A Modified Suture Bridge Technique for Application With Bone Anchors in Foot and Ankle Surgery.

We present a suture bridge technique for reattachment of tendon or ligament to bone for use in foot and ankle surgery. The method is a simple, strong, and reproducible technique that could decrease the risk of irritation of the overlying cutaneous barrier and minimizes the likelihood of tendon strangulation when combined with soft tissue bone anchors. The present report serves as a guide to the use of this suture technique for reattachment of the Achilles tendon.

Vascular bone transfer options in the foot and ankle: a retrospective review and update on strategies.

BACKGROUND: The foot and ankle area has a tenuous blood supply that can easily be damaged with trauma or open exposures. The navicular and talus are susceptible to avascular necrosis, making arthrodesis difficult. In addition, in this region, large bone gaps occur as a result of avascular necrosis, trauma, or infection. Often, vascularized bone flaps are required for reconstruction or salvage. METHODS: A retrospective review of all vascularized bone flaps to the foot and ankle performed by the senior surgeon (L.S.L) from July of 2006 to July of 2012 was performed. Twelve cases were identified

Healing diabetic foot ulcers using cortical bone fenestration and cell therapy.

Chronic wounds with exposed bone present a challenge for clinicians. Without intact periosteum, cortical bone is relatively avascular and provides a poor base for wound healing and skin grafting. It has been shown that cortical bone fenestration and decortication can expedite wound healing in exposed calvarial bone. We present a case study where cortical bone fenestration and the subsequent application of human skin equivalent were used to heal a chronic wound of the lower extremity. We propose that this technique can facilitate the production of granulation tissue, and can aid the healing of

The results of utilizing proximal tibial bone graft in reconstructive procedures of the foot and ankle.

BACKGROUND: The proximal tibia is becoming a popular site for harvesting autogenous bone graft. Reports on the use of the proximal tibia for bone graft have focused on donor-site morbidity. The purpose of this study is to determine the clinical rate of bony healing following the use of proximal tibial bone graft in foot and ankle procedures. METHODS: This study is a retrospective review of consecutive patients who underwent proximal tibial bone grafting as an adjunct to a foot and ankle reconstructive procedure. Patients undergoing this combination of procedures at a single institution over a

[Outcome of minor amputations at the diabetic foot in relation to bone histopathology: a clinical audit].

AIM OF THE STUDY: to assess the clinical course of minor amputations at the diabetic foot in relation to bone histopathology at the affected site, and at the osteotomy site. PATIENTS AND METHODS: A total of 54 toes or metatarsal bones were amputated by general surgeons. Amputation level and mode of wound closure (primary wound closure) was left at the surgeons' discretion. The subjects were 45 diabetic patients, suffering from critical foot ischaemia (n = 27), polyneuropathy (n = 41) and chronic hemodialysis because of endstage renal failure (n = 4). RESULTS: Histopathology revealed osteomye

Evaluation of arterial obstructive leg and foot disease by three-phase bone scintigraphy.

BACKGROUND: The level selected for amputation should generally be the lowest compatible with tissue viability, with a reasonable expectation of wound healing in patients with arterial obstructive leg and foot disease, but determining the amputation level of an ischemic lower limb remains controversial. The general consensus is that a decisive and final decision about the amputation level should be made intraoperatively based on the extent of hemorrhage from the incised skin and soft tissue, and the degree of viability of the stump. OBJECTIVE: To estimate the extent of such hemorrhage, and thu

Rapid healing of intractable diabetic foot ulcers with exposed bones following a novel therapy of exposing bone marrow cells and then grafting epidermal sheets.

