Pedicular stress fracture in the lumbar spine.

Spondylolisthesis with or without spondylolysis is common in the lumbar spine. Associated fracture in the pedicle ('pediculolysis') is unusual. The margins of pedicular stress fractures, like spondylolysis, usually appear sclerotic. A patient with a pedicular stress fracture with minimal marginal sclerosis suggesting an injury of recent onset is presented here. There was associated bilateral spondylolysis. The findings in this patient suggest that established pediculolysis probably represents a stress fracture that has failed to heal.

Stress fracture of the humerus. A case study.

A 13-year-old male Little League pitcher sustained a displaced spiral fracture of the right midhumerus while in midpitch. Deep ache prodrome symptoms in the midhumerus at rest and during pitching occurred during the preceeding week, suggestive of a stress fracture that later completed to an overt fracture during the added stress of a forceful side-arm curve ball pitch, his specialty. Osteonal remodeling in stress fractures is discussed.

The Subcapital Femoral Neck Stress Fracture: A Novel Subtype of Compression-sided Stress Fracture-A Report of Three Cases.

Femoral neck stress fractures (FNSFs) are increasingly common, particularly in military training. The usual mode of classifying these injuries is based on the involvement of the compression or tension side of the femoral neck; however, this may oversimplify and fail to address factors such as the orientation of the fracture line. We present a novel subtype of a compression-sided FNSF affecting the subcapital femoral neck and report the treatment outcomes in a military trainee population. A retrospective analysis of patients with a subcapital, compression-sided FNSF was identified from a single

[Stress reactions and stress fractures in the high performance athlete. Causes, diagnosis and therapy].

From 1987 until July 1991 70 athletes with stress reactions or stress fractures were treated in the orthopaedic department of the Hannover Medical School. The average age of the 42 male and 28 female athletes was 22.6 years. The number of athletes involved in track and field sports was 29 (41.4%), in gymnastics 9 (12.9%) and in soccer 5 (7.1%). The most common bone injured was the tibia in 29 (41.4%), followed by the tarsal navicular in 21 (30.0%), the midfoot in 17 (24.3%) and the fibula in 4 (5.7%) athletes. In three cases double stress fractures were found in adjacent locations; in one case

Bilateral Medial Tibial Plateau Stress Fractures: A Case Report.

Medial tibial plateau stress fractures are rare and often misdiagnosed due to nonspecific symptoms and initially normal radiographs. This case report describes a rare instance of bilateral medial tibial plateau stress fractures in a 43-year-old overweight male police officer with comorbidities including diabetes. The patient developed progressive knee pain after abruptly increasing his running activity. Initial radiographs were inconclusive, but magnetic resonance imaging (MRI) revealed bone marrow edema and non-displaced fractures, highlighting the importance of advanced imaging for early dia

Femoral Neck Stress Fractures in Athletes and the Military.

➤: Femoral neck stress fractures (FNSFs) are an infrequent condition in athletic and military populations. ➤: A high index of suspicion with liberal use of magnetic resonance imaging (MRI) is vital for early recognition and treatment initiation. ➤: An associated hip effusion on MRI is a risk factor for an evolving stress injury and requires close assessment and consideration for repeat MRI. ➤: Stress reactions and stable, incomplete FNSFs (<50% of femoral neck width) can be treated nonsurgically. ➤: Surgical intervention is accepted for high-risk, incomplete (≥50% of femoral neck width), a

Multiple stress fractures of unilateral femur: A case report.

BACKGROUND: Stress fractures of the femoral neck are not common in clinical practice, and simultaneous stress fractures of the femoral neck and proximal femur of the unilateral femur are even more rare. We introduce a case of this type of fracture that was treated in our department, analyze the causes, and review similar stress fractures reported in the literature to provide references for the diagnosis and treatment of such conditions. CASE SUMMARY: A 62-year-old female, with a free medical history, was admitted to the hospital mainly due to pain in the right hip and worsening pain in the ri

Treatment of Navicular Stress Fractures With an Algorithmic Approach.

BACKGROUND: Navicular stress fractures are becoming increasingly more common. There is no universal consensus on treatment. We provide an algorithm that we feel will be useful in determining treatment. METHODS: A retrospective study was performed on all patients having operative treatment of navicular stress fractures during a 10-year period. Acute fractures were treated with open reduction internal fixation. Chronic fractures greater than 3 months were treated with open reduction and internal fixation (ORIF) and iliac crest bone grafting. Chronic fractures with evidence of sclerosis, avascul

Surgical repair of lumbar stress fractures in professional cricketers .

BACKGROUND: Professional cricket fast bowlers sustain high rates of lumbar stress fractures (spondylolysis). Limited research exists around the success of surgical repair when these injuries fail conservative treatment. We present an ambispective cohort study of spondylolysis surgical repair in a consecutive group of multi-national professional cricket fast bowler using a technique not previously reported in this unique sporting group. METHODS: Between 2004 and 2019, a consecutive series of male professional fast bowlers with lumbar spondylolysis who had repeatedly failed conservative treatme

The pathophysiology, diagnosis, and management of foot stress fractures.

