Goal-determined metrics to assess outcomes of esotropia surgery.

PURPOSE: To report outcomes of esotropia surgery with a goal-determined tool. METHODS: A goal-determined outcomes analysis tool was devised to address a quality improvement initiative at Boston Children's Hospital. Surgeons preoperatively ranked four possible goals for intervention: enhancement of binocular potential, restoration of eye contact (reconstructive), management of diplopia, and resolution of torticollis. Criteria for success were goal specific; the primary outcome measure was surgical success at 2-4 months. Secondary outcomes included appraisal of risk factors and a comparison of

Cerebellar astrocytoma manifesting as isolated, comitant esotropia in childhood.

PURPOSE: We encountered a 4 1/2-year-old girl with gradual onset of intermittent, comitant esotropia in the absence of diplopia and other neurologic findings. METHODS: Because of the patient's relatively advanced age and lack of response to hyperopic correction for accommodative esotropia, computed tomography of the head was performed. RESULTS: A large cerebellar astrocytoma was identified and successfully resected. After strabismus surgery, fusion was reestablished. CONCLUSIONS: The onset of comitant esotropia in an older child may indicate an underlying neurologic disorder.

Ocular myasthenia gravis mimicking pseudointernuclear ophthalmoplegia and variable esotropia.

A 70-year-old man with known vascular disease, who had bilateral internuclear ophthalmoplegia, and an 18-month-old girl with a long-term variable esotropia culminating in a postinfectious oculopharyngeal neuropathy both responded with a positive edrophonium chloride (Tensilon) test, strongly suggesting the diagnosis of myasthenia gravis. Both subsequently received maintenance doses of neostigmine methylsulfate (Prostigmin).

Is the Incidence of Concomitant Esotropia with Diplopia in the United States Increasing?

PURPOSE: To investigate the incidence of comitant esotropia (ET) with diplopia using a large insurance claim database in the United States. DESIGN: A retrospective cross-sectional study. SUBJECTS: A large health insurance database. METHODS: Claims for comitant ET with diplopia were extracted using a combination of ET- and diplopia-related codes. The claim ratio of ET to strabismus was calculated from 2007 to 2022. Age and sex distributions were compared between the first (2007-2014) and second (2015-2022) half periods. MAIN OUTCOME MEASURES: Change of ratio of ET with diplopia to all strab

Effectiveness of treatment modalities for childhood esotropia: a systematic review.

BACKGROUND: Esotropia has several types that commonly manifest in early childhood, with numerous treatment options described in the literature. The aim of this systematic review is to synthesize recent research findings on the management of childhood esotropia (ET) to clarify the relative success rates and specific indications for each treatment option, providing guidance for eye care professionals in selecting the most effective interventions. METHODOLOGY: A comprehensive search was conducted across multiple databases, including PubMed, Web of Science, ProQuest, Scopus, Google Scholar, EBSCO

Esotropia-hypertropia complex in a patient with myopic strabismus fixus.

Patients with highly myopic strabismus generally develop esotropia and hypotropia. We report the case of a 54-year-old woman with a combination of esotropia-hypertropia complex in the right eye and esotropia-hypotropia complex in the left eye, with unilateral congenital severe ptosis. The patient had inferotemporal prolapse of the globe in the right eye. Inferotemporal prolapse in this case could have been due to extraocular muscle pulley dehiscence in this area, causing the inferior oblique muscle to slip around the globe along the path of least resistance. Subsequently, the inferotemporal st

A case of bilateral horizontal gaze palsy and concurrent esotropia.

PURPOSE: To report a unique case of bilateral horizontal pontine gaze palsy with concurrent esotropia, surgical management, and post-operative follow-up. OBSERVATIONS: A 39-year-old male presented with diplopia and a history of neurocysticercosis. He was found to have bilateral horizontal gaze palsy and concurrent esotropia, R > L. Classic bimedial rectus recess-resect surgery was done to include resection of the right lateral rectus muscle. Follow-up three months post-op demonstrates markedly improved diplopia. CONCLUSION AND IMPORTANCE: We present a recommended therapeutic approach for the

Botulinum toxin type A as treatment of partially accommodative esotropia.

OBJECTIVE: To determine the effectiveness of a botulinum toxin type A injection in both medial rectus muscles in patients with partially accommodative esotropia. Residual deviation and stability of strabismus were evaluated at 18 months follow up. METHOD: A prospective, analytical, quasi-experimental study was conducted on a cohort of 21 patients who underwent total cycloplegic refraction and with a residual deviation of at least 14 DP. A botulinum toxin type A dose of 5 IU was injected into each medial rectus muscle for a residual deviation greater than 18 DP, with a dose of 2.5 IU being use

Nonsurgical treatment of teenagers with high AC/A ratio esotropia.

