[Helicobacter pylori infection in gastric remnant cancer after gastrectomy].

Patients who have undergone distal gastrectomy for peptic ulcer are at higher risk of developing gastric remnant cancer, and chronic bile reflux is believed to increase the risk of cancer in remnant stomach. In remnant stomach, carcinogenesis may be prevented by selecting the anastomosis method with a few reflux of intestinal juice including a bile acid. How Helicobacter pylori(H. pylori) infection participate in stomal gastritis and gastric remnant cancer, same as early gastric cancer in the intact stomach, is attended. H. pylori positive rate of remnant stomach is different by examination me

[Treatment of "difficult" duodenal stump].

The Plenk method in the author's modification was successfully applied in resection of the stomach "for exclusion" in 22 patients. When there was a threatening incompetence of the duodenal stump sutures, to prevent generalization of peritonitis the stump in 24 patients was covered with a "hood" of the greater omentum and external drainage of this space was installed; the procedure was an alternative of duodenostomy. Incompetence occurred in 3 cases with subsequent recovery. In resection of the stomach for recurrent ulcer after previous vagotomy combined with gastroduodenostomy after Jaboulay,

[Robot-assisted Minimally Invasive Oesophagectomy - Surgical Variants of Intrathoracic Circular Stapled Oesophagogastric Anastomosis].

INTRODUCTION: Anastomotic insufficiency after oesophagectomy contributes significantly to morbidity and mortality of affected patients. A safe surgical technique can reduce the incidence of such anastomotic insufficiencies. INDICATION: In the treatment of oesophageal cancer, the German guideline recommends minimally invasive or hybrid surgical procedures. In most cases, Ivor-Lewis oesophagectomy and continuity reconstruction using a gastric sleeve are performed. Circular stapler anastomosis seems to be superior. METHOD: The preparation of the anastomosis starts intra-abdominally with mobilis

Indications for combining remnant resection in a primary Roux-N-Y gastric bypass: Preliminary results.

INTRODUCTION: Gastric intestinal metaplasia (GIM) is a precancerous lesion that has been independently associated with gastric cancer development. The presence of GIM in the preoperative study of Roux-n-Y gastric bypass (RYGB) raises question of whether or not remove the gastric remnant (GR) due to the impossibility of endoscopic surveillance. METHODS: Descriptive analysis of 6 consecutive cases of GR resection in RYGB from 2012 to 2021. RESULTS: There are six cases (100 % women) with an age of 61 (41-63) years and pre-surgical BMI of 40.5(34.8-43.3) Kg/m2. Endoscopy confirmed the presence

Inhibitory remnant stomach motility by duodenal distention after B-I gastrectomy.

BACKGROUND/AIMS: Although control of gastric motility is a highly regulated process, B-I gastrectomy causes accelerated gastric emptying. We found few reports so far concerned with the mechanical effect of balloon distention of the duodenum on the remnant stomach in B-I dogs. METHODOLOGY: Four weeks after conventional distal gastrectomy with B-I reconstruction procedure, a balloon was inserted into the duodenum, and strain gauge force transducers (SGTs) were sutured onto the wall of the gastric remnant and duodenal serosa in beagles. After a two-week postoperative recovery period, gastro-duod

Laparoscopic Radical Gastrectomy for Remnant Gastric Cancer.

Remnant gastric cancer (RGC) is a serious long-term complication following gastrectomy, for which radical resection remains the standard treatment. With continued advancements in laparoscopic techniques, laparoscopic radical gastrectomy (LRG) has emerged as an important therapeutic option for early-stage and select advanced cases of RGC. This procedure offers reduced surgical trauma, faster postoperative recovery, and a lower incidence of complications. However, several challenges remain in its clinical application. On the one hand, postoperative anatomical changes and intra-abdominal adhesion

An outermost-layer-oriented medial approach to facilitate margin-negative distal transection in gastric cancer with duodenal invasion: a single-centre retrospective cohort study.

