Dilemma still prevails, regarding the exact management of mangled extremity injuries between limb salvage versus amputation, each having there own set of complications. We here present a case of severely comminuted fractures of radius (bag of bones) along with the multiple criss-cross shaped lacerated wounds on the forearm and wrist presenting as a "signature pattern injury" caused by entrapment of the limb in the concrete mixer. MESS score of patient was 8, a score valid for amputation, but contrary, we successfully salvaged the patient's limb with use of radio-carpal distracter. Management o

Comminuted quadrilateral plate fracture with medial displacement is a technically difficult fracture to treat. Minimal bone stock, proximity to the hip joint with limited surgical access, and difficulty in obtaining a stable fixation at this area, contribute to the surgical challenge of open reduction and internal fixation. Fixation of a medial buttress plate in an infrapectineal fashion is a well-described technique to address such fractures. However, this plate alone may be inadequate to buttress all the fragments in a grossly comminuted quadrilateral plate fracture. An additional spring pla

HYPOTHESIS: Using radiologic and clinical results, we studied the outcome of patients who underwent open reduction and plate osteosynthesis for comminuted olecranon fractures.
MATERIALS AND METHODS: We retrospectively studied 18 patients (5 women [27.8%] and 13 men [72.2%]; mean age, 41 years [range, 19-67 years]) with comminuted fractures of the olecranon who underwent locking-plate osteosynthesis after open reduction between March 2005 and August 2009. According to the Mayo classification, 11 cases were classified as type IIB (61.11%) and 7 cases were classified as type IIIB (38.88%). In 7

Mason type III fractures of the radial head are treated by open reduction and internal fixation, resection or prosthetic joint replacement. When internal fixation is performed, fixation of the radial head to the shaft is difficult and implant-related complications are common. Furthermore, problems of devascularisation of the radial head can result from fixation of the plate to the radial neck. In a small retrospective study, the treatment of Mason type III fractures with fixation of the radial neck in 13 cases (group 2) was compared with 12 cases where no fixation was performed (group 1). PURPOSE: The purpose of this retrospective study was to evaluate the early results of arthroscopic treatment in patients with comminuted, displaced greater tuberosity (GT) fractures using the arthroscopic double-row suture anchor fixation (ADSF) technique.
METHODS: Between August 2004 and December 2007, we used the ADSF technique in 16 cases of isolated comminuted, displaced GT fractures. The early clinical results were evaluated in these patients at a mean of 24 months (range, 16 to 51 months) after surgery. There were 11 male and 5 female patients with a mean age of 56.5 years (range, 27 to

Periprosthetic fractures represent a severe complication after joint replacement. A case of comminuted displaced humeral fracture around the stem of a SMR reverse shoulder prosthesis is reported. The patient was a 81-year-old lady who had had a total shoulder replacement 28 months previously. The surgical solution consisted of a partial revision of the modular implant with conservation not only of the glenoid component but also of the prosthetic humeral body, which was well fixed in the humeral metaphysis. The humeral stem was removed and a long uncemented revision stem was implanted providing

Fractures of the mandible and their management are discussed in detail in textbooks and articles dealing with facial trauma. This paper presents the management and treatment of a case of a unilateral comminuted and complicated fracture of the mandible due to dog attack on a geriatric patient. The attack also severed the patient's right arm. Due to the severity of the trauma, an emergency surgery was performed on the mandible and arm.

The cases of nine patients who had a complex fracture of the distal part of the femur and a deficient medial-cortical buttress were reviewed. Stable fixation was not achieved with the lateral condylar buttress plate alone. Collapse of the distal fragment into varus angulation was noted intraoperatively, with the axis of rotation being the junction of the distal screws and the plate. Additional stabilization with a medial plate and a bone graft from the iliac crest was applied in all nine patients: in six, at the time of the index operation and in the remaining three, after the open wound and o

