Current strategies for dentist-applied treatment in the management of hypersensitive dentine.

Few of the many dentist-applied treatments for dentine hypersensitivity meet the requirements of an ideal desensitizing agent'. This paper, although not exhaustive, covers methods that may be classified from simplest to most complex. Topical methods are simple, but they are often uncomfortable and the effects usually quite short-lasting, and many fall short of the 'ideal'. Fluorides and oxalates appear to be the best topical methods. Iontophoresis of fluoride represents an intermediate stage of complexity and it meets most of the criteria of an ideal desensitizing agent. Newer restorative mate

Unexpected sequel to silver fluoride followed by stannous fluoride treatment of root stumps supporting an overlay denture in an aged-care patient.

An institutionalized high care dementia patient, who is unable to maintain his own oral health, presented with five lower anterior carious root stumps supporting a lower overlay denture. Due to limited cooperation, his root stumps were treated with only silver fluoride followed by stannous fluoride applied topically on a 4-monthly cycle. Almost one and a half years after his initial application, there were unexpected calculus formations on the root stumps accompanied by marked gingival inflammation and gingival hyperplasia. At this point prognosis was considered very poor. Because the patient'

Controlled remineralization of enamel in the presence of amelogenin and fluoride.

Reconstructing enamel-like structures on teeth have been an important topic of study in the material sciences and dentistry. The important role of amelogenin in modulating the mineralization of organized calcium phosphate crystals has been previously reported. We used amelogenin and utilized a modified biomimetic deposition method to remineralize the surface of etched enamel to form mineral layers containing organized needle-like fluoridated hydroxyapatite crystals. The effect of a recombinant amelogenins (rP172) on the microstructure of the mineral in the coating was analyzed by SEM, XRD and

Effectiveness of combined preventive methods on erupting teeth in children in a fluoridated community.

Several preventive methods were used in combination to reduce dental caries in children residing in a fluoridated area. The method included dental health education, prophylaxis, pit and fissure sealants, topical fluoride and restorative care. Prophylaxis, sealant and fluoride procedures were repeated every 6 months. Sealant was applied to all fully erupted caries free posterior teeth in the mouth. This report compares results obtained for teeth that erupted after the study began with those teeth that were already present at the time treatment procedures were applied. Percent reductions in cari

Efficacy of topical strategies to prevent the onset of root caries lesions: an in vitro study.

This study evaluated the efficacy of several topical dental products in preventing the initiation of root caries lesions using a validated biological caries model. Biofilms of Streptococcus mutans UA159 were formed on bovine root dentin slabs treated with one of five agents: 1% chlorhexidine (CHX) gel, 22,600 ppm fluoride varnish, 5000 ppm fluoride toothpaste, 38% silver diamine fluoride (SDF), or a negative control (0.9% NaCl). The model simulated a highly cariogenic environment through repeated sucrose challenges for 4 days. After the experimental period, biofilms were assessed for biomass,

[Current and future prospects concerning the prevention of dental caries].

Caries is a disease which on the basis of numerous epidemiological data it should be possible to control. The preventive interventions which have proved to have the greatest effect on the diffusion of this disease are essentially: fluoroprophylaxis, oral hygiene, food hygiene and periodic dental examination. The common denominator, which has the greatest effect on success, is a good level of health education of the populations affected by the programme, with specific reference to the teeth. The importance of the diet as a possible element predisposing to caries is an ascertained fact by now, a

Serendipitous results of a pilot study: precaution indicated.

A pilot study was conducted to estimate sample size for a clinical trial in a F area. In 1992, 98 children 14 years of age living in Fall River, MA were examined for dental caries: Fall River was fluoridated in 1973. Residence histories showed that 74% lived there from birth (B), 12% were residents from kindergarten or 1st grade (K1) and 14% moved into the community at a later time (LT). Findings on caries prevalence showed an inverse relation between DMFS and initial age of residence; mean DMFS was 3.00 for Group B, 5.33 for Group K1 and 6.93 for Group LT. Recommendations for using fluoride to prevent and control dental caries in the United States. Centers for Disease Control and Prevention.

