What health reforms promise and what evaluations measure: A comparative policy analysis of reform evaluation in six predominantly tax-funded high-income countries.

BACKGROUND: Predominantly tax-funded high-income countries are undertaking large-scale reforms to address demographic, workforce and fiscal pressures, digital transformation and fragmented care. Reforms in Australia, England, Canada, Denmark, Norway and Sweden emphasise integration, primary and community care, and sustainability, but their evaluation alignment remains unclear. OBJECTIVE: To compare alignment between national reform objectives and evaluation models across six predominantly tax-funded high-income systems, identifying where reform promises translate into measurable progress. Perceived risks of transit use during COVID-19: Correlates and strategies from a case study in El Paso, Texas.

INTRODUCTION: COVID-19 has led to public transportation ridership plummeting and thus created fiscal crises and operational difficulties for transit operators. Although transit services remain essential for people with limited access to alternative transportation modes, the unfavorable public view keeps potential riders from transit. The public transportation industry is in dire need of restoring trust and recovering ridership. METHODS: In July 2020, we conducted an online survey in El Paso, Texas, to investigate COVID-19 risk perceptions related to transit use. A total of 712 valid responses

The effect of food taxes in the UK on health and economic outcomes, and equity: a modelling study.

BACKGROUND: WHO recommends using fiscal policies (such as taxes on unhealthy foods) that discourage consuming foods that contribute to unhealthy diets. Food tax evaluations require consideration for sociodemographic differences, consumer and industry behaviour, reformulation, and long-term health-related outcomes and health inequalities. We aimed to model the potential benefits and harms of three food tax policy designs, to health, costs, and inequalities, in the UK. METHODS: Using a microsimulation model, we performed an economic evaluation of three UK food tax policy designs: (1) a Sugar Sn

Epidemiology of cardiovascular disease and diabetes in 2026: expanding prevalence, escalating costs, and the limits of contemporary prevention.

The recently published 2026 American Heart Association Heart Disease and Stroke Statistics Update extends beyond routine surveillance to provide a systems-level diagnosis of contemporary cardiovascular medicine. When examined collectively, epidemiologic and economic data reveal that cardiovascular disease has evolved from an acute, episodic illness into a chronic, multisystem condition with profound population and fiscal consequences. Nearly half of US adults now meet criteria for cardiovascular disease when hypertension is included, underscoring that cardiovascular pathology has become a defi

The trade-off between coverage and benefit: spatiotemporal variation of China's health insurance and its influencing factors.

BACKGROUND: Achieving universal health coverage has become a global priority, yet internal regional inequalities remain a major challenge within health systems worldwide. China has achieved universal health insurance coverage; however, substantial inequalities persist across provinces, shaped by decentralized welfare governance and uneven socioeconomic development. Existing research has primarily examined urban–rural and population-based inequities, but comprehensive national-level evidence on regional disparities remains limited, particularly since UHC was achieved. This study investigates th

Ultra-Processed Foods as a Cross-Cutting Driver of Health Inequities: Low- and Middle-Income Countries Across the Life-Course.

Ultra-processed foods (UPFs) have become a growing part of everyday diets worldwide and have been linked to obesity, non-communicable diseases, and adverse mental health outcomes. Yet their role in deepening nutritional and health inequalities across socioeconomic groups and different stages of life is still not fully addressed in policy discussions. Drawing on recent population-based studies, longitudinal surveys, and mechanistic evidence, this perspective examines how UPF consumption intersects with food insecurity, psychosocial distress, and neurodevelopmental health from pregnancy through

Initiation of pharmacotherapy for alcohol use disorder in Japan: a longitudinal descriptive study.

