Development of an In Vitro Methodology to Assess the Bioequivalence of Orally Disintegrating Tablets Taken without Water.

To assess the probability of bioequivalence (BE) between orally disintegrating tablets (ODTs) taken without water and conventional tablets (CTs) taken with water, an in vitro biorelevant methodology was developed using the BE Checker, which reproduces fluid shifts in the gastrointestinal tract and drug permeation. In addition to the fluid shift from the stomach to the small intestine, the process of ODT disintegration in a small amount of fluid in the oral cavity and the difference in gastric emptying caused by differences in water intake were incorporated into the evaluation protocol. Metabolic Profiles of Encapsulated Chondrocytes Exposed to Short-Term Simulated Microgravity.

The mechanism by which chondrocytes respond to reduced mechanical loading environments and the subsequent risk of developing osteoarthritis remains unclear. This is of particular concern for astronauts. In space the reduced joint loading forces during prolonged microgravity (10-6 g) exposure could lead to osteoarthritis (OA), compromising quality of life post-spaceflight. In this study, we encapsulated human chondrocytes in an agarose gel of similar stiffness to the pericellular matrix to mimic the cartilage microenvironment. We then exposed agarose-chondrocyte constructs to simulated microgra

Alterations in upper airway cross-sectional area in response to lower body positive pressure in healthy subjects.

BACKGROUND: Fluid accumulation in the neck during recumbency might narrow the upper airway (UA) and thereby contribute to its collapse in patients with obstructive sleep apnoea (OSA). It is hypothesised that acute fluid shifts from the legs to the upper body in healthy subjects would increase neck circumference and reduce the cross-sectional area of the UA (UA-XSA). METHODS: In 27 healthy non-obese subjects of mean (SE) age 39 (3) years and body mass index 23.2 (0.6) kg/m2 studied while supine, leg fluid volume was measured using bioelectrical impedance, neck circumference using a mercury str

Effects of artificial gravity on the vascular system during 60 days of strict head-down tilt bedrest.

During spaceflight, the lack of gravity-induced hydrostatic pressure gradients in the body results in a headward fluid shift hypothesized to play a role in pathophysiological changes in the cardiovascular, ocular, and central nervous systems. Here, 24 subjects (n = 8 control) underwent 60 days of strict 6° head-down tilt (HDT) bedrest, an analog of spaceflight where we tested the ability of daily 30-min artificial gravity (AG) exposure via short-arm human centrifugation (intermittent AG, n = 8, and continuous AG, n = 8) to reverse the HDT-induced headward fluid shift. Hypervolemia and Sleep Apnea in Kidney Disease.

In end-stage renal disease (ESRD) and heart failure, conditions characterized by fluid overload, both obstructive sleep apnea (OSA) and central sleep apnea (CSA) are highly prevalent. This observation suggests that fluid overload may be a unifying mechanism in the pathogenesis of both OSA and CSA in these conditions. An overnight rostral fluid shift from the legs to the neck and lungs has been shown to contribute to the pathogenesis of OSA and CSA, respectively, in various different patient populations. This article reviews the evidence that supports a role for fluid overload and overnight flu

Ovarian hyperstimulation syndrome: facts and fallacies.

Severe or critical ovarian hyperstimulation syndrome (OHSS) is a serious complication of ovarian hyperstimulation for assisted reproduction techniques (ART). The syndrome is characterized by cystic enlargement of the ovaries and fluid shifts from the intravascular to the third space. The morbidity in OHSS is mainly determined by the hemodynamic changes caused by increased capillary permeability. The incidence of OHSS depends on definitions, risk factors, ovarian stimulation protocols, luteal support and conception. Currently, research on the pathogenesis of OHSS is focused on increased capilla

Anaesthetic Management of a Patient With a Giant Ovarian Tumour Containing 28 Litres of Ascitic Fluid.

We report the anaesthetic management of a 47-year-old woman scheduled for tumour resection of a giant ovarian tumour containing 28 litres of ascitic fluid. Her preoperative abdominal circumference was 186 cm due to massive ascites, and metabolic equivalents (METs) were less than 3.5. She was planned for cytoreductive surgery under general anaesthesia combined with epidural analgesia. Anaesthetic challenges included ventilatory management to maintain airway pressures, haemodynamic stability during massive fluid shifts, extensive monitoring, pre-induction epidural catheter placement, prevention

Updates on Spaceflight-Associated Neuro-Ocular Syndrome: Intracranial Changes.

In this review, we discuss current hypotheses on the pathophysiology of spaceflight-induced neuro-ocular syndrome and recent findings regarding microgravity-induced changes in venous, lymphatic, and glymphatic flow. We propose that spaceflight-associated neuro-ocular syndrome (SANS) may not be explained entirely by intracranial pressure changes only and instead is likely multifactorial including cephalad fluid shifts and fluid outflow obstruction as possible inciting mechanism for the characteristic signs and symptoms of this syndrome. In all, we urge a multifactorial view of SANS whereby pred

Tissue-specific diffusion abnormalities and localized free-water reductions in hypertension despite no detectable group difference in the DTI-ALPS index.

BACKGROUND: Essential hypertension (HTN) is an established risk factor for cerebral small vessel disease and cognitive decline. Although conventional diffusion tensor imaging (DTI) has identified white matter (WM) abnormalities in HTN, these metrics are often confounded by extracellular free-water (FW) contamination and cannot distinguish tissue microstructural abnormalities from extracellular fluid shifts. This study applied a free-water elimination (FWE) model in combination with the diffusion tensor imaging analysis along the perivascular space (DTI-ALPS) index to investigate tissue-specifi

Aneurysmal bone cyst in early childhood. Ultrasound findings.

