Clinical decision making and ethics in communications between donor families and recipients: how much should they know?

Advances in organ recovery and transplantation have provided us with the skills and opportunity to save, extend, and improve the quality of life for many. But with these opportunities have come challenges to redefine our practice and relationships with donor and recipient families. Although most donor families and transplant recipients receive some information about each other, many still do not. In the past, communication between donor families and recipients has been anonymous and highly controlled, with much inconsistency among and within the transplant community, leaving many involved in t

Psychiatry without thresholds: Ethical and clinical lessons from low-triage systems.

Psychiatric practice in high-income countries typically relies on implicit triage pathways that filter patients through primary and stepped care before specialist review. In many low- and middle-income settings, however, psychiatrists manage the entire spectrum of distress without such filters. This absence reshapes hierarchies of severity, alters diagnostic and ethical reasoning and influences how clinical priority is determined. Drawing on practice realities from India, this article argues that severity in such systems becomes a dynamic and socially negotiated construct rather than a fixed c

Ethical and Clinical Boundaries in Genomics & Newborn Screening: A Brief Report from IPIC2025.

The Ethics Session of the International Primary Immunodeficiency Congress (IPIC), held in November 2025 and organised by the International Patient Organisation for Primary Immunodeficiencies (IPOPI), examined one of the most challenging developments emerging from genomic-enhanced newborn screening: the identification of serious, non-treatable disorders as incidental detection in programmes originally designed to detect severe combined immunodeficiency (SCID). Using a fictionalised clinical scenario based on the recent literature, the session explored the early diagnosis of ataxia-telangiectasi

The Algorithm's Blindspot: An Ethical Critique of Data-Fetishism and the Deskilling of the Clinical Gaze in Ophthalmology.

The rapid integration of artificial intelligence (AI) in ophthalmology has produced remarkable diagnostic accuracy while simultaneously raising significant ethical and epistemic concerns. This article argues that the prevailing AI paradigm fosters a form of data-fetishism: the tendency to treat datasets and algorithmic outputs as objective truth, privileging measurable metrics over the patient's lived experience and clinical context. Inspired by Marx's concept of commodity fetishism, the analysis demonstrates how this phenomenon produces "algorithmic substitution": a process through which the

Generative Artificial Intelligence in Urology: Navigating the Frontier of Ethical, Legal, and Clinical Challenges.

The emergence of generative artificial intelligence (AI), particularly large language models and image-generation tools, is poised to transform the field of urology. These technologies enable innovative applications in medical education, clinical decision support, patient communication, and surgical planning, extending beyond traditional analytical AI by creating new content, from synthetic clinical notes to simulated surgical environments. In urology, these capabilities translate into automated summarization of complex patient histories, generation of patient-specific three-dimensional anatom

Navigating the welfare system and mental health: clinical and ethical reflections on disability assessments.

Welfare assessments can harm the mental health of neurodivergent individuals and people with intellectual disabilities, yet this remains under-recognised in clinical practice. This article integrates three perspectives: media reporting on private profit in UK disability assessments; a clinical case of an autistic man whose deterioration was triggered by Personal Independence Payment and Universal Credit reassessment; and research on the experiences of people with learning disabilities. Common themes include fear, shame, loss of agency and reactivated trauma. Psychiatrists should view welfare s

Duchenne Muscular Dystrophy: Clinical Innovations, Ethical Considerations, and Evolving the Path to Adulthood.

Duchenne Muscular Dystrophy (DMD) is a progressive and life-limiting neuromuscular disorder with profound implications for affected individuals and their families. Advances in genetic and molecular therapies, including exon skipping agents, gene therapy, and Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR)-based approaches have ushered in a new era of disease-modifying treatments. However, these breakthroughs also introduce complex ethical and psychosocial challenges. As we continue to extend life expectancy and improve motor outcomes for individuals with DMD, there is also a

Psilocybin in the real world: Regulatory, ethical, and operational challenges in Australia's clinical landscape.

