Gait abnormalities in children with FOXP1 syndrome: A case series.

FOXP1 syndrome is a neurodevelopmental disorder caused by mutations or deletions in the Forkhead Box Protein 1 (FOXP1) gene. It is characterized by intellectual disabilities, language difficulties, autism spectrum disorder, congenital anomalies and motor impairments.Walking difficulties have been reported, but specific gait impairments have not previously been described. In this case series, specific gait abnormalities, and how they were managed, are reported in three children with FOXP1 syndrome. The most prominent clinical abnormalities in their gait and gait analysis were toe walking with i

The inertial-based gait normalcy index of dual task cost during turning quantifies gait automaticity improvement in early-stage Parkinson's rehabilitation.

BACKGROUND: The loss of gait automaticity is a key cause of motor deficits in Parkinson's disease (PD) patients, even at the early stage of the disease. Action observation training (AOT) shows promise in enhancing gait automaticity. However, effective assessment methods are lacking. We aimed to propose a novel gait normalcy index based on dual task cost (NIDTC) and evaluate its validity and responsiveness for early-stage PD rehabilitation. METHODS: Thirty early-stage PD patients were recruited and randomly assigned to the AOT or active control (CON) group. The proposed NIDTC during straight w

Investigating gait-responsive somatosensory cueing from a wearable device to improve walking in Parkinson's disease.

Freezing-of-gait (FOG) and impaired walking are common features of Parkinson's disease (PD). Provision of external stimuli (cueing) can improve gait, however, many cueing methods are simplistic, increase task loading or have limited utility in a real-world setting. Closed-loop (automated) somatosensory cueing systems have the potential to deliver personalised, discrete cues at the appropriate time, without requiring user input. Further development of cue delivery methods and FOG-detection are required to achieve this. In this feasibility study, we aimed to test if FOG-initiated vibration cues

A study of brain networks associated with Freezing of gait in Parkinson's disease using transfer entropy analysis.

BACKGROUND: Parkinson's disease (PD) is a common neurodegenerative disease in the elderly. Freezing of Gait (FOG) is one of the common motor symptoms of PD, but the potential mechanism remains unclear. This study aimed to investigate the changes of brain functional network topology in PD patients with FOG. METHODS: The resting electroencephalogram (EEG) were acquired from15 PD patients with FOG (PD-FOG), 13 PD patients without FOG (PD-nFOG), and 16 healthy control (HC). Cognitive and motor functions were assessed using subjective scales. The whole-brain functional networks were constructed ba

Added forearm weights for gait pattern normalization in patients with Parkinson's disease.

Patients with Parkinson's Disease presented gait impairment. Applying additional weights to enhancing sensory input may improve gait impairment. We assumed that gait impairment could be improved when patients walked with additional forearm weights, and the gait improvement was associated with clinical characteristic of Parkinson's Disease. Thirty patients with Parkinson's Disease and 30 age-sex matched controls were recruited. Spatiotemporal and joint kinematics parameters were evaluated by a three-dimensional motion capture system in normal walking and walking with sandbags, respectively. The

Freezing of Gait in Parkinson's Disease: Implications for Dual-Task Walking.

BACKGROUND: The simultaneous completion of multiple tasks (dual-tasking, DT) often leads to poorer task performance (DT cost, DTC). People with Parkinson's disease (PwPD) exhibit difficulty with DT, and DTC may be particularly pronounced in PwPD with freezing of gait (FOG). OBJECTIVE: This study assessed the relationship between FOG status and DTC during gait. METHODS: Gait parameters were collected using inertial sensors in 106 PwPD (off-medication), including definite-freezers (dFOG; n = 25), possible-freezers (pFOG; n = 16), and non-freezers (nFOG; n = 65) during single (ST)-and DT walkin

Dynamic evaluation of spine kinematics in individuals with Parkinson's disease and freezing of gait.