INTRODUCTION: There is an increasing prevalence of osteoporosis, and with it a rise in the diagnosis of stress fractures. Postmenopausal women are particularly at risk of stress fractures. This review article describes the pathophysiology of foot stress fractures and the latest diagnostic and treatment strategies for these common injuries. DISCUSSION: There are numerous risk factors for stress fractures that have been identified in the literature. Reduced bone mineral density is an independent risk factor for delayed union. Prevention of stress fractures with training periodization and nutrit

The classification systems of stress fractures: a systematic review.

BACKGROUND: Stress fractures of the upper and lower extremity are troublesome overuse injuries in athletes and nonathletes alike. These injuries have a broad spectrum of severity and prognosis. We performed a systematic search of the literature, which revealed multiple classification systems; however, we did not uncover a general system that offered both validated radiographic and clinical parameters. METHODS: A literature search was conducted using Ovid/Medline, Embase, and the Cochrane Library, with publication dates ranging from 1960 to December 2009. Inclusion criteria included all studie

Imaging strategies for diagnosing calcaneal and cuboid stress fractures.

Stress fractures of the calcaneus and cuboid bones are not among the most common pathologies seen by the foot and ankle specialist on a day-to-day basis. Therefore, the clinician must be suspicious of these particular entities when conservative therapy has failed to alleviate the pain of a more common condition. The purpose of this article is to discuss the signs, symptoms, MRI findings, nuclear imaging characteristics, and radiographic features associated with calcaneal and cuboid stress fractures. Case studies of both conditions are also discussed within this article.

Stress examination of supination external rotation-type fibular fractures.

BACKGROUND: Deltoid incompetence in association with an isolated fibular fracture is assumed to be present if there is medial tenderness, ecchymosis, or substantial swelling. We sought to determine whether these soft-tissue indicators predict deltoid incompetence by comparing such findings with the findings on stress radiographs. METHODS: Over a thirty-two-month period, 138 patients who presented acutely with a Weber type-B supination-external rotation (SE) fibular fracture were evaluated for tenderness (in nine locations), ecchymosis, and swelling. Patients who presented with an apparently i

Contact stress distributions in malreduced intraarticular distal radius fractures.

Residual articular incongruity of the distal radius following intraarticular fracture has been correlated with early osteoarthritis (OA) and a poor clinical outcome. We developed a simple in vitro fracture model of the distal radius to investigate the relationship between degree of articular incongruity and the resulting distribution of radiocarpal contact stress. Twelve fresh-frozen cadaver arms were dissected, packets of Fuji Pressensor film were inserted into the wrist, and the wrist was loaded through its flexor and extensor tendons. We created a simple intraarticular fracture that allowed

[Stress fractures in athletes and military recruits. An overview].

Stress fracture is an overuse injury caused by muscle forces together with bending and impact forces acting on the bone, which has not adapted to the loading. The localization of stress fractures is more common in lower extremities. They are found in many other bones of the body as well. In the history of these patients a considerable amount of running exercise is usually found. The symptoms are stress pain and aching at rest after training. Typical findings are local palpation pain and edema. Sometimes tender resistance is felt. Clinically used radiological imaging methods are radiographs, sc

Stress fractures of the ribs in amateur golf players.

BACKGROUND: Stress fractures of the ribs are sometimes seen at the Outpatient Department in patients with a history of playing golf enthusiastically. Many are diagnosed as "muscle strain" or "myofascial pain" and patients are simply advised to take some rest or are treated with analgesics and local injection. This case study investigated 11 amateur golfers whose chief complaint was anterior, posterior or lateral chest pain. After X-ray and bone scan evaluation, "stress fracture of the ribs" was diagnosed. A questionnaire presented to them trying to find the possible mechanisms of these stress

Stress fractures associated with osteosarcoma of the lower limb.

Three patients with osteosarcoma of the femur developed abnormal radiopharmaceutical uptake in the bones of the contralateral leg. This uptake was not due to metastases. The histology in one patient, the form of the lesion and the disappearance of the abnormal uptake without treatment in the other two, indicated that the uptake was probably due to stress fractures. Changes in weight bearing and walking in the normal leg as a result of the osteosarcoma in the other leg could have been the cause of the stress fractures. It should be recognized that new abnormal uptake on bone scintigraphy in pat

Lesser metatarsal stress fractures: a study of 37 cases.

A study of lower-extremity stress fractures is presented with emphasis placed on metatarsal stress fractures. The metatarsals were found to be the second most common site in the study. The study documents not only the number of metatarsal stress fractures found, but also the location within the affected metatarsal. Because stress fractures are a common problem, especially for the military recruit or athlete, metatarsal stress fractures can represent a common cause of metatarsalgia.

Stress fractures in athletes. A study of 320 cases.

We analyzed cases of 320 athletes with bone scan-positive stress fractures (M = 145, F = 175) seen over 3.5 years and assessed the results of conservative management. The most common bone injured was the tibia (49.1%), followed by the tarsals (25.3%), metatarsals (8.8%), femur (7.2%), fibula (6.6%), pelvis (1.6%), sesamoids (0.9%), and spine (0.6%). Stress fractures were bilateral in 16.6% of cases. A significant age difference among the sites was found, with femoral and tarsal stress fractures occurring in the oldest, and fibular and tibial stress fractures in the youngest. Running was the mo
