Current concepts of brain herniation have depended largely on correlating clinical signs and symptoms with indirect radiographic studies and the results of postmortem neuropathology. This article describes measurements on midsagittal magnetic resonance imaging (MRI) scans that distinctly define normal and abnormal rostral-caudal relationships between the diencephalic-mesencephalic junction and the plane of the tentorial incisura, herein termed the incisural line. We similarly provide quantitative MRI scan measurements relating the cerebellum and the plane of the foramen magnum, termed the fora

A 56-year-old woman presented with a foramen magnum meningioma associated with headache and left hypoglossal nerve palsy. A vessel arising from the intradural vertebral artery was identified as the posterior meningeal artery based on an integrated assessment of its origin, course, and relationship to the tumor on three-dimensional angiography and at surgery. During temporary occlusion, the left abductor pollicis brevis motor-evoked potential (MEP) was present at 8 min and absent at 9 min. It was again detectable in the recording obtained 7 min after clip release, but subsequent responses were

There are not so many variants regarding the foramen magnum (FM). However, the existence of a prominent eminence at the anterior border of FM is one of the variations. In the current study, we detected such a tubercle in a dried skull of an old man's skeleton, with its base arising from the midpoint of the internal aspect of the anterior margin of the FM. The apex of this tubercle was oriented backwards. As for the dimensions, the anteroposterior distance was measured at 4 mm, and the transverse distance at 5 mm. That tubercle is usually circular or ovoid in shape, and its appearance is common

Central nervous system tuberculosis is a devastating complication of systemic tuberculosis. Intradural extramedullary (IDEM) tuberculoma at the foramen magnum is rare, and mimics en plaque meningioma. We report the case of a 53-year-old woman who presented with dysesthesia of the tongue and lower cranial nerve (CN) palsy, with onset 4 months prior to admission. The neurologic examination revealed left upper-limb weakness and hypoesthesia on the sole and dorsum of the left foot. Other physical examinations revealed no features of tubercular infection. Laboratory investigations likewise showed n

BACKGROUND: Foramen magnum stenosis (FMS) is a serious complication in children with achondroplasia that may necessitate cervicomedullary decompression (CMD). It is unclear how FMS and CMD influence growth in these children. This study aimed to assess the effects of FMS and CMD on the growth of children with achondroplasia. METHODS: Eighty-seven children (45 males, 42 females) with achondroplasia, aged 4 to 6 years, were evaluated. Height, weight, head circumference, and body mass index were expressed as standard deviation scores (SDS) according to Merker et al., while sitting height SDS was d

BACKGROUND: The foramen magnum is the largest opening at the skull base. Congenital skeletal disorders such as achondroplasia can cause foramen magnum stenosis, leading to neurological problems. In high-risk infants, repeated magnetic resonance imaging (MRI) is often used to screen for or monitor stenosis. Ultrasound (US), already used for neonatal spinal assessment, may offer a non-invasive method for evaluating the craniovertebral junction. However, to our knowledge, no studies have directly compared foramen magnum assessment between US and MRI.
METHODS: We retrospectively reviewed neonates

INTRODUCTION: Managing foramen magnum meningiomas (FMMs) poses challenges due to their anatomical complexity. Surgical resection aimed at achieving gross total resection (GTR) is the primary therapeutic option; however, the proximity to nerves and arteries complicates resection and is associated with significant morbidity and mortality rates, reported to reach as high as 50% and 25%, respectively. Stereotactic radiosurgery (SRS) is a non-invasive radiotherapeutic modality that has been extensively used to treat intracranial meningiomas and is associated with high rates of favorable outcomes an

RATIONALE: Dural arteriovenous fistulas (DAVFs), are rare intracranial arteriovenous malformations which are depicted with direct shunts between meningeal arteries and dural sinuses, dural veins, or cortical veins. We reported a rare case of DAVF in the foramen magnum (FM) which was drained by the jugular veins and the perimedullary veins.
PATIENT CONCERNS: A 53-year-old male patient was admitted at the Department of Neurosurgery, West China Hospital 5 hours after experiencing a severe headache. His neck was stiff and Brudzinski and Kernigs signs where positive indicating meningeal irritation

The role of cervicomedullary decompression (CMD) in the care of hydrocephalic achondroplastic children who present with simultaneous foramen magnum stenosis is not well understood. We sought to determine the percentage of symptomatic achondroplastic children with foramen magnum stenosis who had stabilization or improvement in ventriculomegaly following CMD. The authors retrospectively reviewed the records of pediatric patients at Cedars-Sinai Medical Center with achondroplasia and signs of progressive ventriculomegaly who underwent CMD for symptomatic foramen magnum stenosis between the years

INTRODUCTION: Endodermal cysts are congenital benign cystic lesions in the central nervous system and cause various symptoms. Although some have been reported in the posterior fossa, endodermal cysts located dorsal to the brainstem are extremely rare.
CASE PRESENTATION: The case was of a 10-year-old girl who presented with bilateral upper limb weakness and tremor. Magnetic resonance imaging demonstrated a 4.5-cm cystic lesion with T1-weighted hypointense and T2-weighted hyperintense content in the midline of the cisterna magna dorsal to the medulla oblongata. The cyst was cerebrospinal fluid-

The aims of this study were to determine reference values for magnetic motor evoked potentials (mMEPs) in calves and the influence of position during examination (standing or lateral recumbency). Reference values were determined using 41 healthy Holstein Friesian bull calves aged 1-10 months; standing and lateral recumbency were examined in 11 calves. Maximal magnetic stimulation was performed at the level of the foramen magnum with a magnetic field of 4 T at the coil surface. In standing position, distinct, reproducible mMEPs were obtained in all calves. Onset latency (LAT) (mean ± standard d

BACKGROUND: The management of foramen magnum meningiomas (FMMs) has been a challenge for skull base neurosurgeons. Since the initial description of a FMM in 1872, various surgical approaches have been described. Posterior and posterolateral FMMs are safely removed through a standard midline suboccipital approach. Nevertheless, we still face controversy regarding the management of anterior or anterolateral lesions.
CASE DESCRIPTION: A 47-year-old patient presented with progressive headaches, unsteadiness, and tremor. Magnetic resonance imaging showed an FMM that caused significant displacement

Objectives Safe maximal resection is the basic principle of cranial base surgery and the grade of resection is an important factor influencing the prognostic outcome. This operative video highlights the surgical principles and technical nuances in the microsurgical resection of foramen magnum meningioma (FMM). Case Description The surgery was performed in a 45-year-old lady who presented with hoarseness of voice and spastic quadriparesis (grade 4/5). On imaging, FMM with mass effect on brainstem and spinal cord was identified. The tumor was gross totally resected through modified far lateral

BACKGROUND: Type I Chiari malformation (CMI) is a rare complication of lumbar cerebrospinal fluid (CSF) drainage that is usually reported after lumbar drain or lumboperitoneal shunt placement. It usually remains asymptomatic; however, even if it becomes symptomatic, symptoms are usually mild. There are only a few reports of acute foramen magnum syndrome following continuous lumbar CSF drainage, and acute foramen magnum syndrome after a single diagnostic lumbar puncture (LP) has not been previously reported. We encountered this catastrophic complication in one of our patient.
