Predictors for surgical site infection after fasciotomy in patients with acute leg compartment syndrome.

BACKGROUND: Surgical site infection (SSI) is one of the most common complications of orthopedic surgery, which can result in fever, pain, and even life-threatening sepsis. This study aimed to determine the predictors of SSI after fasciotomy in patients with acute leg compartment syndrome (ALCS). METHODS: We collected information on 125 ALCS patients who underwent fasciotomy in two hospitals between November 2013 and January 2021. Patients with SSI were considered as the SSI group and those without SSI as the non-SSI group. Univariate analysis, logistic regression analysis, and receiver operat

Rare complication of four extremity compartment syndrome requiring fasciotomy from influenza A viral myositis.

Influenza A and B commonly cause benign respiratory disease in humans, but can cause more severe illness in high-risk populations. We report an unusual case of a previously healthy adult patient who presented with myositis and severe rhabdomyolysis secondary to influenza A infection that resulted in atraumatic compartment syndrome of all four extremities, each requiring emergent fasciotomy. The patient was subsequently managed with delayed primary closure and skin grafting in the operating room. Prompt recognition of this rare complication by the team resulted in no limb amputations. Endoscopic Plantar Fasciotomy Through Two Medial Portals for the Treatment of Recalcitrant Plantar Fasciopathy.

Plantar fasciopathy is a common cause of heel pain. Endoscopic plantar fasciotomy has the advantage of less surgical trauma and rapid recovery. The aim of the present prospective study was to delineate the results of endoscopic plantar fascia release through 2 medial portals. The present study included 2 groups. The first group included 27 feet in 25 patients that had undergone endoscopic plantar fascia release followed up for 19.7 (range 12 to 33) months. The second group, the control group, included 20 feet in 16 patients treated conservatively and followed up for 16.4 (range 12 to 24) month

Outcomes of fasciotomy in patients with crush-induced acute kidney injury after Bam earthquake.

INTRODUCTION. Fasciotomy may increase the morbidity and mortality in patients with crush-induced acute kidney injury (AKI), by creating an open wound, increasing the risk of bleeding, coagulopathy, and potentially fatal sepsis. This study evaluates the outcomes of fasciotomy in these patients after Bam earthquake in Iran. MATERIALS AND METHODS. We reviewed medical records of victims of Bam earthquake complicated with crush-induced AKI. Demographic, biochemical, and clinical data of patients who underwent fasciotomy were evaluated and compared with other patients with AKI. RESULTS. Fasciotomy w

Cutting Through Thigh Fasciotomies: A National Surgical Quality Improvement Program Analysis.

INTRODUCTION: Acute compartment syndrome is a relatively common condition of the leg, but thigh ACS is a rare pathology. Once recognized, treatment of thigh ACS relies on surgical release of the three main compartments of the thigh. The goal of this study was to analyze the frequency of thigh fasciotomies in patients diagnosed with thigh ACS. METHODS: The American College of Surgeons (ACS) National Surgical Quality Improvement Program database was used to analyze the incidence of thigh fasciotomies performed at participating hospitals from 2012 to 2023. Patients aged 18 y or older with a Curr

The Outcomes of Single-Portal Endoscopic Fasciotomy for Chronic Exertional Compartment Syndrome of the Forearm.

OBJECTIVE: Chronic exertional compartment syndrome (CECS) of the forearm is a rare and underdiagnosed condition that mainly affects young athletes. Minimally invasive endoscopic surgical treatment has been shown to reduce complication rates and allow for an expedited return to activity. Evidence on the outcomes of single-portal endoscopic fasciotomy for forearm CECS remains limited. The aim of this study was to describe the results of single-portal endoscopic-assisted fasciotomy in treating forearm CECS. METHODS: A total of 17 patients (21 forearms) diagnosed with forearm CECS and treated wit

Dermatotraction: an effective technique for the closure of fasciotomy wounds: a preliminary report of fifteen patients.

