From August 1981 to May 1993 a total of 1263 percutaneous retrogasserian glycerol rhizotomies after Hakanson were performed. The intervention was performed with X-ray monitoring under local anaesthesia and rarely lasted longer than 20 min. Circulatory changes and renal uptake of free fatty acids (FFA), glycerol and triglycerides were studied in ten adult beagle dogs during pentobarbital anesthesia. Six dogs were injected intravenously with E. coli endotoxin 0.5 mg/kg over 15 min and four control dogs received saline. We studied the effect of urea, glycerol, and mannitol on basilar and labyrinthine arteries of dogs in vitro by use of tension recording methods. When the tissue was exposed to urea or glycerol, temporary relaxation of smooth muscles was initially observed, but was followed by gradually increasing contraction. Sixty patients with trigeminal neuralgia or atypical facial pain were followed for an average of 1 year after percutaneous retrogasserian glycerol rhizolysis. Sixteen cases are reported treated with glycerol gangliolysis of Gasser's ganglion for trigeminal neuralgia. Glycerol was injected into Meckel's cavum from the percutaneous approach of Härtel with radiological monitoring. In the follow-up from 6 months to 2 years in 75% of cases no recurrence of neuralgia was observed. No complications were noted. The method seems to be particularly indicated in elderly patients and those with concomitant internal diseases in whom a decompressing operation in the area of the cerebellopontine angle is connected with greater risk.

Acute intracranial hypertension may respond to intravenous mannitol, but frequent administration can cause cerebral edema or renal problems. The effectiveness of oral glycerol as a dietary component or as a supplement to a 1000-kcal/day diet was examined in two studies involving obese patients. Glycerol did not differ from an equicaloric dose of glucose in its effect on hunger ratings, diet compliance or overall weight loss. Glycerol has been injected intravenously in guinea pig and its effects on the pressure in the cochlear fluids have been studied. Simultaneously, arterial and cerebro-spinal pressures have been recorded. Glycerol lowered the intracochlear as well as cerebro-spinal and blood pressures, the latter only temporarily. Different possible mechanisms for the glycerol effect on the intracochlear pressure and its transient effect on hearing in Menière's disease are discussed.
