Periprosthetic fractures of the distal femur most commonly present as fragility fractures associated with relatively minor trauma. These injuries are often complicated by osteopenia of the distal femur secondary to stress shielding or osteolysis. Effective management of periprosthetic fractures of the distal femur requires knowledge of both fracture fixation techniques and revision arthroplasty. This article reviews the treatment options for these challenging fractures with a particular focus on the management of displaced fractures with a stable prosthesis.

We rarely see trauma patients with secondary hyperparathyroidism. We report about a case of a 68-year-old African female with a pertrochanteric femur fracture after a falling accident. As a secondary result we noted a Looser zone (pseudofracture) in the fractured femur. Extended diagnostic work-up including determination of vitamin D metabolites indicated the existence of a secondary hyperparathyroidism. The dark tanned phenotype of the African patient in combination with usually less solar radiation in central Europe seems to be the causative factor for the secondary hyperparathyroidism. Thus

This study reviews the results of subtrochanteric femur fractures treated with a retrograde nail at a level 1 trauma center. Sixteen patients with 17 fractures were followed up until union. The average Injury Severity Score (ISS) was 19, and average knee range of motion was 127 degrees. Four patients required dynamization of their nail, and three healed over a prolonged period of time. PURPOSE: To determine whether hemodynamically significant bleeding occurs after pediatric femur fractures.
METHOD: A retrospective chart review was performed; demographic and injury data were collected for all patients with the diagnosis of femur fracture over a 30-month period at a level/ pediatric trauma center. Included were patients with multisystem injury and patients with femur fractures as the only injury. The incidence of hemodynamic insufficiency, the reasons for and timing of transfusions, and the changes in hematocrit levels over time were evaluated.

Among 56 slipped capital femoral epiphysis (SCFE) treated over 12 years, the authors report 10 particular cases: 3 hypopituitarisms, 1 adrenal hyperplasia, 1 case after radiotherapy, 2 osteochondrodysplasia, 2 cases with major defect of the posterior wall of femoral neck and 1 coxa valga slip. BACKGROUND: Periprosthetic femur fracture (PFF) is one of the most common indications for reoperation following total hip arthroplasty.
METHODS/RESULTS: This article provides a review of a symposium on PFF that was presented at the American Association of Hip and Knee Surgeons 2023 annual meeting, including an overview of the Vancouver classification and its implications on treatment and subsequent complications, an updated approach to the management of intraoperative fractures, and finally, contemporary strategies for both osteosynthesis as well as revision arthroplasty for PFFs.

The standard of care for treatment of midshaft femur fractures is locked intramedullary nailing. Entry point and patient position remain variables without a single universal method agreed as optimum. Morbid obesity poses additional challenges for intramedullary nailing, where special considerations must be taken. Periprosthetic fractures are typically defined as fractures occurring adjacent to a total joint arthroplasty. However, fractures may occur adjacent to a previous internal fixation device used to treat a prior fracture in the same patient, or between implants (the interprosthetic fracture). All of these injuries tend to occur in frail patients with severe osteopenia, and therefore are very challenging to treat. This article provides a brief but thorough review of the current state of the art regarding the treatment of these challenging problems.