Dermatotraction was evaluated as an alternative technique for the closure of dermatofasciotomy wounds, with a review of literature and of our clinical experience. The dermatotraction technique provides closure of fasciotomy wounds and avoids the use of skin grafting. Patients treated with dermatofasciotomy for an acute compartment syndrome of the limbs, without obvious tissue necrosis and without shock or urgent life saving surgery, had their fasciotomy wound closed with dermatotraction with vessel loops, the skin approximation system, or the prepositioned intracutaneous suture. In our experie

Comparison of three types of postoperative management for endoscopic plantar fasciotomy. A retrospective study.

This study compared three types of postoperative management of patients undergoing endoscopic plantar fasciotomy. Participating in the study were 38 patients (45 feet) who had undergone endoscopic plantar fasciotomy from 1995 to 1998. One group of patients wore a below-the-knee walking cast with a molded medial longitudinal arch for 2 weeks; another group was allowed immediate plantigrade weightbearing; the third group was nonweightbearing with crutch-assisted ambulation for 2 weeks. The results showed that the patients who wore the below-the-knee walking cast for 2 weeks required less time to

Development of endoscopic dissection of perforating veins and fasciotomy for treatment of chronic venous insufficiency.

As the subfascial endoscopic perforator vein surgery (SEPS) has become increasingly popular, it becomes important to trace the development of this procedure first initiated in 1985. (Improvements in the technique and modifications of initial instrumentation have occurred. These changes are based on experience with greater than 1000 patients treated since 1980 at Krankenhaus der Barmherzigen Brüder in Munich, Klinikum Merheim in Cologne, and the Weilheim Hospital. Results obtained in 96 patients (140 legs) were examined retrospectively and are presented as well as the prospective evaluation of

Postoperative Outcomes of Patients Undergoing Fasciotomy for Exercise Induced Compartment Syndrome: A Case Series.

Exercise induced compartment syndrome (EICS) is a rare source of pain in athletes that can significantly limit their ability to compete and train. We evaluated a series of patients undergoing open fasciotomy for EICS of the leg in our institution. Twenty-five patients were followed for an average of 3.2 ± 1.5 years postoperatively. Sixteen patients underwent bilateral fasciotomies (64%), with four patients (16%) receiving two compartment and 21 (84%) receiving four compartment release. A significant reduction in intra-compartmental pressure, measured intraoperatively by hand-held manometry, wa

Complete Fasciotomy Using a Radial Approach for Acute Forearm Compartment Syndrome: A New Technique.

Acute forearm compartment syndrome is damaging but rare. Early treatment reduces mid and long-term sequelae. The palmar compartment is most frequently involved but the dorsal compartment and mobile wad are also at risk. Multiple fasciotomies approaches have been described but are associated with long-term complications. In particular, wound management is important. Mini-invasive techniques are not available for acute forearm compartment syndrome. Acute leg release via a single approach has been developed and proven effective. Likewise, we present a lateral S-shaped approach for fasciotomies of

A single volar incision fasciotomy will decompress all three forearm compartments: a cadaver study.

INTRODUCTION: Although there are three distinct compartments of the forearm, the typical approach for decompression of forearm compartment syndrome is a single incision fasciotomy of the volar compartment. This study had two purposes. The first purpose of this study was to investigate the compartment pressure changes within the forearm compartments following the creation of an acute compartment pressure using fresh-frozen cadaver forearm specimens. The second purpose was to compare the compartment pressure changes of the volar, dorsal, and mobile wad compartments at frequent intervals followin

Impact of Surgical Timing, Fasciotomy, and External Fixation on Infection Risk in Tibial Plateau Fractures.

Background/Objectives: Tibial plateau fractures (TPFs) are commonly associated with complex patterns requiring advanced surgical strategies. High-energy trauma often results in severe soft tissue damage, complicating surgical outcomes. Despite advancements in soft tissue management, postoperative complications such as surgical site infections (SSIs) remain prevalent, with rates ranging from 9.9% to 30%. This study aims to analyze risk factors and surgical approaches influencing acute SSIs following TPF fixation. Methods: A retrospective analysis was conducted on 365 patients treated for TPFs w
