Complications and reoperation after Nissen fundoplication in childhood.

Over a 10 year period, 429 Nissen fundoplications were performed on children with gastroesophageal reflux. Postoperative complications occurred in 69 children (16 percent), including wrap herniation or breakdown in 29; postoperative bowel obstruction in 18; stricture in 10; intraabdominal abscess and enterocutaneous fistula in 3 patients each; and wound infection, wound dehiscence, and inadvertent splenectomy in 2 patients each. The postoperative mortality rate was 0.9 percent (4 of 429 patients) and was related to sepsis in 1 patient, a metabolic disorder in 1 patient, and underlying pulmonar

Danger of fundoplication after selective vagotomy and antrectomy.

Antireflux procedures are required in some patients at the same time or later after operations for chronic duodenal ulcer. The consequences to gastric blood supply are different between the three vagotomies usually performed to treat duodenal ulcer. A serious ischemic complication, incurred when a patient underwent fundoplication several years after a selective vagotomy and antrectomy, is reported to emphasize that the stomach relies on greater curvature arterial blood supply after this operation. Additional arteries ligated during fundoplication may surpass the capacity of remaining gastric a

It Is a Wrap! Or Is It?: The Role of Fundoplication in Infant Feeding Intolerance.

Purpose: Fundoplication is one of the most common procedures performed by pediatric surgeons, frequently for gastroesophageal reflux with feeding intolerance. No consensus exists in its management, with multiple institutions opting for medical therapy over surgical intervention. Methods: A case-based survey was administered at a national pediatric surgery conference. Clinical vignettes described former-premature infants with reflux and feeding intolerance with or without failure to thrive (FTT), neurological impairment, complex cardiopathy, and respiratory symptoms. Odds ratios (ORs) for fundo

Nissen fundoplication in the treatment of children with familial dysautonomia.

Thirty-four children with familial dysautonomia (FD) underwent Nissen fundoplication and gastrostomy. The indications for operation were persistent cyclic vomiting that resulted in repeated aspiration pneumonia (94% of the patients), chronic dehydration (82%), failure to thrive (97%), and frequent hospitalizations (76%). There was no operative or early postoperative mortality. Long-term follow-up for up to 12 years was available. Eight patients died during this period from 7 months to 7.5 years postoperatively. In 5 patients (15% of the operated patients), the fundoplication ceased to function

Transabdominal esophagomyotomy and partial fundoplication for treatment of achalasia.

The most common surgical treatment for achalasia is a modified anterior extramucosal esophagomyotomy. Unfortunately, a poor outcome may result secondary to recurrent dysphagia or gastroesophageal reflux. The reported incidence of reflux is 4 to 50 per cent. Our treatment for achalasia is an esophagomyotomy carried onto the cardia combined with a partial gastric fundoplication. Of 22 patients who presented with achalasia and moderate to severe symptoms of dysphagia and odynophagia, 19 had this procedure performed. Of these 19 patients, only two required a second procedure (postoperative dilatat

Herniation of the gastric wall through a Nissen fundoplication.

Two patients presented several years after Nissen fundoplication with dysphagia and/or hemorrhage. In both, the fundoplication was intact in the original relationship to the cardia but herniated gastric wall was found between the fundic wrap and the distal end of the esophagus. The diagnosis and alternatives for preventing this problem are presented.

A modified fundoplication which preserves the ability to belch.

This report describes a transabdominal antireflux procedure which combines some of the features of the operations described by Nisse, Belsey, and Hill. From 1972 through 1976, 61 patients underwent a conventional Nissen fundoplication and 54 patients were operated according to the method described in this report. None of the patients who had a Nissen operation was able to belch freely; 15% had a "gas-bloat syndrome". Return of the ability to belch often signified partial or complete brakdown of the plication, which occurred in 11% of the cases. By contrast, all but one of the patients who had

The Value of Fundoplication in the Treatment of Type I Achalasia.

BACKGROUND: Type I achalasia comprises 20% of achalasia and has nearly absent esophageal motor activity. Concerns that fundoplication decreases the effectiveness of Heller myotomy in these patients has increased adoption of peroral endoscopic myotomy (POEM). Hence, we compared outcomes after Heller myotomy with Dor fundoplication vs POEM. METHODS: From 2005 to 2020, 150 patients with type I achalasia underwent primary surgical myotomy (117 Heller myotomy, 33 POEM). Patient demographics, prior treatments, timed barium esophagrams, Eckardt scores, and reinterventions were assessed between the 2

Influence of Wrap Fixation Technique on the Results of Fundoplication.