BACKGROUND: Diabetic foot ulcers with exposed bones commonly result in amputation. OBJECTIVES: To determine whether exposure of bone marrow cells and subsequent grafting of epidermal sheets accelerates healing and reduces the need for amputation. METHODS: Thirty-eight patients with chronic wounds caused by diabetes mellitus were enrolled in this study. Epidermal sheets obtained from suction blisters of each patient were grafted on to diabetic foot ulcers without exposed bones (n = 10) and were compared with the standard treatment of local wound care, debridement with a scalpel when indicated

Black bone disease of the foot: a case study and review of literature demonstrating a correlation of long-term minocycline therapy and bone hyperpigmentation.

The chronic use of minocycline and tetracycline has been widely reported in published studies to cause discoloration of skin and teeth. There are very few case reports with regard to discoloration of bone. Those cases reported have been termed black bone disease or blue bone disease because the resulting change to the bone is a blue, green, or brown discoloration that resembles necrotic bone. Documentation of the occurrence in bone, however, is rare, with very few studies noted and only 1 other case that reported changes to the bones of the foot. The mechanism responsible for bone discoloratio

The association between foot morphological development and bone maturation in children: a cross-sectional study.

PURPOSE: Accurate assessment of pediatric foot development is essential for guiding clinical interventions. While chronological age (CA) is commonly used, it may not account for individual variations influenced by skeletal maturity. This study hypothesizes that bone age (BA) correlates more strongly with foot morphology than CA, particularly in predicting pediatric flatfoot progression. METHODS: A retrospective cross-sectional study was conducted with 141 healthy children aged 9-13 years. BA was assessed via hand-wrist X-rays (China 05 RUS-CHN method), and 3D foot parameters were measured usi

Severity of Bone Marrow Edema on MRI Predicts the Diagnostic Potential of Foot Joint Injections.

BACKGROUND: Intraarticular steroid and local anesthetic injections are often performed for foot pain. Localizing the exact joint acting as a pain generator in foot pain is eminent for optimal and cost-effective treatment. Because of the complex anatomy of the foot with multiple small articulations side by side, this can be challenging. This study's purpose was to determine magnetic resonance imaging (MRI) findings as possible predictive factors for the immediate outcome of intraarticular injections of the foot. METHODS: All single joint foot injections at our institution from January 2019 to

Management of primary malignant bone and soft tissue tumors of foot and ankle: Is it worth salvaging?

BACKGROUND AND OBJECTIVE: Performing limb salvage with safe margins and preserving meaningful function is very difficult in the setting of primary malignant bone and soft tissue tumors due to the complex and constrained anatomy of the foot and ankle. The study aims to evaluate the efficacy of limb salvage procedures in terms of functional and oncological outcomes. METHODS: Clinical data of 48 patients, who underwent surgical treatment between 1992 and 2015 in our institution, were retrospectively analyzed. Twenty-one (43.7%) patients had unplanned resections elsewhere previously. Limb salvage

Stress fractures of the foot and ankle, part 1: biomechanics of bone and principles of imaging and treatment.

A stress fracture is a focal failure of bone induced by the summation of repetitive forces, which overwhelms the normal bone remodeling cycle. This review, the first of two parts, discusses the general principles of stress fractures of the foot and ankle. This includes bone structure, biomechanics of stress applied to bone, bone remodeling, risk factors for stress fracture, and general principles of imaging and treatment of stress fractures. Cortical bone and trabecular bone have a contrasting macrostructure, which leads to differing resistances to externally applied forces. The variable and o

Rare Case of Aspergillus ochraceus Osteomyelitis of Calcaneus Bone in a Patient with Diabetic Foot Ulcers.

Diabetes is the most common metabolic disease in humans. One of the major complications of the disease is foot ulcer that is prone to infection. The most common causes of infection which have been reported in these patients are bacteria and fungi such as Candida, Aspergillus, and Rhizopus species. We report one such rare case with calcaneal osteomyelitis caused by Aspergillus ochraceus in a patient with diabetic foot osteomyelitis. The case was a 68-year-old male with a history of type II diabetes for 2 years. The patient had two ulcers on the right heel bones for the past 6 months with no sig
