Specific early signs and long-term follow-up findings of progressive pseudorheumatoid dysplasia (PPRD) in the Turkish cohort.

OBJECTIVES: Progressive pseudorheumatoid dysplasia (PPRD) is a spondyloepiphyseal dysplasia caused by biallelic variants in CCN6. This study aimed to describe the early signs and follow-up findings in 44 Turkish PPRD patients. METHODS: The patients with progressive stiffness of multiple joints, characteristic wide metaphysis of interphalangeal (IP) joints and platyspondyly were clinically diagnosed with PPRD. Fifteen patients who had first symptoms under 3 years of age were grouped as early-onset, while others were grouped as classical. CCN6 sequencing was performed in 43 patients. RESULTS:

Practical primary pediatric orthopedics.

Pediatric orthopedic problems often puzzle the primary health provider and worry families. A thorough orthopedic history and examination by the primary care provider is all that is necessary to determine whether a problem requires further evaluation and referral. This article addresses a practical approach to common orthopedic problems, assessment, and management strategies from the specialist perspective. A general description of developmental dysplasia of the hip, foot misalignments, tibial torsion, toe-walking, genu varum (bowlegs), growing pains, sprains and fractures, and the child with a

Oblique tibial osteotomy for Blount's disease (tibia vara).

Nine oblique proximal tibial osteotomies were performed on six children with genu varum, all but one of whom had Blount's disease. The osteotomy was a single-plane cut allowing simultaneous correction of varus and internal rotation and permitting postoperative cast wedging if necessary to improve position. All osteotomies healed within 10 weeks. The major complications were three episodes of weakness of the extensor hallucis longus, two of which resolved in 4 days and one of which resolved only partially. We were able to combine the osteotomy with other procedures, such as epiphyseal bar resec

Windswept lower limb deformities in patients with hypophosphataemic rickets.

BACKGROUND: X-linked hypophosphataemic rickets is an X-linked dominant disorder that is secondary to renal phosphate-wasting. Genu varum and/or genu valgum have been described as the most common deformities in patients with hypophosphataemic rickets. Windswept deformity, which is valgus deformity in one knee and varus deformity in the other, was the most common deformity encountered in our department. PATIENTS AND METHODS: We collected seven patients who had phenotypic and genotypic features consistent with the diagnosis of X-linked hypophosphataemic rickets. All presented with windswept lowe

The preclinical sheep model of high tibial osteotomy relating basic science to the clinics: standards, techniques and pitfalls.

PURPOSE: To develop a preclinical large animal model of high tibial osteotomy to study the effect of axial alignment on the lower extremity on specific issues of the knee joint, such as in articular cartilage repair, development of osteoarthritis and meniscal lesions. Preoperative planning, surgical procedure and postoperative care known from humans were adapted to develop a HTO model in the adult sheep. METHODS: Thirty-five healthy, skeletally mature, female Merino sheep between 2 and 4 years of age underwent a HTO of their right tibia in a medial open-wedge technique inducing a normal (grou

Prospective video-based analysis of coronal lower limb alignment may be as accurate as radiography in children.

Temporary hemiepiphysiodesis is a growth-guided surgical technique to address lower limb malalignment in children with remaining growth potential. It is a minimally invasive approach that relies on full standing radiography of the lower limb, resulting in exposure to ionizing radiation. Radiation-free, video-based alternative techniques have recently evolved which seem to have potential to be applied in clinical pathways, but they are still very expensive and have a complex setup. In this prospective pilot study, a low budget and uncomplicated method to assess malalignment of the lower limb was

Comparison of femoral sagittal axis between navigated total knee arthroplasty and conventional total knee arthroplasty in patients with knee osteoarthritis.

To compare femoral sagittal axis between navigated total knee arthroplasty (TKA) and conventional TKA. A total of 136 cases were assigned to group 1 (navigated TKA) and 77 cases were assigned to group 2 (conventional TKA). Specifically, this study targeted patients with degenerative osteoarthritis. Only patients with primary TKA were analyzed. Hip knee ankle angle and lateral femoral bowing were measured using preoperative scanogram. Anterior femoral bowing was measured using preoperative femoral lateral X rays. The presence of anterior femoral notching and the insertion angle of the femoral i

Varus morphology and its surgical implication in osteoarthritic knee and total knee arthroplasty.

BACKGROUND: Knee varus alignment represents a notorious cause of knee osteoarthritis. It can be caused by tibial deformity, combined tibial-femoral deformity and/or ligament imbalance. Understanding malalignment is crucial in total knee arthroplasty to restore frontal plane neutral mechanical axis. The aim of this study was to determine which factor contributes the most to varus osteoarthritic knee and its related surgical implications in performing a total knee arthroplasty. METHODS: We retrospectively evaluated 140 patients operated for total knee arthroplasty due to a varus knee. Full-leg

Risk factors for reoperations following guided growth around the knee.