BACKGROUND: Gastric cancer with duodenal invasion (Duo+) presents a distinct surgical challenge because adequate distal-margin clearance must be achieved without compromising duodenal stump safety. Standardised laparoscopic strategies for this setting remain poorly defined. METHODS: In this single-centre retrospective cohort study, we evaluated the technical feasibility and observed outcomes of an integrated extended-mobilisation and margin-control strategy in 500 patients undergoing laparoscopic distal gastrectomy with D2 lymphadenectomy for stage II-III gastric adenocarcinoma. The cohort in

The role of gastric mucosal dysplasia in the development of gastric carcinoma.

It is widely acknowledged that most stomach carcinomas developed from dysplastic lesions of gastric mucosa. They can be found in known precancerous conditions as chronic gastritis, gastric adenoma, giant rugal hypertrophy, chronic peptic ulcer, gastric stump after partial resection and pernicious anemia. Several grading systems of gastric dysplasia have been suggested. Nagayo's or the ISGGC classification was applied to 367 biopsy specimens of 258 patients between 1979-1995. The frequency of moderate and severe dysplasia was 0,84% regarding all gastric endoscopies in the same period of time. F

Unexpected Gastrointestinal Tract injury years following Laparoscopic Adjustable Gastric Banding.

INTRODUCTION: Band migration is a late complication of Laparoscopic Adjustable Gastric Banding insertion, although rare it could be life threatening presenting as peritonitis secondary to gastro-intestinal tract injuries. A case of an unexpected extension of severe gastro-intestinal tract injuries secondary to intra-gastric migration and distal band dislocation is reported. PRESENTATION OF CASE: A 53 years old male, with a history of laparoscopic gastric banding 15 years before and known erosion of the band into the gastric lumen was admitted for abdominal pain and raised serum amylase. Adenocarcinoma developing in de novo Barrett's mucosa in the remnant esophagus after esophagectomy: clinical and molecular assessment.

For many patients after subtotal esophagectomy and gastric pull-up, reflux of gastric contents to the esophageal stump is the leading clinical problem. Besides symptoms such as heartburn and regurgitation, de novo formation of columnar mucosa in the esophageal remnant is a well-known and frequent phenomenon. In this context, the remnant supra-anastomotic esophagus serves as an in vivo model for the study of Barrett's carcinogenesis. We present a retrospective case analysis of a patient who developed de novo Barrett's metaplasia followed by de novo invasive carcinoma 28 months after gastric pul

[Life expectancy after gastric resection for gastroduodenal ulcer].

Retrospective analysis of 1045 patients who had had a Billroth II partial gastric resection for benign gastroduodenal ulcers revealed a significantly shorter life expectancy for them than for a control group matched for age and sex (mean and maximal periods of observation 14.6 and 22 years, median 17.6 years; 15 217 observation years). After resection for duodenal ulcer there were more deaths from bronchial carcinoma, liver cirrhosis or suicide than after resection for gastric ulcer. Gastric stump carcinoma was more frequent in patients with gastric ulcer and was the cause of death in 5.8% of

A life-saving but inadequately discussed procedure: tube duodenostomy. Known and unknown aspects.

OBJECTIVE: The most successful method of managing the difficult duodenum, including the stump leakage, has been the tube duodenostomy technique, but it has not gained wide acceptance and is rarely used. The purpose of this study is to describe the details of the procedure for indication, technical approach, and postoperative care. METHODS: During the period from 1998 to 2006, a tube duodenostomy was performed in 31 patients for possible insecure duodenal stump closure during gastric resection, postoperative duodenal stump leakage, duodenal leak after primary closure of duodenum for perforatio

Epithelium related deposition of activated complement in Helicobacter pylori associated gastritis.

