In the United States, four diseases account for the vast majority of cases of lower intestinal bleeding: arteriovenous malformation, diverticulosis, neoplasms, and internal hemorrhoids. In this article the authors discuss less frequent causes of gastrointestinal bleeding. "Common" less frequent causes of gastrointestinal bleeding include solitary rectal ulcer syndrome, colonic varices, mesenteric vascular insufficiency, small bowel diverticula, Meckel's diverticulum, aortoenteric fistula, vasculitis, small intestinal ulceration, endometriosis, radiation-induced injury, and intussusception. INTRODUCTION: Testicular neoplasms metastasizing to the retroperitoneum rarely involve the upper gastrointestinal tract. Gastrointestinal tract metastases usually present with complications including intestinal obstruction, gastrointestinal hemorrhage, and rarely ulceration of the bowel mucosa.
CASE PRESENTATION: We describe an unusual case of duodenal perforation as the presenting manifestation of metastatic classic type seminoma in a 45-year-old man.
CONCLUSION: Germ cell tumor diagnosis should be considered when an ulcerating small bowel mass is identified in a young man.

CONTEXT: Hepatocyte nuclear factor (HNF)-4α is a marker of gastrointestinal tumor differentiation; however, its expression in endocervical tumors remains unclear.
OBJECTIVE: We aimed to assess the utility of HNF4α expression as a marker for endocervical adenocarcinomas (ECAs) and adenocarcinoma in situs (AISs), and to establish a minimal panel for distinguishing them from nonneoplastic endocervical glandular lesions and metastases.
DESIGN: HNF4α expression was analyzed immunohistochemically (positive, H-score ≥10) in 323 tissue samples: 57 endocervical neoplasms including 35 glandular neopla

BACKGROUND: Pancreatic cyst cytology evaluates for neoplastic mucin and epithelial grade. This study describes cytological features of low- and high-grade mucinous neoplasms (MNs) using gastrointestinal contaminants for comparison.
METHODS: Histologically confirmed pancreatic cystic neoplasms were reviewed by a panel of cytopathologists to identify which, among 26 selected cytologic features, correlate significantly with low- and high-grade MN. A test for greater than or equal to four of eight high-grade features (three-dimensional architecture, high nuclear:cytoplasmic ratio, moderate nuclea

PURPOSE: The purpose of the present study was to analyze the enhancement patterns of small bowel neoplasms on post-contrast multiphasic multidetector CT and to assess the diagnostic capacity for differentiating five tumor types.
METHODS: We performed a retrospective study of data on 92 small bowel neoplasms. The neoplasms were categorized into five groups according to pathology findings, not imaging findings (23 adenocarcinomas; 22 lymphomas; 19 metastases; 18 gastrointestinal stromal tumors [GIST]; 10 neuroendocrine tumors [NET]).
RESULTS: GIST and NET demonstrated a hypervascular pattern i

The transcription factor LMO2 is involved in vascular and hematopoietic development and hematolymphoid neoplasia. We have demonstrated that LMO2 is expressed nearly ubiquitously in native and neoplastic vasculature, including lymphatics. LMO2 reactivity is otherwise virtually absent in nonhematolymphoid tissues except in breast myoepithelium, prostatic basal cells, and secretory phase endometrial glands. Vasculature is LMO2- in adult and fetal heart, brain of older adults, hepatic sinusoids, and hepatocellular carcinoma. LMO2 is uniformly expressed in benign vascular and lymphatic neoplasms an

Gastrointestinal stromal tumors (GISTs) are a rare and heterogeneous group of spindle cell neoplasms that have also been reported outside of gastrointestinal (GI) tract. These tumors are characterized by somatic mutations of c-KIT (CD117), a proto-oncogene that encodes a receptor tyrosine kinase normally expressed in the interstitial cell of Cajal that control the GI smooth muscle peristalsis, and an exquisite sensitivity to the action of the tyrokinase inhibitor imatinib mesylate (STI571; Gleevec). We report two cases of gastrointestinal stromal tumor identified on prostatic biopsies, where a

Gastrointestinal stromal tumors (GISTs) are unique neoplasms that occur throughout the gastrointestinal tract, mesentery, omentum, and retroperitoneum. They are the most common mesenchymal neoplasm of the gastrointestinal tract and are defined by their expression of KIT (CD117), a tyrosine kinase growth factor receptor. The expression of KIT is important to distinguish GISTs from other mesenchymal neoplasms such as leiomyomas, leiomyosarcomas, schwannomas, and neurofibromas and to determine the appropriateness of KIT-inhibitor therapy. The series described herein was accumulated over 2 years a

Gastrointestinal stromal tumors (GISTs) are rare neoplasms arising from connective tissue elements of the gastrointestinal wall. They show a great heterogeneity with respect to their histogenetic, morphologic and prognostic characteristics. GISTs are known with myoid, neural or mixed features of differentiation. Clinical findings are gastrointestinal bleeding, abdominal pain and weight loss. We report on the case of a 50-year-old male patient who presented with melena and acute anemia (hemoglobin 10.5 g/dl). Esophagogastroduodenoscopy revealed a broad-based, centrally ulcerated polypoid format