INTRODUCTION: Guided growth using tension band plates is a common treatment for genu varum and valgum deformities in children. Reoperation is a significant adverse event in surgical treatments. This study aims to identify the causes and risk factors for reoperations following guided growth around the knee. METHODS: This retrospective study reviewed children who underwent guided growth for genu varum and valgum deformities between 2012 and 2019. Reoperation was defined as revision surgeries for deformities at the same knee, excluding procedures for implant removal. Potential risk factors inclu

[Salter and Harris type-II distal femoral physeal fracture-separations at adolescent age: a new therapeutic approach (preliminary study)].

PURPOSE OF THE STUDY: The prognosis of distal femoral physeal fracture-separation is poor in children. In adolescents, more than half of the cases are classified as Salter and Harris type-II. The gold-standard treatment for a displaced fracture combines anatomic reduction with internal fixation with a pin or screw, preserving the growth cartilage. Despite this treatment, the rate of mid- and long-term complications has been high in the literature, most problems being related to leg length discrepancy and misalignments (genu valgum and genu varum). In order to avoid these problems, for adolesce

Two families with spondylo-epi-metaphyseal dysplasia due to compound heterozygocity in the vWFA domain of MATN3.

Heterozygous variants of MATN3 is one of the common causes of multiple epiphyseal dysplasia (MED). Here we report three individuals from two unrelated families who harbor compound heterozygous variants in MATN3 (p.Arg121Trp and p.Val220Ala). Contrary to the MED phenotype, these individuals exhibit spondyloepimetaphyseal dysplasia (SEMD) resembling the phenotypes caused by homozygous MATN3 variants. Clinical manifestations included short stature, aggravating genu varum, joint laxity, and spinal abnormalities. Radiographic findings were distinct from typical MED. These compound heterozygous vari

A De Novo Mosaic PHEX Variant Causing Sporadic X-Linked Hypophosphatemic Rickets in a 2-Year-Old Girl.

Pathogenic variants in the PHEX gene are causative of X-linked hypophosphatemic rickets (XLH). We present a case of a 2-year-old girl with hypophosphatemic rickets with genu varum and short stature without any family history of XLH. Next generation sequencing of the PHEX gene identified a splice donor variant, NM_000444.6:c.1173 + 5G > A in intron 10. This variant had a mosaic pattern with only 22% of the sequence reads showing the variant allele and was not present in the girl's parents, both of whom had a normal phenotype. This is a sporadic case of a de novo mosaic splice-site variant in th

A Novel Presentation of Metaphyseal Chondrodysplasia, Schmid Type with Factor VII Deficiency.

Metaphyseal chondrodysplasia, Schmid type (MDSC) is a rare inherited autosomal disorder with characteristic skeletal deformities striking on radiological imaging, which includes metaphyseal cupping and fraying. Physical examination reveals short stature in early childhood, frontoparietal bossing, rachitic rosary, genu varum and valgum, and coxa vara usually. We believe that the constellation of clinical and radiographic findings of MDSC might look similar to vitamin D resistant rickets; hence, genetic analysis is needed to overcome diagnostic challenges faced by physicians to avoid unnecessary

Medial tibial hemi-condylar elevation osteotomy as an operative technique to treat varus mal-united tibial plateau fracture.

Hemi-condylar tibial osteotomy was first reported as a corrective surgical option for neglected Blount's disease by Langenskiold in 1964. The indication for this procedure is largely reserved for the rare cases of excessive ligament laxity. To the best of our knowledge, literature search reveals only 6 cases, all of which were of Blount's disease and there has been no report of this procedure being used in Trauma. We present a case of failed osteosynthesis of Type V tibial plateau fracture, which resulted in genu varum deformity secondary to collapse of only the medial tibial condyle. We succe

[Evaluation of the locomotor system during pediatric health examinations and elements of kinesitherapy].

The pediatrician, being a primary health care physician, has the task of bringing the small patient and his parents to the resolution of all those problems which are erroneously referred to a subspecialist. With this in mind, the authors believe that even common orthopedic problems must be evaluated by the pediatrician. He would decide if and when to refer the child to an orthopedic specialist. Certain problems such as congenital hip dysplasia or scoliosis require specific treatment, whereas others (tibial bowing, genu varum-valgum, flatfoot, metatarsus varus) resolve themselves over time. Myopia and systemic manifestation of tissue hyperlaxity: A population-based cross-sectional study.

BACKGROUND: Whether non-syndromic connective tissue hyperlaxity is associated with myopia is unknown. The aim of this study was to examine the association between systemic signs of tissue hyperlaxity and myopia among adolescents. METHODS: Included were adolescents assessed before mandatory military service at the age of 16-18 years between 2011 and 2022. Diagnoses of hernias, pes planus, genu varus, genu valgum, and scoliosis, as well as joint injuries were used as surrogate markers for tissue hyperlaxity. The prevalence of these events among adolescents with myopia was evaluated and compared
