Antimicrobial use and susceptibility rates in isolates from intensive care unit and other nosocomial inpatient and outpatient areas.

Our objective was to evaluate the relation between antimicrobial use and susceptibility in the intensive care unit (ICU) and non-ICU inpatient areas in the Bolzano regional hospital. For the isolates of S. aureus, coagulase negative staphylococci, Enterococcus sp., P. aeruginosa and E. coli we found a pattern of significant stepwise decrease in the frequency of antimicrobial susceptibility to penicilloic beta-lactam antibiotics and first generation cephalosporins; the highest senitivity rates occurred among isolates from outpatients, followed in decreasing order by rates among isolates from no

Evaluation of the efficacy of cephalosporin antibiotics sold in Kano, Nigeria, against clinical bacterial isolates.

Introduction. The evaluation of the efficacy of cephalosporin antibiotics sold in Kano, Nigeria, against clinical bacterial isolates is a timely and crucial public health concern. Cephalosporins are among the most widely used antibiotic classes globally due to their broad-spectrum activity and low toxicity. They play a vital role in the empirical treatment of infections involving both Gram-positive and Gram-negative bacteria. However, the increasing misuse and circulation of substandard or falsified antibiotics, especially in informal markets, threatens their therapeutic effectiveness. Gap sta

Antimicrobial use on Canadian dairy farms.

Antimicrobial use (AMU) data are critical for formulating policies for containing antimicrobial resistance. The present study determined AMU on Canadian dairy farms and characterized variation in AMU based on herd-level factors such as milk production, somatic cell count, herd size, geographic region and housing type. Drug use data were collected on 89 dairy herds in 4 regions of Canada, Alberta, Ontario, Québec, and the Maritime provinces (Prince Edward Island, New Brunswick, and Nova Scotia) for an average of 540 d per herd. Dairy producers and farm personnel were asked to deposit empty drug

Penicillin Allergy: Mechanisms, Diagnosis, and Management.

Allergy to penicillin can occur via any of the 4 types of Gel-Coombs hypersensitivity reactions, producing distinct clinical histories and physical examination findings. Treatments include penicillin discontinuation, and depending on the type of reaction, epinephrine, antihistamines, and/or glucocorticoids. Most beta-lactams may be safely used in penicillin-allergic patients, with the possible exception of first-generation and second-generation cephalosporins. Penicillin testing includes skin testing, patch testing, and graded challenge. The selection of the type of testing depends on the clin

[Uropathogen pattern and antimicrobial susceptibility in positive urinary cultures isolates from paediatric patients].

OBJECTIVE: Knowledge of urophatogens and antibiotic susceptibility should be used to assist with empirical urinary tract infection treatment. METHODS: We retrospectively analysed local bacterial pattern and antimicrobial susceptibility in positive urinary isolates from paediatric patients collected in the period 2009-2013. Results were compared with a previous study carried out in the same sanitary area between 1995 and 1999. RESULTS: We identified 2,762 urinary isolates. Escherichia coli was the most common uropathogen (58.9%), followed by Enterococcus sp. (11.6%) and Proteus mirabilis (10.

[Treatment of uncomplicated lower urinary tract infections].

Empirical antibiotic treatment of lower urinary tract infections should be based on the patient's clinical data and on local sensitivity data. Because of the increase in resistance among uropathogens, recommendations on the empirical treatment of urinary tract infections have been modified. Currently, the empirical use of co-trimoxazole, ampicillin, and first-generation cephalosporins and quinolones is not recommended. Fluoroquinolones have been demonstrated to be highly effective in comparative studies but, because of the increase in resistance, the type of patient who can benefit from these

First identification of an SHV-12 extended-spectrum beta-lactamase in Klebsiella pneumoniae isolated in Italy.

A clinical isolate of Klebsiella pneumoniae highly resistant to third- and fourth-generation cephalosporins, cephamycins and aminoglycosides, was isolated in 1991 from urine. Analysis of a crude extract showed the presence of three beta-lactamases with isoelectric points of 6.6, 7.5 and 8.2. The enzyme with pI 8.2 was transferred by conjugation into Escherichia coli K-12 J53 and was responsible for the resistance to third-generation cephalosporins and monobactams, but not to other antibiotics. Kinetic studies of partially purified beta-lactamase from the transconjugant strain confirmed that th

Antimicrobial susceptibility of Rochalimaea quintana, Rochalimaea vinsonii, and the newly recognized Rochalimaea henselae.

The susceptibility of an isolate of Rochalimaea quintana, Rochalimaea vinsonii and the newly recognized Rochalimaea henselae to antimicrobial agents was determined by dilution in Mueller-Hinton agar supplemented with 5% sheep blood. Two inocula of 10(2) and 10(4) cfu were used and tests were read after incubation at 5 and 7 days at 37 degrees C in an atmosphere enriched to 5% carbon dioxide. Each isolate was susceptible to amoxycillin, third-generation cephalosporins, tetracyclines, macrolides and rifampicin, with complete inhibition of bacterial growth at concentrations lower than 0.125 mg/L.

Drug-resistant salmonellosis.

