The prevention of adverse physiological change in Space Station crewmembers.

Various physiological countermeasures, consisting primarily of isotonic and isometric exercises but also including prescribed nutrient intake, have been used in all manned spaceflights exceeding about one month in duration. So consistent has been this practice that the effects of weightlessness on the human, unconfounded by the use of countermeasures, are difficult to discern. Equally elusive, in the absence of control studies conducted in weightlessness, is an accurate assessment of the efficacy of the countermeasures themselves. Changes in body composition occurring during and following flig

[Crush injury of the lower limbs. Use of anti-shock trousers].

A case of lower limb crush injury is reported. A 16 year old female sustained severe soft tissue, muscle, skeletal and vascular lesions during a road traffic accident. She rapidly presented with severe haemorrhagic shock due to a popliteal arterial injury. At the scene, massive vascular loading by emergency physicians using both colloids and group O negative packed red cells was not enough to improve the haemodynamic state, even though there was adequate control of the bleeding artery. The pneumatic antishock trousers were therefore used despite the severe injuries to the legs, with rapid impr

Sustaining humans in space.

One goal of NASA's Ames Research Center is to extend the presence of humans in space. Biomedical research, human performance, and life-support systems form the core of the Ames program. Major questions highlight how the effects of microgravity on living systems are modified by exercise, artificial gravity, autogenic feedback training, and nutrition. Bioengineering and life science studies include humans, animals, and plants. Ames investigators collaborate extensively with outside university and commercial scientists, both within the United States and internationally.

MAST suit update.

In recent years, the use of the MAST suit has become commonplace. While no controlled human studies have shown the device's efficacy, extensive clinical experience suggests that it is practical and useful in combating shock, stabilizing fractures, and promoting hemostasis. Studies indicate that the antishock trousers elevate blood pressure primarily by increasing peripheral vascular resistance. Proper application and removal of the device are crucial. A number of potential complications exist, but are infrequent and rarely should preclude MAST suit use. Further studies are necessary to prove t

Exploration of the Feasibility of Mini-Percutaneous Nephrolithotomy with Gravity-Assisted Low-Pressure Irrigation in the Treatment of Renal Stones with "Cat's Paw Sign" Pyonephrosis Instead of Multiple Percutaneous Drainage.

INTRODUCTION: To investigate the feasibility of mini-percutaneous nephrolithotomy (mPCNL) with gravity-assisted low-pressure irrigation in managing renal pelvic stones complicated by multiple calyceal pyonephrosis (characterized by the "cat's paw sign" on CT scan) as an alternative to multiple percutaneous drainage. METHODS: A retrospective analysis was conducted on 7 patients (4 males, 3 females; mean age 52.1 years) who underwent one-stage mPCNL between 2019 and 2023. Meticulous preoperative antibiotic prophylaxis was administered, and intraoperative perfusion pressure was maintained below

Abdominal haemorrhage--a preventable cause of death after field stabilization?

The causes of preventable death vary in different operational settings, and the topic has not previously been explored in a fully developed central European rescue system. The factors associated with potentially preventable death were studied in a retrospective study of 430 fatal traffic accident victims (1980-96) in Lörrach County, Germany. Mission protocols could be retrieved for detailed analysis in 239 of the cases. These were studied in order to identify factors associated with preventable death. At the scene of the accident, 38% of the patients died without cardiopulmonary resuscitation

Delayed diagnosis of vascular injuries during lumbar discectomy.

We report an accidental perforation of the left iliac artery and vein during lumbar discectomy in the kneeling position. During the surgical procedure, a brief and transient hypotension occurred, although the surgeon did not observe any abnormal bleeding in the operating field. In the recovery room, arterial blood pressure decreased, and tachycardia, pallor, and abdominal pain occurred. Retroperitoneal bleeding was suspected. Immediate fluid resuscitation was initiated, anesthesia was continued, and red blood cells were ordered. The use of medical antishock trousers enabled temporization until

Scaling up interventions: findings and lessons learned from an external evaluation of Niger's National Initiative to reduce postpartum hemorrhage.

BACKGROUND: Niger has one of the highest maternal mortality ratios in Sub Saharan Africa, of which postpartum hemorrhage is the leading cause. In 2014, Health and Development International and the Ministry of Health of Niger launched an initiative to introduce and scale-up three PPH interventions in health facilities nationwide: misoprostol, uterine balloon tamponade, and the non-pneumatic anti-shock garment. METHODS: A two-phase mixed-methods evaluation was conducted to assess implementation of the initiative. Health facility assessments, provider interviews, and household surveys were condu

A continuum of care model for postpartum hemorrhage.

