A 28-year-old Romanian woman, presented 2 weeks postnatally with pain in her left hip. Plain radiography suggested a left intracapsular, subcapital hip fracture. A CT scan of the pelvis showed diffuse osteopaenia with bilateral subcapital fractures of both femurs. From this imaging and subsequent investigation, a diagnosis of transient osteoporosis of pregnancy was made. The patient was treated with a closed reduction and cannulated screw fixation. Bilateral femoral neck insufficiency fracture due to osteomalacia in pregnancy is an extremely rare condition. In this article, we report a 22-year-old female case with bilateral femoral neck fractures in whom the diagnosis was delayed due to the avoidance of ionising radiation and managing hip complaints by conservatively in pregnancy. She was treated surgically with internal fixation using cannulated screws and received medical treatment for vitamin D deficiency.

INTRODUCTION: Stress fractures are common during military training but femoral neck stress fractures are uncommon and sometimes pose diagnostic and therapeutic challenges. An incomplete stress fracture with excellent prognosis, if left unprotected, can lead to displaced femoral neck fracture with almost 63% complication rate even with best of the treatment. The aim of this study was to analyze various aspects of the femoral neck stress fracture so that early diagnosis can be made to prevent devastating complications like osteonecrosis and non-union.

This is a retrospective study of 18 patients who had ipsilateral femoral and tibial fractures. Femoral head fractures occur with a reported incidence of 6%-16% after hip dislocation. Even the largest series have evaluated only a few dozen patients, often collecting experience from multiple centers. The authors conducted a retrospective review of 60 children under the age of 16 years with femoral shaft fractures treated with compression plate fixation. All patients were followed to hardware removal. The 100% union rate and low complication rate after compression plate fixation of pediatric femur fractures compared favorably to the literature of other treatment options. Compression plate fixation is a safe and effective treatment in children with both isolated femoral shaft fractures and those associated with multiple injuries.

Most insufficiency fractures of the femur are transverse fractures of the femoral neck. We report two 77-year-old female patients with longitudinal fractures of the femoral shaft; one had a vertical fracture line in the proximal part of the shaft and the other a spiroid line in the middle third of the shaft. Twenty-seven patients with 28 open femoral shaft fractures were treated by intramedullary nailing. Ten fractures were Grade I, nine Grade II and nine Grade III according to Gustilo.
