Fasting serum gastrin and basal gastric acid secretion.

The gastric secretion of acid was examined 30 minutes basally (BAO) and in response to stepwise increasing doses of pentagastrin in subjects with (n = 51) and without (n = 40) peptic ulcer disease. None of them showed basal anacidity. Before insertion of the gastric tube, blood was taken for radioimmunological determination of the serum gastrin concentration (SG). A significant positive correlation was found between SG and BAO in the subjects without ulcer. This was mainly due to a close correlation in 20 healthy young volunteers. When BAO and SG was expressed as proportions of calculated maxi

A carbohydrate loading fasting protocol versus 'Sip til Send': a randomised trial of two different fasting protocols at elective caesarean delivery.

INTRODUCTION: Pulmonary aspiration under general anaesthesia is a rare but serious risk in obstetric populations, leading to conservative fasting protocols prior to elective caesarean delivery (CD). However, prolonged fasting may negatively impact maternal comfort and metabolic status. 'Sip til Send' (STS), a liberal fasting protocol permitting clear fluids until theatre admission, has been shown to improve the perioperative patient experience. Pre-operative carbohydrate loading (CHO), as part of Enhanced Recovery After Surgery (ERAS) pathways, may offer additional benefits, yet its role in ob

Fasts, feasts and festivals in diabetes-1: Glycemic management during Hindu fasts.

This communication is the first of a series on South Asian fasts, festivals, and diabetes, designed to spread awareness and stimulate research on this aspect of diabetes and metabolic care. It describes the various fasts observed as part of Hindu religion and offers a classification scheme for them, labeling them as infrequent and frequent. The infrequent fasts are further sub-classified as brief and prolonged, to facilitate a scientific approach to glycemic management during these fasts. Pre-fast counseling, non-pharmacological therapy, pharmacological modification, and post-fast debriefing a

Impact of combining fast vitrification and fast warming protocols on live birth outcomes.

RESEARCH QUESTION: Does the combination of fast vitrification and fast warming protocols, compared with standard vitrification and standard warming protocols, result in differences in pregnancy outcomes following frozen embryo transfer (FET)? DESIGN: This was a retrospective comparative cohort study of unique patients undergoing either natural, medicated or programmed FET cycles of single blastocysts. The first FET following fresh cycles was included for each patient. Three groups were analysed: standard vitrification with standard warming protocol (n = 1025); standard vitrification with fast

Variable spacing Fast-T1ρ for the analysis of fast relaxing species at low-field.

T1ρ is a powerful analytical technique capable of probing motions on the timescale of molecular dynamics. Conventional T1ρ approaches are performed on high-field NMR magnets (typically >2 T) and rely on step-by-step data acquisition, resulting in relatively long experiments and acquisitions that are affected by non-negligible internal gradient effects and strict Specific Absorption Rate limits. The work presented herein reports the development of novel one- and two-dimensional Fast-T1ρ pulse sequences on a 2 MHz benchtop NMR spectrometer. The developed method enables the observation of fast-re

A Comprehensive Perspective on the Biological Effects of Intermittent Fasting and Periodic Short-Term Fasting: A Promising Strategy for Optimizing Metabolic Health.

It is well established that different fasting strategies offer a range of benefits and may even serve as potential therapeutic approaches for metabolic diseases. The biological effects of intermittent fasting (IF) are multidimensional, involving the induction of metabolic switching from glucose to fatty acid and ketone utilization, thereby enhancing fat metabolism and improving glucose tolerance and insulin sensitivity. In addition, IF modulates the growth hormone/insulin-like growth factor 1 (GH/IGF-1) axis by lowering IGF-1 levels, a change associated with enhanced cellular protection, reduc

Studies of substrate regulation in fasting. II. Effect of infusion of glucose io the carotid artery upon fasting lipolysis in the baboon.

The effect of glucose infusion on rates of lipolysis were studied in a group of chair-trained papio baboons that had been prepared for chronic intravenous and intracarotid infusion. All studies were carried out after a 24 hr period of fasting and when the animals were fully awake. After a control interval of 1 hr, a glucose infusion was begun either intravenously or intra-arterially. The infusion was continued at a constant rate for 2 hr and then changed directly to the alternate route and continued an additional 2 hr. Blood samples were collected at 30-min intervals for glucose, free fatty ac

Beyond Postprandial Symptoms: Fasting Hypoglycemia in Post-Bariatric Hypoglycemia.

BACKGROUND: Post-bariatric hypoglycemia (PBH) is characterized by postprandial neuroglycopenia resulting from an exaggerated hyperinsulinemic response to nutrient intake in patients who have undergone Roux-en-Y gastric bypass, sleeve gastrectomy, or other upper gastrointestinal surgical procedures for weight loss. Fasting hypoglycemia is atypical. OBJECTIVE: We present a cohort of patients who developed symptomatic fasting hypoglycemia in the setting of established PBH and discuss the possible pathophysiological mechanisms. We also examine the role of β-cell hyperfunction in PBH. METHODS: A

Association between fasting substrate utilization and adipose tissue distribution.

BACKGROUND AND AIM: Metabolic flexibility is crucial for health. While reduced fatty acid utilization often correlates with obesity, its relationship with specific fat depots in non-obese individuals remains unclear. This study investigated the association between fasting substrate utilization, measured via respiratory exchange ratio (RER), and abdominal fat distribution (visceral and subcutaneous) across BMI categories. METHODS AND RESULTS: In a retrospective study of 15,022 participants, fasting RER was measured via indirect calorimetry. Participants were categorized by metabolic phenotype:

Fasting boosts breast cancer therapy efficacy via glucocorticoid activation.

The majority of breast cancers are driven by oestrogen receptor-α (ERα) activation, and endocrine therapy represents the mainstay treatment for these patients1. However, resistance is common and tumours often progress after years of endocrine suppression2. Periodic fasting enhances the efficacy of standard endocrine therapy and delays acquired drug resistance, although the underlying mechanisms remain unclear3. Here we show that fasting induces extensive epigenetic reprogramming in ERα-positive breast cancer xenografts when combined with endocrine therapy, with large-scale activation of glucoc

Rab4b controls hepatic glucose production during fasting through glycophagy.

Hepatic glucose production, essential for maintaining glycemia, is often altered in metabolic-associated fatty liver and glycogen storage diseases. Therefore, understanding the mechanisms that regulate the glucose production by hepatocyte is pivotal. Recent studies have identified endocytic trafficking as a key modulator of liver glucose production. Particularly, its role in directing protein trafficking toward lysosomal degradation influences gluconeogenesis. However, the contribution of endocytic recycling to liver glucose production remains yet poorly understood. Here, we demonstrate that h

Intermittent fasting in the treatment of type 2 diabetes.

The increasing prevalence of type 2 diabetes mellitus (T2DM) has inspired researchers to investigate therapies and tools supporting the treatment of this disease. One such tool is intermittent fasting (IF). Given the nature and mechanism of action of IF, it would be logical for it to have a beneficial effect on T2DM patients. This study analyzes the role of IF in the treatment of type 2 diabetes, used alongside standard recommendations, based on the current literature available to the authors. The authors discuss the benefits of IF in T2DM treatment, such as improved glycaemic control, improve