Australia's reclassification of psilocybin as a Schedule 8 substance for treatment-resistant depression represents a significant shift in psychiatric policy. While this regulatory change positions Australia as a global leader in psychedelic medicine, its implementation has revealed substantial challenges. This article critically examines the regulatory, ethical and operational complexities surrounding the provision of psilocybin-assisted therapy in clinical practice. Key issues include limited prescriber access, absence of Australian Register of Therapeutic Goods-listed products, lack of stand

Screening embryos for polygenic disease risk: a review of epidemiological, clinical, and ethical considerations.

BACKGROUND: The genetic composition of embryos generated by in vitro fertilization (IVF) can be examined with preimplantation genetic testing (PGT). Until recently, PGT was limited to detecting single-gene, high-risk pathogenic variants, large structural variants, and aneuploidy. Recent advances have made genome-wide genotyping of IVF embryos feasible and affordable, raising the possibility of screening embryos for their risk of polygenic diseases such as breast cancer, hypertension, diabetes, or schizophrenia. Despite a heated debate around this new technology, called polygenic embryo screeni

Invasive experimental brain surgery for dementia: Ethical shifts in clinical research practices?

The increasing dementia prevalence worldwide is driving the testing of novel therapeutic approaches, such as invasive brain technologies, despite limited clinical evidence and the risk of accelerating cognitive decline. Our manuscript (a) reviews the NIH Clinicaltrials.gov database for deep brain stimulation, stem cell implantation, and gene therapy trials on people with dementia; (b) discusses issues on beneficence, nonmaleficence, and autonomy associated with these trials; and (c) proposes nine recommendations that build on elements from the Declaration of Helsinki. We found 49 preregistered

Pharmacological Approaches to Managing Violence and Aggression in Prison Populations: Clinical and Ethical Issues.

Violence and aggression are common problems encountered in prison, which frequently require clinical intervention. This increased prevalence is partially attributable to the high morbidity of psychiatric and personality disorders in prison inmates. As prisons are non-therapeutic environments, the provision of clinical care becomes more complex. This article examines the general principles of management of violence and aggression in prison settings, with a particular focus on the clinical and ethical considerations that guide pharmacological approaches. Use of psychotropic medication to address

Clinical review: Ethical and medical considerations of androgen deprivation treatment of sex offenders.

CONTEXT: Blood testosterone codetermines the threshold for erotosexual imagery and sexual activity. Androgen deprivation may therefore have a place in the treatment of unacceptable sexual behavior. Although androgen deprivation can be effective for sex offenders, their basic human rights must be respected; otherwise such treatment constitutes a violation of their physical integrity and is ethically unacceptable. As experience in treating prostate cancers demonstrates, androgen deprivation may have serious side effects. Endocrinologists are qualified to advise on and monitor androgen deprivatio

Ethical brain stimulation - neuroethics of deep brain stimulation in research and clinical practice.

Deep brain stimulation (DBS) is a clinically established procedure for treating severe motor symptoms in patients suffering from end-stage Parkinson's disease, dystonia and essential tremor. Currently, it is tested for further indications including psychiatric disorders like major depression and a variety of other diseases. However, ethical issues of DBS demand continuing discussion. Analysing neuroethical and clinical literature, five major topics concerning the ethics of DBS in clinical practice were identified: thorough examination and weighing of risks and benefits; selecting patients fair

Legal and Ethical Considerations for Clinical Research in Neuromodulation in the European Union: The Chimaera Checklist.

INTRODUCTION: During the last century, many ethics codes have emerged, sometimes embedding legally binding instruments, sometimes encoding good clinical practice for the first time. Apart from the general ethical considerations in use of neuromodulation devices, regulators have tried to shield the patient, implanters, and society from possible risks related to such devices, namely, the protection of the patient's privacy and ensuring enduring manufacturer responsibility for the lifecycle of the device. This review aims to reveal the limits of ethics guidelines, the personal responsibility of t

The Emergency Medical Treatment and Active Labor Act: Clinical Practice and Ethical Considerations.

The Emergency Medical Treatment and Active Labor Act (EMTALA), enacted in 1986, established the legal foundation for access to emergency medical care in the United States. This review examines EMTALA's historical origins, core requirements, and implications for pediatric emergency practice. The law emerged in response to widespread patient dumping practices in the 1980s, when hospitals routinely transferred or refused care to uninsured patients. In the 4 decades since, amendments and regulatory clarifications have expanded the law's scope and strengthened enforcement mechanisms. The modern ite
