Prevalence of vulvovaginal candidiasis and susceptibility to fluconazole in women.

OBJECTIVE: The purpose of this study was to determine the prevalence of vaginal colonization by Candida with the use of a rapid detection method, to examine the determinants of vaginal candidiasis, and to evaluate susceptibility for fluconazole. STUDY DESIGN: Vaginal swabs were collected from unselected women at the outpatient clinic of the Department of Obstetrics and Gynecology. A differentiation was made between patients with a positive and a negative potassium hydroxide examination. RESULTS: Six hundred twelve women were recruited, of whom 39 women (6.3%) had clinical candidiasis. The ov

Fluconazole versus ketoconazole in the treatment of tinea versicolor.

BACKGROUND: A number of effective topical therapies are available for the treatment of tinea versicolor (TV). However, topical antifungals are difficult to apply to a large body surface area and for this reason TV is perhaps more easily treated with systemic agents. METHODS: A total of 128 patients with TV, aged 15-55 years, were entered into a randomized, double-blind, clinical trial comparing the efficacy and tolerance of two regimens of oral treatment for TV. The patients were randomly divided in two groups: group 1 received two 150 mg capsules of fluconazole in a single dose repeated week

Treatment of candidal infections with fluconazole in neonates and infants.

UNLABELLED: To study the therapy, efficacy and safety of fluconazole in candidal mycoses during neonatal phase and infancy a case review in 53 newborns and infants was performed. The majority of these patients were premature with a median birth weight of 1120 g and born within gestational week 23-38. The median age at the onset of fluconazole treatment was 5 weeks. All patients had underlying diseases and several risk factors, which favored the occurence of a systemic candidal mycosis. Systemic candidiasis was the most frequent diagnosis (75.5%). Fluconazole was administered at a daily dosage

Fluconazole in the therapy of multiple osteomyelitis in African histoplasmosis.

Amphotericin B has been the standard treatment for deep mycoses including African histoplasmosis, but its use is complicated by its toxicity. More recently, Fluconazole, a water-soluble triazole with antimycotic activity and little side effects, has become available. A case of African histoplasmosis from Nigeria, manifesting as multiple osteomyelitis and successfully treated with oral Fluconazole, is presented. Fluconazole may be preferred in the therapy of African histoplasmosis.

[Status of fluconazole in the therapy of endogenous Candida endophthalmitis].

During the clinical course of invasive candidosis, endogenous Candida endophthalmitis (ECE) is associated with a higher mortality. In patients with candidemia, an ECE-incidence of 28 to 37% was reported. In i.v.-drug users, the incidence of Candida infections was 21%. Besides surgical procedures including vitrectomy and enucleation, early initiation of systemic antifungal therapy is decisive for the outcome. The clinical use of fluconazole in ECE is documented in 96 patients and in a minimum of 108 eyes. The mean duration of therapy was 6-8 weeks (maximum duration: lifelong) with an average do

Fluconazole in the treatment of tinea corporis and tinea cruris.

BACKGROUND: Results of topical dermatomycosis treatment are often unsatisfactory, particularly in patients with extended or multiple infection sites. OBJECTIVE: Given the high fluconazole concentrations attainable in the stratum corneum and the long elimination half-life of fluconazole, we investigated whether efficacy is satisfactory when using fluconazole at once weekly doses of 150 mg. METHODS: In an open, noncomparative study, tinea corporis and cruris patients were treated with once weekly fluconazole 150 mg over 2-4 weeks. Clinical (pruritus, erythema, scaling, burning/pain, vesiculati

Plasma and tissue concentrations of fluconazole and their correlation to breakpoints.

The prediction of clinical outcome during antifungal therapy is an issue of paramount importance in clinical research, because no consistently reliable parameters are available. Minimum inhibitory concentration (MIC) values and breakpoint interpretation may serve as surrogate criteria until standardized in vitro antifungal susceptibility testing is developed, especially for fluconazole. With reproducible susceptibility testing methods for Candida species now available, tentative fluconazole interpretive breakpoints derived from MIC values determined by the National Committee on Clinical Labora

Fluconazole: comparison of pharmacokinetics, therapy and in vitro susceptibility.

