Laser-Assisted Esthetic Crown Lengthening: Open-Flap Versus Flapless.

This study compares the clinical outcomes of Er,Cr:YSGG (2,780 nm) laser-assisted open-flap (OF) and flapless (FL) esthetic crown lengthening (ECL) for the treatment of altered passive eruption. Thirty-six healthy patients requiring ECL were randomly divided into two groups: OF and FL. Gingivectomy and ostectomy were performed with an Er,Cr:YSGG laser in both groups. The periodontal condition and gingival margin level (GML) were assessed at baseline, immediately postsurgery, and at 1, 3, and 9 months postsurgery. The effect of periodontal phenotype and tooth location on GML and supracrestal gi

Use of the gingival unit transfer in soft tissue grafting: report of three cases.

Current procedures in periodontal plastic surgery probe the limits of the copious blood supply available to the periodontal tissues. None reaches for these limits more consistently and meets with less forgiveness than soft tissue grafting for the interdental papilla and root coverage in esthetically sensitive areas. Gingiva contains the only soft tissue naturally created to survive and function interproximally and facially over avascular root surfaces. Preliminary results suggest that the supracrestal gingiva, used as a free graft for the above purposes, may have an uncommon capacity for perfu

Advanced periodontic techniques.

Periodontal disease is multifaceted in nature. Problems occur, such as mucogingival problems, osseous deformities, and gingival enlargement, and they require multiple types of treatment. There is no one way to approach each problem or procedure. Philosophy, talent, experience, training, and objectives ultimately determine treatment selection.

Cosmetic periodontal procedures.

This paper reviews the newest advances in and opinions about cosmetic periodontal surgery. It includes a review of basic periodontal anatomy as it relates to restoring ideal health and esthetic gingival and osseous tissues. Treatments for excessive gingival tissue, ridge augmentations, and gingival deficiencies are discussed. These procedures are important in creating the illusion of a natural cosmetic smile in otherwise compromised situations.

Refinement of the transpalatine exposure of the nasopharynx.

The intrasulcular incision provides excellent transpalatine exposure of the nasopharynx with a viable palatal flap. There is less chance for naso-oral fistula formation since the resulting suture line is always over underlying bone. This safely allows extensive removal of the hard palate. Additionally, the continuous sling suture allows excellent flap reapproximation using the teeth as anchors for the replaced palatal flap. Patients tolerate oral feedings within 24 hours of their operations and require minimal postoperative analgesics. The extended intrasulcular incision offers many advantages

Treating a severe iatrogenic gingival exposure and lip incompetence - a challenge worthwhile.

This case reports the retreatment of a young adult who had a previous orthodontic history of inappropriate biomechanical strategies leading to iatrogenic effects with the formation of a dual occlusal plane causing anterior dental extrusion. As a result, an already severe dento-skeletal malocclusion with a vertical growth pattern, severe gummy smile, increased overjet and a very deep bite along with procumbent and incompetent lips became exaggerated. The high smile line and heavy convex profile were changed to an average smile line and a straight profile by intruding the anterior maxillary segm

Periodontal management of severe periodontitis and generalized gingival enlargement in a patient with chronic renal failure.

Gingival enlargement is a common periodontal pathology seen in medically compromised patients. Although it is not the disease itself, certain medications used to treat these chronic diseases are known to precipitate the gingival enlargement. Periodontitis (PD) and gingival enlargement have been reported increasingly in patients with chronic renal failure. Severe enlargement is detrimental to esthetics and function while having a negative impact on the overall oral health-related quality of life. Treatment of such cases requires comprehensive periodontal management by a specialist, keeping in m

Hereditary Gingival Fibromatosis: A Report of a Rare Case in Siblings and Its Management Using Diode Laser.

Hereditary gingival fibromatosis (HGF) is a proliferative fibrous lesion causing severe gingival enlargement, affecting the esthetics, as well as posing various periodontal problems. This case report addresses the diagnosis and treatment of one such rare case of HGF where the patient presented with generalized diffuse gingival enlargement involving the maxillary and mandibular arches extending on the buccal and lingual/palatal surfaces and covering the incisal/occlusal third of the tooth, resulting in altered esthetics, difficulty in speech, and mastication. Gingivectomy was carried out in all

Phenytoin-induced severe gingival overgrowth in a child.

Gingival enlargement or overgrowth (GO) is a common complication of the anticonvulsant drug phenytoin (PHT). GO is evident in almost half of the patients receiving PHT therapy. PHT-induced gingival overgrowth (PGO) is more common in children than in adults and affects both males and females equally. PGO may vary from mild to severe and does not seem to be dose dependant. It is supposed that PHT and its metabolites cause a direct effect on the periodontal tissues; however, poor oral hygiene may contribute to the severity of gingival inflammation in patients with PGO. Management of PGO includes

Primary gingival pseudoepitheliomatous hyperplasia with periodontal findings: a rare case report.

BACKGROUND: This paper is the first-ever report, to our knowledge, of a case of generalized primary gingival pseudoepitheliomatous hyperplasia(PEH) with significant periodontal findings in a 23-year-old Indian female patient. It explains the need to differentiate PEH from squamous cell carcinoma in diagnosis and treatment planning. The present article also reviews current immunohistologic staining methods used to differentiate PEH from squamous cell carcinoma. METHODS: A two-stage surgical approach was used to eliminate the lesion by gingivectomy and gingivoplasty under local anesthesia. Hist

Three siblings with juvenile hyaline fibromatosis.

