The length of training pilot: does anyone really know what time it takes?

With funding from the American Board of Family Medicine, the Review Committee for Family Medicine of the Accreditation Council for Graduate Medical Education has undertaken a project to examine the length of training for family medicine residents. This project comes at a time when concerns are being raised regarding how well family medicine residents are being prepared for independent practice, especially in view of the changing health care environment. The declining performance of recent graduates on the American Board of Family Medicine certification examination and reports of narrowing of t

Financing assistive technology-Policy options on the path towards universal health coverage.

Supporting functional ability in everyday life is essential for advancing toward universal health coverage (UHC). However, of the estimated 2.5 billion people in need of assistive technology (AT) globally, the vast majority do not have access. The main reason for this is financial barriers, particularly in low- and middle-income countries. This study aims to identify financing policy options for increasing access to affordable AT. The UHC concept, more specifically the three coverage dimensions-population, interventions (products and services), and costs, is used to descriptively analyze gaps

Cardiac surgical missions: what works, what does not, where we need to go from here.

PURPOSE OF REVIEW: Although overall neonatal and infant mortality continues to decrease around the world, years of life lost to congenital heart disease (CHD) rank among the top 10 leading causes for that age group in Latin America, Central Asia, Africa and the Middle East. Short-term surgical missions (STSMs) continue to be the only resource available in some areas of the world. RECENT FINDINGS: There is a need for better communications between charity organizations and the supported institutions. We observe a raising concern with the limited published information on the results of the STSMs

Do investments in cancer registry databases and tools bring added value? NORDCAN as an example.

BACKGROUND: In the 1990s, the large-scale collaboration Kreftbildet i Norden (KIN) drew attention to the need for timely cancer statistics for cancer control planning in the Nordic countries. Supported by the Nordic Cancer Union (NCU), a web-based version of NORDCAN was continually developed by the Association of Nordic Cancer Registries (ANCR) from 2003, with website support and hosting by the International Agency for Research on Cancer (IARC). Despite empirical evidence of its global reach, the question of whether recurrent investment in NORDCAN brings added value was raised; we sought to fo

National and international policies to mitigate disease threats.

To devise and implement effective health policy, we must define the problem, choose the tools, craft the policy, build consensus, set goals and deadlines, raise funds and take action. Success or failure depends on the perception of risk, the strength of the underlying science, the efficacy of the technology, ownership and intellectual property, the conflict between individual and public health, the choice of weaker (guidelines) and stronger (law) policy instruments, the level of public interest, political opportunity, institutional inertia, mechanisms for enforcement and who foots the bill. Outreach workers' experiences in a homeless outreach project: issues of boundaries, ethics and staff safety.

Mental health professionals and researchers have emphasized the importance of conducting outreach to locate homeless persons with mental illness, and of creatively engaging these persons into a therapeutic relationship. These outreach and engagement activities raise challenging issues in the areas of client-staff boundaries, professional ethics, and staff safety. While several issues in each of these three key areas have received attention in the growing literature on homelessness, certain issues within each area remain unexplored. The authors draw from the street experiences of outreach staff

Surgically reducing displaced distal radial fractures in children.

Displaced fractures of the distal radius in children are usually reduced under sedation or general anaesthesia to restore anatomical alignment before the limb is immobilized. However, there is growing evidence of the ability of the distal radius to remodel rapidly, raising doubts over the benefit to these children of restoring alignment. There is now clinical equipoise concerning whether or not young children with displaced distal radial fractures benefit from reduction, as they have the greatest ability to remodel. The Children's Radius Acute Fracture Fixation Trial (CRAFFT), funded by the Na

Community Perceptions of Executive Orders Impacting Lung Cancer Care, Research, and Mental Health.

OBJECTIVES: In January 2025, federal executive orders introduced sweeping changes to healthcare, public health infrastructure, and research funding. These shifts raised concern within the lung cancer (LC) community, which depends on early detection, timely care, and continued innovation. This study examined community perceptions of these policy changes. METHODS: We conducted a mixed-methods national survey (March 2025) guided by the Social Ecological Model. A 12-item online survey collected responses from individuals with personal or professional ties to LC. Descriptive statistics and inducti

Rising rates of vaccine exemptions: problems with current policy and more promising remedies.

Parents of school-age children are increasingly claiming nonmedical exemptions to refuse vaccinations required for school entry. The resultant unvaccinated pockets in many areas of the country have been linked with outbreaks of vaccine-preventable diseases. Many states are now focused on reducing rates of nonmedical exemptions by making exemption processes more restrictive or burdensome for the exemptor. These strategies, however, pose ethical problems and may ultimately be inadequate. A shift to strategies that raise the financial liabilities of exemptors may lead to better success and prove

SUDEP risk is influenced by longevity genomics: a polygenic risk score study.

