Femoroacetabular impingement in the unpinned contralateral hip in patients with primary unilateral slipped capital femoral epiphysis: preliminary radiographic outcomes.

The study evaluated femoroacetabular impingement (FAI) in the unpinned contralateral hip in patients with unilateral slipped capital femoral epiphysis (SCFE) and verified initial age, posterior sloping angle (PSA) and center-edge angle (CEA) as predictors of FAI in the contralateral hip. 152 patients with unilateral SCFE with a mean index age of 13.2 years (8.2-17.2 years) were enrolled retrospectively into the study. Mean follow-up was 8 years (3-14 years). PSA and CEA were measured on initial radiographs of the unaffected hip. Alpha-angle and CEA were measured on radiographs taken at the las

Prevalence of low back pain and related disability in patients with femoroacetabular impingement syndrome.

BACKGROUND: Low back pain (LBP) has been associated with worse hip function for persons with femoroacetabular impingement syndrome (FAIS). Reports are limited to surgical populations and based on the presence or absence of LBP, regardless of pain severity. OBJECTIVES: To report the prevalence of clinically significant LBP for persons with FAIS; compare demographics, pain, and function between those with and without clinically significant LBP; and evaluate relationships between hip function and both LBP-related disability and LBP severity. We hypothesized that participants with LBP would be ol

Editorial Commentary: Hip Arthroscopy Evolution and Causes of Failure.

Hip arthroscopy use has grown as a result of expanding indications; improved imaging including higher resolution magnetic resonance imaging with stronger magnetic fields and the advent of 3-dimensional computed tomography revealing the nuances of hip joint morphology; increased specialized training; improvements in instruments and implants; a record of successful outcomes; and increased understanding of microinstability, focal acetabular undercoverage or occult dysplasia, indications for labral reconstruction and capsular plication, and, most of all, femoroacetabular impingement syndrome, the

Characteristics of hip impingement syndrome in patients with multiple hereditary exostoses.

BACKGROUNDS: This study aimed to investigate the characteristic deformities of the hip in multiple hereditary exostoses patients (MHE) and its association with the hip impingement syndrome. MATERIALS AND METHODS: Between 2001 and 2019, total 51 patients (102 hips) were evaluated in this study. Patients with MHE were classified to femoro-acetabular impingement (FAI) symptom group, ischio-femoral impingement (IFI) symptom group and non-impingement symptom group by comparing the symptoms, clinical signs and imaging studies. To assess the morphometry of the hip in patients with MHE, the femoral n

Editorial Commentary: Large Heterogeneity of Minimal Clinically Important Difference Utilization in Hip Arthroscopy Studies: Where Do We Go From Here?

The minimal clinically important difference is a relatively new method to evaluate outcomes after surgery, defined as the lowest value of change in the outcome score that results in a perceived clinical improvement. There is no clear delineation of use for this metric, resulting in a heterogeneous application in hip arthroscopy research, making comparisons with other studies difficult. Cohort-specific values calculated using an anchor-based method are best.

Editorial Commentary: The Hip-Spine Connection in Patients Undergoing Hip Arthroscopy: Should We Learn From Total Hip Replacement and Counsel Patients Differently If They Have Had Previous Back Surgery?

Disorders of the adult hip and spine are common, and there is evidence that there is a hip-spine connection in patients with hip femoroacetabular impingement (FAI). Many reports have shown that patients undergoing lumbar fusion and subsequent total hip arthroplasty have a higher risk of complications and worse outcomes following total hip arthroplasty. However, there has been a paucity of research on the effect of a history of lumbar spine surgery in patients undergoing hip arthroscopy for the treatment of FAI. Variations in lumbar lordosis, pelvic incidence, tilt, and sacral slope may modify

Joint Space Narrowing in Patients with Femoroacetabular Impingement: A Matched Cohort Study of Military versus Civilian Patients.

INTRODUCTION: Decreased hip joint space has been associated with poorer outcomes after hip arthroscopy. The purpose of this study was to determine if a difference exists in hip joint degenerative findings in military versus civilian patients with FAI. METHODS: A single surgeon's case series was reviewed to identify all patients undergoing primary hip arthroscopy for treatment of FAI. All active duty military personnel were included in the study group. A cohort of civilian patients matched for age, gender, lateral center edge (LCE) angle, and alpha angle was identified and included as the cont

Hip Arthroscopy: Common Problems and Solutions.

The use of hip arthroscopy continues to expand. Understanding potential pitfalls and complications associated with hip arthroscopy is paramount to optimizing clinical outcomes and minimizing unfavorable results. Potential pitfalls and complications are associated with preoperative factors such as patient selection, intraoperative factors such as iatrogenic damage, traction-related complications, inadequate correction of deformity, and nerve injury, or postoperative factors such as poor rehabilitation. This article outlines common factors that contribute to less-than-favorable outcomes.

What is the tolerated width of periacetabular osteophytes to avoid impingement in cementless THA?: a three-dimensional simulation study.

BACKGROUNDS: Impingement is a risk factor for instability and prosthetic failure following total hip arthroplasty (THA). If the periacetabular osteophytes are not removed at surgery, impingement could occur between the osteophytes and the femoral stem following THA. However, excessive removal of the osteophytes could lead to bleeding from the bone. The aim of our study, therefore, was to locate the site of the impingement and to determine the width of tolerable osteophytes, which does not induce impingement during activities of daily living (ADL), using a three-dimensional simulation. METHODS

Are "normal hips" being labeled as femoroacetabular impingement due to EE angle?