BACKGROUND: Alcohol use disorder (AUD) is common worldwide, but the proportion of patients receiving pharmacotherapy remains low. In Japan, patterns of pharmacotherapy initiation for AUD have not been fully characterized. Here, we quantified the cumulative incidence of pharmacotherapy initiation among patients newly diagnosed with AUD in Japan. METHODS: We conducted a descriptive study using a Japanese insurance claims database. We created annual cohorts of adults aged 20-64 years who were newly diagnosed with AUD during fiscal years (FYs) 2012-21. Cumulative incidence for pharmacotherapy ini

Historical roots of hospital-centred operational governance in China: a path dependence analysis and its implications for global health system reform.

BACKGROUND: Hospital-centred operational governance has been a persistent feature of China's health system. While this model has supported administrative control, rapid service expansion and organisational standardisation, it has also contributed to the continued concentration of resources in hospitals, constrained local flexibility and the relative marginalisation of primary healthcare. This study examines, from the perspective of path dependence, how this governance model gradually emerged and persisted from 1835 to the present. METHODS: Drawing on historical studies, policy documents, inst

The political economy of universal health coverage reform in Kenya: executive dominance, elite alignment, and populist strategy (2004-2023).

Why do some ambitious social health insurance reforms get enacted, while others stall, even within the same country, and under similar structural constraints? This article compares Kenya's failed 2004 National Social Health Insurance Fund Bill, with the reform enacted in 2023. It combines Kingdon's Multiple Streams Approach (MSA) with political economy and populism theory, drawing on 48 elite interviews, and policy documents. Using outcome-explaining process tracing, the study explains why enactment became possible in 2023 but not 2004. MSA highlights how adoption depends on coupling across pr

Social cohesion in contexts of violence against women: evidence from Honduras and implications for public policy.

This cross-sectional, descriptive study assesses social cohesion among women in Honduras using microdata from the 2022 First National Specialized Survey on Violence Against Women and Girls (ENESVMN). Utilizing a multidimensional framework, the analysis examines relational, institutional, and normative variables based on an effective sample of 5,962 interviewed women. The normalized-weight results show that 27.1% (nominal 95% CI: 26.02-28.28) of participants exhibit high social cohesion, 26.6% (95% CI: 25.50-27.74) exhibit moderate cohesion, and 46.3% (95% CI: 45.00-47.53) exhibit low cohesion.

Factors Influencing Water and Sweet Beverage Purchasing Decisions and Behaviours Among Low-Income Households in Four Peri-Urban Communities in Accra: An Exploratory Study.

BACKGROUND: In May 2023, Ghana implemented a 20% ad valorem tax on bottled water and sweet beverages (SBs), replacing a 17.5% tax; sachet water remained untaxed. The effect on low-income consumers' purchasing decisions and consumption patterns remains poorly understood. OBJECTIVE: We aimed to explore factors influencing water and SB purchasing behaviours among low-income households in four peri-urban Accra communities. METHODS: This study employed a convergent parallel mixed-methods design. Four focus group discussions (n = 36) and a cross-sectional survey (n = 43) were conducted among purpo

Lessons on Community Engagement from the Centers for Disease Control and Prevention's Child Sexual Abuse Prevention Portfolio.

Community engagement is a cornerstone of effective and fiscally responsible public health initiatives, especially in the prevention of child sexual abuse (CSA). It helps to ensure the voices, experiences, and perspectives of those directly affected by CSA prevention are central to research processes. This approach promotes collaboration, trust, and shared decision-making, leading to the development of programs and policies that are responsive to community needs and, therefore, more likely to succeed. This brief report synthesizes the community engagement experiences and lessons learned across

Reproductive health, population growth, economic development and environmental change.

World population will increase by 1000 million, or by 20%, within 10 years. Ninety-five per cent of this increase will occur in the South, in areas that are already economically, environmentally and politically fragile. Morbidity and mortality associated with reproduction will be greater in the current decade than in any period in human history. Annually, 40-60 million pregnancies will be terminated and 5-10 million children will die within one year of birth. AIDS-related infections, e.g. tuberculosis, will undermine health care in Africa (and elsewhere) and in places AIDS-related deaths will

Mismatch between urban eco-sanitation improvement and health service supply capacity: evidence from Chinese city panel data and interpretable machine learning.