Juvenile hyaline fibromatosis (JHF) is an extremely rare hereditary genetic disease of autosomal recessive transmission that is characterized by large cutaneous tumors commonly involving the scalp, papulonodular skin lesions, flexural joint contractures, gingival hyperplasia, and osteolytic bone lesions. JHF is usually diagnosed in young infants and in children younger than 5 years, and the lesions characteristic of this disorder consist of fibrous tissue and homogenous amorphous eosinophilic hyaline material. We report the case of a 9-year-old girl with severe gingival hyperplasia, nasal enla

Generalized hereditary gingival fibromatosis in a child: clinical, histopathological and therapeutic aspects.

Hereditary gingival fibromatosis (HGF) is a rare genetic condition characterized by slow and progressive gingival enlargement. The gingival overgrowth often delays teeth eruption and may cause serious functional and aesthetic problems. We reported a case of a 10-year-old female child presenting a generalized gingival enlargement covering almost all the maxillary and mandibular teeth and resulted in problems for swallowing, speaking, and poor aesthetics. An incisional biopsy was performed and revealed a hypocellular and hypovascular dense collagenous tissue covered by squamous epithelium exhibi

Unusual presentation of familial gingival fibromatosis among male siblings.

Gingival enlargement refers to any increase in the size of normal gingiva. The gingiva may be enlarged in response to various interactions between the host and the environment. It can be of various types including inflammatory, drug induced, associated with systemic disease, neoplastic, false, and idiopathic. Idiopathic gingival enlargement is a rare condition of undetermined etiology. The enlargement can be localized or generalized to entire dentition and is usually associated with the emergence of the teeth into the oral cavity and may regress after extraction. Treatment aims to solve patien

Gingival fibromatosis combined with cherubism and psychomotor retardation: a rare syndrome.

Gingival fibromatosis is frequently an isolated condition, but rarely associated with some uncommon syndromes. This paper describes an 11-year-old patient with pronounced gingival enlargement, cherubic facial appearance, and psychomotor retardation and discusses the major aspects of the case. The most striking finding orally was the presence of grossly hyperplastic gingiva, which completely covered all teeth except the occlusal surfaces of some teeth. The swelling in the lower part of the face and the appearance of sclera beneath the iris suggest cherubism. The diagnosis was confirmed by the d

[Methods aimed at lengthening the clinical crown: a review].

This review deals with the different methods of lengthening the clinical crown and discusses the advantages and/or disadvantages of each method. Different flap procedures, including ostectomy and osteoplasty, reduce the height of the alveolar crest aiming at a distance of about 3 mm to a future reconstruction margin. Root planing may avoid reattachment of surgically separated fibers. Other methods like gingivectomy, electrosurgery, intra-alveolar transplantation, and forced eruption might contain considerable biological disadvantages. However, if the extrusion is combined with regular fiberoto

Rapidly Recurrent Fibrous Gingival Enlargement in a Geriatric Patient: Clinical Significance of Recurrence Timing.

Localized gingival enlargement (epulis) demonstrates variable recurrence rates, yet the clinical significance of recurrence timing has received limited attention. Regrowth may occur gradually due to persistent irritation or more rapidly when residual periodontal ligament or periosteal tissue remains after excision. A 74-year-old female with systemic comorbidities presented with fibrous epulis in the lower left anterior region. Histopathological examination following initial periodontal therapy and gingivectomy confirmed the diagnosis. The lesion recurred at the same site within 4 months. Manag

Hereditary gingival fibromatosis associated with generalized aggressive periodontitis: a case report.

BACKGROUND: Hereditary gingival fibromatosis is a rare, genetically inherited overgrowth condition that is clinically characterized by a benign fibrous enlargement of maxillary and mandibular keratinized gingiva. A syndromic association between gingival fibromatosis and a wide variety of other genetically inherited disorders has been described. However, its coexistence with aggressive periodontitis has not been reported. METHODS: A 24-year-old African-American female, patient (proband X, [Px]) reported with a chief complaint of tooth mobility and gingival enlargement. Clinical examination rev

Pink-White Esthetic Optimization in Excessive Gingival Display Using Crown Lengthening and Direct Composite Veneering.

An esthetic smile plays a pivotal role in enhancing patient confidence and social well-being. Smile esthetics are influenced by multiple factors, including maxillary anterior tooth morphology, lip length and mobility, periodontal health, and the degree of gingival display. Short clinical crowns associated with excessive gingival display are generally perceived as unesthetic and frequently require esthetic crown lengthening. Altered active eruption (AAE) and altered passive eruption (APE) represent common etiologic factors in such presentations. Management typically involves gingivectomy with o

High-Power Surgical Laser for Treatment of Oral Manifestations of Mucopolysaccharidosis VI: A Case Report.

Mucopolysaccharidosis type VI (MPS VI), a rare metabolic disorder, results from glycosaminoglycan accumulation in tissues, frequently leading to hyperplastic gingival alterations that necessitate comprehensive intervention. An 11-year-old male patient diagnosed with MPS VI required hospitalization due to complications associated with a ventriculoperitoneal shunt (VPS) valve. A dental evaluation identified bone malformation in the maxillomandibular complex accompanied by pronounced gingival hyperplasia. The surgical team performed a gingivoplasty in the operating room under general anesthesia,

Complex Odontoma Initially Suspected to be Eruption Sequestrum.

Various pieces of information, including the age of the patient, the site involved, symptoms, intraoral findings, and X-ray findings are necessary in making a clinical diagnosis. This report describes a case in which the initial diagnosis was eruption sequestrum based on clinical information but which was later found to be a complex odontoma based on histopathological findings. The patient was an 8-year-old girl who presented at our hospital with the chief complaint of pain in the right mandibular molar region.