A 1-year-old child with an aneurysmal bone cyst in the scapula is presented. The sonographic examination showed a cystic formation with intraosseal fluid levels that shifted after repositioning of the child. Similar findings have been reported on CT scans and it is suggested that shifting fluid levels are an indicative feature of aneurysmal bone cyst.

Integrating structured point-of-care ultrasound into dengue fever management: A mini review and comprehensive clinical guide.

Dengue fever is one of the most common mosquito-borne viral diseases and poses a significant health threat. The primary management involves hydration tailored to the disease stage and severity. Although many cases are mild, dengue can rapidly progress within hours. Complications such as plasma leakage and bleeding are difficult to detect through routine clinical assessment alone. In this context, point-of-care ultrasound (POCUS) has emerged as a valuable bedside tool for the early detection of fluid shifts and plasma leakage. Ultrasound findings in dengue-related plasma leakage include gallbla

Space motion sickness: incidence, etiology, and countermeasures.

Space motion sickness is experienced by 60% to 80% of space travelers during their first 2 to 3 days in microgravity and by a similar proportion during their first few days after return to Earth. Space motion sickness symptoms are similar to those in other forms of motion sickness; they include: pallor, increased body warmth, cold sweating, malaise, loss of appetite, nausea, fatigue, vomiting, and anorexia. These are important because they may affect the operational performance of astronauts. Two hypotheses have been proposed to explain space motion sickness: the fluid shift hypothesis and the

Assessment of body fluid compartment volumes by multifrequency bioelectrical impedance spectroscopy in children with dengue.

Dengue haemorrhagic fever (DHF), the most severe form of illness following infection with a dengue virus, is characterized by plasma leakage and a period of increased microvascular permeability. Monitoring of plasma volume and body fluid compartment shifts is an integral part of the clinical management of DHF, and is crucial to the performance of clinical research studies on DHF pathogenesis. Multifrequency bioelectrical impedance spectroscopy (BIS) was assessed as a non-invasive method to monitor body fluid compartment shifts in children participating in a prospective, hospital-based, study o

Shock in the emergency department.

Shock continues to be associated with a high mortality rate primarily because of delays in diagnosis and therapy. To diagnose shock early, and thereby increase the chances of reversal before there is extensive deterioration of vital organs, one should look for any decrease in pulse pressure, urine output, urine sodium concentration, alertness or any increase in urine osmolarity, tachypnea or tachycardia. Systolic hypotension, oliguria, metabolic acidosis and a cold clammy skin are late signs of shock. The pathophysiology of early hypovolemic shock includes hyperventilation, vasoconstriction, c

A Quick Reference on Hyponatremia.

Hyponatremia, characterized by low serum sodium and hypoosmolality, commonly results from excess water relative to sodium, often due to conditions like syndrome of inappropriate antidiuretic hormone secretion, dehydration, or fluid shifts. Accurate diagnosis involves assessing serum and urine parameters alongside volume status. Treatment emphasizes identifying and addressing underlying causes, restricting fluid intake, and gradually correcting sodium concentration to prevent neurologic complications such as brain edema. While sodium deficits are rarely calculated, they may be needed in specifi

Safety and efficacy of aspartame-based liquid versus sucrose-based liquids used for dilution in oral sodium phosphate solutions for colonoscopy preparations.

The aim of this study was to investigate whether an oral sodium phosphate solution (OSPS) mixed with aspartame-based clear liquids as the diluent would yield improved colon cleansing results compared to an OSPS mixed with sucrose-based liquids as the diluent. Fifty-one patients undergoing colonoscopy were prospectively randomized into two groups to receive different OSPS colonoscopy preparations, with sucrose-based or aspartame-based liquids used as diluents. The primary end point was the quality of the colonoscopy preparation and secondary end points were serum electrolytes before and after p

[Metabolic state of the other eye in post-traumatic uveitis].

Biochemical analysis of the paired eye lacrimal fluid lipid peroxides (diene conjugates, Schiff's bases, TBA-active products), antioxidant enzymes (catalase, superoxide dismutase), dehydrogenases (lactate and malate dehydrogenases), and lactic and pyruvic acids was carried out during the early and remote posttraumatic period in 468 patients after perforating injuries to the eye. Signs of activation of lipid peroxidation, antioxidants, dehydrogenases, and of glycolysis intensification were detected. These changes corresponded to the results of analysis of lacrimal fluid of the injured eye and d

A mechanical model of retinal detachment.

We present a model of the mechanical and fluid forces associated with exudative retinal detachments where the retinal photoreceptor cells separate, typically from the underlying retinal pigment epithelium (RPE). By computing the total fluid volume flow arising from transretinal, vascular and RPE pump currents, we determine the conditions under which the subretinal fluid pressure exceeds the maximum yield stress holding the retina and RPE together, giving rise to an irreversible, extended retinal delamination. We also investigate localized, blister-like retinal detachments by balancing mechanic

Volume control of the lower limb with graduated compression during different muscle pump activation conditions and the relation to limb circumference variation.

BACKGROUND: The literature supports the use of graduated compression stockings (GCS) for leg edema. Nevertheless, there is a paucity of data on the GCS effect on limb edema related to sitting, standing, and walking. Data of different limb shapes and their impact on GCS-exerted pressure are lacking. This investigation provides evidence-based information on the effect of GCS on edema reduction and the impact of limb circumference gradients on GCS pressure. METHODS: Thirty healthy individuals (15 men and 15 women; mean age, 32 ± 5 years) were included. All the participants underwent lower limb v