BACKGROUND AND AIMS: It is unknown whether Helicobacter pylori infection activates complement in vivo. Mucosal deposition of various activation products of the complement system may contribute to the pathogenesis of chronic gastritis and was therefore studied by immunohistochemistry. PATIENTS AND METHODS: Ethanol fixed antrum or body gastric tissue sections from 24 patients infected with H pylori (determined by bacterial immunohistochemistry) and 22 uninfected patients were examined by immunofluorescence with monoclonal antibodies to activation neoepitopes in C3b and in the terminal complex (

Spontaneous clearance of Helicobacter pylori after pylorus-preserving gastrectomy for gastric cancer.

Residual mucosa in the gastric stump after pylorus-preserving gastrectomy (PPG) is considered a risk factor for the development of gastric stump carcinoma (GSC). Duodenogastric reflux (DGR) and Helicobacter pylori (H. pylori) infection are suspected to contribute to the development of GSC. The aim of this study was to investigate the prevalence of H. pylori in the residual stomach after PPG for gastric cancer and to assess factors associated with the presence of H. pylori. We investigated 72 patients who had undergone PPG at least 1 year prior to the study and were confirmed to be positive for

[Surgical tactics in peptic ulcer devleoped after gastrectomy].

The authors analyse the materials pertaining to 96 patients with peptic ulcers arising after gastric resection. Eighty seven of them were subjected to 94 surgical interventions. Attention is called to different causes of the ulcer occurrence and possibilities of their recognition. Depending on the cause of ulcer various operations were undertaken i.e. reresection of the stomach, vagotomy, removal of the remained protion of antral duodenal mucosa, total resection of the gastric stump. Indications to each kind of surgery under consideration are discussed.

Function-preserving gastrectomy for early gastric cancer.

The number of early gastric cancer (EGC) cases has been increasing because of improved diagnostic procedures. Applications of function-preserving gastric cancer surgery may therefore also be increasing because of its low incidence of lymph node metastasis, excellent survival rates, and the possibility of less-invasive procedures such as laparoscopic gastrectomy being used in combination. Pylorus-preserving gastrectomy (PPG) with radical lymph node dissection is one such function-preserving procedure that has been applied for EGC, with the indications, limitations, and survival benefits of PPG

Gastroduodenostomy after gastric resection.

Gastroduodenostomy after gastric resection is a procedure which can be readily performed if the short gastric vessels are first divided. It makes for a more physiological restoration of the gastrointestinal tract than is accomplished with gastrojejunostomy and permits proper admixture of the food with the bile and pancreatic enzymes. It avoids several potential dangers such as leakage from the duodenal stump, the possibility of a gastrocolic fistula, and malfunction of the anastomosis due to distortion of a jejunal loop. Apparently the "dumping syndrome" does not occur after gastroduodenostomy

[Roux-en-Y reconstruction with defunctionalized jejunal loop, after gastric resection].

Authors report their personal experience on Roux-en-Y reconstruction, after partial gastric resection. They have considered 39 patients: 24 of them had been operated for a peptic ulcer of the lesser gastric curve and 15 for a pyloric cancer for which a partial gastric resection had been adopted for various reasons (age, stage or else). The follow-up lasted on an average of 42 months (4 to 68). Controls concerned secretory activity and motility. A prolonged stasis of solids, in the gastric stump or in the isolated loop, has been observed in two of the long controlled patients, together with sup

Gastric adenocarcinoma in the excluded stomach after Roux-en-Y gastric bypass: a case series, systematic review, and diagnostic algorithm.

BACKGROUND: Primary remnant gastric cancer after Roux-en-Y gastric bypass presents a diagnostic challenge, and no consensus indications exist to guide the evaluation of the excluded anatomy when malignancy is suspected. METHODS: We present a single-institution case series of 7 patients diagnosed with gastric adenocarcinoma of the excluded stomach after Roux-en-Y gastric bypass; all were diagnosed at an advanced stage within the last 3 years. A literature review was performed, and the combined findings were used to create a novel diagnostic algorithm. RESULTS: A total of 55 patients with remn
