The Iatrogenic Consequences of Medicalising Grief: Resetting the Research Agenda.

When the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) was published in 2013, there was a firestorm of controversy about the elimination of the bereavement exclusion. Proponents of this change and of the proposed "complicated grief" designation believed that this change would help clinicians recognise major depression in the context of recent bereavement. Other researchers and clinicians have raised concerns about medicalising grief. In 2022 "prolonged grief disorder" (PGD) was officially included in the DSM-5-TR in the trauma- and stressor-related disorder

Exploring the role of empathy in prolonged grief reactions to bereavement.

Grief reactions to the bereavement of a close individual could involve empathy for pain, which is fundamental to social interaction. To explore whether grief symptoms interact with social relatedness to a person to whom one directs empathy to modulate the expression of empathy, we administered an empathy task to 28 bereaved adults during functional magnetic resonance imaging, in which participants were subliminally primed with facial stimuli (e.g., faces of their deceased or living relative, or a stranger), each immediately followed by a visual pain stimulus. Individuals' grief severity promot

Complicated grief and manic comorbidity in the aftermath of the loss of a son.

Based on the recommendations of the sub-workgroup on trauma and dissociative disorders, the American Psychiatric Association (APA) removed the "bereavement exclusion" from the criteria for major depression in DSM-5. In addition, proposed DSM-5 research criteria for persistent complex bereavement disorder (PCBD) were included in the new manual in a section for conditions and criteria needing further research. We describe a case that warranted such a diagnosis. The patient was a 52- year-old woman who was admitted to the inpatient unit of our clinic on the birthday of her son who had died 18 mon

An item response theory analysis of a measure of complicated grief.

Item response theory modeling was applied to the data of 1,321 bereaved individuals who completed the Dutch version of the Inventory of Complicated Grief--Revised (ICG-R)--a 29-item self-report measure of complicated grief (CG). The authors aimed to examine the information that each of the ICG-R items contributes to the measurement of overall CG severity and to detect possible differences between subgroups of mourners in the way CG items are endorsed. Findings showed that items differed in their ability to distinguish individual differences in CG severity. Items that discriminated best between

Identifying and managing preparatory grief and depression at the end of life.

Grief and depression present similarly in patients who are dying. Conventional symptoms (e.g., frequent crying, weight loss, thoughts of death) used to assess for depression in these patients may be imprecise because these symptoms are also present in preparatory grief and as a part of the normal dying process. Preparatory grief is experienced by virtually all patients who are dying and can be facilitated with psychosocial support and counseling. Ongoing pharmacotherapy is generally not beneficial and may even be harmful to patients who are grieving. Evidence of disturbed self-esteem, hopeless

The death surround: factors influencing the grief experience of survivors.

Factors surrounding the death of a loved one can have a profound impact on the grief experience of survivors. This study examined the relationship between participation in a hospice program of care, place of death, length of illness prior to death, and the grief experience of survivors of patients who have died of cancer. Sixty survivors responded to the Grief Experience Inventory (GEI). Survivors of terminally ill people who participated in a hospice program prior to the death showed decreased feelings of guilt, dependency, loss of control, despair, numbness, shock, and disbelief. Grief as a neuroendocrine transition: Insights from perinatal loss.

Grief is an all-encompassing human experience that reverberates across mind, brain, and body. Historically, the scientific study of grief has been rooted primarily in psychological frameworks. Pioneering work has begun to elucidate how attachment loss is encoded not only psychologically but also biologically, and this may be particularly significant during the perinatal period.

Ecological Validity of Grief Rumination Measures Among Bereaved People.

Grief rumination, characterised by repetitive thinking about the loss and its causes and consequences, is linked to various psychopathological symptoms, including prolonged grief disorder (PGD). Traditional assessments of grief rumination rely on trait self-report questionnaires assessing multiple types of rumination (e.g., reactions, injustice, counterfactuals and meaning), which may be susceptible to memory biases and often fail to capture the dynamic and context-dependent nature of ruminative thoughts. This brief report evaluates the ecological validity of trait versus state grief ruminatio

Beyond grief: Quantifying bereavement needs of rural family caregivers.

OBJECTIVES: Family caregivers (FCGs) may experience numerous psychosocial and practical challenges with interpersonal relationships, mental health, and finances both before and after their care recipient (CR) dies. The specific challenges affecting rural FCGs who often have limited access to palliative care services, transitional care, and other community resources are not well understood. The purpose of this paper is to quantify the challenges rural FCGs experienced immediately before the death of a CR and continuing into the bereavement period. METHODS: A secondary analysis of data from a r

Emotion Regulatory Strategies in Complicated Grief: A Systematic Review.

Prolonged grief disorder, characterized by severe, persistent, and disabling grief, has recently been included in the International Classification of Diseases-11 (ICD-11). Emotional disturbances are central to such complicated grief responses. Accordingly, emotion regulation is assumed critical in the development, persistence, and treatment of complicated grief. Yet, a comprehensive review on this topic is lacking. We conducted a systematic review (PROSPERO: CRD42017076061) searching PsycInfo, Web of Science, and PubMed to identify quantitative research examining relationships between emotion

Complicated grief and related bereavement issues for DSM-5.

Bereavement is a severe stressor that typically incites painful and debilitating symptoms of acute grief that commonly progresses to restoration of a satisfactory, if changed, life. Normally, grief does not need clinical intervention. However, sometimes acute grief can gain a foothold and become a chronic debilitating condition called complicated grief. Moreover, the stress caused by bereavement, like other stressors, can increase the likelihood of onset or worsening of other physical or mental disorders. Hence, some bereaved people need to be diagnosed and treated. A clinician evaluating a be

Insomnia and complicated grief symptoms in bereaved college students.

In this study, we extended previous research by concentrating on sleep- and grief-related symptoms in a cohort of bereaved college students, in view of the potential for each of these problems to exacerbate the other. A sample of 815 college students completed the Inventory of Complicated Grief (H. G. Prigerson & S. C. Jacobs, 2001), along with an assessment of diagnostic criteria for insomnia and associated sleep behaviors. As predicted, the rate of insomnia was significantly higher (22%) in the bereaved sample than in a nonbereaved comparison group (17%), a difference that was particularly p

Prevalence of loss and complicated grief among psychiatric outpatients.

OBJECTIVE: This study examined the prevalence of significant loss through the death of another person as well as complicated grief among patients at two psychiatric outpatient clinics for one year. METHODS: A total of 729 patients were interviewed about significant losses through death during their lives. Standard questionnaires were used to classify 235 patients who had experienced such losses into three groups: those who had minimal disturbance, those who had moderate complicated grief, and those who had severe complicated grief. Multivariate and univariate analyses of variance were used to

Grief reactions treated in a psychiatric out-patient clinic.

Grief reactions to the death of a close relative are not commonly treated in psychiatric out-patient clinics in Japan. However, recently our clinic has had a chance to see such cases. I analyzed 11 cases of grief reactions (7 females, 4 males). The first part of this paper focuses on the distinction between normal and abnormal grief reactions, and an attempt was made to classify grief reactions in the nosology. Following this, three factors were specified as enhancing vulnerability to bereavement: (1) sex (female), (2) age (middle-aged), and (3) cause of death (sudden death). The second part f