Fluconazole shows good penetration into the tissues and body fluids examined and a rapid equilibrium is achieved between the concentrations in the various compartments. The pharmacokinetics of fluconazole after intravenous or oral administration are proportional to the dose. This finding, together with the slow elimination of the triazole (t1/2 30 h), makes it easier to forecast the therapeutically effective dosage. Measurements of fluconazole concentration in blood can be used to predict levels in some tissues (lung, brain, gynaecological samples), body fluids (sputum, saliva, vaginal secreti

Combination therapy with fluconazole and flucytosine for pulmonary cryptococcosis.

We investigated, in vitro, the combined effects of fluconazole (FLCZ) and flucytosine (5-FC) against different strains of Cryptococcus neoformans, and retrospectively analyzed the clinical efficacy of combination therapy of FLCZ and 5-FC in patients with pulmonary cryptococcosis. The minimum inhibitory concentrations (MICs) of antifungal agents and drug interaction were determined by the broth microdilution method and checkerboard titration. FLCZ and 5-FC showed synergistic activity against 8 (32%) of 25 strains of C. neoformans. The clinical efficacy of the 2 drugs when combined together was

Fluconazole monotherapy for cryptococcosis in non-AIDS patients.

Treatment with 200 to 400 mg/day fluconazole was evaluated in 44 patients without AIDS who had cryptococcosis (19 with cryptococcal meningitis, 22 with pulmonary cryptococcosis, 3 with other cryptococcal infections). For all patients, the clinical response rate was 89% (48% were clinically cured and 41% clinically improved). Of the patients with cryptococcal meningitis, 89% were mycologically cured. These rates are comparable to those obtained in the treatment of AIDS patients with cryptococcal disease. In the group of patients with cryptococcal meningitis, there was a high rate of agreement b

[A case report: cryptococcal meningitis and fluconazole therapy].

An 18-year-old female was given prednisolone and azathioprine for treatment of systemic lups erythematosus. She was admitted to Fukuoka University Hospital because of headache and vomiting. Examinations revealed she was suffering from cryptococcal meningitis. Fluconazole (FLCZ) 400 mg a day was administrated for therapy. Her general condition improved and the serum level of cryptococcal antigen decreased one month after therapy. This therapy resulted in abatement of subjected symptoms and sterilization of the cerebral spinal fluid. This case suggested that therapy with FLCZ alone is useful for

[The use of fluconazole in low-weight neonates with candidiasis].

Systemic Candida Albicans infections in premature newborns are characterized by high morbidity; early diagnosis and immediate treatment are therefore necessary. In this study we report the cases of fourteen newborns admitted to our Newborn Care Unit in a three-year period, in whom candidiasis was diagnosed. The mycetes was isolated from blood, liquor, peritoneal fluid, urine, oesophageal aspirate. Treatment was undertaken immediately with fluconazole, administered through the parenteral route or locally into peritoneal cavum and oesophageal stump. Duration of treatment varied from five to twen

Fluconazole in patients at risk from invasive aspergillosis.

We report on four cases of aspergillosis which developed during therapy with fluconazole in patients who were immunosuppressed or granulocytopenic. Fluconazole may obscure the onset of aspergillosis. We feel that it should neither be given prophylactically nor for fever of unknown origin in patients who are at high risk of contracting aspergillosis.

Fluconazole therapy for experimental cryptococcosis and candidiasis in the rabbit.

Fluconazole is a second-generation azole compound with broad-spectrum antifungal activity. It has been examined in several animal models emphasizing important clinical sites of infection with common yeast pathogens. The drug has an excellent pharmacokinetic profile for central nervous system, renal, and ocular infections; at these sites fluconazole has been successful in the treatment of infections with Cryptococcus neoformans or Candida albicans. On the basis of the experience in animals, fluconazole should be critically evaluated in the treatment of human mycoses such as cryptococcosis of th

Topical fluconazole for experimental candida keratitis in rabbits.

Using a reproducible model of Candida albicans keratitis in rabbits we studied the effect of topical fluconazole, a new triazole. Candida albicans DSM 70010 (2.5 X 10(5) cells) was injected into the corneal stroma of both eyes of 21 rabbits. All eyes developed a corneal ulcer. Forty-eight hours after inoculation the animals were divided into three groups: (1) 14 eyes, received fluconazole (2 mg/ml) and the epithelium subsequently removed; (2) 14 eyes, received only fluconazole drops; (3) 14 eyes, received 0.9% NaCl: half of this group was also debrided. We applied one drop of either substance
