Plantar release in the correction of deformities of the foot in childhood.

One hundred and ninety-one feet with residual cavovarus deformities from club foot or poliomyelitis were treated by a plantar release followed by correction with serial cast application. Roentgenographically, there was significant improvement in the adduction of the fore part of the foot and the cavus deformity, but the varus angulation of the hind part of the foot did not improve. In children more than six years old with deformities resulting from club foot, the plantar release was particularly effective in alleviating residual cavus deformity. In cavus deformity resulting from poliomyelitis,

The management of deformities of the foot in children with spina bifida.

A simple photographic technique is described which measures the weight-bearing and non-weight-bearing areas of the feet in the standing child. The child stands on a rectangular sheet of thick glass which is set into the floor. Beneath the floor is a cellar, from which the soles of the feet may be observed and photographed. A transparent grid placed over the photographic print enables the weight-bearing and non-weight-bearing areas of the foot to be measured, and thus to express the weight-bearing areas as a percentage of the total area of the sole of the foot. The procedure was used to assess

Progressive collapsing foot deformity: an instructional review.

Progressive collapsing foot deformity (PCFD) is an acquired condition characterised by collapse of the medial longitudinal arch, hindfoot valgus and forefoot abduction. This condition predominantly affects middle-aged and older patients and can lead to significant pain and functional impairment. Clinical assessment is key to identifying a flexible versus rigid foot deformity. The Johnson and Strom classification provides a framework for staging the disease process and guides management. Treatment can involve non-operative measures such as orthoses, or operative measures tailored to the compone

The Association of Cavovarus Foot Deformity With an Os Peroneum.

ObjectiveWe sought to assess whether the presence of an os peroneum is correlated with cavovarus foot alignment in patients without a neurologic explanation for their foot shape. We hypothesized that a large os peroneum would increase the power of the peroneus longus and lead to a forefoot-driven, hindfoot varus deformity.Materials and MethodsThis was a retrospective cohort study conducted at a single institution and reviewed patients with 3 weightbearing views of the foot on plain radiography. Patients were characterized into having either no os peroneum (235), a small os peroneum (18), or a

Neurologic Conditions Associated with Cavus Foot Deformity.

The cavus foot deformity is an often less understood deformity within the spectrum of foot and ankle conditions. The hallmark concern is the possibility of an underlying neurologic or neuromuscular disorder. Although a proportion of these deformities are idiopathic, a significant majority do correlate with an underlying disorder. The appropriate evaluation of this deformity, in coordination within the multidisciplinary scope of health care, allows for a timely diagnosis and understanding of the patient's condition. We provide an abbreviated survey of possible underlying etiologies for the pati

Classification and Nomenclature: Progressive Collapsing Foot Deformity.

RECOMMENDATION: The historical nomenclature for the adult acquired flatfoot deformity (AAFD) is confusing, at times called posterior tibial tendon dysfunction (PTTD), the adult flexible flatfoot deformity, posterior tibial tendon rupture, peritalar instability and peritalar subluxation (PTS), and progressive talipes equinovalgus. Many but not all of these deformities are associated with a rupture of the posterior tibial tendon (PTT), and some of these are associated with deformities either primarily or secondarily in the midfoot or ankle. There is similar inconsistency with the use of classifi

Hindfoot Arthrodesis for the Elective and Posttraumatic Foot Deformity.

Triple (talonavicular, subtalar, and calcaneocuboid) joint arthrodesis and most recently double (talonavicular and subtalar) joint arthrodesis have been well proposed in the literature for surgical repair of the elective, posttraumatic, and/or neuropathic hindfoot deformities. The articulation of the hindfoot with the ankle and midfoot is multiaxial, and arthrodesis of these joints can significantly alter the lower extremity biomechanical manifestations by providing anatomic correction and alignment. This article reviews the indications and preoperative planning for some of the most common pro

Progression of foot deformity in Charcot neuropathic osteoarthropathy.

BACKGROUND: Charcot neuropathic osteoarthropathy associated foot deformity can result in joint instability, ulceration, and even amputation. The purpose of the present study was to follow patients with and without active Charcot osteoarthropathy for as long as two years to examine the magnitude and timing of foot alignment changes. METHODS: We studied fifteen subjects with Charcot osteoarthropathy and nineteen subjects with diabetes mellitus and peripheral neuropathy without Charcot osteoarthropathy for one year; eight of the subjects with osteoarthropathy and five of the subjects with diabet

[Historical review of congenital foot deformity treatment].

INTRODUCTION: There is still no good solution for the treatment of all the forms of clubfoot. HISTORY OF THE TREATMENT IN THE WORLD: The first written description of the treatment of this deformity was given by Hippocrates. In the Middle Ages, Galenus, Celsus and Averroe only reviewed the lessons of Hippocrates's principles. The first description of orthopedic prosthetic for the treatment of clubfoot was published in "Opera Chirurgica" by Amboise Peréa in 1575. In 1796, Bruckner wrote the first monography about clubfoot. The biggest contribution to the modern way of treating clubfoot was give

Acquired flat foot deformity: postoperative imaging.

Flat foot (pes planus) is a progressive and disabling pathology that is treated initially with conservative measures and often followed by a variety of surgeries. This article briefly reviews the pathology in acquired flat foot deformity, the classification of posterior tibial tendon dysfunction, discusses surgical techniques for the management of adult flat foot deformity, and reviews potential complications and their relevant imaging appearances.

Two cases of neurofibromatosis presenting with a foot deformity.

Two children presented with an isolated foot and ankle deformity. Examination in each suggested a plexiform neurofibroma although this diagnosis had not been considered before referral. Diagnosis of neurofibromatosis type 1 was confirmed by MRI scanning and on investigation both patients were proved to have widespread disease. One had a plexiform neurofibroma encasing the aorta and oesophagus. Both cases remain under observation and have not undergone surgery for their disease. Neurofibromatosis can present with isolated foot and ankle deformity and when such a diagnosis is suspected thorough

Rheumatoid flat foot and deformity of the first ray.

OBJECTIVE: To study the relationship between flat foot and forefoot deformities in rheumatoid arthritis (RA) in order to improve understanding of the progression of deformity and thus provide more appropriate treatment. METHODS: Anteroposterior and lateral weight-bearing radiographs were obtained of 308 feet of patients with RA and 202 feet of patients with neck pain (control feet). RESULTS: In women with RA, we observed with disease duration an increased frequency of flat foot that was correlated with first ray deformity (chiefly metatarsus primus adductus) and severe stages of disability.