Neuroendocrine neoplasms commonly arise from the gastrointestinal tract and lungs. Less commonly, they may occur in the gynecologic tract, typically within the ovary of a mature cystic teratoma. Primary neuroendocrine neoplasms of the fallopian tube are exceptionally rare and only a total of 11 cases have been reported in the literature. We describe the first case to our knowledge of a primary grade 2 neuroendocrine tumor of the fallopian tube in a 47-yr-old female. In this report, we describe the case's unique presentation, review the published literature on primary neuroendocrine neoplasms o

Appendiceal adenocarcinomas are rare invasive gastrointestinal neoplasms, with higher propensity to metastasize to lymph nodes and peritoneum, whereas other common sites include liver and lung. Metastases to the breast are exceptionally rare in appendiceal neoplasms, with limited data in literature. We present the case of a recurrent mucinous adenocarcinoma of the appendix with clinically occult breast, skeletal, and nonregional nodal metastases detected on fluorine-18-fluorodeoxyglucose positron emission tomography-computed tomography scan.

BACKGROUND: Low-grade mucinous neoplasms arising within mature ovarian cystic teratomas are rare and represent an uncommon ovarian source of pseudomyxoma peritonei (PMP). Because mucinous ascites and peritoneal disease are most often associated with appendiceal or other gastrointestinal primaries, ovarian-origin mucinous neoplasms may be underrecognized, contributing to diagnostic uncertainty and potential misclassification.
CASE DESCRIPTION: We report the case of a 30-year-old nulliparous woman who presented with progressively worsening abdominal pain, distension, and respiratory discomfort.

Neuroendocrine tumors (NETs) encompass a diverse spectrum of neoplasms that can originate from various sites, including the gastrointestinal tract. Brain metastases from neuroendocrine tumors, while rare, present significant clinical challenges. In this case report, we present the unique instance of a 50-year-old female with a history of gastrointestinal neuroendocrine tumor who manifested left-sided weakness, tremors, and recurrent focal convulsions. Initial imaging scans revealed a lesion in the right parietal lobe, which was surgically excised and diagnosed as a metastatic large-cell neuroe

Soft tissue sarcomas are common neoplasms accounting for 1% of all adult malignancies; however, soft tissue sarcomas infrequently arise from the abdominal viscera. Many case reports discuss gastric and esophageal neoplasms. In the group of gastrointestinal liposarcomas, primary duodenal liposarcomas are among the rarest, with only three previous cases reported in the literature. Herein, we discuss a case of primary duodenal liposarcoma. A 59-year-old woman presented with symptoms consistent with anemia raising suspicion for an upper gastrointestinal bleed. Upper endoscopy revealed an ulcerated

Over a 20 year period, 64 patients with primary neoplasms of the small intestine were treated by celiotomy (61 patients) or surgical endoscopy (3 patients). Gastrointestinal bleeding and anemia (38 percent of patients) characterized benign lesions, whereas pain (42 percent), nausea and vomiting (26 percent), weight loss (29 percent), and either gastrointestinal obstruction or jaundice (18 percent) were more indicative of malignancy. Barium studies, duodenal endoscopy, and selective angiography were the most useful diagnostic tools. All 26 patients with benign lesions did well after resection,

Neuroendocrine neoplasms are tumours that usually arise in the gastrointestinal tract and the bronchopulmonary system. The orbit is a rare anatomical site for their metastases. In the following article we present a case report of a 73-year-old man who was admitted to the Ophthalmology Department because of eye pain and high intraocular pressure in the right eye. There was also eye motility restriction, diplopia, oedema of the eyelid, and subconjunctival haemorrhage. Magnetic resonance imaging revealed a tumour in the right orbit, probably arising from the inferior rectus muscle. The patient wa

Gastrointestinal stromal tumors (GISTs) are potentially aggressive mesenchymal neoplasms with spindle cell, epithelioid, or mixed morphology. They typically express CD117, DOG1, and CD34 and can be diffusely and strongly positive for h-caldesmon. Leiomyomas are benign smooth muscle neoplasms that can arise in a variety of visceral and soft tissue sites, including the gastrointestinal tract. We illustrate a case of a neoplasm of the gastroesophageal junction that was clinically suspected to be a GIST. Histology showed a tumor composed of ovoid and spindle cells arranged in short intersecting fa

Gastroduodenal intussusception is an infrequent cause of gastrointestinal obstructive disease. Benign neoplasms, gastrointestinal stromal tumors and pedunculated adenocarcinomas of less than 5 cm have been reported to cause gastroduodenal intussusception. We report a case of 76-year-old woman who was presented with a 3-day history of nausea and vomiting due to upper gastrointestinal obstruction. Computed tomography revealed gastroduodenal intussusception with the transpyloric herniation of alarge gastric hyperplastic polyp. The patient underwent laparoscopic wedge resection with the eversion m

A 61-year-old woman with no significant past history underwent gastric biopsies demonstrating a strongly c-kit-positive epithelioid malignancy, initially thought to represent gastrointestinal stromal tumor (GIST). Subsequent clinical and immunohistochemical evaluation proved the neoplasm to represent metastatic lobular carcinoma. This case illustrates that although c-kit is highly specific and sensitive for GIST, its expression may occur in a variety of other neoplasms, some of which morphologically resemble GIST and may present in the gastrointestinal tract as metastases. Therefore, a review