BACKGROUND: Freezing of gait (FoG) is an episodic failure of gait exposing people with Parkinson's disease (PD) to a high risk of falling. Despite growing evidence of the interconnection between impaired trunk control and FoG, a detailed description of spinal kinematics during walking is still lacking in this population. RESEARCH QUESTION: Do spinal alterations impact gait performance in individuals with PD and FoG? METHODS: We analyzed kinematic data of 47 PD participants suffering (PD-FOG, N = 24) or not suffering from FoG (PD-NFOG, N = 23) and 15 healthy controls (HCO) during quiet standi

Brain activity response to cues during gait in Parkinson's disease: A study protocol.

Various cueing strategies (internal and external) have been used to alleviate gait deficits in Parkinson's disease (PD). However, it remains unclear which type of cueing strategy is most effective at different disease stages or with more severe walking impairment, such as freezing of gait (FOG). The underlying neural mechanisms of response to cueing are also unknown. This trial aims to: (i) determine brain activity response to cue stimulus (internal, visual, auditory or tactile) when walking in PD and; (ii) examine changes in brain activity to cues at different stages of PD. This ongoing singl

Development of a Prototype Overground Pelvic Obliquity Support Robot for Rehabilitation of Hemiplegia Gait.

In this work, we present the overground prototype gait-rehabilitation robot for using motion assistance and training for paralyzed patients. In contrast to the existing gait-rehabilitation robots, which focus on the sagittal plane motion of the hip and knee, we aim to develop a mobile-based pelvic support gait-rehabilitation system that includes a pelvic obliquity support mechanism and a lower-limb exoskeleton. To achieve this, a scissor mechanism is proposed to generate the paralyzed patient's pelvic obliquity motion and weight support. Moreover, the lower limb exoskeleton robot is integrated

Impact of motor subtype on non-motor symptoms and fall-related features in patients with early Parkinson's disease.

AIM: Clinical implications for motor phenotypes of Parkinson's disease (PD) remain to be further elucidated, particularly at the early stages of the disease. We aimed to compare the non-motor and fall-related features between tremor-dominant (TD) and postural instability-gait difficulty (PIGD) subtypes in patients with early PD. METHODS: PD was categorized into TD, intermediate and PIGD types, according to the literature. Not only motor symptoms, but also non-motor symptoms for global cognition, depression, anxiety, fatigue and dysautonomia, were measured in detail. In addition, fall-related

Surrogates for rigidity and PIGD MDS-UPDRS subscores using wearable sensors.

BACKGROUND: Telemedicine has the advantage of expanding access to care for patients with Parkinson's Disease (PD). However, rigidity and postural instability in PD are difficult to measure remotely, and are important measures of functional impairment and fall risk. RESEARCH QUESTION: Can measures from wearable sensors be used as future surrogates for the MDS-UPDRS rigidity and Postural Instability and Gait Difficulty (PIGD) subscores? METHODS: Thirty-one individuals with mild to moderate PD wore 3 inertial sensors at home for one week to measure quantity and quality of gait and turning in da

Pelvis-Toe Distance: 3-Dimensional Gait Characteristics of Functional Limb Shortening in Hemiparetic Stroke.

We aimed to investigate whether a newly defined distance in the lower limb can capture the characteristics of hemiplegic gait compared to healthy controls. Three-dimensional gait analyses were performed on 42 patients with chronic stroke and 10 age-matched controls. Pelvis-toe distance (PTD) was calculated as the absolute distance between an anterior superior iliac spine marker and a toe marker during gait normalized by PTD in the bipedal stance. The shortening peak during the swing phase was then quantified as PTDmin. The sagittal clearance angle, the frontal compensatory angle, gait speed, a

Gait deviations in adolescent patients with Wilson disease and their possible connections with biochemical markers: preliminary results.

UNLABELLED: The study aims to evaluate the gait pattern of Wilson Disease adolescents, receiving pharmacological treatment, without clinical signs of gait problems, with instrumented gait analysis. The gait changes could be the first signs of neurological manifestations, providing information about which patients are at greater risk of developing neurological problems. Another aim of the paper was to find possible relationships between gait disturbances and biochemical markers. Twenty-seven patients underwent gait analysis with the VICON system. Captured data were processed and averaged. From