BACKGROUND: Different techniques of wrap fixation in laparoscopic Nissen fundoplication (LNF) have been proposed with of the aim to reduce the complications, but the optimal technique is yet to be determined. The aim of our prospective study was to evaluate several techniques of wrap fixation and determine whether the application of a combined approach to perform wrap fixation reduces the failure rate in short-term and long-term follow-up. MATERIALS AND METHODS: One hundred two patients with sliding or paraesophageal hiatal hernia (type I or type II), who underwent antireflux surgery were ran

[Robot-Assisted Repeated Fundoplication in Children and Adolescents].

BACKGROUND: Recurrent gastroesophageal reflux symptoms in adolescents and young adults who underwent fundoplication in childhood present a technical challenge for the surgeon. The distal oesophagus and hiatus are difficult to access by laparotomy, the anatomy is modified by the primary procedure, and there are often dense adhesions between important structures. Robot-assisted surgery may be advantageous, because of the three dimensional (3D) view and the multiaxial freedom of movement afforded by the system. PURPOSE: To describe our experience with robot-assisted Nissen re-fundoplication in a

Ineffective esophageal motility is not a contraindication to total fundoplication.

INTRODUCTION: Ineffective esophageal motility (IEM) is a physiologic diagnosis and is a component of the Chicago Classification. It has a strong association with gastroesophageal reflux and may be found during work-up for anti-reflux surgery. IEM implies a higher risk of post-op dysphagia if a total fundoplication is done. We hypothesized that IEM is not predictive of dysphagia following fundoplication and that it is safe to perform total fundoplication in appropriately selected patients. METHODS: Retrospective chart review of patients who underwent total fundoplication between September 2012

Laparoscopic Heller Myotomy and Fundoplication: What Is the Evidence?

There is no agreement about the best type of fundoplication to add in patients undergoing laparoscopic Heller myotomy (LHM) for achalasia to reduce the risk of postoperative gastroesophageal reflux. This article reviews the current evidence about the outcomes in achalasia patients undergoing LHM with a partial anterior, a partial posterior, or a total fundoplication. We performed a review of the literature in PubMed/Medline electronic databases, which was evaluated according to the GRADE system. The results of the published randomized controlled trials show with a high level of evidence that t

Alleviation of retching and feeding intolerance after fundoplication.

BACKGROUND: Although surgical intervention for gastroesophageal reflux disease (GERD) in children offers the strongest control of GERD, these results are tempered by postoperative problems such as retching, bloat, dysphagia, dumping syndrome, and postprandial hypoglycemia. We created a specialty clinic and an algorithmic approach to ameliorating these problems. Here, we present our experience with the first 60 patients. METHODS: Patients referred to the clinic for feeding problems after fundoplication were tracked as part of quality improvement monitoring. Patients were treated according to a

[The Influence of Laparoscopic Fundoplication on Reflux-Associated Cough].

Introduction: The gastrooesophageal reflux disease (GERD) is a possible cause of chronic cough. The laparoscopic fundoplication is well established in the treatment of GERD. In a retrospective study, the effectivity of this operation on the GERD associated cough was examined and possible preoperative predictive factors concerning the post-surgical therapy effect were characterized. Patients and Methods: 85 patients after laparoscopic fundoplication due to GERD treated with proton pump inhibitors without (RS-H: n = 31) or with associated cough (RS+H: n = 54) were evaluated in a three-month foll

Failed Nissen fundoplication in children: causes and management.

INTRODUCTION: Nissen fundoplication (NF) is the gold standard procedure for the treatment of gastroesophageal reflux (GER) in children. However, it fails in a significant proportion of patients. The purpose of this study was to identify preoperative predictors of failure and to examine the results of reoperations. PATIENTS AND METHODS: The charts of patients who underwent NF at our institution between 1992 and 2011 were retrospectively reviewed. Surgery was indicated in patients with symptomatic GER in whom medical treatment failed, particularly, in cases of esophageal atresia (EA), congenita