A 4-year-old Standardbred stallion sustained a severely comminuted fracture involving the second, third, and fourth metacarpal bones. The fracture was repaired using two 14-hole broad dynamic compression plates positioned at 90 degrees to one another, allowing one plate to protect the other in the bending mode. An autologous cancellous bone graft obtained from the tuber coxae was added at the site of the defect in the mid- to upper third of the third metacarpal bone. Complications associated with the fixation included a Staphylococcus aureus infection 5 months after surgery, laminitis that dev

Treatment of comminuted fractures of the distal radius bone by the traction-suspension technique is both simple and innocuous. The authors describe this old and satisfactory technique, slightly modified: a transmetacarpal pin is introduced and suspended to a giving set stand, and a counter-traction of 3 to 4 kg is exerted by means of a transulnar pin. After remodelling of the bone under general anaesthesia and radiographic control, the pins are sunk in plaster to be removed 6 to 8 weeks later. Excellent, often unexpected clinical and radiological results can be obtained, and complex traumatic

The patients selected for this modification of the Roger Anderson device are few, because the method is intended for unstable fractures of the distal radius. Extensive disabling stiffening of the wrist and hand encountered following prolonged immobilization by rigid distraction methods can be avoided or greatly reduced by this simple modification of the method. This modification of the extraskeletal fixation should prove to be superior to rigid distraction when early decompression of the carpal tunnel is indicated.

The technique used in the treatment of seventy-five patients with severely comminuted, often intra-articular fractures near the wrist was as follows- Pins were inserted through the metacarpals and proximal part of the ulna, reduction was done, and the pins were incorporated in a cast from elbow to knuckles. This allowed movement of the fingers and the elbow. The main complications - loss of radial length and pintract infection - are avoided by attention to technical details. Of the patients with adequate follow-up, 86 per cent had good or excellent results.

BACKGROUND: Although it is well known that some calcaneal fractures extend to the calcaneocuboid joint, there have been no reports detailing the techniques of reduction in calcaneal fractures with severe displaced depressed calcaneocuboid joint fractures.
PURPOSE: We experienced a case of calcaneal fracture with severe displaced comminuted calcaneocuboid joint depression fracture. The purpose of this report is to introduce reduction and fixation techniques.

BACKGROUND: Surgical treatment of comminuted and multiple facial fractures is challenging, as identifying the bone anatomy and restoring the alignment are complicated. To overcome the difficulties, 3D-printed "jigsaw puzzle" has been innovated to improve the surgical outcome. This study aimed to demonstrate the feasibility of 3D-printed model in facial fracture restoration procedures.
MATERIALS AND METHODS: Patients with traumatic craniomaxillofacial fractures treated at a single institution were enrolled in this study. The exclusion criteria included the presence of mandibular fractures, gre

OBJECTIVE: To investigate the outcomes of humeral head replacement in the treatment of patients with comminuted proximal humeral fracture.
METHODS: Between February 2013 and September 2016, 56 patients underwent humeral head replacement in our hospital. Of them, 18 cases were diagnosed as comminuted proximal humeral fracture before the operation. The mean age of the patients was 69.5 years old (ranging from 61 to 79 years old). Of them, there were six males and 12 females. All the patients in this group had fresh fractures. They were all treated by artificial humeral head replacements. After

OBJECTIVE: To investigate the surgical approach, fixation, and clinical effect of comminuted coronal shear fracture of distal humerus.
METHODS: From March 2017 to February 2019, we had used open reduction and internal fixation to treat 19 cases of comminuted distal humeral coronal shear fracture. There were 8 males and 11 females, with an average age of 44.6 years (19-72 years). There were 10 cases on left side and 9 cases on right side. All cases were closed fractures. According to Dubberley's classification, there were six cases of type 1, six cases of type 2, and seven cases of type 3. The

Emergency physicians in the field are sometimes confronted with cases wherein patients cannot be intubated and ventilated. In some cases, cricothyrotomy, the method of choice for securing an emergency airway, may not have a successful outcome. We report a rare case of a 35-yearold male patient with avulsion of the larynx and a comminuted fracture of the jawbone, due to entrapment in a dung excavator. Prehospital tracheotomy was successfully performed. In cases with crush injuries to the larynx, anatomic structures, including the ligamentum conicum, are destroyed. In addition, massive subcutane
