Microneedling for Anterior Neck Surgical Scars: A Systematic Review of Effectiveness and Evidence Gaps.

Neck surgeries often leave anterior neck scars, causing cosmetic and psychological issues. These scars are more exposed to mechanical stress and the environment, potentially leading to hypertrophy, dyschromia, and scar widening. Traditionally, surgical scar interventions have been postponed until full scar maturation, usually between six and 12 months after surgery, but recent evidence indicates that early microneedling may improve collagen remodelling and scar quality. While microneedling has been studied for facial and abdominal scars, its effectiveness on anterior neck scars remains unclear

Evidence Gaps in Economic Evaluations of HIV Interventions Targeting Young People: A Systematic Review.

PURPOSE: Young people living with HIV (YPLWH) face the burden of navigating the unique physical, psychological, and social challenges of adolescence while coping with a stigmatized infectious disease that requires lifelong care. Consequently, YPLWH experience worse HIV outcomes compared to children and adults. This systematic review seeks to collate evidence on the health and economic impact of HIV interventions targeting YPLWH and to identify gaps in the available evidence that may inform future economic evaluations of interventions for YPLWH. METHODS: We searched the MEDLINE, Embase, Scopus

Factors associated with gaps in naloxone knowledge: evidence from a 2022 great plains survey.

BACKGROUND: The rising prevalence of fast-acting opioids in the USA suggests the increased need for non-professional first responder administration of naloxone. Effective administration of naloxone during an overdose requires that bystanders are familiar with, have access to, and know how to use naloxone. METHODS: Drawing on a statewide, address-based sample of Nebraskan adults, we used logistic regression to predict the likelihood of respondents' familiarity with, access to, and competency to administer naloxone. Our independent variables included measures indicating proximity to drug use, p

Chiropractic and Spinal Manipulation: A Review of Research Trends, Evidence Gaps, and Guideline Recommendations.

Chiropractors diagnose and manage musculoskeletal disorders, commonly using spinal manipulative therapy (SMT). Over the past half-century, the chiropractic profession has seen increased utilization in the United States following Medicare authorization for payment of chiropractic SMT in 1972. We reviewed chiropractic research trends since that year and recent clinical practice guideline (CPG) recommendations regarding SMT. We searched Scopus for articles associated with chiropractic (spanning 1972-2024), analyzing publication trends and keywords, and searched PubMed, Scopus, and Web of Science

Identification of research gaps from evidence-based guidelines: a pilot study in cystic fibrosis.

OBJECTIVES: Evidence-based guideline committees are multidisciplinary and explicitly consider the existing evidence. They are thus in an ideal position to identify research gaps. However, gaps have not been systematically identified through guidelines. We pilot tested a method to systematically identify and classify gaps from evidence-based guidelines. METHODS: We reviewed all evidence-based guidelines published by the Cystic Fibrosis Foundation. We identified research gaps as topics for which there was insufficient evidence (recommendations were not made or consensus recommendations were mad

GSTP1 promoter hypermethylation in prostate cancer detection: current evidence, clinical positioning, and translational gaps.

GSTP1 promoter hypermethylation is one of the most consistently documented molecular alterations in prostate cancer, with high prevalence in malignant tissue and a well-established relationship with transcriptional silencing. However, despite more than two decades of biological and diagnostic investigation, GSTP1-based testing has not acquired a clearly defined stand-alone role in contemporary prostate cancer detection. This narrative review examines why extensive biological evidence has not translated into routine clinical use and where, if anywhere, GSTP1 testing might provide meaningful val

Myocardial Injury After Non-Cardiac Surgery in Otolaryngology: Evidence Gaps and a Systematic Review.

Background/Objectives: Myocardial injury after non-cardiac surgery (MINS) is a common and serious postoperative complication. Its largely asymptomatic course hampers early recognition, highlighting the importance of systematic biomarker monitoring. The aim of this review is to summarize current evidence on the diagnosis, risk factors, and management of MINS, with a focus on otolaryngology, where intraoperative hypotensive techniques may increase risk. Methods: A basic science review was conducted using PubMed, Embase, and the Cochrane Library (2005-2025). From 2712 records, 30 studies met the

Cholera in Kenya: A scoping review of current research, evidence gaps and future directions.

Cholera remains a major public health challenge in Kenya, driven by environmental pollution, poor water, sanitation, and hygiene (WASH) facilities, weak monitoring systems, and climate effects. While there have been improvements in studying the disease, testing, monitoring, and environmental checks, the spread continues, especially in informal settlements, rural areas, and refugee camps. This review looked at 106 peer-reviewed studies published between 1979 and 2024. It aimed to describe the scope, progress, and gaps in cholera research in Kenya. Five databases, including Google Scholar, Web o

Management of aggressive PitNETs: evidence gaps, molecular clues, and a roadmap for clinical trials.

Aggressive and metastatic pituitary neuroendocrine tumors constitute a rare, yet biologically distinct group of lesions, marked by rapid growth, therapeutic resistance, and unpredictable clinical behavior. Despite their rarity, they contribute disproportionately to morbidity due to the absence of reliable prognostic and therapeutic frameworks. Recent evidence has reframed aggressiveness as a multidimensional process shaped by somatic variants, chromosomal instability, and epigenetic remodeling. Recurrent alterations in ATRX, TP53, and SF3B1; widespread copy number losses; and lineage-specific

Cardiovascular Considerations Before Cancer Therapy: Gaps in Evidence and JACC: CardioOncology Expert Panel Recommendations.

Baseline cardiovascular assessment before the initiation of potentially cardiotoxic cancer therapies is a key component of cardio-oncology, aiming to reduce cardiovascular complications and morbidity in patients and survivors. Recent clinical practice guidelines provide both general and cancer therapy-specific recommendations for baseline cardiovascular toxicity risk assessment and management, including the use of dedicated risk scores, cardiovascular imaging, and biomarker testing. However, the value of such interventions in altering disease trajectories has not been established, with many re

Management of Coronary Artery Disease in Older Adults: Recent Advances and Gaps in Evidence.

Coronary artery disease (CAD) is highly prevalent in older adults, yet its management remains challenging. Treatment choices are made complex by the frailty burden of older patients, a high prevalence of comorbidities and body composition abnormalities (e.g., sarcopenia), the complexity of coronary anatomy, and the frequent presence of multivessel disease, as well as the coexistence of major ischemic and bleeding risk factors. Recent randomized clinical trials and epidemiological studies have provided new data on optimal management of complex patients with CAD. However, frail older adults are