BACKGROUND: Health service supply capacity depends not only on medical resource stocks but also on urban eco-sanitation conditions and resource-allocation mechanisms. Existing studies focus mainly on environmental exposure and health outcomes, with limited attention to whether eco-sanitation improvements can be converted into health service supply capacity. METHODS: This study constructed a panel of Chinese prefecture-level and above cities for 2002-2024, using the health service supply capacity index as the dependent variable and the eco-sanitation environment index as the core explanatory v

Do free healthcare policies play a role in expanding national health insurance enrollment among informal sector workers? The case of the Afya Care pilot program in Kenya.

BACKGROUND: Several Sub-Saharan African countries have transitioned from fiscally unsustainable free healthcare policies to national health insurance schemes. These national schemes often expect informal sector workers to contribute directly to the scheme. Although enrollment in the national scheme is critical to achieving universal health coverage, informal sector workers’ inclusion remains challenging. In this paper, using the case of the Afya-Care free healthcare policy pilot in Kenya, we examine an issue that has thus far received limited attention in the literature: the extent to which ea

A scoping review on the impact of austerity on healthcare access in the European Union: rethinking austerity for the most vulnerable.

BackgroundThere is consensus that the 2008 financial and economic crisis and related austerity measures adversely impacted access to healthcare. In light of the growing debt caused by the COVID-19 crisis, it is uncertain whether a period of austerity will return.ObjectiveThis study aims to provide a structured overview of the impact of austerity policies in the EU-28 zone, applied in response to the Great Recession, on access to health care for the adult population, using the five access dimensions by Levesque et al. (2013).MethodsThis study followed the PRISMA extension for Scoping Reviews gu

Erased Alternatives, Locked-in Path: The Contestation of Medical Workforce Supply Policy in Authoritarian South Korea, 1961-1988.

This article traces how physician supply policy was formulated and implemented during Korea's authoritarian era (1961-1988), spanning the military regime of Park Chung-hee and the New Military regime of Chun Doo-hwan. It examines several questions that have hitherto gone unexplained: the circumstances behind the initiation (1965), suspension (1971), and resumption (1978) of the founding of new medical schools; why most of these newly established schools were private; and the range of alternatives explored in lieu of building medical schools. The founding of medical schools that began in the mi

Who Upgrades the Plant? A Tripartite Evolutionary Game Model for PPCPs Removal Technology Adoption.

Pharmaceuticals and personal care products (PPCPs) contamination in aquatic environments has attracted considerable research attention, yet most studies focus on treatment technologies while the governance mechanisms coordinating stakeholder behavior remain underexplored. This study develops a tripartite evolutionary game model to analyze strategic interactions among pharmaceutical enterprises, wastewater treatment plants, and government environmental agencies in PPCPs pollution control. Replicator dynamics and stability analysis identify six stable equilibrium configurations and reveal that c

An inconvenient mandate: Margaret Thatcher, neoliberalism, and the politics of climate change, 1987-90.

Between 1987 and 1990, Margaret Thatcher became one of the most high-profile world leaders to warn publicly of anthropogenic climate change. This article argues that her apparent 'green turn' was not a moral conversion or a departure from Thatcherism, but a strategic adaptation of it. Drawing on declassified Prime Minister's Office files, Cabinet correspondence, and private papers, it shows that climate policy was shaped through a 'neoliberal filter' that favoured responses compatible with fiscal discipline, technological modernization, and market logics. As a result, ministers supported clima

Firefighters as a Key Alliance Group to Advocate for Tobacco Control Policies in Korea.

Tobacco control is a public health priority. However, Korea has recently experienced a reversal in its long-term decline in smoking prevalence. To address this trend, it is essential to broaden advocacy networks by engaging new stakeholders. We propose that firefighters represent a strategic yet under-represented partner in tobacco control because their professional interests closely align with tobacco-related policy goals. First, cigarette butts have consistently been the leading cause of fire incidents, and tobacco use directly undermines firefighters' core mission to protect life and public