Widespread use of fluoride has been a major factor in the decline in the prevalence and severity of dental caries (i.e., tooth decay) in the United States and other economically developed countries. When used appropriately, fluoride is both safe and effective in preventing and controlling dental caries. All U.S. residents are likely exposed to some degree to fluoride, which is available from multiple sources. Both health-care professionals and the public have sought guidance on selecting the best way to provide and receive fluoride. During the late 1990s, CDC convened a work group to develop r

Mouthrinses in the prevention and treatment of periodontal disease.

Although the ideal topical antimicrobial does not exist, some marketed products possess many of the desirable characteristics of such an agent. Antimicrobial agents from a number of different chemical categories are being evaluated continuously. Recent clinical trials evaluating the effectiveness of chlorhexidine, stannous fluoride, prebrushing rinses, phenolic compounds, cetylpyridinium chloride, and new agents such as triclosan are described. Other trials involving special patient groups such as the elderly, mentally handicapped, and orthodontic patients, are discussed. Recent articles on al

Saturation of human saliva with respect to calcium salts.

It may be assumed that free ionic concentrations of calcium and phosphate in resting saliva tend to equilibrate with those in plaque fluid, and that salivary data can therefore be used to illustrate chemical conditions in both saliva and plaque. In the present study, salivary data collected from the literature or obtained in our laboratory were used to calculate degrees of super- and undersaturation with respect to apatites, brushite, beta-tricalcium phosphate, octacalcium phosphate, calcium carbonate and calcium fluoride in the pH range from 3 to 9. The effect of stannous and sodium fluoride on coronal caries, root caries and bone loss in rice rats.

Sixty rice rats (Oryzomys palustris) were divided by littermate into 3 groups of 20 each. The 3 groups received either SnF2 (1000 parts/10(6) F), NaF (1000 parts/10(6] or double-distilled water (control). Test solutions were topically applied to molar teeth, twice daily, for 7 days. All rats were also provided with double-distilled drinking water and diet 2000 ad libitum. Experiments ended after 9 weeks. Alveolar bone loss, root and coronal caries were recorded and scored. SnF2 significantly reduced bone loss (p less than 0.05), but NaF did not. Root caries was significantly different in all 3

Water fluoridation between public health and public law: an assessment of regulations across countries and their preventive medicine implications.

BACKGROUND: Among health professionals and health policymakers concern is growing as to the difficulty of balancing the prevention of dental caries through cost-effective interventions and the need to limit unnecessary exposure of the population, and especially children, to environmental chemicals. In this respect, the use of water fluoridation to prevent tooth decay epitomizes the dilemma by raising questions relevant to both public health and public law, ranging from the balance of public health and medical self-determination, the division of powers between local or national authorities over

Effect of adhesive components in experimental fluoride varnish on fluoride release within 30 days in vitro study.

We aimed to determine whether adhesive components could increase the release time of effective fluoride concentration from an experimental fluoride varnish applied to bovine teeth. An experimental fluoride varnish containing 5% sodium fluoride (EX1) was prepared and combined with 35% hydroxyethyl methacrylate (HEMA) (EX2), 5% glutaraldehyde (EX3), or 35% HEMA/5% glutaraldehyde mixture (EX4). Two commercially available fluoride varnishes were used for comparison. Each group was applied to bovine incisors, and the fluoride release and pH were monitored for 30 days. Chapter 9.1: The Use of Fluorides in the Control of Coronal Caries.

Fluoride is the main agent used to control dental caries, with a very successful history after its widespread implementation worldwide, resulting in significant caries declines around the globe. In this chapter, the mechanism of action and peculiarities of different methods of fluoride use are revised. Fluoride controls caries progression by interfering with the processes of mineral exchanges (demineralization, remineralization) to which teeth are frequently subjected upon exposure to dental biofilm and a diet rich in fermentable sugars. Because caries is a ubiquitous disease of modern societi