After the development of plasmid-mediated resistance against first-line drugs, the emergence of chromosomally-mediated quinolone resistance in typhoid fever has become a major concern. Quinolone insensitivity and treatment failures with ciprofloxacin are being increasingly reported. However, third generation cephalosporins have come as a saviour. To overcome fluoroquinolone insensitivity, higher doses with longer duration of treatment is recommended. Moreover, return of chloramphenicol and furazolidone sensitivity shown by different studies is a welcome observation.

Adverse drug reactions to a cephalosporins in hospitalized patients with a history of penicillin allergy.

Estimates on the cross-reactivity between cephalosporin and penicillin range from 1 to 16%. Patients with a history of penicillin allergy usually receive less optimal and more costly alternatives even if cephalosporins are a more viable alternative. One hundred eighty-six patients admitted to Winthrop University Hospital in a 7.5-month period, who reported penicillin allergy and received cephalosporin, were sent surveys. Eighty-three patients completed the survey and their charts were reviewed. Seven of 83 patients (8.4%) from a larger group of 186 penicillin-allergic patients developed a reac

Emergence of resistance to fluoroquinolones and third-generation cephalosporins in Shigella flexneri subserotype 1c isolates from China.

We report here on the first identification of Shigella flexneri subserotype 1c in China. We also report the emergence of resistance to ciprofloxacin and third-generation cephalosporins in this subserotype 1c for the first time. Isolates of seven strains circulating in China yielded three new sequence types and seven pulsed-field gel electrophoresis patterns, thus demonstrating the existence of high genetic diversity within the isolates. Overall, the seven isolates showed reduced susceptibility to ciprofloxacin; one isolate was ciprofloxacin resistant, whilst another developed resistance to cip

Cefotaxime for treatment of neonatal sepsis and meningitis.

Neonatal sepsis is a clinical syndrome characterized by systemic signs and symptoms, and bacteremia during the first month of life. The incidence is relatively low (one to eight cases/1000 live births), yet the risk of mortality is approximately 25%. Meningitis in the neonate is usually a sequela of bacteremia; however, it is discussed with neonatal sepsis, because they commonly share etiology and pathogenesis. The incidence of meningitis is usually a fraction of the number of infants with sepsis, varying in different settings from one-fourth to one-third. The mortality rate is high, varying i

Antibiotic treatment of community-acquired pneumonia in clinical practice: a European perspective.

The prescription pattern of empirical antibiotic treatment of community-acquired pneumonia (CAP) in seven European countries was studied based on data from published studies and prescription surveys. Overall, the three most commonly used compounds for outpatient treatment were aminopenicillins, aminopenicillins with clavulanic acid, macrolides and cephalosporins. The relative order of these three compounds varied very much between the different countries. Aminopenicillin was the first or second choice in four of seven countries. Cephalosporins were used mainly in Germany and in Southern Europe

The Epidemiology of Meningitis in Infants under 90 Days of Age in a Large Pediatric Hospital.

BACKGROUND: Meningitis is associated with substantial morbidity and mortality, particularly in the first three months of life. METHODS: We conducted a retrospective review of patients <90 days of age with meningitis at Texas Children's Hospital from 2010-2017. Cases were confirmed using the National Healthcare Safety Network (NHSN) definition of meningitis. RESULTS: Among 694 infants with meningitis, the most common etiology was viral (n = 351; 51%), primarily caused by enterovirus (n = 332; 95%). A quarter of cases were caused by bacterial infections (n = 190; 27%). The most common cause of

The evolution of antimicrobial resistance in Salmonella Typhi.

PURPOSE OF REVIEW: Increasing antimicrobial resistance in Salmonella Typhi is a serious public health concern, especially in industrializing countries. Here we review recent clinical and laboratory data concerning the evolution of antimicrobial resistance, with particular reference to the emergence resistance against fluoroquinolones, third generation cephalosporins, and azithromycin. RECENT FINDINGS: The last 40 years have witnessed the sequential emergence of resistance to all first-line antimicrobials used in the treatment of S. Typhi infections. Emergence of Ceftriaxone Resistance during a Case of Pneumococcal Meningitis with Fatal Evolution.

We report a case of a 62-year-old man treated for Streptococcus pneumoniae meningitis by ceftriaxone and dexamethasone. After neurological improvement, neurological degradation by vasculitis occurred, despite effective concentrations of ceftriaxone in the serum and cerebrospinal fluid (CSF). S. pneumoniae with increased MICs to third-generation-cephalosporins (3GC) was isolated from the ventricular fluid 10 days after the isolation of the first strain. Isolate analysis showed that a mutation in the penicillin-binding protein 2X (PBP2X) has occurred under treatment.

The inoculum effect of methicillin-susceptible Staphylococcus aureus on cefazolin and other antimicrobial agents.

UNLABELLED: The inoculum effect (IE) refers to a reduced susceptibility of methicillin-susceptible Staphylococcus aureus (MSSA) to certain antibiotics under high bacterial inocula and may contribute to treatment failure. This exploratory study assessed IE prevalence among 234 nonduplicate MSSA isolates across 11 agents spanning major therapeutic classes, including cefazolin, and characterized IE-positive clones via whole-genome sequencing to inform clinical strategies. Minimum inhibitory concentrations (MICs) were determined by broth microdilution at standard and high inocula. Whole-genome seq
