yet poorly understood in terms of both incidence and physiopathology. These aspects of gastrooesophageal reflux were studied in a group of patients subjected to endoscopic examination. The 13.6% incidence of the condition was very similar to the encountered incidence of ulcers. In two thirds of the cases, anamnesis is sufficient for correct diagnosis. In its more severe forms, reflux oesophagitis is associated with hiatus hernia and overindulgence in tobacco or alcohol. Routine radiology is practically or completely useless for the diagnosis of

[Medical management of reflux esophagitis].

When the cause of oesophagitis has been discovered, patients for whom surgical management is impossible may be successfully treated by improving oesophageal peristalsis, increasing the tone of the LES, and speeding up the emptying of the stomach. The best results are obtained in peptic oesophagitis, since the latest secretion-inhibiting drugs ensure constant reduction of the acidity of the gastric juice, as has been demonstrated by continuous pH monitoring.

Esophagitis caused by oral medication.

Three cases of esophagitis caused by oral medication are reported: 2 were the result of prolonged use of quinidine tablets (producing focal subacute inflammation mimicking carcinoma in one and stricture in the other) and one occurred soon after ingestion of liquid KCl. Previous reports have implicated KCl tablets ("slow KCl"), tetracycline and doxycycline capsules, and ferrous sulfate tablets. The factors predisposing to this uncommon complication of oral medication are discussed. In all reported cases caused by KCl tablets, left atrial enlargement was present as the result of mitral stenosis.

Evaluation of effects of curcumin on acute esophagitis in the corrosive esophagitis model in rats.

Ingestion of a corrosive substance may cause corrosive esophagitis. Curcumin has anti-inflammatory and mucosal protective effects. In this study, the effects of curcumin on the acute phase of corrosive esophagitis were investigated. Twenty-seven Wistar Albino rats were divided into four groups; sham (group I), control (group II), and experiment groups (group III, 100 mg/kg curcumin; group IV, 200 mg/kg curcumin). Forty percent sodium hydroxide solution was used to erode the esophagi of rats in groups other than the sham group. Curcumin was applied to animals in the experiment groups 10 min aft

Lymphocyte-predominant Esophagitis: A Distinct and Likely Immune-mediated Disorder Encompassing Lymphocytic and Lichenoid Esophagitis.

Lymphocytic esophagitis is a well-known manifestation of Crohn disease among children but is not considered to be an immune-mediated mucositis in adults. We hypothesize that adult-onset lymphocyte-predominant esophagitis is also an immune-mediated inflammatory pattern, the nature of which has been masked by other conditions that feature esophageal lymphocytosis and occur in older patients. We performed this study to consolidate diagnostic criteria for lymphocyte-predominant esophagitis and determine its clinical significance. We identified 61 patients with lymphocyte-rich inflammation in the m

Can endoscopists differentiate cytomegalovirus esophagitis from herpes simplex virus esophagitis based on gross endoscopic findings?

Differential diagnosis between herpes simplex virus (HSV) esophagitis and cytomegalovirus (CMV) esophagitis is challenging because there are many similarities and overlaps between their endoscopic features. The aims of this study were to investigate the implications of the endoscopic findings for the diagnosis of HSV and CMV esophagitis, and to develop a predictive model for differentiating CMV esophagitis from HSV esophagitis.Patients who underwent endoscopic examination and had pathologically-confirmed HSV or CMV esophagitis were eligible. Clinical characteristics and endoscopic features wer

Clinical Characteristics of Severe Erosive Esophagitis among Patients with Erosive Esophagitis: A Case-control Study.

Objective The risk factors associated with severe erosive esophagitis are not well defined in Japan. We aimed to evaluate the risk factors associated with the endoscopic severity of esophageal mucosal injury. Methods Eighty consecutive Japanese patients with severe erosive esophagitis [Los Angeles (LA) classification grade C or D] who had undergone upper endoscopies in the Gastroenterology Division of Omori Red Cross Hospital between June 2010 and March 2013 were retrospectively analyzed. For each case, a control with mild erosive esophagitis (LA classification grade A or B) who was matched by

Esophageal physiologic profiles within erosive esophagitis in China: Predominantly low-grade esophagitis with low reflux burden.

BACKGROUND & AIMS: The Lyon Consensus defines LA grades C&D erosive esophagitis (EE) or acid exposure time (AET) >6% as the conclusive evidence for gastro-esophageal reflux disease (GERD). However, most of EE is LA grade A&B and the rate of AET exceeding 6% is low in China. We aimed to evaluate patients with EE in the Chinese population based on the Lyon Criteria. METHODS: A retrospective study was performed among patients with EE who had esophageal function tests performed in a tertiary hospital from 2013 to 2017. Erosive esophagitis was staged according to the Los Angeles classification. Th

Lymphocytic Esophagitis: Navigating an Uncharted Territory.

Lymphocytic esophagitis (LE) is an uncommon subtype of esophagitis defined by persistent esophageal inflammation characterized by a high count of intraepithelial lymphocytes with scarce granulocytes. Although LE can present with atypical features such as chest pain, its clinical presentation can mimic that of gastroesophageal reflux disease or eosinophilic esophagitis, highlighting the importance of biopsy in diagnosing LE. Studies are still limited in understanding the pathophysiology behind this disease warranting further research. A 47-year-old female patient sought medical care with a chie

[Diagnosis and Treatment of Eosinophilic Esophagitis].

INTRODUCTION AND OBJECTIVES: Eosinophilic esophagitis is a chronic inflammatory esophageal disease mediated by a type-2 immune response, mainly triggered by food allergens. Its incidence and prevalence have significantly increased in recent decades, making it a frequent cause of dysphagia and esophageal obstruction. The aim of this narrative review is to summarize current evidence of the diagnosis and treatment of EoE, with emphasis on clinical applications in Latin America. MATERIAL AND METHODS: A narrative review of literature published in PubMed and complementary databases up to May 2025 w

[Eosinophilic esophagitis-an underestimated food allergy?].

Eosinophilic oesophagitis (EoE) is a chronic T2-inflammatory disease and can be figuratively referred to as atopic dermatitis of the oesophagus. It is an immune-mediated disease caused by food allergens and is histologically defined by eosinophilic infiltration and clinically characterised by symptoms of oesophageal dysfunction. In addition to reducing the currently still considerable diagnostic latency, the focus is on prioritising existing and new treatment options with the aim of ensuring effective patient care with improved quality of life in the long term.