Gluteal muscle contracture (GMC) is a clinical syndrome characterized by gait abnormality and limb dysfunction, as well as secondary deformities of pelvis and femur. Femoroacetabular impingement (FAI) typically could be diagnosed on the basis of computed tomography (CT) such as the equatorial-edge angle (EE angle), but it did not work well in GMC patients. In this study, we retrospected all image data and found small EE angles in GMCs, which meant retroverted acetabulum; however, none of them showed no symptoms and signs of FAI. Therefore, we had reasons to think that, some normal hips with un

Experimentally induced cam impingement in the sheep hip.

Sheep hips have a natural non-spherical femoral head similar to a cam-type deformity in human beings. By performing an intertrochanteric varus osteotomy, cam-type femoro-acetabular impingement (FAI) during flexion can be created. We tested the hypotheses that macroscopic lesions of the articular cartilage and an increased Mankin score (MS) can be reproduced by an experimentally induced cam-type FAI in this ovine in vivo model. Furthermore, we hypothesized that the MS increases with longer ambulatory periods. Sixteen sheep underwent unilateral intertrochanteric varus osteotomy of the hip with t

The Effect of Sagittal Spinopelvic Parameters and Kinematics on the Morphology and Symptomatology of Femoroacetabular Impingement: A Systematic Review and Meta-analysis.

BACKGROUND: Femoroacetabular impingement (FAI) morphology is common in asymptomatic adults. Spinopelvic alignment modulates hip mechanics and influences hip range of motion. Specific spinopelvic parameters, such as pelvic incidence and dynamic parameters, have been inconsistently associated with FAI. Yet, their specific influence on FAI morphology and symptomatology remains uncertain. PURPOSE: To determine how morphologic, postural, and dynamic spinopelvic parameters differ according to FAI morphology (cam, pincer) and symptomatology. STUDY DESIGN: Meta-analysis; Level of evidence, 3. METHO

Editorial Commentary: Flat Earth-Why 2-Dimensional Imaging Reveals Acetabular Retroversion's Minimal Impact of Arthroscopic Outcomes.

Acetabular retroversion (AR) has been found to be associated with the development of multiple symptomatic pathologies of the hip joint including femoroacetabular impingement syndrome, labral tears, chondral injuries, posterior instability, and the development of hip osteoarthritis. Many studies have showed good results with hip arthroscopy in patients with 2-dimensional imaging preoperatively utilized to diagnose AR. While plain radiographs can be helpful in preliminary evaluation for AR, its diagnosis can be more accurately made using 3-dimensional imaging. Surgeons should utilize 3-dimension

More is not always better-association between hip range of motion and symptom severity in patients with femoroacetabular impingement syndrome: A cross-sectional study.

BACKGROUND: Restricted hip range of motion (ROM) is a common finding in patients with femoroacetabular impingement (FAI) syndrome. However, the association between hip ROM and symptom severity in these individuals is unclear. OBJECTIVE: Explore associations between symptom severity and hip flexion and rotation ROM in patients with FAI syndrome and determine if ROM measures can discriminate those with worse symptoms. METHODS: Data from 150 participants with FAI syndrome were analysed. A digital inclinometer was used to measure hip flexion, internal rotation, and external rotation ROM. Symptom

Editorial Commentary: There Are Limited Changes In Gait Biomechanics After Femoroacetabular Impingement Surgery: A Call for More Comprehensive Functional Testing.

Biomechanical studies have reported that patients with femoroacetabular impingement syndrome (FAIS) exhibit altered walking patterns compared with control patients. The underlying mechanisms of these observed kinematic differences in FAIS patients are believed to be maladaptive and multifactorial in nature. Although arthroscopic hip techniques, surgical indications, and long-term postoperative outcomes for FAIS continue to rapidly evolve, the influence of surgical intervention on restoration of abnormal gait patterns in FAIS patients remains understudied. Recent biomechanical studies using 3-d

Sagittal Pelvic Tilt Directly Influences the Ischiofemoral Space: A Cadaveric Study.

BACKGROUND: Ischiofemoral impingement (IFI) is understood to be a pain generator in the deep gluteal space. Femoral position is known to influence the ischiofemoral space (IFS), but there has been no study examining the effect of sagittal pelvic tilt on the IFS. The purpose of this study was to determine whether changes in pelvic tilt in the sagittal plane lead to changes in the dimensions of the IFS. MATERIALS AND METHODS: Five fresh frozen cadavers (10 hips) were used for this anatomic study. The specimens were skeletonized and placed in the prone position with the pelvis fixed to a custom-

Editorial Commentary: Revision Hip Arthroscopy in Patients With Borderline Dysplasia Has a Role: Indications Are Narrow Versus Periacetabular Osteotomy.

Hip arthroscopy (HA) is preferred for surgical management of femoroacetabular impingement syndrome, whereas periacetabular osteotomy (PAO) is the gold standard for frank developmental hip dysplasia in young adults. Borderline hip dysplasia (BHD) is a conundrum, with data supporting the use of either or both, not to mention that BHD is defined by varying lateral center-edge angle thresholds between 18° and 25° or 20° and 25° and features generalized ligamentous laxity and variations in acetabular and femoral version. That said, HA for BDH has been shown to have 10-year survivorship of 82%. In a

Hip External Rotation Decreases Ischiofemoral Distance: A Positive Correlation Between Cadaveric and Fluoroscopic Studies.

PURPOSE: To determine the effect of hip external rotation or extension/adduction on minimizing the ischiofemoral distance (IFD) and assess the correlation between cadaveric and fluoroscopic IFD measurements in different hip positions. METHODS: This cadaveric study involved 33 hip joints from 17 embalmed cadavers. IFD, the distance between the lesser trochanter and lateral ischium, was measured in different hip positions: neutral, external rotation at 30°, and external rotation at 60° with the hip in both neutral extension and adduction as well as 10° hip extension and 10° hip adduction.