The leading cause of maternal mortality is hemorrhage, generally occurring in the postpartum period. Current levels of PPH-related morbidity and mortality in low-resource settings result from institutional, environmental, cultural and social barriers to providing skilled care and preventing, diagnosing and treating PPH. Conventional uterotonics to prevent PPH are typically not available or practical for use in low-resource settings. In such deliveries, most often taking place at home or in rural health centers, underestimation of blood loss leads to a delay in diagnosis. Deficiencies in commun

Congenital hip dislocation: the importance of early detection and comprehensive treatment.

Hip dislocation is the musculoskeletal condition most commonly missed during neonatal examinations. Failure to diagnose the condition can lead to long-term disability and is a common target in pediatric legal suits concerning damage to the musculoskeletal system. Early neonatal assessment for hip dislocation includes an examination using the Barlow and Ortolani tests. Later examinations include assessment of gluteal folds, knee height and the degree of hip abduction. The preferred treatment is use of the Pavlik harness, an outpatient treatment regime that provides effective reduction in 90 per

Modeling orientation perception adaptation to altered gravity environments with memory of past sensorimotor states.

Transitioning between gravitational environments results in a central reinterpretation of sensory information, producing an adapted sensorimotor state suitable for motor actions and perceptions in the new environment. Critically, this central adaptation is not instantaneous, and complete adaptation may require weeks of prolonged exposure to novel environments. To mitigate risks associated with the lagging time course of adaptation (e.g., spatial orientation misperceptions, alterations in locomotor and postural control, and motion sickness), it is critical that we better understand sensorimotor

Resuscitation of the multiply injured patient.

The resuscitation of the traumatized patient with multisystem injuries begins on the scene of the incident, continues into the emergency department, and finally includes the surgeon and the operating room. Treatment of the traumatized patient requires a systematic approach to best utilize all facilities and personnel while limiting duplication of service. The patient must go to the facility that best meets his or her needs. Stabilization in the emergency department should proceed in an orderly and organized fashion. Care must be taken when establishing the airway to protect the cervical spine.

Pathogenesis of hyperadrenergic orthostatic hypotension. Evidence of disordered venous innervation exclusively in the lower limbs.

The pathogenesis of hyperadrenergic orthostatic hypotension was studied in eight patients. Correction of the abnormal orthostatic changes by an inflated pressure suit (MAST) confirmed previous evidence of excessive gravitational pooling of blood in the leg veins. Intravenous L-norepinephrine infusion raised diastolic blood pressure in the same relationship to the infusion-induced increments in plasma norepinephrine concentrations as in normal subjects, indicating normal arteriolar responses. Contractile responses of the veins to infused L-norepinephrine were measured with a linear variable dif

Application of USAF G-suit technology for clinical orthostatic hypotension: a case study.

INTRODUCTION: The purpose of this study was to determine the effectiveness of a USAF anti-gravity suit (G-suit) on the stability of a patient with chronic orthostatic hypotension. METHODS: A 37-yr-old female with a history of insulin-dependent diabetes mellitus (IDDM) and symptomatic orthostasis was evaluated and the results were compared with those of non-diabetic controls, matched for age, height, and weight. Cardiac vagal tone was assessed by determination of standard deviation of 100 R-R intervals (R-R SD). We assessed the carotid-cardiac baroreflex response by plotting R-R intervals (ms)

Assessing the role of the non-pneumatic anti-shock garment in reducing mortality from postpartum hemorrhage in Nigeria.

BACKGROUND/AIMS: Postpartum hemorrhage (PPH) is the leading cause of maternal mortality. The non-pneumatic anti-shock garment (NASG), a first-aid lower-body pressure device, may decrease mortality. METHODS: This pre-intervention/NASG study of 288 women was conducted in four referral facilities in Nigeria, 2004-2008. Entry criteria: women with PPH due to uterine atony, retained placenta, ruptured uterus, vaginal or cervical lacerations or placenta accreta with estimated blood loss of ≥750 ml and one clinical sign of shock. Differences in demographics, conditions on study entry, treatment and o

Excessive gravitational blood pooling caused by impaired venous tone is the predominant non-cardiac mechanism of orthostatic intolerance.

1. In a group of 40 patients with orthostatic intolerance due to hypotension and/or tachycardia, we have compared the pathogenetic roles of impaired contractility of the arterioles and the veins by measuring contractile responsiveness of the arterioles, reflected by increases in diastolic blood pressure and of the veins reflected by measurements of reduction in venous diameter during intravenous noradrenaline infusions. 2. Compared with 27 healthy subjects, patients with diffuse autonomic insufficiency showed striking supersensitivity in diastolic blood pressure (six out of eight) and venous c