BACKGROUND: Sudden Unexpected Death in Epilepsy (SUDEP) is a rare and tragic outcome in epilepsy, identified by those with the condition as their most serious concern. Although several clinical factors are associated with elevated SUDEP risk, mechanisms underlying SUDEP are poorly understood, making individual risk prediction challenging, especially early in the disease course. We hypothesised that common genetic variation contributes to SUDEP risk. METHODS: Genetic data from people who had succumbed to SUDEP was compared to data from people with epilepsy who had not succumbed to SUDEP and fr

Paramedics in general practice in Cornwall: what roles and what work do they undertake?

INTRODUCTION: General practice faces increasing pressure, especially in deprived rural areas (Laccobucci, 2019). The Additional Roles Reimbursement Scheme (ARRS) was introduced to employ extra health professionals, including paramedics, to support the delivery of general practice services. In Cornwall, over 60% of general practices employ paramedics, as identified in a preliminary search of general practice websites by the author. However, it was unclear what roles and tasks they undertook and how they were funded. This study's aim, the first to focus on a specific rural area in the UK, was to

Strategic analysis of financing communicable diseases in Afghanistan.

BACKGROUND: Afghanistan as a low-income country suffers from the heavy burden of Communicable Diseases (CDs) and their significant economic consequences. Therefore, this research is aimed to strategically analyze the financing system of CDs management in Afghanistan and provide effective solutions. METHODS: We conducted the qualitative research using interpretative phenomenological analysis (IPA). An interview guide was used to conduct the semi- structured interviews with 49 experts from the Afghanistan health system. We used the framework analysis method to analyze the results. RESULTS: In

Selective Reporting of Outcomes in Tinnitus Trials: Comparison of Trial Registries With Corresponding Publications.

Objectives: We aimed to study the prevalence of selective reporting of primary and secondary outcomes in tinnitus trials and to examine if selective reporting of outcome measures is influenced by the nature and direction of its results. Background: Selective reporting of outcome measures has been reported in several biomedical fields and can influence the clinical usefulness and implementation of outcomes of clinical trials. It is reported as one of the obstacles in finding an effective intervention for tinnitus. Methods: ClinicalTrials.gov (CT.gov) was used to identify all registered interven

A Comparison of Dependence across Different Types of Nicotine Containing Products and Coffee.

INTRODUCTION: Few studies have compared the dependence to different tobacco and nicotine products. Even less is known about how it relates to dependence on other common drugs, e.g., caffeine. In this study degree of dependence was compared between snus, cigarettes, nicotine replacement (NR), electronic cigarettes and coffee. METHODS: A random sample of Swedish citizens belonging to an internet panel were contacted from September to October 2017. The responders were asked among other related things about their use of snus, NR, traditional cigarette or e-cigarette use and coffee consumption. Th

The state of UK palliative care - time for change at scale and pace.

After 60 years of development, palliative care services are fully embedded in local communities, raising awareness, delivering care and often also relying on local fundraising. Specialist palliative care teams are integrated within primary and community care services, as well as acute hospitals. Palliative care is shown to be cost effective, improving patient experience and reducing acute hospital bed days, saving money for the health service as long as patients access services early enough to make a difference. Unfortunately across the country, services are patchy with significant inequalitie

Building Bridges: A framework for redefining and decolonizing implementation science in low- and middle-income countries.

Implementation science (IS) is gaining recognition for its role in enhancing health outcomes by connecting research evidence with practical application. However, most IS frameworks have been developed in high-income countries (HICs), raising concerns about their applicability, fairness, and sustainability in low- and middle-income countries (LMICs). In LMICs, which are home to over 75% of the global population, implementation challenges are influenced by unique social, cultural, geopolitical, and health system factors. This context reflects both opportunities and disparities in current IS mode

A PLAN to address the Parkinson pandemic.

The Parkinson pandemic continues to spread. Almost 12 million individuals now have the disease, nearly double the estimate from just six years ago. Its human-made nature is also increasingly clear as more studies tie environmental toxicants to the disease. Chief among these are certain pesticides, the dry-cleaning chemicals trichloroethylene and perchloroethylene, and air pollution. An etiological role for these toxicants-inhaled or ingested-is also consistent with the emerging brain- and body-first models of Parkinson's disease. To address the pandemic will require a "PLAN" that (1) Prevents t

A collaborative genetic carrier screening model for the British Ashkenazi Jewish community.

We present a unique model of a British genetic carrier screening programme for individuals with Ashkenazi Jewish ancestry that exemplifies a partnership between a publicly funded healthcare service (the NHS) and a charity, Jnetics. This model provides affordable access to carrier screening for severe autosomal recessive diseases increased in this community. Prior to the development of this programme, the British healthcare system only provided Tay Sachs' screening for this community, leaving them at higher risk of having a child with a serious autosomal recessive disease. The Jnetics screening