Nonsurgical treatment options commonly prescribed for teenagers with high AC/A ratios are progressive lens spectacles, single vision lens spectacles, bifocal contact lenses, and monovision. The gold standard treatment for high AC/A ratios in patients with esotropia is bifocal spectacles, but they are not cosmetically acceptable for most teenagers. However, excellent binocularity is often sacrificed with the alternative treatment options. Quality of life surveys acknowledge that poor cosmesis can have a negative psychosocial impact on individuals. Therefore, it is often necessary for practition

Use of atropine to predict the accommodative component in esotropia with hypermetropia.

This cohort study included children with esotropia and hypermetropia of ≥ +2.0 diopters (D). The deviation was measured at presentation, under atropine cycloplegia and 3 months after full refractive correction. Of 44 children with a mean age of 5.2 ± 2.4 years, 25 were males. Eighteen (41%) had fully refractive accommodative esotropia (RAE), 10 (23%) had partial accommodative esotropia (PAE), and 5 (11%) had nonaccommodative esotropia (NAE). Eleven (25%) had convergence excess (CE). Under cycloplegia, all with RAE and RAE with CE had orthotropia. There was no significant change in the deviatio

Timing of surgery for infantile esotropia: sensory and motor outcomes.

Infantile esotropia is a common ophthalmic disorder in childhood. It is often accompanied by profound maldevelopment of stereopsis, motion processing, and eye movements, despite successful surgical realignment of the eyes. The proper timing of surgery has been debated for decades. There is growing evidence from clinical and animal studies that surgery during the early critical periods enhances sensory and ocular motor development. The Congenital Esotropia Observational Study has defined a clinical profile of infants who will benefit most from early surgery, and several other studies have shown

[The esotropia of the convergence-excess type subdivided into two forms].

There are two types of convergence excess esotropia in the classification of strabismus in our country. One is nonrefractive accommodative esotropia and another is nonaccommodative convergence excess esotropia. Inadequate heterophoria method to determine the AC/A ratio does not distinguish between a high AC/A ratio and nonaccommodative convergence excess. The diagnosis of nonrefractive accommodative esotropia is based on the presence of a high AC/A ratio as established with the gradient method to distinguish this condition from nonaccommative convergence excess.

The remnants of crossed fixation observed in teenaged children with esotropia.

PURPOSE: To report 2 cases of teenage children with esotropia from a much earlier age, displaying remnants of crossed fixation with special reference to Helmholtz's theory that each eye is controlled independently and Hering's Law of equal innervation. CASE REPORTS: Case 1 had paretic and partially accommodative esotropia that was first observed at 11 months of age; and Case 2 had accommodative esotropia with an onset at age 3 years. Both cases had hypermetropia and good corrected visual acuity in both eyes. They were able to fixate independently with each eye but the dominant eye always cont

Acute esotropia may be a presenting sign of intracranial neoplasm.

Here we report eight consecutive patients with intracranial neoplasm at different ages clinically presenting with acute esotropia. Among these patients, two had divergent paresis, four had bilateral abducens paresis and the other two had unilateral abducens nerve paresis. All of these patients have intracranial neoplasm demonstrated by neuroimaging studies. Besides of acute esotropia, four had papilledema, one had optic atrophy, and three had associated other neurological signs. Of the eight patients, three patients are children, and the other five are adults. In the adult group, four patients

Effect of ongoing treatment of amblyopia on surgical outcome in esotropia.

PURPOSE: Classic teaching recommends completion of amblyopia therapy prior to surgical correction of esotropia. Recent reports, however, suggest that incomplete treatment does not adversely affect surgical outcome. This study assesses the effect of incompletely treated amblyopia on the success rate of bimedial rectus recession in infantile and acquired esotropia. METHODS: All patients (n = 102) with esotropia undergoing bimedial rectus recession in 1994 who met inclusion criteria were reviewed. Subjects were classified as having infantile; acquired, partially accommodative; or acquired, nonac

[Study of retinal correspondence in esotropia with phase difference haploscope].

There is some concern regarding the evaluated depth of anomalous correspondence, because we often experience paradoxical results. We have already reported that, in intermittent exotropia, we sometimes found dual correspondence with a phase difference haploscope (PDH) reducing the stimulus intensity of either image with a series of neutral density filters. In this study, we analyzed the retinal correspondence in esotropes with the same procedure. 27 esotropes were examined. 11 cases had normal correspondence and 16 cases anomalous correspondence in PDH. All of the 11 normal correspondence cases

The long-term results of surgical correction of childhood esotropia.

The results of surgical management by one ophthalmologist of 102 patients with congenital esotropia and 74 patients with acquired esotropia in childhood were analysed. The majority of patients had undergone a monocular recession-resection procedure, with 91% and 93% of patients showing a reduction in the angle, of more than five degrees, following surgery for congenital and acquired esotropia respectively. Unexpectedly, varying the amount of lateral rectus resection from 3 to 6 mm, when combined with a constant 5 mm medial rectus recession, did not produce significantly different amounts of ch
